The perspective of IT decision makers on factors influencing adoption and implementation of AI-technologies in German Hospitals: Descriptive Analysis (Preprint)

BACKGROUND New AI tools are being developed at a high speed. Yet, strategies and practical experiences surrounding the adoption and implementation of AI in health care are lacking. This is likely due to AI’s high implementation complexity, legacy IT infrastructure and unclear business cases, thus complicating AI adoption. Research has recently started to identify factors influencing organizations' readiness for AI. OBJECTIVE Our study aimed to investigate the factors influencing AI readiness as well as possible barriers to AI adoption and implementation in German hospitals. We also tried to assess the status quo concerning the dissemination of AI tools in German hospitals. We focused on IT decision makers which is a seldom studied but highly relevant group. METHODS We created an online survey based on recent AI readiness and implementation literature. The survey was pretested. Possible participants were identified through a publicly accessible database and contacted via e-mail or via invitational leaflets sent by mail, in some cases accompanied by a telephonic pre-notification. Survey responses were analyzed through descriptive statistical methods. RESULTS Overall, we contacted 609 possible participants and our database recorded 40 fully completed surveys. Most participants agreed or rather agreed with the statement that AI will be relevant in the future, both in Germany (37/40, 92,5%) and in their own hospital (36/40, 89,5%). Participants were asked whether their hospital used or planned using AI technologies. 65% (26/40) answered this question with “yes”. Most AI technologies were used or planned in patient care, followed by biomedical research, administration, and logistics and central purchasing. The most important barriers to AI were lacking resources (staff, knowledge, financial). Relevant possible opportunities of using AI were increases in efficiency due to time saving effects, competitive advantages, and increases in quality of care. Most AI tools in use or in planning were developed with external partners. CONCLUSIONS Few tools have been implemented in routine care and many hospitals do not use or do not plan on using AI in the future. This can likely be explained by missing or unclear business cases or needed modern IT infrastructure to integrate AI tools in a usable manner, hence complicating decision making and resource attribution. Since most AI technologies already in use were developed in cooperation with external partners, these relationships should be fostered. IT decision makers in hospitals should assess their hospital’s readiness for AI individually with a focus on resources. Further research should continue to monitor the dissemination of AI tools and AI readiness factors to see if improvements can be made over time, especially in regards to governmentally supported investments in AI technologies that could alleviate financial burdens. Qualitative studies with hospital IT decision makers should be conducted to explore the reasons for slow AI adoption in more detail.

Objective Previous studies have highlighted the short career intentions and high attrition rates of physiotherapists from the profession. The aim of the present study was to examine the job satisfaction and attrition rates of early career physiotherapists graduating from one Western Australian university. Methods A self-administered online survey was conducted of 157 Notre Dame physiotherapy graduates (2006–2012), incorporating a job satisfaction rating scale. Results Results showed that lowered job satisfaction was related to working in the cardiorespiratory area of physiotherapy and working in multiple jobs since graduation. The majority of graduates did not predict a long-term career in physiotherapy, highlighting a lack of career progression and limited scope of practice as influential factors. Conclusions Job satisfaction in early career physiotherapists varies across different clinical areas of practice related to several factors, including challenge and flexibility. New roles in the profession, including extended scope roles, may impact on the future job satisfaction of physiotherapists. Further studies are needed to explore the effect of these roles on workforce trends, including attrition rates. What is known about the topic? Physiotherapists predict careers of 10 years or less on entry into the profession. No previous studies have explored the individual factors influencing job satisfaction in early career physiotherapists across different clinical settings. What does this paper add? This study highlights specific factors influencing the job satisfaction of early career physiotherapists, including clinical area of practice. Physiotherapists working in the cardiorespiratory area were less satisfied, as were physiotherapists undertaking multiple positions since graduation. What are the implications for practitioners? This study informs employers and workforce planners on the factors affecting job satisfaction in early career physiotherapists. In addition, knowledge of issues affecting job satisfaction in the early career stage may assist educational institutions in their preparation of graduates for the future health workforce.

AbstractIn this contribution, we propose a reflection on the potential of data visualisation technologies for (informed) public policy making in a growingly complex and fast changing landscape—epitomized by the situation created after the outbreak of the Covid-19 pandemic. Based on the results of an online survey of more than 50 data scientists from all over the world, we highlight five application areas seeing the biggest needs for innovation according to the domain specialists. Our main argument is that we are facing a transformation of the business cases supporting the adoption and implementation of data visualisation methods and tools in government, which the conventional view of the value of Business Intelligence does not capture in full. Such evolution can drive a new wave of innovations that preserve (or restore) the human brain’s centrality in a decision making environment that is increasingly dominated—for good and bad—by artificial intelligence. Citizen science, design thinking, and accountability are mentioned as triggers of civic engagement and participation that can bring a community of “knowledge intermediaries” into the daily discussion on data supported policy making.

Abstract Background Time-saving formats of evidence syntheses have been developed to fulfill healthcare policymakers’ demands for timely evidence-based information. A discrete choice experiment (DCE) with decision-makers and people involved in the preparation of evidence syntheses was undertaken to elicit preferences for methodological shortcuts in the conduct of abbreviated reviews. Methods D-efficient scenarios, each containing 14 pairwise comparisons, were designed for the DCE: the development of an evidence synthesis in 20 working days (scenario 1) and 12 months (scenario 2), respectively. Six attributes (number of databases, number of reviewers during screening, publication period, number of reviewers during data extraction, full-text analysis, types of HTA domains) with 2 to 3 levels each were defined. These were presented to the target population in an online survey. The relative importance of the individual attributes was determined using logistic regression models. Results Scenario 1 was completed by 36 participants and scenario 2 by 26 participants. The linearity assumption was confirmed by the full model. In both scenarios, the linear difference model showed a preference for higher levels for “number of reviewers during data extraction”, followed by “number of reviewers during screening” and “full-text analysis”. Subgroup analyses showed that preferences were influenced by participation in the preparation of evidence syntheses. Conclusion The surveyed persons expressed preferences for quality standards in the process of literature screening and data extraction.

Background: Enteric fever is an acute febrile-illness caused by infection with the human-restricted Salmonella serovars Typhi and Paratyphi. Controlled human infection models (CHIM) of S. Typhi and Paratyphi infection are used to accelerate vaccine development and to better understand host-pathogen interactions. The primary motivations for participants to take part in these studies are unknown. We studied participant motivations, attitudes and the factors influencing CHIM study participation. Methods: Participant surveys were nested in six enteric fever CHIM studies conducted at a single centre in Oxford, UK, between 2011 and 2017. All eligible participants received one invitation to complete an anonymous, self-administered paper or online survey on either day 28 or 60 after challenge. A descriptive analysis was performed on these pooled data. All studies were included, to minimize selection bias. Results: Survey response rates varied from 33.0%-86.1%, yielding 201 participants. In the cohort, 113/198(57.0%) were educated to bachelor’s level, 61.6% were employed, 30.3% were students and 4.6% were unemployed. The most commonly cited motivations for CHIM study participation were a desire to contribute to the progression of medicine (170/201; 84.6%); the prospect of financial reimbursement (166/201; 82.6%) and curiosity about clinical trials (117/201; 57.2%). The majority of respondents (139/197; 70.6%) reported that most people advised them against participation. Conclusion: Motivation to participate in a CHIM study was multi-factorial and heavily influenced by internal drivers beyond monetary reimbursement alone. High educational attainment and employment may be protective factors against financial inducement; however, further research is needed, particularly with CHIM studies expanding to low-income and middle-income countries.

Abstract. Background: Suicide can have a lasting impact on the social life as well as the physical and mental health of the bereaved. Targeted research is needed to better understand the nature of suicide bereavement and the effectiveness of support. Aims: To take stock of ongoing studies, and to inquire about future research priorities regarding suicide bereavement and postvention. Method: In March 2015, an online survey was widely disseminated in the suicidology community. Results: The questionnaire was accessed 77 times, and 22 records were included in the analysis. The respondents provided valuable information regarding current research projects and recommendations for the future. Limitations: Bearing in mind the modest number of replies, all from respondents in Westernized countries, it is not known how representative the findings are. Conclusion: The survey generated three strategies for future postvention research: increase intercultural collaboration, increase theory-driven research, and build bonds between research and practice. Future surveys should include experiences with obtaining research grants and ethical approval for postvention studies.

AbstractWe communicate and decide every day, but the complexity of the context in which we do these things is increasing. Today, the cultural structure of the organization’s members, due to the need to have competent employees in correlation with the established objectives, is somewhat puzzled and dynamical. Thus, the decision-makers must take into account (mandatory) the cultural basis of the subordinates when choosing the best alternative for solving an organizational problem. From this perspective, Hofstede’s model can be one of the explanatory modalities of the organization’s cultural characteristics as a basis to identify the action’s solutions in that organization for the future.

Studying the current state of cartography and ways of further developing the industry, the role of the map in the future of the society, new methods of promoting cartographic products is impossible without a deep scientific analyzing all the paths, events and factors influencing its formation and development throughout all the historic steps of cartographic production in Russia. In the article, the history of cartographic production in Russia is considered together with the development of private, state and military cartography, since, despite some differences, they have a common technical, technological and production basis. The author describes the stages of originating, formation and growth of industrial cartographic production from the beginning of the XVIII century until now. The connection between the change of political formations and technological structures with the mentioned stages of maps and atlases production is considered. Each stage is studied in detail, a step-by-step analysis was carried out, and the characteristics of each stage are described. All the events and facts are given in chronological order, highlighting especially significant moments influencing the evolution of cartographic production. The data on the volumes of printing and sales of atlases and maps by commercial and state enterprises are presented. The main trends and lines of further development of cartographic production in Russia are studied.

This study aimed to clarify the role that career resilience plays in preventing inhibition of career development when individuals confront changes during their working life, such as changes in work tasks or health condition. Career resilience consists of five factors: ability to cope with problems, social skills, interest in novelty, optimism about the future, and willingness to help others. In all, 1,000 Japanese company employees completed an online survey. The results showed that optimism about the future and ability to cope with problems exhibited a negative correlation with NPC when confronting changes. The results of simple slope analysis suggested that social skills and ability to cope with problems decreased the negative influence that psychological symptoms caused by changes had on job satisfaction, which was one index of career development. This study underlines the necessity of developing the ability to cope with problems and social skills.

On the 25 March 2017, leaders of the EU27 and European Union (EU) institutions ratified the Rome Declaration. They committed to invite citizens to discuss Europe’s future and to provide recommendations that would facilitate their decision-makers in shaping their national positions on Europe. In response, citizens’ dialogues on the future of Europe were instituted across the Union to facilitate public participation in shaping Europe. This paper explores Ireland’s set of dialogues which took place during 2018. Although event organisers in Ireland applied a relatively atypical and more systematic and participatory approach to their dialogues, evidence suggests that Irelands’ dialogues were reminiscent of a public relations exercise which showcased the country’s commitment to incorporating citizens into the debate on Europe while avoiding a deliberative design which could have strengthened the quality of public discourse and the quality of public recommendations. Due to an absence of elite political will for a deliberative process, as well as structural weaknesses in design, participants’ recommendations lacked any clear and prescriptive direction which could shape Ireland’s national position on the future of Europe in any constructive or meaningful way.
