Thirty patients with completely edentulous jaws (the main group) and 30 patients without denture defects and treated oral cavity (the control group) were examined in order to study oral stereognosis. Standard Н. Landt sets were used. The rate of tactile assessment of the objects shape in the oral cavity of the patients with completely edentulous jaws was established to decrease by 2.6 times on average in comparison with the control group, and the percentage of correctness of the sample determination decreased by 44.4%. After one month of prosthodontic treatment of the patients with completely edentulous jaws with the use of complete laminar dentures, the rate of tactile assessment of the objects’ shape increased by 18.3% and the index of the correctness of the sample determination increased by 31.5%. The results of the oral stereognosis study showed that the duration of collecting and analysis of receptor signals of oral mucosa in the patients with completely edentulous jaws was longer than in the patients without denture defects. However, the tactile sensitivity of the oral mucosa had the ability to recover in one month after prosthodontic treatment with complete laminar dentures.

Importance. Despite the improvement of diagnostic methods and the introduction of new technologies in the treatment of pathology of the oral mucosa, it is difficult to achieve stable remission, since 70 % of cases are based on somatic and bodily suffering factors. The article presents data from a comparative analysis of the use of the traditional treatment regimen for patients with pathology of the oral cavity and the inclusion of the latest generation of antipsychotics in the treatment regimen. It was established that, when applied, a tendency towards normalization of psychological and autonomic status was observed. The inclusion of anxiolytics and antioxidants in the complex of general therapy for patients with pathology of the oral mucosa contributed to a more pronounced positive dynamics of achieving and prolonging the phase of remission. Objectives — to evaluate the effectiveness of introducing drugs of the latest generation of antipsychotics into the traditional treatment regimen for patients with pathology of the oral cavity. Methodology. Under our supervision, there were 30 patients aged 40 to 60 years with a diagnosis of pathology of the oral mucosa and pathology of the periodontium. All examined were divided into 2 groups: the main (15 people) and control (15 people). Results. Clinical well-being, manifested in a subjective decrease in pain in the tongue, feelings of rawness, tingling, numbness, awkwardness, in the tongue, was noted already at the 5th visit in 67 % of patients of the main group, while in the control group in 17 % of patients. Conclusions. The inclusion of anxiolytics and antioxidants in the complex of general therapy for patients with pathology of the oral mucosa contributed to a more pronounced positive dynamics of achieving and prolonging the phase of remission. Our data allow us to implement this treatment regimen in practical healthcare.

e17530 Background: It is believed that hypoxia-inducible factor (HIF-1α) hyperactivity promotes synthesis of protective factors and increases bacterial growth-inhibitory activity, provides enhancement methods of the congenital immune response to microbial infection. Methods: Lactoferrin and α-defensin were detected in saliva of 41 patients with oral mucosa (OM) cancer (T2N0-2M0) before a comprehensive treatment; enzyme immunoassay (EIA) method was used. A level of hypoxia-inducible factor (HIF-1α) was evaluated by EIA method in tumor tissues after surgical treatment. The patients were divided into two groups according to an after-surgery period: the main group (n = 16) had after-surgery suppurative complications. An after-surgery period proceeded without infectious complications in patients of a control group (n = 25). Results: It was found that Lactoferrin content in saliva statistically was significantly higher by 36,4% (p < 0,05) (3,0±0,24 µg/ml against 2,2±0,19 µg/ml) if to draw a comparison between the main group and the control group with respect to development of suppurative complications and absence of a inter-group difference according to α-defensin concentration (4,1±0,35 pg/ml against 3,8±0,19 pg/ml). HIF-1α expression index in the main group (8,4±0,79 CU/mg protein in a well) exceeded this index by 18,3% (p < 0,05) in the control group (7,1±0,79 CU/mg protein). A positive correlation between HIF-1α expression and Lactoferrin content (R = 0,53 at р = 0,0001) was detected in patients with OM cancer. Conclusions: Detected HIF-1α hyperexpression in tumor tissues and Lactoferrin content in saliva, as well as their interaction allows suggesting significance of these factors for their progression prediction in patients with OM cancer in case of after-surgery suppurative complications.

In the structure of modern diseases, there are 50-75% of psychosomatic diseases. To understand the role of the emotional factor in the onset and progression of somatic disorders, as well as the opposite process – the emergence of psychological and emotional disorders as a response to an acute or chronic disease of internal organs – acquires a particular importance and relevance at the present stage of the causality of the disease. Individual features of the response of the human body to social, biological factors have common mechanisms of combined damage to the mucous membrane of the oral cavity and periodontal tissue. The severity of the damage to these organs and tissues is also determined by the psychological characteristics of individuals. These points are important not only for understanding the causality of the combined pathology of the periodontal and oral mucosa, but also for choosing the optimal tactics for treating such dental patients. The aim of the work was to analyze the role of psychosomatic correlations in the occurrence and development of the combined pathology of the periodontal and oral mucosa, determining the tactics of their treatment. 32 patients were involved in the examination with pathological changes of periodontal and oral mucosa. The essential role of the emotional component in the genesis of diseases was confirmed. Taking into account the psychosomatic nature of the pathology in the experimental group of patients, recommendations were given on diet therapy, psychotherapy, autogenic training. General medical treatment for internists and dentists was maximally individualized, taking into account the psychological characteristics of the response to the disease, differentiated according to the nature of the existing diseases, severity, pathology. After 6 months of follow-up, a stable, reliable positive clinical result was obtained in all patients in the experimental group and in 64% in the control group. Accounting for the state of psychosomatic relationships in the mechanisms of origin, development, treatment of diseases is extremely important. This approach in the supervision of dental patients who have combined changes in periodontal tissues, the oral mucosa allows achieving positive results of treatment, both in relation to the condition of the oral cavity, and for the body as a whole.

The search for and the application of available noninvasive methods for early diagnosis of oral mucosa (OM) neoplasia is a clinically significant problem. The aim of this study was to evaluate the effectiveness of the original score-based algorithm for assessing clinical data generated by a conventional and an autofluorescencebased examination in diagnosing OM cancer and assessing indications for a biopsy. We analyzed 134 medical histories and pathology reports of patients with oral neoplasia. The patients were assigned to 2 groups: the control group included 63 patients who underwent a standard visual and tactile examination with history taking and then were referred for an incisional biopsy followed by a histopathological examination of the specimens. In the main group consisting of 71 patients, a standard visual and tactile examination was complemented by an autofluorescence-based examination and the original score-based algorithm with the original index of required histopathological verification (RHV) were used to assess indications for a biopsy. In both groups, the most commonly affected site was the tongue (72.4%). The histopathological examination revealed that 28 patients from the main group and 14 patients from the control group had OM cancer (р = 0.051). Histologically, early-stage cancer was diagnosed in 17 patients from the main group and in 4 patients from the control group (р = 0.004). The proposed algorithm allowed us to effectively (in 90% of cases) diagnose precancer and cancer and avoid unnecessary biopsies.

Generalized periodontitis is the most common form of periodontal pathology, especially in the second half of life. Nowadays, the problem of periodontal disease is relevant due to its high prevalence, tendency to progression, multifaceted influence on the dentoalveolar system and the whole organism as well as uncertain treatment. Therefore, there is a need to find optimal ways of prevention and treatment of this disease. Close relationships between periodontal pathology and systemic diseases, such as hypertension, which affects every 2nd-3rd adult were determined.The objective of the research was to determine the status of oral cavity hygiene in patients with generalized periodontitis of II degree and stage II hypertension.Materials and methods. The study included 30 patients with generalized periodontitis of II degree and stage II hypertension being treated in the Ivano-Frankivsk Regional Clinical Cardiology Dispensary; the average age ranged from 35 to 54 years (the main group). The control group included 10 patients of the same age without generalized periodontitis and somatic pathology .The status of oral cavity hygiene was determined using the Green Vermillion index. The diagnosis of periodontal disease was made on the basis of the classification proposed by M.F. Danilevskyi. The obtained results were subjected to variation-statistical analysis using statistical package “Stat Soft 6.0”; classical methods of variational statistics were applied; mean values and their reliability were evaluated.Results. The results of examination showed poor oral hygiene in almost all patients. The analysis of hygienic indices showed the following results: in patients of the main group, the Green Vermillion index was 1.99 ± 0.13 points (p<0.001) which corresponds to unsatisfactory level of oral hygiene. In patients of the control group, this index was 1.10 ± 0.17 points which corresponds to satisfactory level of oral hygiene. According to the index, 9 (30.0%) patients of the main group had satisfactory oral hygiene, 12 (40.0%) patients had poor oral hygiene, and in 9 (30.0%) patients, poor oral hygiene was observed.In the control group, 2 (20.0%) patients had good oral hygiene, 6 (60.0%) patients had satisfactory oral hygiene and in 2 (20.0%) patients, unsatisfactory oral hygiene was observed. There were no patients with poor oral hygiene. The analysis of the indicators of the Green Vermillion index showed that in case of the pathological process exacerbation the oral hygiene status in patients deteriorated.Conclusions. The level of oral hygiene in patients of both groups was low due to incorrect selection of personal hygiene products or their untimely replacement. In patients with generalized periodontitis of II degree and stage II hypertension, the level of oral hygiene was lower than in somatically healthy persons: the worse status of oral cavity hygiene – the more pronounced changes in the periodontal tissues. We can suppose that high blood pressure affects the status of the oral cavity, creates a higher risk and exacerbates the periodontal diseases.

Relevance: inflammatory processes in the oral mucosa are often found in diseases of the gastrointestinal tract, which is explained by the common functions and unity of all parts of the digestive system.Purpose – to study the effect of the studied drugs on the state of antioxidant-prooxidant systems of the oral mucosa in patients with chronic pancreatitis.Materials and methods: we examined 118 patients, of whom 62 were men (mean age 39.6 ± 16.4 years) and 56 women (mean age 40.3 ± 15.7 years); of these, 34 patients (control group) are practically healthy people without somatic pathology and 84 patients with chronic pancreatitis who were divided into two groups. The first (main) group - 46 people who, in addition to the drug therapy prescribed by the gastroenterologist, were prescribed the drug “Multiflora” 1 tablet 1 time per day with meals for 30 days. Also, the patients of the main group, after each meal, rinsed their mouths with “Enoant” food concentrate at a dilution of 1: 5 and applied it inside with 1 tablespoon 2 times a day, diluted with water in a ratio of 1: 5. The second (comparison group) - 38 people who, in addition to treatment of chronic pancreatitis, were treated with the oral mucosa according to the generally accepted method - prescribed fluconazole 150 mg 1 time a day, rinsing with 2% borax solution in glycerol after eating for 14 days. Unstimulated oral fluid of the examined patients served as the material for the study. In all groups, the activity of the catalase enzyme was determined by the method of S.V. Girina (1999), the level of malonic dialdehyde (MDA activity by reaction with thiobarbiturate acid to form a trimethine complex) and antioxidant-prooxidant index of unstimulated oral fluid by the method of А.P. Levitsky (2010). Laboratory research was carried out at the time of the initial examination before the start and on the 30th day of treatment.Results: in the comparison group after treatment, an increased concentration of malondialdehyde in the oral fluid, as well as reduced indicators of the API index and catalase activity confirm the presence of inflammatory processes in the oral mucosa. The application of the proposed therapeutic complex to patients of the main group significantly reduces the concentration of malondialdehyde in the oral fluid, proving its anti-inflammatory effect on the oral mucosa. An increase in the antioxidant-prooxidant index and an increase in catalase activity indicates an increase in the body’s defenses in patients of the main group.Conclusion: the use of the proposed therapeutic complex in patients with inflammatory diseases of the oral mucosa suffering from chronic pancreatitis contributes to the elimination of inflammatory and degenerative processes and the restoration of the normal level of protective systems in the oral mucosa.

Objective. To study the indicators of the dental status in professional athletes. Materials and methods. The object of the study was 200 professional female athletes representing various kinds of sport and 200 practically healthy female individuals who are not involved in sports. The total sports experience of the subjects was on average 1315 years. The prevalence of periodontal diseases among female athletes was determined using the CPITN index. The prevalence and intensity of caries was determined by the CPI index. The hygienic state of the oral cavity was determined using the oral hygiene efficiency index (PHP). In the main group, patients were administered applications of a drug based on propolis and vegetable oil for two weeks; in the comparison group, the traditional antiseptic, chlorhexidine bigluconate, was used for the same period. Results. In the control group, the prevalence of chronic generalized gingivitis was on average two times less than in the main group, in which the value of this indicator varied within 50 %. Among all the studied diseases of the oral cavity in athletes in general, the most essential and significant differences were observed in relation to the prevalence of dental caries and its complications, which were more common in the group of athletes than in the control group 95.0 1.54 %, versus 89.5 2.17 %, respectively (p 0.001). The need for treatment of moderate and severe inflammatory periodontal diseases in the main group was approximately 1.5 times higher than in the control group (77.0 2.98 vs. 42.5 3.50 %, p 0.001), and for gingivitis almost 2 times higher than in the control group (61.5 3.44 vs. 39.0 3.45 %, p 0.001). The results of the study showed that among the oral diseases in sportswomen, the most essential and significant differences were observed in the prevalence of dental caries and its complications. The intensity of periodontal tissue damage over the entire period of sports career in athletes of the older age group corresponded to moderate and severe periodontitis. The high percentage of cases of major dental diseases detected among female athletes indicates their high need for periodontal care. Conclusions. Regular use of a biologically neutral propolis-based preparation for preventive purposes leads to a relatively more pronounced improvement in laboratory and clinical indicators of the oral cavity status.

Aim. To conduct a clinical assessment of short- and long-term surface sealing in patients having a different hygienic status of the oral cavity.Materials and methods. For clinical trials, a group of 250 male and female patients aged 25–45 years old and diagnosed with Black’s class III and V carious lesions was selected. All patients were divided into three groups depending on the state of oral hygiene and the applied methods of aesthetic restoration: control group — defect restoration by Filtek Z-250 microhybrid composite (3M ESPE); comparison group — Filtek Z-250 restoration (3M ESPE) + Easy Glaze (Voco) surface sealing on the day of restoration; and the main group with three subgroups: 2α subgroup — Restavrin restoration + Easy Glaze sealant on the day of treatment; 2β subgroup — Restavrin restoration + Easy Glaze sealant on the day of treatment and re-sealing with a frequency of 1 time per year; 2γ subgroup — Restavrin restoration + Easy Glaze sealant on the day of treatment and re-sealing with a frequency of 1 time per 6 months. The quality of caries cavities was assessed by USPHS criteria, including anatomical shape (AS), marginal pigmentation (MP), marginal adaptation (MA), the presence of secondary caries (SC) and sensitivity (S). The evaluation was conducted on the day of the visit and following 6, 12 and 24 months.Results. A comparison of the clinical evaluation of photocomposite restorations by the Filtek Z-250 (3M ESPE) microhybrid composite in the control group of patients, in comparison group and in the main Restavrin (Technodent) group showed that the Easy Glaze (Voco) surface sealing of the Restavrin composite restorations carried out in the 2α subgroup of the main group onе time on the day of restoration, in the 2β subgroup with re-sealing after 12 months, and in the 2γ subgroup with sealing every 6 months had allowed the maximum number of restorations to be corresponded to the “satisfactory” value according to the AS, MP, MA, SC, S evaluation criteria throughout the entire observation period.Conclusion. The Restavrin (Technodent, Russia) polymeric nanohybrid fi lling material is shown to produce the most effective and durable restorations compared to other materials under study. The applicatiyon of Restavrin followed by the Easy Glaze (Voco) sealing protective system with a frequency dependent on the hygienic status of the oral cavity allows therapeutic methods for dentin caries treatment to be optimized.

Objective to refine the method of incisional biopsy in the diagnosis of oral mucosa cancer using the auto-fluorescent stomatoscopy. Materials and method.The study was conducted on the base of the Samara Regional Clinical Oncology Center. The inclusion criterion for patients was the diagnose of the oral mucosa cancer of various localization. Patients were divided into 2 groups. The main group included patients (n=43), who were being diagnosed for cancer with the help of optimized incisional biopsy of the oral mucosa formations, using the "AFS-400" autofluorescence complex and glasses with a green light filter for identification. The patients of the control group (n=46) received the standard biopsy procedure under direct vision. Results.The first incisional biopsies revealed cancer in 25 (54%) patients of the control group and in 36 (84%) patients of the main group. A histological verification of the diagnosis was necessary in 7 (16%) patients of the main group and required the second biopsy. In the control group, for the same purpose, 17 (37%) patients underwent the second biopsy and 4 (9%) patients required the third biopsy procedure. Exophytic-papillary forms of cancer were the most complex for histological verification. The primary biopsy of these cases was effective in 16 (37%) patients in the main group and in 8 (17%) patients in the control group (p = 0.036). In patients with initial stages of cancer (I-II), with the first incision biopsy, the histological verification of cancer was achieved in 16 (37%) cases in the main group and in 8 (17%) cases in the control group (p = 0.036). Conclusion.The use of the "AFS-400" autofluorescent complex and glasses with a green light filter for incisional biopsy of oral mucosal formations allows histological verification of cancer with the first biopsy in 84% of cases, including in stages I - II in 16 (37%) cases and in exophytic papillary forms in 16 (37%) cases. The significant difference was registered for the similar indicators of the control group (p = 0.036).

90 pregnant women with inflammatory periodontal diseases, aged 27 to 35 years were examined. The main group consisted of 60 pregnant women with gestational diabetes mellitus and the control one consisted of 30 patients with physiological pregnancy. All patients were examined before providing treatment and preventive measures at 14–15 weeks of pregnancy and after them at 37–38 weeks. Studies have shown deterioration in periodontal health, oral hygiene and increase in contamination of the oral cavity in patients of the main group as compared with patients of the control group. The use of the anti-inflammatory succinic acid based preparation is effective for inflammatory periodontal diseases in pregnant women with gestational diabetes. The improvement of periodontal tissues health was confirmed by the posttreatment indices of hygiene, immunoglobulins, microbiocenosis and periodontal indices that promote normalization of the oral cavity.
