Abstract Granulomatous rosacea is considered to be the only true variant of rosacea. Diascopy of larger lesions often reveals apple-jelly nodules, indicating their granulomatous histology. Other signs and symptoms of rosacea are not required to make a diagnosis of granulomatous rosacea. The response to treatment may be slow, which must be the most important consideration for both the clinician and the patient. We present an otherwise healthy 62-year-old non-atopic woman with a 15-year history of episodic facial flushing, often accompanied by a burning sensation without sweating. Exclusively affecting the face, lesions had a high tendency to spread each year. The patient was a lifelong non-smoker. A seller on the local market, she spent most of her time outdoors. She had no positive family history of rosacea. At the time of presentation, she was not taking any medications, except for topical neutral creams. Multiple reddish-brown papules without comedones, associated with telangiectasia were scattered over the erythematous background on the chin, forehead, cheeks, nose, glabella and eyelids, while small pustules clustered over the eyelids. The nasolabial folds, neck and ears were not affected. There was neither lymphadenopathy, nor ocular involvement. Based on the history, physical, laboratory and other relevant investigations including histopathology, the diagnosis of granulomatous rosacea was established. The therapy was conducted with topical azelaic acid 20% cream, twice daily for six months. Clinical evaluation was done every 14 days during the first month, and once monthly during the next five months. Affter two weeks, there was a significant decrease in the mean inflammatory lesion count. At the end of the therapy, telangiectasia and facial erythema almost disappeared. There were no side effects. Apart from several short episodes of erythema, during the five-year-long follow-up, there were no other signs of the disease. In conclusion, azelaic acid 20% cream may be an effective and safe treatment option for some patients with granulomatous rosacea.

Genital TB may present as on abdominopelvic mass mimicking ovarian malignancy because clinical and laboratory findings are similar. Family history is very important and should be considered for differential diagnosis. Three cases of genital TB with presentation of abdominopelvic masses and with no signs and symptoms of TB were presented. Two of them had positive family history of pulmonary TB. Tissue diagnosis was the best method for diagnosis of genital TB, but it should be reminded that if positive family history of TB was present, mini laparotomy should be done to take biopsy and to make rapid diagnosis before treatment.

Tuberculosis is a highly contagious infectious disease worldwide. Tuberculosis causes ill-health among millions of people each year and ranks as the second leading cause of death from an infectious disease worldwide. A descriptive exploratory study design was conducted among 106 middle adulthood Community people of Saptakoshi Municipality of Saptari District, between 2074/06/3 to 2074/06/1 using non-probability purposive sampling method, and data was collected using semi-structured interview schedule. More than half of the respondents (62.3%) were of the age group 40-49 years, nearly half (50.9%) were Female. Near about one fifth (19%) of the respondents had positive family history of tuberculosis. Majority of the respondents (98.1%) stated cause of tuberculosis as bacteria. Almost all the respondents were aware about the signs and symptoms of tuberculosis. Only 12.3% of the respondents were aware about DOTS Programme. A significant association (p=0.037) was revealed between level of awareness and Past history of previous exposure to Pulmonary Tuberculosis

Melasma is characterized by brown patches, typically on malar prominences and forehead. The pigmented patches are usually affect darker complexioned individual especially Asian. So it is a great problem in our country. A clinical trial was conducted to find out the effect of the combination of 20% azelaic acid cream with 0.05% tretinoin cream in the treatment of melasma. Thirty clinically diagnosed cases of melasma attending the outpatient department of Dermatology and Venereology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, were enrolled Majority of !he study subjects (43%) were between 26 to 30 years and most of them (73.3%) were female. Regarding occupation 56.7% were housewife. Out of the study subjects 70% were married and 86.7% were of middle class. Around 66.7% cases had positive family history of melasma and maximum 93.3% patient had no history of systemic drug and the maxim11m 73.3% patient had no history of use of cosmetics. It was observed that highest (93.3%) number of patients had malar area involvement and 6. 7% had centro-facial area involvement. The st11dy showed that after treatment, the average MASI (Melasma Area and Severity Index) score was decreased by 38 66% indicating moderate reduction of the severity of melasma Out of the study subjects, 50%, 30%, 16% developed burning sensation, itching and erythema respectively It could be concluded that combination of 20% azelaic acid and 0.05% tretinoin cream, has a moderate lightening effect in the treatment of melasma. Melasma is characterized by melanosis with sharply demarcated blotchy, brown macules usually in a symmetric distribution over the cheeks and forehead and sometimes on the upper lip and neck. It is most often seen in women during pregnancy, at menopause and while taking oral contraceptives; it occasionally occurs in women who are not pregnant or taking oral contraceptives, as well as men. So it is a great problem in our country. A clinical trial was conducted to find out the effect of the combination of 20% azelaic acid with 0.05% tretinoin cream in the treatment of melasma. Twenty clinically diagnosed cases of melasma attending the outpatient department of Dermatology and Venereology, Faridpur Medical College Hospital, Faridpur were enrolled. Majority of the study subjects (40%) were between 26 to 30 years of age and most of them (60%) were females. Regarding occupation, 60% were housewife. Out of the study subjects 75% were married and 60% were middle class. Around 60% cases had positive family history of melasma and maximum (90%) patient had no history of systemic drug and the maximum (80%) patients had no history of use of cosmetics. It was observed that highest (90%) number of patients had malar area involvement and 10% had centro-facial area involvement. The study showed moderate reduction of the severity of melasma after the treatment. Out of the study subjects, 40% and 20% developed burning sensation and erythema respectively. SummaryAntithrombin-III activity was determined in 752 patients with a history of venous thrombosis and/or pulmonary embolism. 54 patients (7.18%) had an antithrombin-III activity below the normal range. Among these were 13 patients (1.73%) with proven hereditary deficiency. 14 patients were judged to have probable hereditary antithrombin-III deficiency, because they had a positive family history, but antithrombin-III deficiency could not be verified in other members of the family. In the 27 remaining patients (most of them with only slight deficiency) hereditary antithrombin-III deficiency was unlikely. The prevalence of hereditary antithrombin-III deficiency was higher in patients with recurrent venous thrombosis.

Juvenile polyposis syndrome (JPS) and hereditary haemorrhagic telangiectasia (HHT) are rare autosomal dominant diseases, where symptoms manifest at childhood. A 32-year-old man with no family history of JPS or HHT with SMAD4 gene mutation who developed signs and symptoms only at the age of 32, when he was an adult. In this article, we highlight the steps taken to diagnose this rare pathology, explain its pathophysiology and management.

Interest in psychiatric neurosurgery has waxed and waned since the 1930s. This chapter reviews the history of these methods, with a focus on OCD. This review of lesion procedures and deep brain stimulation includes neuropsychological and neuroimaging research in the context of putative neurocircuitry underlying symptoms and response to treatment. The chapter highlights how an abundance of caution is needed, as well as key issues in long-term management of patients so treated.

Abstract Background Previous studies have indicated inconsistent relationships of diabetes with thyroid cancer risk, yet little is known in China. In this study, we aimed to investigate the associations between diabetes, diabetes duration and the risk of thyroid cancer in Chinese population. Methods A 1:1 matched case-control study was performed between 2015 and 2017 in Zhejiang Province including 2,937 thyroid cancer cases and 2,937 healthy controls. Odds ratios (ORs) with 95 % confidence intervals (CIs) for thyroid cancer were estimated in logistic regression models. Specific effects stratified by age, as well as sex, body mass index (BMI) and family history of diabetes were also examined. Results Overall, neither diabetes (OR = 0.75, 95 % CI: 0.21–2.73) nor diabetes duration (OR = 0.14, 95 % CI: 0.02–1.22 for diabetes duration ≦ 5 years; OR = 2.10, 95 % CI: 0.32–13.94 for diabetes duration > 5 years) was significantly associated with thyroid cancer. In stratified analyses, significant lower risk of thyroid cancer was observed among subjects with diabetes and shorter diabetes duration ( ≦ 5 years), but limited to those who were aged more than 40 years, female, overweight/obese and had positive family history of diabetes. Conclusions Diabetes and shorter diabetes duration were significantly associated with decreased risk of thyroid cancer in individuals characterized by older age, female sex, higher BMI and positive family history of diabetes.

Endometriosis is a common entity among fertile women which unfortunately manifests through variable symptomatology. Intestinal involvement in endometriosis is quite common and can simulate several diseases such as Crohn’s disease, appendicitis, tubo-ovarian abscess, or malignant tumors. Intestinal obstruction due to endometriosis is rare, and preoperative diagnosis is difficult because the signs and symptoms are nonspecific and can be easily confused. In the case of patients without a history of endometriosis, diagnosis is further complicated. We present a case of a 41-year-old female patient. She presented to the emergency room with complete bowel obstruction and a mass in the cecum. Surgery was decided, and the patient underwent full recovery. Endometriosis was the final diagnosis for the observed condition.
