Resumen: El presente artículo presenta una propuesta de intervención desde los municipios, la prevención y control del Síndrome Metabólico (SM), cuya detección utilizan criterios nacionales e internacionales; para su abordaje se adoptan los ejes esenciales de la Atención Primaria de Salud. Asimismo, se presentan avances en materia de coordinación intergubernamental, intersectorial e interinstitucional, entre los que destaca la constitución de la Red integrada, inicialmente, por Xalapa, Coatzacoalcos, Minatitlán, Jáltipan, Agua Dulce y Moloacán, municipios donde se estableció una Mesa Interinstitucional. A esta iniciativa, se han sumado instituciones gubernamentales, organizaciones no gubernamentales, representantes legislativos, sociedades de profesionistas y asociaciones civiles.Palabras clave: Municipios, Síndrome, Metabólico, Red, Atención, Primaria AbstractThis paper is a proposal for intervention from the municipalities, the prevention and control of Metabolic Syndrome (SM), the detection of which uses national and international criteria; For its approach, the essential axes of Primary Health Care are adopted. The present study “Promotion of Primary Health Care in Pakistan: A Case Study of the Role of Non-Governmental Organizations (NGOs) in Promoting Primary Health Care in Karachi” is aimed at determining the role of NGOs in the promotion of primary health Care sector in Pakistan with special reference to Karachi. The primary objective of this research is to evaluate the role and performance of relevant NGOs working in Karachi in the health sector. It is a quantitative research. A questionnaire based to obtain the opinions of respondents. The health care is the basic responsibility of the state. However, the NGOs play their role in promoting the health care facilities. It was noted that the primary health care system is still not working properly. NGO sector was given a wide room and finances but unfortunately they did not make the required contribution. Their lack of technical knowledge and accountability was a factor in their failure. The prevailing attitude towards health care among the masses has not changed despite the efforts of the NGO sector. Therefore the basic health conditions of the masses at the grass roots level continue to be poor. The stake holders in the health care sector should rethink about their strategies to improve health care facilities in Karachi.

Background: Despite hypertension being a common condition among patients attending primary health care (PHC) clinics, blood pressure (BP) control is often poor. Greater insight into patient-related factors that influence the control of hypertension will assist in the development of an intervention to address the issues identified.Aim: The aim of the study was to assess patient-related variables associated with hypertension control among patients attending a peri-urban PHC clinic.Setting: The setting for this study was a peri-urban PHC clinic in KwaZulu-Natal.Method: This was an observational, descriptive and cross-sectional study with 348 patients selected over a 1-month period. A validated questionnaire was used to collect data on patients’ hypertension knowledge and self-reported adherence, and BP recordings from their medical record were recorded to ascertain control.Results: Of the 348 participants, only 49% had good BP control and 44% (152/348) had concurrent diabetes mellitus. The majority of patients had moderate levels of knowledge on hypertension and exhibited moderate adherence. There was a significant relationship between knowledge and reported adherence, between reported adherence and control, but not between reported knowledge and control.Conclusion: Despite over 90% of the study population having moderate knowledge, and 62% with moderate reported adherence, BP was well controlled in only less than 50% of the study population. These findings suggest a need to emphasise adherence and explore new ways of approaching adherence.

Abstract Background In fulfilling the COVID-19 containment, primary health care (PHC) facilities in China played an important role. We sought to investigate the exact tasks performed at the PHC facilities and the processes of COVID-19 prevention and control.Methods Semi-structured face-to-face interviews for primary care physicians (PCPs) and a simple survey for residents were conducted in the field survey. Based-on purposive stratified sampling, 32 PCPs were selected from 22 PHC facilities in Wuhan as a high-risk city, in Shanghai as medium-risk city and in Zunyi as low-risk city. In the field survey, semi-structured face-to-face interviews were conducted with PCPs to summarize the tasks of COVID-19 prevention and control at the PHC facilities. A simple survey was used to investigate the local residents’ awareness about COVID-19 prevention and control.Results In pre-outbreak period, the PHC facilities mainly engaged in storing medical supplies; in out-break period, they were responsible for screening, transferring, quarantine and treatment; in regular prevention and control period, attention was given to the employees and items of cold-chain & fresh food markets, etc. In Wuhan, PHC facilities focused on graded diagnoses and treatments of patients; in Shanghai, they were mainly engaged in at-home/centralized quarantine; in Zunyi, they focused on the screening of high-risk individuals. In urban areas, COVID-19 were more likely to be transmitted; in urban-rural areas, it was difficult to perform screening on the migrant populations; in rural areas, the risk was much lower. The community residents had satisfactory compliance with the preventive measures.Conclusion We identified differences in the prevention and control tasks performed at the PHC facilities in China. During the different phases of the pandemic, the tasks were adjusted depending on the gradually comprehensive understanding of COVID-19. Among the cities at different risk levels, screening, quarantine, transferring or treatment was chosen to be a priority accordingly. Located in different intra-city geographic locations at different risk levels, the PHC facilities conducted their own tasks accordingly. Additionally, compliance on the part of the local community residents could not be overemphasized in COVID-19 prevention and control.

Objetivo: compreender, dentro das melhores práticas, as experiências de busca por conhecimento e utilização da experiência profissional dos enfermeiros no cuidado da pessoa idosa na Atenção Primária à Saúde. Método: trata-se de um estudo qualitativo, descritivo e exploratório, com 30 enfermeiros atuantes nas Estratégias Saúde da Família. Utilizou-se um instrumento de entrevista semiestruturado. Analisaram-se os dados pela técnica de Análise de Conteúdo na modalidade Análise Temática. Resultados: revelaram-se a busca por conhecimento com outros profissionais e o acesso à rede de internet como fontes de conhecimento. Evidenciou-se a necessidade do estabelecimento de uma rotina de estudos sistematizada e agenda de educação permanente sob a temática do envelhecimento. Conclusão: conclui-se que o estudo confirma que os enfermeiros realizam ações que carecem de notoriedade e que estão preocupados com o impacto de suas ações para o usuário idoso, mas, para que as melhores práticas sejam completamente estabelecidas,  se faz necessária a imersão científica e metodológica dos profissionais e da gestão, além da busca por conhecimento e valorização da experiência, que já coexistem. Method: this is a qualitative, descriptive and exploratory study, with 30 nurses working in the Family Health Strategies. A semi-structured interview instrument was used. Data was analyzed using the Content Analysis technique in the Thematic Analysis modality. Results: the search for knowledge with other professionals and access to the internet network as sources of knowledge were revealed. The need to establish a systematic study routine and a permanent education agenda under the theme of aging became evident. Conclusion: it is concluded that the study confirms that nurses perform actions that lack notoriety and that they are concerned with the impact of their actions for the elderly user, but, for the best practices to be completely established, scientific immersion is necessary and methodological approach of professionals and management, in addition to the search for knowledge and appreciation of experience, which already coexist. Conclusión: se concluye que el estudio confirma que los enfermeros realizan acciones que carecen de notoriedad y que están preocupados por el impacto de sus acciones para el usuario mayor, pero, para que las mejores prácticas se establezcan por completo, es necesaria la inmersión científica y enfoque metodológico de profesionales y directivos, además de la búsqueda de conocimiento y valoración de la experiencia, que ya coexisten. The AIDS Commission (KPA) is an institution formed in 2013 that has the function of leading, managing and coordinating efforts to prevent and control the Human Immunodeficiency Virus (HIV)/Acquired Immunodeficiency Deficiency Syndrome (AIDS) which is chaired directly by the Mayor of Pekanbaru. This paper aims to understand, examine in-depth and explain how the Mayor's leadership is in the prevention and control of HIV/AIDS in Pekanbaru, including Regional Apparatus Organizations, Health Organizations and Non-Governmental Organizations. This study uses a qualitative method with informant withdrawal techniques through Stratified Sampling, converted to the Snow Ball technique. This study showed that the mayor leadership in preventing and overcoming AIDS was autocratic due to the policies were made in the form of mayor regulations. The decisions were made only from the Mayor, causing weaknesses including unclear policy objectives, budgeting, and ambiguity regarding oversight by the legislative institution (DPRD) of Pekanbaru. It has an impact on communication, resources, disposition and bureaucratic structures.
