Topicality. Bedsores are an urgent problem of modern medicine both at the outpatient and inpatient stage. For the treatment of infected bedsores it is advisable to use drugs that have a complex effect focused on various parts of the pathological process. For the treatment of chronic wounds and bedsores together with the inhibition of the growth of microorganisms the drug must provide the optimal conditions for the growth of granulation, have a reparative and anti-inflammatory effect. Aim. To experimentally substantiate the composition of the gel base with hyaluronic acid and decamethoxine. Materials and methods. To select the structure-forming component in the gel base, the possibility of using “Ultrez 10 NF” carbomer, “Aristoflex AVC” (copolymer of acrylamidomethylpropanesulfonic acid and vinyl pyrrolidone) from Clariant Surfactants company, “Structure XL” (hydropropylated corn starch) as a gelling agent was studied. Organoleptic, physicochemical, structural and mechanical parameters of gel model samples were determined according to the methods of the State Pharmacopoeia of Ukraine. The statistical processing of the results was performed using the Statistica 6.0 program. Results and discussion. The choice of the optimal gelling agent was performed taking into account the results of the study of organoleptic, physicochemical, structural and mechanical parameters of gel model samples. The experimental studies conducted allowed to choose the optimal gelling agent – “Aristoflex AVC” in the concentration of 1 %. It is easily dispersed with water and does not require the addition of a neutralizer. It has been proven that the base selected has the optimal consumer, structural-mechanical, and physicochemical properties. Conclusions. Based on the organoleptic, physicochemical, structural-mechanical studies the composition of the gel base with hyaluronic acid and decamethoxine has been experimentally substantiated.

Modern medicine offers a wide spectrum of wound healing resources for acute or chronic wounds. Negative pressure wound therapy (NPWT) is a very effective method, allowing complicated defects and wounds to heal. The basic set is usually provided with various special accessories to facilitate the use and support safe application of NPWT to high-risk tissue. Selected case reports are presented herein to document the special use and combinations of materials in negative pressure wound therapy.

Одним из основных требований к разработке экспериментальных моделей цереброваскулярных заболеваний является их максимальная приближенность к реальной клинической практике. В работе систематизированы данные по основным методам моделирования острой ишемии головного мозга (ОИГМ), представлена их классификация, анализируются данные о преимуществах и недостатках той или иной модели. Обсуждаются результаты экспериментальных исследований по изучению патогенеза ОИГМ с использованием различных моделей (полной и неполной глобальной, локальной и мультифокальной ишемии) и способов их реализации (перевязка артерий, клипирование, коагуляция, эмболизация и др.). Особое внимание уделяется «стабильности» последствий острого нарушения мозгового кровообращения: необратимых ишемических повреждений головного мозга или обратимых с реперфузией заданной продолжительности. Отмечается, что важное значение в этих исследованиях должно принадлежать современным методам прижизненной визуализации очагов острого ишемического повреждения, что позволяет оценивать динамику патологического процесса. Предлагаемый метод отвечает требованиям гуманного обращения с животными. Подчеркивается, что выбор релевантной модели ОИГМ определяется задачами предстоящего исследования и технологическими ресурсами научной лаборатории. Development of experimental models for acute forms of cerebrovascular diseases is essential for implementation of methods for their prevention and treatment. One of the principal requirements to such models is their maximum approximation to actual clinical practice. This review systematized major models of acute cerebral ischemia (ACI), their classification, and presented information about their advantages and shortcomings. Also, the review presented results of experimental studies on pathophysiological mechanisms of different types of modeled ACI (complete and incomplete global, local, and multifocal ischemia) and methods for creating these models (arterial ligation, clipping, coagulation, embolization, etc.). Particular attention was paid to “stability” of the consequences of acutely impaired cerebral circulation - an irreversible ischemic brain injury or a reversible injury with reperfusion of a given duration. The authors emphasized that in such studies, a special significance should be given to intravital imaging of acute ischemic damage foci using modern methods, which allow assessing the dynamics of the pathological process and meet the requirements to humane treatment of animals. The choice of a relevant ACI model is determined by objectives of the planned study and the technological resources available at the research laboratory.

Purpose. Еxperimental substantiation of the effectiveness of biocompatible biodegradable hydrogels based on hyaluronic acid and chitosan succinate as a carrier of ranibizumab in antiglaucoma operations. Material and methods. Hydrogel drainage (HD) was obtained immediately before surgery. A solution of ranibizumab (0.23 ml) was mixed with a solution of hyaluronic acid dialdehyde (0.5 ml), then a solution of chitosan succinate (0.5 ml) was added. Experimental studies were performed in 12 (12 eyes) healthy rabbits. The first group consisted of 6 eyes – 0.187 ml of ranibizumab per 1 ml of gel. In the control group, HD was used intraoperatively without the addition of ranibizumab (6 eyes). Morphological studies were performed on 7th, 21st, and 42nd days. Results. In experimental studies in vitro and in vivo, it was proved that ranibizumab, administered as a part of 0.1 ml of hydrogel drainage in the antiglaucoma surgery area is released within 3 weeks and suppresses vascularization, scarring of the operating area, and preserves the intrascleral cavity. The optimal concentration of ranibizumab was selected-0.02 ml in 0.1 ml of gel. Conclusion. The safety and effectiveness of the use of hydrogel drainage with ranibizumab based on hyaluronic acid dialdehyde and chitosan succinate in anti-glaucoma operations has been proven. The manuscript proposes analytical methods for calculating fuel economy and traction-speed properties when modeling the movement of cargo-carrying vehicles on real routes, based on theoretical and experimental studies in a hot and dry climate, which allows for determining the efficiency of cargo-carrying vehicles objectively in terms of traction and speed, fuel and economic indicators. Using the statistical processing of experimental, theoretical research data, the authors calculate the coefficient X2, which allows for evaluating the adequacy of the mathematical model and experimental data. As an example, the manuscript provides for an assessment of fuel economy and traction and speed properties. The authors presented the results in graphs for the ease of evaluating the effect of external temperature on fuel consumption and the average speed of a road train. The authors’ methodology allows for determining the efficiency of cargo-carrying vehicles in a hot and dry climate.

The article presents results of theoretical and experimental studies of the processes of iron solid-phase reduction from an iron-containing concentrate obtained as a result of hydrometallurgical dressing of ferromanganese and polymetallic manganese-containing ores with coals of grades D (long-flame) and 2B (brown). The method of thermodynamic modeling using TERRA software complex was used to study the reducing properties of hydrocarbons by calculating equilibrium compositions in the temperature range of 373 - 1873 K. The authors obtained the dependences of compositions and volume of the gas phase formed as a result of the release of volatile components during heating on the temperature for the coals of the grades under consideration. As a result of thermodynamic modeling, the optimal temperatures and consumption are determined, which ensure the complete iron reduction from an iron-containing concentrate. The results of experimental studies were obtained by modern research methods using laboratory and analytical equipment, as well as methods of statistical processing. Results of the coals analysis carried out using the Setaram LabSys Evo thermal analyzer showed that the process of thermal decomposition of coals of the studied grades proceeds according to general laws. The process of thermal decomposition of long-flame coal proceeds less intensively than of brown coal. The results of an experimental study of the processes of thermal decomposition of reducing agents have shown that volumes of the gas phases, formed when coals are heated to a temperature of 1173 K in an argon atmosphere, practically coincide with the calculated values. As a result of thermodynamic modeling and experimental study, the optimal consumption of D and 2B grades of coal is determined at a temperature of 1473 K. The best reducing agent with a minimum specific consumption is long-flame coal of D grade. When determining the optimal amount of reducing agent in charge mixtures during the study of metallization processes, it was found that with an excess of reducing agent, it is possible to achieve almost complete extraction (98 - 99 %) of iron from the concentrate.

Vibration disease is one of the leading in the structure of occupational diseases. A more detailed study of the pathogenesis, expanding the possibilities of diagnosis of vibration disease, is necessary. The aim of the study is to determine the diagnostic value of stabilometry, quantitative sensory testing and neuroenergocarting in vibration disease associated with the combined effects of local and general vibration. Materials and methods. Two groups of patients were examined: with the diagnosis of vibration disease associated with the combined effect of local and general vibration – 50 people (age 48.7 ± 3.1 years), not in contact with vibration – 30 people (age 49.1 ± 2.8 years). The study of the equilibrium function, the determination of the level of the constant potential of the brain by 12 standard leads and the determination of temperature and pain sensitivity. Statistical processing of the results was carried out using the software package “Statistica 6.0” (StatSoft Inc., USA). Differences were considered statistically significant at p < 0.05. Results. In vibration disease, the vertical stance of patients is more unstable than in patients with no contact with vibration. In patients with vibration disease, there is a decrease in the threshold of cold sensitivity and an increase in the threshold of thermal sensitivity and thermal pain compared to the control group. According to our data, the increased level of permanent potential in patients with vibration disease is observed in the central (22.7 (12.6; 30.7) mV) and right central (20.4 (11.5; 27.1) mV), also the average level of permanent capacity in this group is increased – 17.1 (8.4; 25.8) mV.Conclusion. The results obtained indicate the involvement of the neurosensory complex united by single thalamic and cortical centers in the pathological process and confirm the diagnostic value of the methods used.

Introduction. Toxic hepatitis (TH) is a complex and multifaceted disease, the development of which is mediated by a complex of biochemical and molecular genetic interactions. The current understanding of the pathogenesis of TH and, as a consequence, its treatment is based on standardization of the phenotype of the disease, often without taking into account metabolic disorders within the cells. Material and methods. experimental studies were performed on white outbred male rats weighing 200-220 g. A 50% solution of TCM was used as a toxicant. Biochemical studies were performed on a laboratory medical photometer “Stat Fax 3300” using clinical test kits and control materials manufactured by Vector-Best LLC. Liver tissue for histological examination was subjected to the standard histological procedure and paraffin embedding. Sections 5-7 μm thick were stained with hematoxylin-eosin. Gene expression analysis was performed using real-time PCR amplification on a RotorGene instrument (QIAGEN). Statistical processing of experimental data was performed using the Pearson correlation coefficient and one-way analysis of variance (ANOVA). The results were considered reliable at p <0.05. Results. As a result of the analysis of the correlation of the expression of the studied genes and the level of biochemical parameters, it was found that the correlation of the expression of the Nfe2l2 and Gstm1 genes was r = 0.812 (p = 0.0001). The dynamics of gene expression of Chek, Gstm1, Gstp1, Nfe2l2, had a negative correlation with the level of AST activity in blood serum. And the expression of the genes Chek, Gclc, Gstm1, Nfe2l2, Ripk, Sod1 with an index of ALT activity in the blood serum. After 72 hours, the expression of almost all of the studied genes became multidirectional. And the correlation between indices is often not determined. An analysis of the relationship between the level of cytolysis enzymes and the correlation level of the studied genes showed that after 72 hours the correlation was observed in the Gstm1, Hmox, and Sod1 genes with the levels of AST and ALT.

Today in modern medicine the study of human health is reoriented to the individual principle, which is based on the identification and study of constitutionally determined patterns of manifestation of certain diseases. The purpose of the study is to establish and analyze the features of the girth body sizes in men and women with generalized fatty form of seborrheic dermatitis of varying severity. The comprehensive body size of 40 men and 40 young women (25-44 years) with generalized fatty seborrheic dermatitis (mild and severe) was determined. The control group consisted of the girth sizes of practically healthy men (n=82) and women (n=154) of the same age group, which were selected from the database of the research center National Pirogov Memorial Medical University. Statistical processing of body circumference was performed in the licensed package “Statistica 6.0” using non-parametric evaluation methods. As a result of studies in patients with seborrheic dermatitis of varying severity of men, compared with practically healthy men, found only greater values of the girth of the shoulder in a relaxed state and thighs, neck (only mild), shin in the upper part and waist (in both cases only with a severe degree), as well as smaller values of the girth of the shoulder in a tense state; and in patients of varying severity of women – greater values of the girth of the shoulder in a relaxed state, thighs, lower legs, neck, waist and all girths of the chest and both thighs (only severe), as well as smaller values of the girth of the hand (only with mild). In both men and women with seborrheic dermatitis, differences in girth body sizes are more pronounced in people with severe disease. Between men or women with seborrheic dermatitis of varying severity, there are no significant or trends in differences in girth body sizes. In the analysis of the manifestations of sexual dimorphism of the circumferential body size between men and women with seborrheic dermatitis found greater values in men with mild and severe disease of the upper extremities, hands, shin, feet and neck (in most cases more pronounced in representatives with mild severity), as well as only in men with mild severity – greater values of all chest girths. For a more correct understanding of changes in girth body sizes in Ukrainian men or women with seborrheic dermatitis of varying severity, it is necessary to analyze other constitutional parameters of the body.
