Reexpansion pulmonary edema (RPE) is a rare, but frequently lethal, clinical condition. The precise pathophysiologic abnormalities associated with this disorder are still unknown, though decreased pulmonary surfactant levels and a pro-inflammatory status are putative mechanisms. Early diagnosis is crucial, since prognosis depends on early recognition and prompt treatment. Considering the high mortality rates related to RPE, preventive measures are still the best available strategy for patient handling. This review provides a brief overview of the pathophysiology, diagnosis, treatment, and prevention of RPE, with practical recommendations for adequate intervention.

Introduction. In the world (and in the Russian Federation) the situation caused by the increase in the number of socially significant diseases continues to be quite tense. The most important in this group is tuberculosis, which causes significant damage to society, due to the loss of temporary and permanent disability, an increase in the number of disabled people and destabilization of the demographic situation in the country. In this regard, an integrated approach to the implementation of therapeutic and preventive measures in patients suffering from tuberculous lesions is a priority in addressing the spread of this pathology. Purpose of the study. Analysis of data on the epidemiological situation, morbidity, prevalence, mortality, prevention and treatment of tuberculosis patients in the Khanty-Mansiysk Autonomous District – Ugra in 2017, comparing the figures for the first half of 2017 and 2018. Materials and methods. To assess the parameters characterizing the organization of early diagnosis of tuberculosis, hospitalization of patients of the relevant cohort of the population, the effectiveness of therapeutic measures taken, the quality of preventive measures among the population of the Khanty-Mansiysk Autonomous District – Ugra, the data of annual reports of relevant medical organizations for 2017–2018 are used. Results. In 2017, the coverage of examinations for tuberculosis of the population with the use of all survey methods amounted to 78,6%, while the target indicator of the state program «Health Development 2014–2020» was 78,0%. The number of patients effectively treated at the studied stages was more than 50% of cases for each. Conclusion. In the Autonomous District – Ugra, the work of all services and institutions for the treatment and prevention of tuberculosis is recognized as systematic and effective according to the results of events held.

The article is devoted to the application of preventive measures to persons with mental disorders. The article presents the views and positions of leading domestic scientists on the types of the most effective precautionary measures for persons in respect of whom proceedings on the application of coercive measures of a medical nature are considered. The article draws attention to international standards as guidelines on the path to legality and justice in the process of applying precautionary measures against insane persons. Scientific positions on the types and features of special precautionary measures that can be applied to insane persons are given. The author found that the problematic issues of application of precautionary measures against persons subject to proceedings on the application of coercive measures of a medical nature are the lack of regulations in the CPC of Ukraine or family members within the precautionary measure in the form of transfer to the care of guardians, close relatives or family members with compulsory medical supervision, as well as the lack of direct right of the person in respect of which the application of coercive measures of a medical nature is decided. appeal against a court decision on the application of a precautionary measure. To improve the legislative provision of the rights of persons subject to coercive measures of a medical nature, as well as to eliminate legislative gaps in the mechanism of application of precautionary measures against such persons, the author proposed specific regulations on the procedure of application of precautionary measures under Chapter 39 of the CPC of Ukraine. care for guardians, close relatives or family members with mandatory medical supervision; as well as placement in a psychiatric institution in conditions that exclude its dangerous behavior. The article expresses the point of view on the need to improve the legislative technique by developing sanctions against persons responsible for the implementation of precautionary measures against insane persons.

In the presented review of the literature, the most significant problems of modern pharmacotherapy of status epilepticus are indicated. The urgency of treatment of status epilepticus is confirmed by its frequency and ineffectiveness of traditional drugs (benzodiazepines, hydantoins, barbiturates), which have a number of side effects from the cardiovascular and respiratory systems. In Russian intensive care medicine, treatment of status epilepticus is carried out in accordance with international standards and recommendations. For a number of years, the injectable form of valproate has proven its effectiveness, including in pediatric practice. In comparative studies, phenytoin, valproate and levetiracetam are safe and equally effective in treating status epilepticus. A particular difficulty in therapy is caused by benzodiapine-resistant status epilepticus and refractory, which requires consideration of the possibility of using a combination of antiepileptic drugs. The review of foreign and domestic sources presents the results of clinical studies that allow the use of new antiepileptic drugs, which can effectively stop status epilepticus already in the early stages. New antiepileptic drugs are especially relevant in the treatment of patients refractory to drug treatment. However, treatment with antiepileptic drugs does not always show its effectiveness in refractory and super-refractory status epilepticus, which requires new approaches in complex treatment. The tolerance of drugs and the frequency of side effects are important for patients, since most of them require long-term combined use. Correct selection of antiepileptic drugs increases the level of compliance. A special approach requires the treatment of status epilepticus in the context of palliative care and comorbid pathology. In the treatment and prevention of status epilepticus, surgical treatment and the use of a ketogenic diet can be considered.

The overall high relapse rates observed in schizophrenia are attributed to several causes. One important factor influencing satisfactory prevention of relapse is the lack of consistent treatment strategies among medical doctors, especially neurologists-psychiatrists. Nearly one-third of the members of the Hellenic Society of Neurology and Psychiatry were asked to fill in anonymously a structured questionnaire on their treatment attitudes and prescribing tactics regarding schizophrenic patients both after the first schizophrenic episode and after multiple episodes. The majority of Greek neurologists-psychiatrists seem to adopt prescribing habits that approximate the current international standards for prevention of schizophrenic relapse. Their attitudes regarding the treatment and prevention of relapse for the first schizophrenic episode and first relapse are determined from multiple factors. These are: the expected relapse rates after the first episode, the expected prevalence of extrapyramidal side effects following a long-term neuroleptic treatment, the patient’s expected treatment compliance after the first episode, the doctor’s experience with treating schizophrenics, and lastly the knowledge of current literature on the topic. These results point to the need for continuing education, especially of the younger mental health professionals and those working in the private sector, addressing the issue of the actual risk of developing side effects from the treatment. In due course, benefits could result for everyday psychiatric practice and the patients’ compliance with treatment.
