Summary Gestalt Theoretical Psychotherapy, in the broader sense of the term, has developed in various forms on both sides of the Atlantic since the 1920s. Gestalt Theoretical Psychotherapy, in the narrower sense of the term, came into being in the second half of the 1970s in German-speaking countries. In Austria, it is a state-approved, independent scientific psychotherapy method since 1995, and an integrative psychotherapeutic approach based on the Gestalt theory of the Berlin School. With reference to this comprehensive, consistent, scientific theory, this article presents the basic concepts of therapeutic practice in the field of Gestalt psychotherapy. Starting from the overarching whole to the parts, the paper first examines the concept of therapeutic relationship and therapeutic attitude, and then describes the basic principles of the practical design of the therapeutic process.

Chapter 2 redefines the three basic concepts of any rhetoric: ethos, logos, and pathos. It relates these elements to the questioning process by which they are rhetorically linked. Special attention is given to logos as a way of answering and expressing questions. This leads to the development of a radically new view of language and the principles of thought. The passage of a propositionalist view of language and reason, indifferent to questioning, to a problematological one, based on questioning is studied through examples of sentences. This leads to an integrative view, in which texts are also seen as answers to questions taken up (partially, i.e. as points of view) by the audience or the reader. The chapter ends with a reformulation of the basic principles of thought (identity, sufficient reason, and non-contradiction) as the three principles necessary to deal with questions, answers, and their relationship.

This paper presents a model of the implementation of mediation in cases where a power disbalance between partners exists. The model includes relevant theoretical concepts which are important for understanding of violence phenomenon in the family, and the contemporary approaches to the work on balancing unequal power in relationships. In creating the model of family mediation in cases including violence, some basic concepts of mediation procedures are modified or adjusted (neutrality, confidentiality), as well as the concept of circular causality and complementary relationships from a systemic perspective. Implementation of interdisciplinary approach is proposed for efficient work on balancing the power in relations and using various working domains. The confession of violent behavior and the acceptance of personal responsibility by the offender, and the readiness of both sides to take part in restoration of a relationship are basic principles of restorative justice that is fundamental for victim- offender mediation. Therefore, those conditions are also necessary for a family mediation in cases including elements of violence, if the security for the victim is provided and guaranteed.

A number of approaches exist within the field of music therapy. Some models for evaluating the efficacy of therapy have been adopted in the UK in recent years. These have measured the occurrence of specific behaviours within therapy, or compared music therapy with other interventions. There is a need to find reasonably reliable methods of describing change and the therapeutic process occurring within music therapy. This paper describes change occurring in five people with learning disabilities, in terms of their levels of engagement in therapy and in the therapeutic relationship. A method is provided, to evaluate independent observers ‘perceptions of change in the patients over a 30-session period of therapy. A significant increase in levels of engagement over time was found. It was also found that the degree of change over time was not related to the mean level of engagement. We discuss some subtle factors involved in therapeutic engagement for the five patients in the study, and stress the importance of a therapy which emphasises the dynamics of interpersonal communication for people with limited opportunities to express thoughts and emotions.

The termination phase of clinical practice is an important component of the therapeutic process. The ending of the therapeutic relationship, whether planned or unplanned, can elicit feelings of loss, separation, and guilt, impacting both the client and the practitioner. The reasons for ending service and preparation for termination can affect the client's gains. Systematic research on the termination process and the maintenance of gains is needed to further determine variables for successful termination.

A review of research relevant to the practice of psychotherapy is presented. Research on the central role of cognition in emotional behavior is reviewed as the basis for the therapeutic approach known as cognitive behavior modification. The three stages of the therapeutic approach offer the potential of a unified explanation for the effectiveness of a variety of therapeutic schools as well as a basis for integrating psychology and theology in therapeutic practice. The potential for a theology of the thought life informing therapeutic practice is explored. Finally, the cognitive behavior modification model is embedded in an interpersonal view of therapy to account for the overwhelming evidence of the importance of the therapeutic relationship in effective therapy. The influence of client-therapist value similarity and therapist's characteristics on the quality of the therapeutic relationship are explored. The biblical and psychological data reviewed suggest a fruitful approach to formulating effective and integrated intervention techniques.

In this paper I undertake to analyze the way in which the arrival of HETs may influence the therapeutic relationship between the medical doctor and the patient. I begin with presenting he notion of transhumanism, insisting especially on the fact that some of the technologies that can be classified as HETs are already in use. As a result, the traditionally difficult task of defining health and a disease is becoming even more complicated. This circumstance poses the risk that medical doctors in their relationship with the patient, because of the possibilities offered by new technologies, will oscillate in their professional practice between helping the patient to recover and satisfying needs that are not justified by the considerations of health. I will try to show how the therapeutic relationship between the medical doctor and the patient may be transformed because of new technologies by using the example of IVF procedure applied to postmenopausal patients. In order to understand why the relationship between the medical doctors and their patients is so vulnerable in the context of transhumanism, I propose to re-analyze the most basic notions which help us understand the nature of the therapeutic relationship: the status of medicine as contrasted with technology, basic principles of medical ethics, the notion of a disease and an illness.

Therapeutic Relationship has been regarded as a profound element of the psychotherapeutic procedure and crucial for the outcome of therapy. Research evidence so far claims that both therapist’s and client’s personality can impact the Therapeutic Relationship. Based on the well-researched Attachment Theory there has been some research mostly focused on how clients’ attachment patterns can affect the Alliance. Limited research, though, on how therapists experience the Relationship in relation with their Attachment Styles do exist. The current study investigates trainee therapists’ experience of the Therapeutic Relationship in the light of their Attachment Style. A qualitative approach was used for this research to investigate in depth the experience of five trainee Integrative psychotherapists who were recruited to take part in semi-structured interviews. Interviews were transcribed and verbatim was divided into superordinate and subordinate themes and analyzed by Interpretative Phenomenological Analysis method. Four master themes were identified. Findings were then compared and discussed according to existing literature. All participants referred to the relevance of Attachment Theory in psychotherapy. They also agreed on the activation of their Attachment Styles during the therapeutic process. The securely attached therapist confirms previous studies on her capability to offer a secure base for her clients. On the other hand therapists with insecure Attachment Styles appeared sensitive in their collaboration mostly with insecure clients. Anxiously attached therapists proved to experience difficulty in the initial stages of the Therapeutic Relationship while the combination of avoidant therapist-avoidant client appeared to be the most demanding one, regarding the concept of trust and relationship ruptures. We suggest that the exploration of trainee therapists’ Attachment Styles as well as the study of the Attachment Theory should be added to all Counseling and Psychotherapy curricula regardless the therapeutic orientation because of its contribution to forming positive Therapeutic Relationships.

Abstract Background Evidence-based clinical practice is an inherent component of mental health professional practice in developed countries. However, little is known about professional perspectives of evidence-based practice in mental in developing countries such as Ghana. This paper describes the processes involved in the delivery of best practice in Ghana. The paper reports on a realistic evaluation of mental health nurses and allied health professionals’ views on the evidence-based therapeutic process in Ghana. Methods A purposive sample of 30 mental health professionals (MHPs) was recruited to participate in semi-structured, in-depth interviews. Thematic analysis was used to analyse the data. A program theory of Context + Mechanism = Outcome (CMO) configuration was developed from the analysis. Results The thematic analysis identified two contexts, mechanism and outcome configurations (themes): 1) technical competency stimulates evidence-based mental health services, and 2) therapeutic relationship building ensures effective interaction. The study demonstrates that contextual factors (technical competencies and therapeutic relationship building) together with mechanisms (intentional and unintentional) help to promote quality in mental health service provision. However, contextual factors such as a lack of sign language interpreters yielded unintended outcomes including barriers to communication with providers for consumers with hearing impairment and those from linguistic minority backgrounds. Conclusion Government stakeholders and policymakers should prioritise policies, periodic monitoring and adequate financial incentives to support the mechanisms that promote technical competence in MHPs and the building of therapeutic relationship.
