Abstract Background: The Coronavirus Disease 2019 (COVID-19) pandemic presents a novel challenge to modern healthcare systems and medical training. Resource allocation and risk mitigation has dramatically affected resident training with the subsequent cancellation of elective procedures, 14-day isolation recommendations, and social distancing requirements. To combat the unique challenges to resident education and wellness, academic leaders must develop new strategies to maintain a healthy, competent residency program. Methods: Our institution implemented a revolving 3-Team system. While the “Inpatient-Team” delivered direct care to orthopaedic patients, the “Back-up Team” and “Quarantine-Team” managed the telemedicine virtual clinic and education-wellness strategy, respectively. The education strategy included active learning methods on virtual platforms, junior resident-specific sessions, and subspecialty-interest panels. Research teams were built and rapidly deployed virtually for large scale retrospective studies. For the wellness strategy plan, our prior resident “family” organization (peer support group) was supplemented by friendly interdepartmental competitions and virtual faculty social hours. In order to evaluate the effectiveness of our implemented strategies a blinded survey was completed by the residents affected by the pandemic. Results: Our 3-Team system allowed for the delivery of safe, high-quality patient care while optimizing resident education, research, and wellness. One hundred percent of residents felt they had the tools necessary to protect themselves throughout the pandemic and 94% felt that program leadership cared about their wellness and safety. In terms of our education and wellness strategy plan, the efficient use of technology led to both improved virtual education outside of the hospital and intentional wellness opportunities despite social distancing restrictions. Eighty-eight percent of residents felt the program was able to offer valuable educational opportunities despite the pandemic. Overall, 76% of residents did not feel the COVID-19 pandemic negatively impacted their training or preparedness for their career, however 75% of PGY4’s felt they missed important subspecialty exposure and 50% felt that it negatively impacted their training. Conclusions: The COVID-19 pandemic is unlikely to be the last challenge the medical training community faces. Utilization of virtual platforms for patient care, education, research, and wellness grew out of necessity in this pandemic, yet represents an opportunity for lasting improvement with re-entry.

Abstract The purpose of this work is to develop a computationally efficient model of viral spread that can be utilized to better understand influences of stochastic factors on a large-scale system - such as the air traffic network. A particle-based model of passengers and seats aboard a single-cabin 737-800 is developed for use as a demonstration of concept on tracking the propagation of a virus through the aircraft's passenger compartment over multiple flights. The model is sufficiently computationally efficient so as to be viable for Monte Carlo simulation to capture various stochastic effects, such as number of passengers, number of initially sick passengers, seating locations of passengers, and baseline health of each passenger. The computational tool is then exercised in demonstration for assessing risk mitigation of intervention strategies, such as passenger-driven cleaning of seating environments and elimination of middle seating.

BACKGROUND The availability of large-scale and fine-grained aggregated mobility data has allowed researchers to observe the dynamic of social distancing behaviors at high spatial and temporal resolutions. Despite the increasing attentions paid to this research agenda, limited studies have focused on the demographic factors related to mobility and the dynamics of social distancing behaviors has not been fully investigated. OBJECTIVE This study aims to assist in the design and implementation of public health policies by exploring the social distancing behaviors among various demographic groups over time. METHODS We combined several data sources, including mobile tracking data and geographical statistics, to estimate visiting population of entertainment venues across demographic groups, which can be considered as the proxy of social distancing behaviors. Then, we employed time series analyze methods to investigate how voluntary and policy-induced social distancing behaviors shift over time across demographic groups. RESULTS Our findings demonstrate distinct patterns of social distancing behaviors and their dynamics across age groups. The population in the entertainment venues comprised mainly of individuals aged 20–40 years, while according to the dynamics of the mobility index and the policy-induced behavior, among the age groups, the extent of reduction of the frequency of visiting entertainment venues during the pandemic was generally the highest among younger individuals. Also, our results indicate the importance of implementing the social distancing policy promptly to limit the spread of the COVID-19 infection. However, it should be noticed that although the policy intervention during the second wave in Japan appeared to increase the awareness of the severity of the pandemic and concerns regarding COVID-19, its direct impact has been largely decreased could only last for a short time. CONCLUSIONS At the time we wrote this paper, in Japan, the number of daily confirmed cases was continuously increasing. Thus, this study provides a timely reference for decision makers about the current situation of policy-induced compliance behaviors. On the one hand, age-dependent disparity requires target mitigation strategies to increase the intention of elderly individuals to adopt mobility restriction behaviors. On the other hand, considering the decreasing impact of self-restriction recommendations, the government should employ policy interventions that limit the resurgence of cases, especially by imposing stronger, stricter social distancing interventions, as they are necessary to promote social distancing behaviors and mitigate the transmission of COVID-19. The COVID-19 outbreak has had a serious impact on almost all countries in the world, including Indonesia. In response to this case, various policies began to emerge. Starting from the implementation of work from home, social distancing and physical distancing, until the implementation of large-scale social restrictions (PSBB). overseas investors are busy focusing their finances on the needs of their respective countries to fight the virus. Domestic investment (PMDN) is also predicted to experience a slowdown. The social distancing policy resulted in the community not being able to run the economic system well, especially in the Indonesian investment sector so that the perokoniman namely investment in Indonesia decreased and there were some delays in investment by other countries in Indonesia.

Understanding how individuals perceive the barriers and benefits of precautionary actions is key for effective communication about public health crises, such as the COVID-19 outbreak. This study used innovative computational methods to analyze 30,000 open-ended responses from a large-scale survey to track how Wisconsin (U.S.A.) residents' perceptions of the benefits of and barriers to performing social distancing evolved over a critical time period (March 19th to April 1st, 2020). Initially, the main barrier was practical related, however, individuals later perceived more multifaceted barriers to social distancing. Communication about COVID-19 should be dynamic and evolve to address people's experiences and needs overtime.

AimsTo explore the level of supervision between training and non-training posts at LSCFT.Background•Supervision is defined as ‘provision of guidance and feedback on matters of personal, professional and educational development in the context of a trainees' experience of providing safe and appropriate patient care’.•Along with the trainees, doctors working in non-training posts such as staff grade, specialty doctors, trust grade doctors (TJD)and MTI (Medical training initiative) doctors form an integral part of patient care in the NHS.Method•A mixed method approach was adopted with both qualitative and quantitative data collected simultaneously in the form of an online questionnaire.•An anonymous online questionnaire was sent to junior doctors currently in training and non-training posts at LSCFT in 2019 using Meridian software.Result1- Quantitative Data: - Participants included were doctors in training post such as Foundation Doctors (5), Psychiatry Core Trainees (6), GP STs (2) and doctors in non-training post such as TJD (4), Specialty Doctors (2) and MTI doctors (4). Based on the Meridian score, 84% of doctors were satisfied with the supervision. It was found that 72% of doctors received weekly supervisions, 10% monthly (1 TJD, 1 Foundation trainee) and16% bi-monthly (1 MTI, 1 SAS, 2 CTs). The data suggested that there was no difference in the frequency of supervisions between training and non-training posts at LSCFT.2- Qualitative Data: - The feedback was common as there was no major difference between training and non-training doctors. •Positives – WPBAs, discussion on reflections, management of complex cases and medication, personal issues affecting work.•Negatives – Limited discussion on QI, Audit, Research and Psychotherapy.- More specific help, need more support at times.Conclusion1.To prepare a checklist of contents to be discussed during supervision.2.To prepare a timeline chart of supervision.3.Preparing a ‘menu’ of QI projects that junior doctors can sign up to at the start of each post.4.To formulate training packages available to support junior doctors with QI/Audits.

The COVID-19 pandemic has forced governments around the world to impose containment measures to prevent the rapid spread of the corona virus. The Indonesian government implemented “large-scale social restrictions,” which have impacted farming and farmers’ food security. Farmers are both producers and consumers of food and, therefore, have been facing new challenges due to transport restrictions, price spikes for inputs, price drops for their produce, or conditions which aggravated cooperation, such as social distancing. This study aims at analysing the challenges of the containments from a smallholder farmer perspective and examining farmers’ coping potential. A digital survey with 323 farmers has been designed as comparative observational research in Toraja, South Sulawesi, and selected regions of Java. The Bonferroni Multiple Comparison Test was used to test for significance regarding socio-economic factors and space. A logistic regression model extracted determinants for crisis coping. Results reveal, that female farmers worry more about COVID-19 outbreak compared to men at a significant level. In contrast, male farmers, particularly in Java, are more concerned about social restrictions due to limited mobility. Food price spikes were reported in both regions, with sharp increases for fish, fruits, and vegetables in Java, for staples in Toraja, and for meat and sugar in both regions. Food groups, that trade through agents and brokers or are transported longer distances were affected most due to their complex and long supply chains that were disrupted during the restrictions. In Java, farmers face multiple shocks, of which climate change was reported even more often than the pandemic related shocks. Not being able to help each other on the farm due to social distancing is a significant concern of farmers in Toraja. As a result of food market disturbances, farmers began to grow and eat more vegetables and fruits. In conclusion, food security for farmers slightly decreased due to affordability, and market disruptions already point to long-term income losses. The study team recommends to promote smallholders’ healthy food production, value addition and direct end-consumer linkages to build back better their livelihoods post-COVID-19.

Abstract. A scale-invariance analysis of space and time rainfall events monitored by meteorological radar over the area of Rome (Italy) is proposed. The study of the scale-invariance properties of intense precipitation storms, particularly important in flood forecast and risk mitigation, allows to transfer rainfall information from the large scale predictive meteorological models to the small scale hydrological rainfall-runoff models. Precipitation events are monitored using data collected by the polarimetric Doppler radar Polar 55C (ISAC-CNR), located 15 km Southeast from downtown. The meteorological radar provides the estimates of rainfall intensity over an area of about 10 000 km2 at a resolution of 2×2 km2 in space and 5 min in time. Many precipitation events have been observed from autumn 2001 up to now. A scale-invariance analysis is performed on some of these events with the aim at exploring the multifractal properties and at understanding their dependence on the meteorological large-scale conditions.
