Traditionally oral appliances i.e. removable orthodontic appliances, bite splints and snoring / sleep apnea appliances are made with alginate impressions and wax registrations. Our aim was to describe the process of manufacturing customized oral appliances with a new technique i.e. rapid tooling method. The appliance should ideally be custom made to match the teeth. An orthodontic patient, scheduled for conventional orthodontic treatment, served as a study subject. After a precise clinical and radiographic examination, the approach was to digitize the patient’s dental arches and then to correct them virtually by computer. Additive manufacturing was then used to fabricate a mould for a soft customized appliance. The mould was manufactured using stereolithography from Somos ProtoGen O-XT 18420 material. Casting material for the mould to obtain the final appliance was silicone. As a result we managed to create a customized soft orthodontic appliance. Also, the accuracy of the method was found to be adequate. Two versions of the described device were manufactured: one with small and one with moderate orthodontic force. The study person also gave information on the subjective patient adaptation aspects of the oral appliance.

Introduction: Braces often used in the treatment of malocclusion. However, in recent years malocclusion no longer becomes the reason for orthodontic treatment. Many people use it merely for a lifestyle purpose, and most of them got it from the dental quack. This study was aimed to understand the influencing factors of the citizen of Bandung preferences towards dental quack on constructing custom-made orthodontic appliances. Methods: This study was a cross-sectional study with purposive sampling technique consisted of 30 samples. The inclusion criteria were subject wearing dental quack custom-made orthodontic appliance; citizen of Bandung; and willing to participate in the study. Exclusion criteria were subject refused to participate in the study. Results: The study showed that most of the subject (73%) were using the treatment for the teeth alignment, and only a few were using for the lifestyle purpose only. From all subjects wearing orthodontic appliance, 63% of them were chosen dental quack custom-made orthodontic appliance due to the lower price, and 37% of the rest due to peer’s suggestion who had it before. The combination of the low income with the needs of orthodontic treatment and inadequate information regarding orthodontic appliance such as dental braces affected the subjects preferences towards the dental quack custom-made orthodontic appliance. Overall, the socioeconomic environment is the most influencing factor of preference. Conclusion: A lower price was the most influencing factor of high preferences for the dental quack custom-made orthodontic appliance.

Abstract Objective: To assess the prevalence, intensity, and extent of the impacts on daily performances related to wearing different types of orthodontic appliances. Materials and Methods: A total of 1657 students, 15 to 16 years old, were randomly selected from those attending all secondary schools in Bauru, São Paulo, Brazil. Only those wearing orthodontic appliances at the time of the survey were included. Face-to-face structured interviews were done to collect information about impacts on quality of life related to wearing orthodontic appliances, using the Oral Impact on Daily Performances (OIDP). Adolescents were also clinically examined to assess the type of orthodontic appliance they were wearing. Comparisons, by type of orthodontic appliance and covariables, were performed using nonparametric statistical tests. Results: Three hundred fifty-seven adolescents (36.1% boys and 63.9% girls) undergoing orthodontic treatment participated in the study. The prevalence of condition-specific impacts related to wearing orthodontic appliances was 22.7%. Among adolescents with impacts related to wearing orthodontic appliances, 35.8% reported impacts of severe or very severe intensity and 90.1% reported impacts on only one daily performance, commonly eating or speaking. The prevalence, but not the intensity or the extent, of condition-specific impacts differed by type of orthodontic appliance (P = .001). Conclusions: One in four Brazilian adolescents undergoing orthodontic treatment reported side effects, specific impacts on daily living, related to wearing orthodontic appliances. Such impacts were higher among adolescents wearing fixed rather than removable or a combination of fixed and removable orthodontic appliances. This information could help to inform patients about the frequency and intensity of sociodental impacts during the course of their treatment.

Abstract Background : Treatment compliance plays fundamental roles in achieving the best results in orthodontic treatment. Some psychological characteristics such as general self-efficacy (GSE) are correlated with the level of tolerance of individuals. This study aimed to assess the correlation of psychological characteristics such as GSE and level of malocclusion with compliance of removable orthodontic appliances. Methods : The Index of Orthodontic Treatment Need (IOTN) of 50 patients between 10-12 years who required removable orthodontic appliances was determined prior to the onset of treatment. They were also requested to fill out the GSE scale (GSES). The removable orthodontic appliance compliance questionnaire was also filled out by patients at 1 (T1), 3 (T2), and 6 (T3) months after the delivery of the removable orthodontic appliance. Results : The GSES score had significant correlations with the total score of the compliance questionnaire, and subscale score of satisfaction with the appliance during eating and oral hygiene practice, duration of usage of the appliance, and interest in using it (P<0.05). The IOTN had no significant correlation with the compliance questionnaire. Conclusions: The level of compliance of removal orthodontic appliances by patients can be predicted using the GSES. The IOTN is not efficient for prediction of the compliance level of removal orthodontic appliances by patients.

The aim of this study was to investigate the orthodontic force exerted by thermoplastic orthodontic appliances incorporating Eucommiaulmoides in terms of usefulness as the aligner-type orthodontic device. Erkodur, Essix C+®, Eucommia elastomer, and edgewise brackets were used (n = 3, each; thickness = 1.0 mm, each). The orthodontic force on the upper right incisor was measured every 24 h for two weeks using a custom-made measuring device. The force of the Eucommia elastomer (4.25 ± 0.274 N) and multi bracket system (5.32 ± 0.338 N) did not change from the beginning to the end (p > 0.01). The orthodontic force exerted by the Eucommia elastomer was lower than that of the multi-bracket orthodontic appliance from the beginning to the end. The force of Erkodur significantly decreased from the beginning to 24 h (6.47 ± 1.40 N) and 48 h (3.30 ± 0.536 N) (p < 0.01). The force of Essix C+® significantly decreased from the beginning (13.2 ± 0.845 N) to 24 h (8.77 ± 0.231 N) (p < 0.01). The thermoplastic orthodontic appliance made of Eucommia elastomer continuously exerted a constant orthodontic force for two weeks under water immersion conditions. The orthodontic force of Eucommia elastomer was found to be similar to the orthodontic force exerted by the multi-bracket orthodontic appliance with 0.019 × 0.025 in nickel–titanium wire. These results suggest that the Eucommia elastomer has possibly become one of the more useful materials to form thermoplastic orthodontic appliance exerting low continues orthodontic force.

Enamel decalcification around and under orthodontic appliances have been known asa significant clinical problem, in fact an orthodontic patient is a high risk person.Whether this high risk due to oral hygiene related to orthodontic appliance or as acause of phosporic acid contained in zinc oxyphosphate cement that have beencommonly used to bracket adhesion. The purpose of this article was to provide adescription of enamel decalcification process and prevention of decalcificationfollowing orthodontic treatment. Prevention of enamel decalcification followingorthodontic treatment can be performed by polishing tooth surface after treatment,using tooth paste contained fluoride, rinsing, maintenance good oral hygiene, toothbrushing, and other additional oral hygiene method.

Abstract Background: The patients’ acceptance of a treatment plan and their subsequent cooperation play a crucial role in achieving the best results in orthodontic treatments. Evidences show some personality traits such as general self-efficacy (GSE) and some dental traits such as severity of malocclusion are correlated with motivation of orthodontic treatment. These factors may predict the patients’ compliance and acceptance in using removable orthodontic appliances. This cross sectional study was conducted to assess the correlation of GSE and the severity of malocclusion with patients’ acceptance in using removable orthodontic appliances. Methods: This study recruited 50 patients aged 10-12 years who required removable orthodontic appliances. The severity of malocclusion was determined using the index of orthodontic treatment need (IOTN) before the onset of treatment and GSE of participants were assessed using GSE scale self-report. The acceptance questionnaire was proposed to the patients on first (T1), third (T2), and sixth (T3) month after the delivery of the appliance. Results: The GSE score had a statistically significant correlation with the total score of the acceptance questionnaire, subscale score of satisfaction with the appliance during eating and oral hygiene practice, duration of usage of the appliance, and interest in using it (P<0.05). The IOTN had no significant correlation with the acceptance questionnaire. Conclusions: Our findings substantiate the role of the GSES, concurrently declining the role of the IOTN in prediction of 10-12-year-old children's acceptance and cooperation in treatment of malocclusion with removable appliances.

The success of orthodontic treatment, particularly with removable orthodontic appliances not only relies on theexpertise of the clinician, but also highly depends on the patient's own. It is necessary to know the extent of thesuccess rate of an orthodontic treatment. Assessment of malocclusion severity and treatment outcome has become avery popular object of research conducted by the PAR index. This study aims to determine the success rate oforthodontic treatment with removable appliances based on PAR Index. The research was carried out on March-May2011 with samples of patients dental models who had completed orthodontic treatment in the year 2008-2011 at theDepartment of Ortodonsia RSGMP UNHAS. Sixty six pairs of dental models fulfilled the inclusion criteria and therules of PAR Index. Percentage change in PAR score obtained by 35% in the assessment with no weighted and 36%was assessment by weighted. This results showed the level of orthodontic treatment success include into thecategory of "no change", but with a trend into the category of "a change "with a significant value ρ = 0.000 (ρ <0.05). It was concluded that there was occlusion improvements in orthodontic treatment with removable appliance,although very low and therefore need evaluation and improvement of governance maintenance carried out.

A 46-year-old male Caucasian with traumatically induced maxillary retrusion was referred for orthodontic treatment, eight weeks after the original fracture had occurred. Initial surgical reduction and fixation had been successful, when a second traumatic episode was encountered. This resulted in a further degree of posterior maxillary displacement, which was resistant to further surgical reduction. The resulting Class III malocclusion was treated using maxillary protraction headgear, in conjunction with removable orthodontic appliances and intermaxillary traction. Appliances were worn full time and inter-arch correction was achieved in six months. The resulting occlusion proved to be stable following the cessation of active treatment.

Background/Aim. Orthodontic treatment enables the establishment of functional occlusion and improvement of oral health, however, it increases the risk of periodontal disease development. The aim of this paper was to examine the efficiency of the applied programme for the prevention of gingivitis in children undergoing the fixed orthodontic appliance therapy and to determine the most efficient devices and techniques for maintaining oral hygiene during orthodontic treatment. Methods. The study included 80 patients of both genders - 60 patients comprised the experimental group and 20 patients comprised the control group. All of them were patients of the Clinic for Orthodontics at the School of Dentistry in Belgrade, aged between 13 and 18. The Silness-L?e Plaque Index (PI) was utilised for the assessment of oral hygiene quality and Silness-L?e Gingival Index (GI) and M?hlemann Papilla Bleeding Index (PBI) were utilised for the assessment of gingival state. Checkups were conducted as a single-blind study at the beginning and after the first, the third and the sixth month of the preventive and prophylactic programme. Results. During the observed period, a statistically significant change in PI, GI and PBI values was noticed (p < 0.005), as well as the difference in the dynamics of value changes during the periods between the observed groups. Conclusion. The preventive programme, applied to children undergoing the fixed orthodontic appliance therapy, had a positive effect both on oral hygiene quality and gingival state. The values of the examined parameters of the patients from the experimental group were significantly lower in comparison with those of the patients from the control group. The most efficient combination of devices for oral hygiene during orthodontic treatment was: a Curaprox CP5460 toothbrush, CD Ortho 60 orthodontic toothbrush and Curaprox CPS 14 interdental brush.

Introduction: There has been little research to explore how adults financially value private orthodontic treatment and whether they have preferences for different attributes of treatment. This study used a discrete choice experiment and aimed to determine whether the recognized skill level of the dental professional and the type of orthodontic appliance influence the values that the public places on private adult orthodontic treatment. Methods: In total, 206 adult patients or the parents/guardians of children attending general dental practices in the northeast of England were recruited to complete a discrete choice experiment. Three attributes were included: the type of dental professional providing treatment, the type of orthodontic appliance, and cost. Also collected were demographic and orthodontic history characteristics. Results were analyzed with conditional logistic regression and elicited marginal willingness to pay (MWTP). Results: Participants value the training and expertise of the dental professional providing private orthodontic treatment greater than the type of orthodontic appliance. MWTP for orthodontic treatment increased in conjunction with the recognized skill level of the dental professional. Participants were willing to pay more for aesthetic appliances over a fixed metal appliance. Conclusions: Participants value the training and expertise of the dental professional providing private adult orthodontic treatment greater than the type of orthodontic appliance. These preferences concur with other discrete choice experiments undertaken in medical specialties that included attributes focusing on the qualification, skill, or expertise of the health care professional. MWTP for orthodontic treatment increased in conjunction with the recognized skill level of the dental professional. Participants were willing to pay more for aesthetic appliances than metal fixed appliances. The cost of orthodontics is significant, and adults appreciate the importance of having options and making choices. Knowledge Transfer Statement: The results of this study suggest that patients are willing to pay more for orthodontic services provided by clinicians with higher levels of formal training. In a competitive market where the public appears to prefer the provider over treatment modality, there is an incentive for clinicians to optimize their knowledge and skills to deliver the high-quality orthodontic treatment that patients are demanding. Orthodontic clinicians should be mindful of the demand for the different adult orthodontic appliances and tailor their skill sets accordingly.
