The aim of this study was to investigate the role of Chlamydia trachomatis as a risk factor in ectopic pregnancy in Iraqi women. The study involves two groups, the first group included 24 women diagnosed as ectopic pregnancy, while the second group included 24 women as control group diagnosed as normal pregnancy. Both groups were admitted to Al-Elweya Teaching Hospital, Baghdad from November 2009 to July 2010. Serum antichlamydial antibodies were investigated in both studied groups using enzyme_linked immunosorbent assay (ELISA). Results revealed a significant(p<0.05) percentage of IgG antichlamydial antibodies in women detected as ectopic pregnant, as compared with control normal pregnant (45 vs. 8.3%). On the other hand, ectopic pregnant women exhibited low (p<0.05)IgM antichlamydial antibodies 4% as compared with control group. In conclusion all women with seropositive to Chlamydia trachomatis antibodies had pelvic inflammatory disease (PID).

Aim of the current research is to assess the Chlamydia Trachomatis infection role in the development of early pregnancy complication including ectopic pregnancy and miscarriage in Sulaimanyia Maternity Teaching Hospital. It is a comparative study conducted in Gynecology Clinic and Emergency department of Sulaimanyia Maternity Teaching Hospital during the period from 1st of September 2018 to 31st of March 2019. The study groups included of 70 pregnant women; the first group included 35 ectopic pregnant women and the second group included 35 normal pregnant women that both groups had been selected randomly. Pregnant women with history of ectopic pregnancies, women used intrauterine device, in vitro fertilization, assisted reproduction and history of pelvic surgery. Blood sample (2 ml venous blood) collected to test for antibodies level for Chlamydia Trachomatis by Alegria test system for both studied groups and patients with ectopic pregnancy detected by beta human chorionic gonadotropin and ultrasound scanning. The collected data analyzed by SPSS program and for compare between means of two variables independent sample t-test was used while for comparison of categorical variables Chi square test was used with considering ≤ 0.05 P-value as significant level. The results shows that the mean age of normal pregnancy were (28.3±4.6) group compared with mean age ectopic pregnancy (29.5±4.9) group. The mean IgG (6.3±5.1) of patients with ectopic pregnancy was found to be significantly higher than mean IgG (2.8±1.1) for normal pregnant patients (P-value 0.01) and IgM mean (4.5±2.4) of patients with ectopic pregnancy was significantly higher than mean IgM (1.6±1.2) for normal pregnant patients with P-value 0.01. In conclusion, infection of Chlamydia Trachomatis has a significant relationship with the development of ectopic pregnancy therefor screening and treatment of Chlamydia infection may reduce ectopic pregnancy rate with low cost

The study has been carried out to observe the effect of calcium therapy on urinary calcium/creatinine ratio in healthy gravid women and pre-eclamptic women. Calcium/creatinine ratio has been considered for early detection of pre-eclamptic toxaemia (PET). So for the diagnosis of PET we try to investigate relation of urinary calcium, urinary creatinine, urinary calcium/creatinine ratio before and after calcium therapy. For this purpose total number of 60 healthy gravid and clinically suspected PET women of 20- 28 weeks of pregnancy having age ranged from, 17-39 years were selected. Of these 30 healthy pregnant women were taken as control group and another clinically diagnosed 30 PET women were taken as observational group. Urinary calcium/creatinine ratio was within normal range in healthy pregnant women and after calcium therapy this ratio was not significantly changed. On the other hand urinary calcium/creatinine ratio in PET women was significantly decreased and this ratio was significantly increased after calcium therapy. These observations suggest that low calcium/creatinine ratio help in early prediction of PET and calcium supplement during pregnancy may reduce the incidence of PET. Background/Aim. Considering the fact that role of oxidative stress in pathogenesis of thrombophilia in pregnancy has still not been clarified, the aim of our study was to assess the redox status of pregnant women with thrombophilia. Methods. The study involved 120 pregnant women who were divided into two groups: thrombophilia and normal pregnancy group. The thrombophilia group consisted of 60 pregnant women with thrombophilia, while the normal pregnancy group included 60 physiologically healthy pregnant women. Blood samples for biochemical analysis were collected at the end of first, second and third trimester of pregnancy. Concentrations of hydrogen peroxide (H2O2), nitrites (NO2-) and index of lipid peroxidation measured as TBARS were measured in plasma. Level of reduced glutathione (GSH), activities of superoxide dismutase (SOD) and catalase (CAT) were measured in erythrocytes. Results. In women with thrombophilia, NO2- values were increased in the first and third trimester when compared with healthy pregnant women (p<0.05). The higher levels of TBARS and H2O2 were noticed in women with thrombophilia in the first trimester when compared to healthy pregnant women (p<0.05). The values of SOD and CAT were lower in women with thrombophilia in the third and GSH in the first trimester compared to control group (p<0.05). Conclusion. Our results suggest increased generation of pro-oxidants in thrombophillia at the beginning of gestation, which declines as gestation progresses and reaches the similar values as in normal pregnancy at the end of pregnancy. Generally viewed, thrombophilia was associated with impaired antioxidant capacity - SOD and CAT were lower in the third and GSH in the first trimester compared to healthy women.

Introduction. Chlamydia trachomatis infections are the most prevalent bacterial sexually transmitted infections recognized throughout the world. In the last few years, several studies have indicated that predisposition of C. trachomatis to persist within the host cell is recognized as a major factor in the pathogenesis of chlamydial infection. During persistent chlamydial infection, the stress protein hsp60 is synthesized continually as immunopathologic antigen. Antibodies to hsp60 are found in women with tubal occlusion, but these antibodies are not detected in women with the acute C. trachomatis infection, which indicates that hsp60 has an important role in pathogenesis of persistant chlamydial infection. Objective. The aim of this study was to determine the role of chlamydial stress proteins (heat shock proteins) in pathogenesis of ectopic pregnancy. Methods. The study included 40 women with ectopic pregnancy (experimental group) and 34 women with normal pregnancy (control group). C. trachomatis was detected in endocervical smears by direct immunofluorescence test, while specific antibodies against Chlamydia spp. and against hsp60 were detected by ELISA. Results. DIF method detected the presence of C. trachomatis in 12.5% of women with the ectopic pregnancy and 17.6% of women with normal pregnancy. IgG antibodies against Chlamydia spp. were found in 57.5% of women with the ectopic pregnancy and 26.5% of women with normal pregnancy. IgA antibodies were positive in 37.5% of women with the ectopic pregnancy and 2.9% of women with normal pregnancy. IgG antibodies against hsp60 were positive in 37.5% of women with the ectopic pregnancy and 14.7% of women with normal pregnancy. Conclusion. The results of this study have proven the correlation between the ectopic pregnancy and the presence of IgG antibodies to chlamydial hsp60.

Abstract. Intact hCG and free α and β hCG subunits were simultaneously measured by specific radioimmunoassays in blood samples from pregnant women with normal pregnancy (n = 87; control group) or with a ruptured ectopic pregnancy (n = 35) between 5 and 14 weeks. Since the lower 95% confidence limit of the hCG levels encountered in normal pregnancies was at 1200 ng/ml, the pathological group has been subdivided in 2 subgroups: A (n = 17) if hCG was greater and B (n = 18) if hCG was lower than this limit. In subgroup A, mean hCG and β hCG were significantly lower than in the control group in contrast to a tendency to secrete and/or release more α hCG, as reflected by a significantly increased ratio of α hCG/hCG. Thus, the ectopically implanted trophoblast, even when able to produce normal amounts of hCG, undergoes an impairment of its ability to synthesize and/or release free β hCG but not free α hCG, of which the synthesis and/or release appears to become even enhanced. In subgroup B, mean hCG, α hCG and β hCG levels were all dramatically suppressed but there was surprisingly less alteration of the synthesis and/or release of free β hCG. These alterations of the relative secretions of the free hCG subunits, compared to those occurring for various glycoproteic hormones in pathological circumstances, reflect probably early alterations of the trophoblast.

Pregnant women with intrauterine growth restriction (IUGR) have an increased risk of adverse perinatal and long-term complications compared with the birth of children with normal body weight. Thus, IUGR is one of the main challenges for the global health system, especially in poor and developing countries. Morpho-functional studies of the placentas help in determining the causes of IUGR, and therefore, timely prevent complications in pregnant women with IUGR. The objective: The purpose of this study is to investigate various morphometric and pathomorphological changes in the placenta, including inflammatory, in cases of IUGR, and to establish a correlation of these results with the etiology and complications for the fetus. Materials and methods. In the current study, 54 placentas of the fetuses with IUGR (the main group) were compared with 50 placentas of the fetuses with normal development (control group). The criteria for the inclusion of IUGR were gestational age more than 30 weeks and all fetuses with a weight less than 10th percentile for this period of pregnancy. The placenta material was studied pathomorphologically with laboratory screening for infection and inflammation. Similarly, the results were determined for placentas of the fetuses with normal development compared to placentas with IUGR. Results. The placenta study showed the presence of calcification in the case of IUGR, as well as in the case of prolonged pregnancy. However, calcification of the placenta in the case of IUGR was more progressive compared with placenta in the normal pregnancy. In addition, the presence of intrauterine infection and inflammation was observed, which could also lead to an adverse outcome for the further progression of pregnancy with IUGR. Conclusion. A comparative macro- and microscopic pathomorphological study of the placentas in the two groups has shown a significant increase in the pathological changes in all the anatomical structures of the fetuses with IUGR. <p>ABSTRACT<br />Objective: The goal of the present study was to examine the viability of Morus alba (M. alba) ethanolic extract in repression of obesity-associated<br />hepatic steatosis and related metabolic disorder; dyslipidemia, hyperinsulinemia, and glycemic status.<br />Methods: Adult female albino rats were randomly assigned into four groups, eight rats each as follows: Group (1) control group received standard<br />rodent diet for 24 weeks. The other three groups administered high cholesterol diet for 12 weeks and served as obese group, M. alba-treated group,<br />and simvastatin-treated group.<br />Results: The current results showed an increment in thoracic circumference (TCX) and abdominal circumferences (AC) as well as body mass index<br />(BMI) in obese group. In addition, dyslipidemia, hyperinsulinemia, hyperglycemia, and insulin resistance have been elucidated in obese group.<br />Moreover, hepatic malondialdehyde (MDA), nitric oxide (NO), serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), and bilirubin<br />values were significantly increased in obese groups versus control group. On the other hand, administration of ethanolic extract of Morus alba or<br />simvastatin could significantly lessen BMI and in addition to improve dyslipidemia in obese group. Glucose, insulin levels, and insulin resistance value<br />in serum samples demonstrated a significant reduction in obese group upon treatment with M. alba ethanolic extract or simvastatin. Furthermore,<br />noticeable depletion in hepatic MDA, NO contents, serum ALT, AST activities, and serum bilirubin level was recorded as a result of treatment with<br />either ethanolic extract of M. alba or simvastatin. Histopathological examination of liver tissue showed ballooning degeneration in the hepatocytes<br />(hepatic steatosis) associated with inflammatory cells penetration in portal zone in obese group. Meanwhile, the treatment of obese groups with<br />ethanolic extract of M. alba or simvastatin was found to restore the structural organization of the liver.<br />Conclusion: The present findings provide a novel aspect for understanding of the role of M. alba against obesity-associated liver diseases and related<br />metabolic disorder. The mechanisms underlying these effects seem to depend on the hypolipidemic potential, anti-inflammatory property, and<br />antioxidant activity of its phytochemicals.<br />Keywords: Obesity, Morus alba, Dyslipidemia, Hyperinsulinemia, Hyperglycemia, Hepatic steatosis.</p>

Preeclampsia (PE) is characterized by a series of clinical features such as hypertension and proteinuria associated with endothelial dysfunction and the impairment of placenta vascular endothelial integrity. This study aimed to investigate the effect of serum copper (Cu) level on some angiogenesis-related factors including vascular endothelial growth factor-A (VEGF-A), soluble Fms-like tyrosine kinase-1 (sVEGF-R1), soluble endoglin (sEng) and cerruloplasmin (Cp) in Iraqi women with preeclampsia (PE) and control pregnant women. Therefore, 60 women with PE in addition to 30 healthy pregnant women were enrolled in the study. Serum concentration of sEng, VEGF-A, sVEGF-R1, and Cu in PE group significantly increased (p&lt;0.05) in the PE group compared with that in the control group. Increased production of antiangiogenic factors, soluble VEGF-A and sEng contribute to the pathophysiology of PE, indicating the involvement of these parameters in the angiogenic balance in patients with PE. Tests for between-subject effects showed that the circulating angiogenesis factors and Cu were significantly associated with the presence of PE. Serum Cu level was significantly correlated with VEGF- A and VEGF-R1 levels but not with sEng. Multiple regression analysis revealed that only Cp and BP can significantly predict the complications in women with PE. In conclusion, serum Cu has a role in the angiogenesis in women with PE and may be a new drug target in the prevention or treatment of PE.

Background: An in-depth study of dismetabolic mechanisms in the genesis of pre-eclampsia (PE) has been updated because pregnancy is considered as a natural model of metabolic syndrome (MS), as well as the metabolic disorders are important in development of essential hypertension.Aims: to reveal clinical and laboratory parallels in pregnancy complicated by PE without MS and pregnancy proceeding on the background of MS to assess the role of metabolic disturbances in the development of PE.Materials and methods: 82 women with MS were examined in the dynamics of pregnancy and were divided into 2 groups depending on the implementation of PE: group I consisted of 50 women with PE on the background of MS, group II 32 women with MS without PE. We formed group III consisting of 44 pregnant women with PE without accompanying diseases to assess the pathogenetic value of metabolic disorders in the development of PE. The IV (control) group consisted of 30 healthy women with physiological pregnancy. Metabolic, hematological parameters, hormones, markers of the proinflammatory state, endothelial hemostasiological dysfunction, decidualization and placental angiogenesis, accumulation dynamics and distribution loci of adipose tissue were determined in all pregnant women.Results: In the groups of pregnant women with PE, changes similar to MS were revealed: pronounced diabetic and atherogenic disorders with the development of pathological insulin resistance, hyperinsulinemia and leptinemia, endothelial-platelet link hyperactivation, thrombotic and inflammatory status, visceral type of fat deposition, hyperuricemia, hypersympathicotonia. It is proved that in the hierarchy of mechanisms of PE formation, placental dysfunction is a secondary alteration factor, which additionally potentiates the insulin resistance increase and the effects of structural and functional destabilization of the vascular endothelium.Conclusions: The direction of metabolic changes during pregnancy, the common development of PE and MS indicate the important role of dismetabolic mechanisms in the formation of PE.
