Kata - kata kunci : Letak rumah, cerobong asap pabrik karet, derajat keparahan ISPAABSTRACTAcute respiratory tract infections (ARI) is one of the main health problems. The prevalence of ARI in Indonesia reached 25% and the characteristics of the population with the highest ARI occurred in the 1-4 year age group (25.8%). The study was observational analytic with cross sectional design. The research subjects consisted of 50 toddlers suffering from ARI recorded at Pelambuan Health Center, selected by simple random sampling. Data were analyzed by chi square test at the 95% confidence level. The results showed that 76% of the subjects lived in the house with a moderate distance to the chimney and 24% at close range. As many as 60% have an attack ARI > 1 time and 48% with moderate severity. Statistical test results obtained relationship between the distance from the house to the chimney with respiratory disease patterns in terms of frequency and severity of attacks of ARI in children under five years in the region Public Health Pelambuan.Key words: distance of the house - a rubber factory chimneys – severity of ARI

Data collection was done through interview, observation, thermohygrometer, roll meter, lux meter, and EPAM 5000. Data was analyzed using chi-square and binary logistic regression analysis. The results showed that children under five’s bedroom density (p=0.241) was affecting the occurrence of ARI among children under five, meanwhile ventilation area (p=0.241), air temperature (p=0.948), humidity (p=0.830), and lighting (0.393) were not affecting the occurrence of ARI among children under five. Communities of East Kalianget Village that have unsuitable occupancy density are suggested to reset the number of their bedroom’s occupants, for example maybe a father sleeping in the living room to prevent the occurrence of ARI among children under five.

BACKGROUND Excessive screen time is detrimental to the child’s health. However, screen time situation among Malaysian children is poorly understood. OBJECTIVE This study aims to identify the prevalence and determinants of screen time among children under five years old using the latest WHO guidelines. METHODS A cross sectional design was used to randomly select 489 children from nine government health clinics. Total screen time and factors were assessed using validated self-administered questionnaires and analyzed using multiple logistic regression. RESULTS Results show an overall prevalence of 91.4% with a median of 3.00 hours (IQR: 1.36-5.04). Majority of children watched television (66%), followed by mobile phones (30%) and computers (4%). The determinants of screen time were Malay ethnicity, (AOR 3.56, 95% CI: 1.65-7.68), parental age of 30 years or more (AOR 3.12, 95% CI: 1.58-6.16), parental screen time exceeding 2 hours a day (AOR 2.42, 95% CI: 1.24-4.73), parent’s moderate self-efficacy to influence child’s physical activity (AOR 2.29, 95% CI: 1.01-5.20) and parent’s positive perception on influence of screen time on child’s cognitive well-being (AOR 1.15, 95% CI:1.01-1.32). CONCLUSIONS Parents played an important role in determining their child’s screen time. Future interventions that focus on the parents may ensure age appropriate screen time for their children.

Patients in health services have different behaviors in deciding to take advantage of health services. Patients used various considerations to make decisions based on the health products or services for re-treatment. The study aimed to analyze the factors that influence the patient's decision in using health services for re-treatment. This research was quantitative with cross-sectional design and conducted at Tanjung Pura Regional Hospital. The population was 3.946 people, and the sample were 247 respondents taken by using purposive sampling. Data analysis was univariate and bivariate using chi-square and multivariate using multiple logistic regression at the 95% confidence level (a= .05). The results showed that the factors that influenced the patient's decision to use health services for re-treatment at the Tanjung Pura Regional Hospital were the service quality of doctors / nurses (p= .000), distance (p=.049), facilities (p=0.000), and service speed (p = .000). The factors that did not influence we age (p=.461). gender (p=.895), education (p=.371), occupation (p=.593). The most dominant factor influencing is facilities with Exp (B)/OR =14.477, meaning that respondents who stated that the hospital facilities were good, had the opportunity to use health services for re-treatment by 14.4 times higher the respondents who stated that the hospital facilities were not good. It is suggested to Tanjung Pura Regional Hospital to propose and allocate a budget for improving hospital facilities that are still lacking and adding new service facilities so that it can increase public confidence for repeat treatment
