Tumor expression of hypoxia-inducible factor and antimicrobial peptides in oral fluid for prediction of after-surgery suppurative complications in patients with oral mucosa cancer.

e17530 Background: It is believed that hypoxia-inducible factor (HIF-1α) hyperactivity promotes synthesis of protective factors and increases bacterial growth-inhibitory activity, provides enhancement methods of the congenital immune response to microbial infection. Methods: Lactoferrin and α-defensin were detected in saliva of 41 patients with oral mucosa (OM) cancer (T2N0-2M0) before a comprehensive treatment; enzyme immunoassay (EIA) method was used. A level of hypoxia-inducible factor (HIF-1α) was evaluated by EIA method in tumor tissues after surgical treatment. The patients were divided into two groups according to an after-surgery period: the main group (n = 16) had after-surgery suppurative complications. An after-surgery period proceeded without infectious complications in patients of a control group (n = 25). Results: It was found that Lactoferrin content in saliva statistically was significantly higher by 36,4% (p < 0,05) (3,0±0,24 µg/ml against 2,2±0,19 µg/ml) if to draw a comparison between the main group and the control group with respect to development of suppurative complications and absence of a inter-group difference according to α-defensin concentration (4,1±0,35 pg/ml against 3,8±0,19 pg/ml). HIF-1α expression index in the main group (8,4±0,79 CU/mg protein in a well) exceeded this index by 18,3% (p < 0,05) in the control group (7,1±0,79 CU/mg protein). A positive correlation between HIF-1α expression and Lactoferrin content (R = 0,53 at р = 0,0001) was detected in patients with OM cancer. Conclusions: Detected HIF-1α hyperexpression in tumor tissues and Lactoferrin content in saliva, as well as their interaction allows suggesting significance of these factors for their progression prediction in patients with OM cancer in case of after-surgery suppurative complications.

The examination of the wound process in 140 patients after the alloplasty, according to the standard method «on-lay» and developed method with the use of the flaps of the hernial sac was carried out. All patients were divided into 2 groups. The main group was consisted of the patients treated by the developed method using the flaps of the hernial sac, the control group of patients – by means of the standard method «on-lay». According to the data of the cytological study of the exudates in the both groups it has been found that the flaps of the hernial sac situated in the subprosthetic space contribute to the high content of the cells having the macrophage activity (neutrophils, macrophagocytes) in the surgical intervention field. In the main group, the mean volume of the secretion was 113ml, it was in 2 times less than the volume of the serous drainage in the patients of the control group. Decrease of the exudates volume in the main group influenced to the drainage terms (3 days average) and to the hospital stay (as many as 8 bed-days average). The absence of the infectious complications in the patients treated by method with the use of the hernial sac flaps is explained by the effective activity of the cells immunity factors in the wound.

Aim: to study the activity of the hypoxia marker HIF-1α (hypoxia-inducible factor-1α) in pregnant women with phlebopathy.Materials and Methods. We examined 70 women with phlebopathy in the dynamics of pregnancy. The main group consisted of 30 patients whose newborns had signs of hypoxia; the control group consisted of 40 women with healthy children. All women underwent ultrasound examinations of the veins of the lower extremities and pelvis with an assessment of vascular patency, the condition of the venous valves, and the phenomenon of platelet sludge. The expression of the HIF-1α transcription factor at 18–20 and 36 weeks was performed by real-time PCR.Results. Disorder of veins functional state was found in all pregnant women of the main group, which was accompanied by the formation of platelet sludge of varying degrees in the area of the venous valves in the majority, signs of endothelial dysfunction and venous hypoxia – an increase in the expression of the hypoxia gene HIF1-α by 2.18 times. In the women of the control group the indicators were not violated.Conclusion. The transcription factor HIF1-α can be considered a marker of unfavorable perinatal outcomes in pregnant women with signs of phlebopathy.

. Background. Human papillomavirus (HPV) has been established to be the etiological factor of cervical cancer (CC). HPV infection and CC progression involve the direct participation of the E6 oncoprotein. Aim. An analysis of the E6 oncoprotein levels in tissues of the tumor and its perifocal area in HPV-associated cervical squamous cell cancer as an objective indicator of the effect of treatment depending on preoperative chemotherapy. Material and methods. The study included clinical and laboratory data of 237 patients with high-risk HPV infection of the cervix. The patients were divided into 4 groups: two main groups (CC T2а2–2bN0–1M0) and two control groups. Patients in the main group 1 (n=84) received standard neoadjuvant chemotherapy (NACT), in the main group 2 (n=93) — modified NACT with prior plasmapheresis session and a parallel course of nonspecific immunotherapy with Allokin-alpha. Control group 1 (n=40) included patients with CC T1b2–2a1N0–1M0, surgical treatment; control group 2 (n=20) — HPV-positive patients without CC. Levels of E6 were measured in samples of the cervical tumor and perifocal tissues. Results. The lowest levels of the E6 oncoprotein were registered in the group of HPV-positive patients without CC. After modified NACT, E6 levels in tumor tissues remained 4.6 times higher than in intact tissues, and even so, these patients demonstrated minimal E6 levels compared to other CC patients. E6 in tumor tissues was significantly lower than in main group 1 (by 3.3 times) and 8 times lower than in control group 1. E6 levels in the perifocal tissues of patients in main group 2 were 1.9 times lower than in the corresponding tissues of patients in main group 1 and 2.2 times lower than in control group 1. Conclusions. Inclusion of plasmapheresis and inducers of endogenous interferonogenesis into neoadjuvant treatment for cervical cancer can be considered pathogenetically justified, since it affects the key unit of cervical carcinogenesis.

Introduction. In recent years, the emergence of bacterial strains resistant to antibiotics used in medical practice significantly reduced the effectiveness of chemoprophylaxis and therapy. Following data indicates the need to search for new antibacterial drugs and ways of treating patients with bacterial keratitis. The aim of research is improvement of clinical parameters in patients with bacterial keratitis using hyperbaric oxygenation. Materials and Methods. We observed 68 patients (68 eyes) with bacterial keratitis aged 18 to 75 years. There were 36 men and 32 women. The patients were divided into 2 groups: main group (38 patients) and control group (30 patients). All the patients received antibiotic therapy, mydriatics, tissue-based products, vitamins. In addition, patients of the main group underwent hyperbaric oxygenation. The severity of the inflammatory response was evaluated using objective indicators: the presence and type of discharge into the conjunctival cavity, the severity of the mixed injection, corneal edema, the degree of inflammatory infiltration of the surface layers of the cornea. Results. The changes of clinical parameters in patients of the main and control groups were evaluated and scored (conjunctival discharge in the cavity, the severity of the mixed injection, corneal edema and inflammatory infiltration). The therapy contributed to faster disappearance of discharge into the conjunctival cavity and mixed injection, regression of edema, resorption of infiltrates (p <0.001). Conclusions. Hyperbaric oxygenation in comprehensive treatment of patients with bacterial keratitis is highly efficient, provides stable clinical and functional results in the main group compared with the control group. The search for and the application of available noninvasive methods for early diagnosis of oral mucosa (OM) neoplasia is a clinically significant problem. The aim of this study was to evaluate the effectiveness of the original score-based algorithm for assessing clinical data generated by a conventional and an autofluorescencebased examination in diagnosing OM cancer and assessing indications for a biopsy. We analyzed 134 medical histories and pathology reports of patients with oral neoplasia. The patients were assigned to 2 groups: the control group included 63 patients who underwent a standard visual and tactile examination with history taking and then were referred for an incisional biopsy followed by a histopathological examination of the specimens. In the main group consisting of 71 patients, a standard visual and tactile examination was complemented by an autofluorescence-based examination and the original score-based algorithm with the original index of required histopathological verification (RHV) were used to assess indications for a biopsy. In both groups, the most commonly affected site was the tongue (72.4%). The histopathological examination revealed that 28 patients from the main group and 14 patients from the control group had OM cancer (р = 0.051). Histologically, early-stage cancer was diagnosed in 17 patients from the main group and in 4 patients from the control group (р = 0.004). The proposed algorithm allowed us to effectively (in 90% of cases) diagnose precancer and cancer and avoid unnecessary biopsies.

BACKGROUND. There are many unsolved medical problems and, of course, pneumonia is one of them. Communityacquired pneumonia (CAP) is a multifactorial disease, but the role of viruses as causative agents is constantly growing. Specific antiviral therapy for CAP is limited. Therefore, the search for drugs with virucidal activity remains relevant. An antimicrobial agent with a broad spectrum of action – decamethoxin – is successfully used today for treatment of patients with infectious exacerbations of bronchial asthma and chronic bronchitis. At the same time efficacy of decamethoxin in CAP patients was not studied. OBJECTIVE. To evaluate the effectiveness and safety of the inhaled antimicrobial drug decamethoxin in the complex treatment of patients with group III viral-bacterial CAP. MATERIALS AND METHODS. There was enrolled 62 patients with group III viral-bacterial CAP. All patients received the same sequential antibiotic therapy: protected aminopenicillin with macrolide or III generation cephalosporin with macrolide. Patients of the main group were prescribed inhalations through a nebulizer of the antiseptic drug decamethoxin in addition to antibacterial therapy from the first day of treatment for 5-7 days. RESULTS AND DISCUSSION. No adverse events were detected in any of the patients during treatment. In all cases, recovery was diagnosed. At the same time, the term of achieving positive results in the main group was 12.2±0.7 days, and in the control – 17.2±0.7 (р <0,05). The average duration of antibiotic use was different in main and control groups: respectively 9.4±0.4 and 10.7±0.4 days (р <0,05). There were no infectious complications in the patients of the main group, while 24 (72 %) patients of the control group were diagnosed with acute rhinopharyngitis (47.0 % of cases), lateral pharyngitis (13 %) and sinusitis (9 %), other complications (otitis, infectious exudative pericarditis). In 22 (66 %) cases there was one complication and in 2 (6 %) cases there were two complications. CONCLUSIONS. For patients with group III viral-bacterial CAP additional inclusion in the empirical sequential antibiotic therapy of inhaled decamethoxin can significantly reduce the frequency of infectious complications, duration of antibiotic therapy, as well as the duration of positive treatment results.

Relevance: inflammatory processes in the oral mucosa are often found in diseases of the gastrointestinal tract, which is explained by the common functions and unity of all parts of the digestive system.Purpose – to study the effect of the studied drugs on the state of antioxidant-prooxidant systems of the oral mucosa in patients with chronic pancreatitis.Materials and methods: we examined 118 patients, of whom 62 were men (mean age 39.6 ± 16.4 years) and 56 women (mean age 40.3 ± 15.7 years); of these, 34 patients (control group) are practically healthy people without somatic pathology and 84 patients with chronic pancreatitis who were divided into two groups. The first (main) group - 46 people who, in addition to the drug therapy prescribed by the gastroenterologist, were prescribed the drug “Multiflora” 1 tablet 1 time per day with meals for 30 days. Also, the patients of the main group, after each meal, rinsed their mouths with “Enoant” food concentrate at a dilution of 1: 5 and applied it inside with 1 tablespoon 2 times a day, diluted with water in a ratio of 1: 5. The second (comparison group) - 38 people who, in addition to treatment of chronic pancreatitis, were treated with the oral mucosa according to the generally accepted method - prescribed fluconazole 150 mg 1 time a day, rinsing with 2% borax solution in glycerol after eating for 14 days. Unstimulated oral fluid of the examined patients served as the material for the study. In all groups, the activity of the catalase enzyme was determined by the method of S.V. Girina (1999), the level of malonic dialdehyde (MDA activity by reaction with thiobarbiturate acid to form a trimethine complex) and antioxidant-prooxidant index of unstimulated oral fluid by the method of А.P. Levitsky (2010). Laboratory research was carried out at the time of the initial examination before the start and on the 30th day of treatment.Results: in the comparison group after treatment, an increased concentration of malondialdehyde in the oral fluid, as well as reduced indicators of the API index and catalase activity confirm the presence of inflammatory processes in the oral mucosa. The application of the proposed therapeutic complex to patients of the main group significantly reduces the concentration of malondialdehyde in the oral fluid, proving its anti-inflammatory effect on the oral mucosa. An increase in the antioxidant-prooxidant index and an increase in catalase activity indicates an increase in the body’s defenses in patients of the main group.Conclusion: the use of the proposed therapeutic complex in patients with inflammatory diseases of the oral mucosa suffering from chronic pancreatitis contributes to the elimination of inflammatory and degenerative processes and the restoration of the normal level of protective systems in the oral mucosa.

e17578 Background: Cytostatic treatment was shown to influence significantly reactivation of latent viral infections. Our purpose was to study the possible effect of cetuximab on activation of herpesvirus infections. Methods: The blood serum of patients with histologically confirmed laryngeal squamous cell carcinoma was studied before and after chemotherapy. The main group included 9 pts receiving cetuximab+cisplatin+5-fluorouracil, the control group – 9 pts, cisplatin+5-fluorouracil. Antibodies (AB) against viral proteins were detected by ELISA. Results: 6 (66.7%) pts of the main group had primary advanced cancer, 3 (33.7%) – recurrent laryngeal cancer; in control group - 7 (77.7%) and 2 (22.2%), respectively. All pts in both groups were seropositive for HSV 1,2, CMV and EBV before chemotherapy; 4 (44.4%) pts in main group and 7 (77.8%) controls were HPV 6 seropositive. Serological markers of activation of herpes virus infection (IgM or IgG against early viral proteins) after chemotherapy were detected in the main group in 8 (88.9%): HSV 1,2 - 5 (55.6%), CMV - 2 (22.2%), EBV - 3 (33.3%). HSV 1,2, CMV and EBV infections in the control group were activated equally often and found in 3 (33.3%) pts. Clinical signs of herpes virus infection were not observed in either group. A sharp decline in AB titers, sometimes up to their complete disappearance, was registered in 3 (33.3%) pts in the main group and in 6 (66.7%) controls. 1 pt of the main group showed a decline in EBV AB titers and absence of HPV 6 AB titers, 1 pt - a decline in CMV and EBV AB, 1 pt - a decline in HSV 1,2, CMV and EBV AB and absence of HPV 6 AB titers. 2 pts of the control group showed a decrease in HPV 6 AB, 2 pts – their absence, 2 pts – absence of HPV 6 AB and a decline in HSV 1,2, CMV and EBV AB. It could be associated with a greater degree of bone marrow hematopoiesis inhibition during cytostatic therapy. Lymphopenia was registered in the control group more frequently than in the main one. Conclusions: Cetuximab inclusion into chemotherapy does not have an additional negative impact on the development of viral infectious complications. A higher rate of registration of specific antibodies typical of the acute infection phase in the cetuximab group could indicate an adequate response to viral infection.

Introduction: Squamous cell carcinoma of the oral cavity is one of the most common head and neck cancers with an aggressive course and high mortality rates. The aim of the study was to determine the EGFR expression levels in tumor tissues in patients with squamous cell carcinoma of the tongue and oral mucosa depending on the efficacy of the therapy. Material and methods: The study included 60 patients with squamous cell carcinoma of the tongue and oral mucosa T3-4N0-1M0. The main group included 30 patients receiving chemotherapy (cisplatin/fluorouracil) in combination with targeted therapy with cetuximab. The control group included 30 patients receiving chemotherapy without cetuximab. Both groups were divided into two subgroups: sensitive and resistant. Results: In treatment-resistant patients of the main group with cetuximab, the average EGFR expression was twice lower than the initial levels (p = 0.0080) and 1.7 times higher than in treatment-resistant patients of the control group (p = 0.0157). In treatment-sensitive patients, the average EGFR expression was 19.8 times lower (p = 0.0020) than initial values and 14.9 times higher (p = 0.0067) than in treatment-sensitive controls. Conclusions: A natural decrease in the EGFR expression in tumor tissues due to the targeted therapy was revealed. However,  some patients were resistant to cetuximab, which dictates the need to search for predictors of targeted therapy efficacy in patients with locally advanced squamous cell carcinoma of the tongue and oral mucosa.

Objective to refine the method of incisional biopsy in the diagnosis of oral mucosa cancer using the auto-fluorescent stomatoscopy. Materials and method.The study was conducted on the base of the Samara Regional Clinical Oncology Center. The inclusion criterion for patients was the diagnose of the oral mucosa cancer of various localization. Patients were divided into 2 groups. The main group included patients (n=43), who were being diagnosed for cancer with the help of optimized incisional biopsy of the oral mucosa formations, using the "AFS-400" autofluorescence complex and glasses with a green light filter for identification. The patients of the control group (n=46) received the standard biopsy procedure under direct vision. Results.The first incisional biopsies revealed cancer in 25 (54%) patients of the control group and in 36 (84%) patients of the main group. A histological verification of the diagnosis was necessary in 7 (16%) patients of the main group and required the second biopsy. In the control group, for the same purpose, 17 (37%) patients underwent the second biopsy and 4 (9%) patients required the third biopsy procedure. Exophytic-papillary forms of cancer were the most complex for histological verification. The primary biopsy of these cases was effective in 16 (37%) patients in the main group and in 8 (17%) patients in the control group (p = 0.036). In patients with initial stages of cancer (I-II), with the first incision biopsy, the histological verification of cancer was achieved in 16 (37%) cases in the main group and in 8 (17%) cases in the control group (p = 0.036). Conclusion.The use of the "AFS-400" autofluorescent complex and glasses with a green light filter for incisional biopsy of oral mucosal formations allows histological verification of cancer with the first biopsy in 84% of cases, including in stages I - II in 16 (37%) cases and in exophytic papillary forms in 16 (37%) cases. The significant difference was registered for the similar indicators of the control group (p = 0.036).
