Objectives: The objectives of the study were to compare progression of spontaneous versus induced labor in primigravida and multigravida women. Methods: Pregnant women admitted in Labor room of Bebe Nanaki mother and Child Care Centre, Amritsar during April 2019–March 2020 were selected for this study. A total of 200 pregnant women were selected and divided into two groups. Women in group A were induced while in group B were women with spontaneous onset of labor. Labor progression in both was compared. Results: In group A, the mean duration of the active phase in primigravida was 4.08±2.30 h and in multigravida was 4.02±2.20 h. In group B, the mean duration of active phase in primigravidas was 7.24±1.39 h and in multigravidas was 6.48±1.40 h. In group A, the mean duration of the second stage in a primigravida was 25.5±8.15 min and in a multigravida was 17.38±9.95 min. In group B, the mean duration of the second stage in a primigravida was 41.3±9.6 min, while in a multigravida was 22.72±6.2 h. Discussion: The mean duration of active phase in group A in the primigravida and multigravida was almost similar, showing that induction does not have any effect on the duration of active phase. The mean duration of the second stage of group A in primigravida was 25 min and multipara was 17 min showing that induction reduces the duration of the second stage. Conclusion: Induction of labor when done at the right gestational age for correct indication is beneficial to women as it reduces the complications caused due to the continuation of high-risk pregnancies.

Abstract Background This study aims at determining the biological effect of 75/25 w/w nano-hydroxyapatite/chitosan (nHAp/CS) scaffolds on bone regeneration, in terms of fraction of bone regeneration (FBR), total number of osteocytes (Ost), and osteocyte cell density (CD), as well as its biodegradability. Methods Two critical-size defects (CSDs) were bilaterally trephined in the parietal bone of 36 adult Sprague-Dawley rats (18 males and 18 females); the left remained empty (group A), while the right CSD was filled with nHAp/CS scaffold (group B). Two female rats died postoperatively. Twelve, 11, and 11 rats were euthanized at 2, 4, and 8 weeks post-surgery, respectively. Subsequently, 34 specimens were resected containing both CSDs. Histological and histomorphometric analyses were performed to determine the FBR, calculated as [the sum of areas of newly formed bone in lateral and central regions of interest (ROIs)]/area of the original defect, as well as the Ost and the CD (Ost/mm2) in each ROI of both groups (A and B). Moreover, biodegradability of the nHAp/CS scaffolds was estimated via the surface area of the biomaterial (BmA) in the 2nd, 4th, and 8th week post-surgery. Results The FBR of group B increased significantly from 2nd to 8th week compared to group A (P = 0.009). Both the mean CD and the mean Ost values of group B increased compared to group A (P = 0.004 and P < 0.05 respectively). Moreover, the mean value of BmA decreased from 2nd to 8th week (P = 0.001). Conclusions Based on histological and histomorphometric results, we support that 75/25 w/w nHAp/CS scaffolds provide an effective space for new bone formation.

This cross sectional, descriptive study was done in the Department of Anatomy, Dhaka Medical College, Dhaka from January to December 2009, to determine the proportion of cortex and medulla of the ovary in di_erent age group of Bangladeshi women. This study was based on collection of 140 ovaries of 70 unclaimed female dead bodies from the morgue of Dhaka Medical College, Dhaka. The samples were divided into three age-groups including A (10-13 years), B (14-45 years) & C (46-52 years). Histological slides were prepared by using routine haematoxylin and eosin stain. Ten best prepared histological slides from each age group were examined to determine the thickness of the cortex and medulla & proportion of the thickness of the cortex and the medulla of the ovary were expressed in percentages. The mean proportion of the cortex and the medulla of the right ovary were found 80.83±0.58% and 19.17±0.58% in group A, 86.95±1.14% and 13.05±1.14% in group B, 70.53±1.53% and 29.47±1.53% in group C respectively. The mean proportion of the cortex and the medulla of the left ovary were found 80.63±0.58% and 19.37±0.58% in group A, 86.78±1.14% and 13.22±1.14% in group B, 70.41±1.50% and 29.59±1.50% in group C respectively. The difference in mean proportion of the cortex and the medulla was not signi_cant in between the ovaries. However, the difference in mean proportion of the cortex and the medulla of the ovary between group A & group B, group A & group C and group B & group C were statistically significant.Bangladesh Med J. 2015 Jan; 44 (1): 8-10

Objectives The objectives of this study were to test: (1) the repeatability of ultrasonographic examination of the optic nerve sheath diameter (ONSD) in the cat; (2) the association between the ONSD and age, sex and body weight in healthy cats; and (3) the difference in the ONSD between healthy cats and those suffering from presumed intracranial hypertension (ICH). Methods This study had a prospective, blinded, observational cross-sectional study design. Two groups of animals were considered: healthy cats (group A) and cats with a diagnosis of presumed ICH (group B). The ONSD was evaluated, measured and compared between the two groups via an ultrasonographic transpalpebral approach. Repeatability of the procedure was evaluated through the intraclass correlation coefficient (ICC). Data were statistically compared using the Student’s t-test and linear regression analysis. Results A strong inter- and intraobserver ICC indicating good repeatability was observed. The interobserver ICC was 0.965 ( P  <0.05) for the right eye and 0.956 ( P  <0.05) for the left eye. The intraobserver ICC was 0.988 ( P  <0.05) and 0.984 ( P  <0.05) for the right and left eyes, respectively. In healthy cats the mean ± SD ONSD was 1.23 ± 0.11 mm (range 1–1.47 mm) and 1.23 ± 0.10 (range 1–1.4 mm) for right and left eyes, respectively. The ONSD was not related to sex or weight; a weak relationship was observed with age. In group B, the mean ONSD was 1.68 ± 0.13 mm (range 1.5–1.9 mm) and 1.61 ± 0.15 mm (range 1.4–1.9 mm) for the right and left eyes, respectively. In group B, the ONSD was statistically significantly larger than in group A, the healthy cats ( P  <0.001). Conclusions and relevance The transpalpebral ultrasonographic technique is a non-invasive, feasible and reproducible method to measure ONSD both in healthy cats and in cats suffering from suspected ICH.

Objective(s): To assess the association between the haematocrit value and severity of preeclampsia.Materials & Methods: This cross sectional study was conducted in the department of Obstetrics & Gynaecology of Dhaka Medical College Hospital, Dhaka during the period of January 2012 to December 2013. Total of 100 patients were studied and they were divided into two groups – group-A and group-B. Fifty preeclamptic patients were included in the group-A and 50 normal pregnant women were included in the group-B.Results: Majority of preeclamptic women (68%) and normal pregnant women (76%) were in the age group of 20-30 years with the mean ages of group-A and group-B subjects were 26.50±5.71 and 26.26±4.91 years respectively. Eighty six percent of group-A and 70% of group-B women were of lower socioeconomic status. Majority of the study subjects (74% of group-A and 80% of group-B) were housewife. More than half (54%) of the preeclamptic women were primigravid and 60% of normal pregnant women were multigravid. Among the preeclamptic subjects 16 (32%) had mild hypertension (DBP<110mmHg) and 34 (68%) had severe hypertension (DBP ? 110 mmHg). Out of 50 preeclamptic subjects 45 (90%) had severe proteinuria (+++) and only 5 (10%) had moderate (++) proteinuria.The mean haematocrit value of preeclamptic patients was 34.881±3.03 and that of normal pregnant women was 31.94±1.2. It was statistically significant (P value 0.001). The mean haematocrit value of normal pregnant, mild and severe preeclamptic women were 31.94±1.2, 33.31±2.57 and 35.62±2.95 respectively. It was also statistically significant (P value 0.001).Conclusion: This study shows that haematocrit value of preeclamptic patients is significantly higher compared to that of the normal pregnant women (P<0.05). Context: A cross-sectional descriptive type of study was designed to find out the difference in weight of the parathyroid glands of Bangladeshi people in relation to age and sex and to compare with the previous studies. Methods: This study was done in the Department of Anatomy, Dhaka Medical College, Dhaka, from January to December 2008. The study was performed on 207 post mortem human parathyroid glands collected from 60 unclaimed dead bodies which were in the morgue under examination in the Department of Forensic Medicine, Dhaka Medical College, Dhaka. The samples were divided into three age-groups including group A (10-30 years), group B (31-60 years) & group C (61-90 years) and the weight of the parathyroids were measured and recorded. Results: The mean weight of the right superior parathyroid glands were 13.43±13.77 mg in group A, 13.86±19.15 mg in group B and 9.25±5.12 mg in group C in males and 16.00±6.69 mg in group A and 8.57±5.97 mg in group B in females respectively. The mean weight of the right inferior parathyroids were 17.78±13.94 mg, 16.75±15.67 mg and 23.00±11.53 mg in group A, B and C respectively in males and 26.00±4.18 mg in group A and 15.20±4.32 mg in group B in females. The mean weight of the left superior parathyroid glands were 13.63±6.08 mg in group A, 10.58±7.57 mg in group B and 11.00±3.74 mg in group C in males and 16.20±7.89 mg in group A and 8.29±5.47 mg in group B in females respectively. The mean weight of the left inferior parathyroid gland was 26.11±24.29 mg, 15.81±10.28 mg and 25.25±25.28 mg in group A, B and C respectively in males and 32.25±13.82 mg in group A and 13.29±5.41 mg in group B in females. No difference was found in the weight of superior parathyroid glands at right side between sexes and age groups excepting in superior and inferior parathyroid glands at left side between group A & B (p<0.05). Objective Video-assisted thoracic surgery lobectomy is a minimally invasive procedure for major pulmonary resection. The purpose of this study was to present a novel approach with a thoracoscope in the right upper lobe and to compare different lobectomy methods at our institution. Methods We reviewed the medical records of patients who underwent a thoracoscopic right upper lobectomy for lung cancer between September 2015 and September 2016. We performed 128 thoracoscopic right upper lobectomies: group A (n = 50) was treated with the bronchus-first and vessels simultaneously stapled method and group B (n = 78) was treated with the conventional isolation–ligation method. Preoperative mediastinal staging and lymphadenectomy followed the National Comprehensive Cancer Network guidelines. The intra- and postoperative outcomes were recorded and statistically compared. Results All patients underwent successful thoracoscopic right upper lobectomies. No significant differences in mean intraoperative blood loss, massive hemorrhage (>500 mL), and postoperative complications were observed between the two groups (p < 0.05). The mean operative time of group A was less than that of group B (110.80 ± 34.74 versus 167.01 ± 48.38 minutes, p = 0.000). The mean duration of chest drainage in group A was 4.34 ± 2.06 days, which was shorter than that of group B (5.85 ± 3.13 days, p = 0.017). No significant differences were observed in the local recurrence and distant recurrence between the two groups during the postoperative follow-up. Conclusions Thoracoscopic right upper lobectomy with the lobectomy bronchus-first and vessels simultaneously stapled method is a safe and efficient procedure that leads to better recovery.

Objective: A Cross-sectional descriptive type of study was done in the Department of Anatomy, Dhaka Medical College, Dhaka, from July 2008 to June 2009, to see the variation in the volume of the adrenal glands with age in Bangladeshi people. Materials & Methods: The study was performed on 140 post mortem human adrenal glands collected from 70 unclaimed dead bodies which were in the morgue under examination in the Department of Forensic Medicine, Dhaka Medical College, Dhaka. The samples were divided into four age-groups including group A (11-20 years), group B (21-30 years), group C (31-40 years) & group D (41-60 years). The length, breadth and thickness of each adrenal gland were measured by using a slide calipers. Then the volume of each adrenal gland was determined by the product of its length, breadth and thickness multiplied by 0.52, according to the prolate ellipsoid formula. Results: The mean volume of the right adrenal glands were found 6.36±0.85 cm3 in group A (11-20 years), 6.49±0.76 cm3 in group B (21-30 years), 6.50±0.80 cm3 in group C (31-40 years), 6.76±0.79 cm3 in group D (41-60 years). The mean volume of the left adrenal glands were found 6.97±1.02 cm3 in group A (11-20 years),6.93 ±0.83 cm3 in group B (21-30 years), 6.65±0.79 cm3 in group C (31- 40 years), 7.09±0.81 cm3 in group D (41-60 years). The differences between the right and left adrenal glands and the difference between age groups were not statistically significant. Background: Childbirth is one of the most wonderful and anticipated moments in a woman’s life. But the excruciating pain associated with labour makes it a really undesirable one. Objective of present study was to compare the effects of programmed labour protocol and expectant management of labour.Methods: A clinical study was conducted on 120 low risk primigravida, 60 of them were managed with programmed labour protocol (group A) and the rest 60 (group B) were managed expectantly. Primary parameters were pain relief and changes in the mean duration of labour. Secondary parameters were changes in the amount of blood loss and APGAR score of delivered babies.Results: Among group A, 25% had good pain relief, 60% had moderate pain relief, 15% had mild pain relief and 0% had no relief of pain. The mean duration of labour from active phase through third stage in group A was 234.27±50.38 minutes and 304.58±41.72 minutes in group B. The average blood loss during labour was 89.33±19.06 ml in group A and 142.15±32.27 ml in group B. In group A, 4 babies had APGAR score less than 7 at 1 minute, but had score more than 7 after 5 minutes following resuscitation. In group B, 9 babies had APGAR less than 7 at 1 minute. Of these, 6 babies had score more than 7 after 5 minutes following resuscitation. 3 babies had score less than 7 even after 5 minutes and were shifted to neonatal ICU.Conclusions: Programmed labour protocol is an effective method to reduce the deleterious effects of labour on both mother and foetus.

OBJECTIVEThe outcome for jailing arterial branches that emerge near intracranial aneurysms during flow-diverting stent (FDS) deployment remains controversial. In this animal study, the authors aimed to elucidate the role of collateral supply with regard to the hemodynamic changes and neointimal modifications that occur from jailing arteries with FDSs. To serve this purpose, the authors sought to quantify 1) the hemodynamic changes that occur at the jailed arterial branches immediately after stent placement and 2) the ostia surface values at 3 months after stenting; both parameters were investigated in the presence or absence of collateral arterial flow.METHODSAfter an a priori power analysis, 2 groups (Group A and Group B) were created according to an animal flow model for terminal and anastomotic arterial circulation; each group contained 7 Large White swine. Group A animals possessed an anastomotic-type arterial configuration to supply the territory of the right ascending pharyngeal artery (APhA), while Group B animals possessed a terminal-type arterial configuration to supply the right APhA territory. Subsequently, all animals underwent FDS placement, thereby jailing the right APhAs. Mean flow rates and velocities inside the jailed branches were quantified using time-resolved 3D phase-contrast MR angiography before and after stenting. Three months after stent placement, the jailed ostia surface values were quantified on scanning electron micrographs. The data were analyzed using descriptive statistics and group comparisons with parametric and nonparametric tests.RESULTSThe endovascular procedures were feasible, and there were no findings of in situ thrombus formation on postprocedural optical coherence tomography or ischemia on postprocedural diffusion-weighted imaging. In Group A, the mean flow rate values at the jailed right APhAs were reduced immediately following stent placement as compared with values obtained before stent placement (p = 0.02, power: 0.8). In contrast, the mean poststenting flow rates for Group B remained similar to those obtained before stent placement. Three months after stent placement, the mean ostia surface values were significantly higher for Group B (527,911 ± 306,229 μm2) than for Group A (89,329 ± 59,762 μm2; p < 0.01, power: 1.00), even though the initial dimensions of the jailed ostia were similar between groups. A statistically significant correlation was found between groups (A or B), mean flow rates after stent placement, and ostia surface values at 3 months.CONCLUSIONSWhen an important collateral supply was present, the jailing of side arteries with flow diverters resulted in an immediate and significant reduction in the flow rate inside these arteries as compared with the prestenting values. In contrast, when competitive flow was absent, jailing did not result in significant flow rate reductions inside the jailed arteries. Ostium surface values at 3 months after stent placement were significantly higher in the terminal group of jailed arteries (Group B) than in the anastomotic group (Group A) and strongly correlated with poststenting reductions in the velocity value.

Background: Thyroid diseases have a strong predominance in woman of childbearing age. Pregnancy may be associated with thyroid dysfunction. The aim of the present study was to assess the serum FT3, FT4 and TSH levels in pregnant women. Methods: This cross-sectional analytical study was done in the Department of Physiology of Dhaka Medical College, Dhaka, from July 2006 to June 2007. Total 50 apparently healthy women of low socioeconomic class, age ranged from 18-40 years were selected from the Outpatient Department of Urban Primary Health Care Project (UPHCP) at Mirpur, Dhaka. Out of them 30 pregnant women of different trimester were taken as study group (group A) and 20 age matched non pregnant women were taken as control (group B). Serum FT3, FT4 and TSH levels were parameters in both groups. Statistical analysis was done by the SPSS version 12.0. Results: The mean serum FT3 levels were 6.36±1.16 pmol/L and 6.381.36 pmol/L in group A (Study) and group B (control) respectively. The mean serum FT4 levels were 20.25±4.77 pmol/L and 19.39±8.17 pmol/L in group A (Study) and group B (control) respectively. The mean serum TSH levels were 0.96±0.96 mIu/L and 1.27±0.86 mIu/L in group A (Study) and group B (control) respectively. The difference was not significant (p>0.05) between group A and B. Conclusion: From the results obtained in the present study, it may be concluded that there is no change of serum FT3, FT4 and TSH level in pregnancy.
