Features of the management of girls with anomalies of the genitourinary system

Aim. To study the clinical and anamnestic, and surgical features of patients with genitourinary system malformations. Materials and methods. A retrospective analysis of data from 252 medical records of patients with genitourinary system malformations who were admitted for examination and treatment to the Department of Pediatric and Adolescent Gynecology of the Kulakov National Medical Research Center for Obstetrics, Gynecology and Perinatology of the Ministry of Health of the Russian Federation from 2012 to 2019. During analysis, special attention was paid to the data of surgical treatment, clinical and anamnestic data and the intraoperative picture. Results. Developmental anomalies of the genital system are combined with urinary system malformations in 21.1% of cases. Genital malformations in adolescent girls are characterized by a high rate of various complications: hematocolpos (53.6), hematocervix (30%), hematometra (26.2%), hematosalpinx (15.1%), endometriosis (19.2%), masses (16.4%) and adhesions (25.4%). Conclusion. Genital system malformations are often associated with anomalies of the urinary system and are characterized by a high risk of complications, therefore, timely identification and multidisciplinary management of such patients from an early age is extremely important.

In recent years the Russian Federation among the causes of maternal mortality extragenital diseases occupy the first place (37.7 % in 2012). There is an increasing group of children with chronic diseases. Objective: to make an analysis of the gynecological pathology in girls with decompensated chronic extragenital pathology. Methods. A total of 1682 patients aged under 18 years who are treated in the Saint Petersburg State Pediatric Medical University’s multidisciplinary clinic and directed clinicians to consult a gynecologist. We used anamnestic, clinical and laboratory methods of investigation and analysis of medical records. Results. As a result of analysis, we found that the structure of gynecological pathology identified in patients with decompensated extragenital diseases, 44 % are menstrual disorders, 37.9 % - an inflammatory disease, 14.2 % - non-inflammatory problems, 12.7 % - other. In this case, gynecological pathology, of course, is associated with somatic. So, the girls suffering from connective tissue, often recorded long and heavy menstruation (38.9 %), diseases of the gastrointestinal tract and urinary system - painful (45.9 and 55 %, respectively), endocrine pathology often accompanied by rare, scanty menses (56.8 %). Conclusions. Given the importance of extragenital pathology in the implementation of fertility, it is important to pay particular attention to: a) development of an algorithm for monitoring, treatment, rehabilitation of girls suffering from extragenital pathology, which should include a compulsory inspection gynecologist; b) the participation of gynecologist in the regular medical check observation girls with chronic extragenital pathology in the case of existing combinations with gynecological diseases. Such an approach should be further consolidated when transferring patients under the supervision of an adult network.

Background. Adnexal torsion (AT) takes fifth place among all emergency gynecological conditions. Suspicion of AT requires immediate diagnosis and urgent surgical treatment. The most common causes of AT are various volumetric formations, such as functional or dermoid ovarian cysts, contributing to an increase in its volume and/or anomalies in the development of the ligamentous apparatus. Timely diagnosis and detorsion contributes to the full restoration of impaired venous outflow and lymphatic drainage of the ovarian tissue, preventing the development of severe ischemia and necrosis. Over the past few decades, a surgical organ-preserving approach in managing patients with AT has been the gold standard of care. Materials and methods. The article describes the results of a retrospective study of cases of AT in children and adolescents treated at the Department of Pediatric and adolescent gynecology Kulakov National Medical Research Center for Obstetrics, Gynecology and Perinatology with an assessment of the clinical and anamnestic features of this cohort of patients and the choice of therapeutic tactics. Conclusion. A multidisciplinary approach is critical to optimizing the delivery of care in cases of AT, including minimally invasive detorsion and preserving the functionality of the ovary as a treatment standard that should be used in the management of children and adolescents.

Child marriages and early age pregnancy are an alarming issue among adolescent girls and young females in South Asian region. This research was carried out to understand the knowledge, attitude, and practices towards child marriages and early age pregnancies. For this purpose an exploratory research design was employed and data was gathered through using qualitative and quantitative research techniques. The research found that the respondents were lacking authentic knowledge about sexual and reproductive health rights. Most of the respondents were having access to public and private health service providers and this channel can be a source for the quality information about sexual and reproduce health and rights. An integrated approach through combing all the channels and stakeholders of the society can be vital for the desired social change to counter child marriages and early age pregnancies.

In the structure of diseases of the urinary system in children, microbial-inflammatory lesions of the kidneys and urinary tract are 70–80 %. The prevalence of infections of urinary system in children in the Russian Federation is on average 18–22 per 1000 children’s population. The aim of the study was to analyze the spectrum of infectious agents of urinary infection in children and to determine the dynamics of bacterial resistance. We analyzed 330 cases of urinary system infection in children aged from 2 months up to 18 years in two time peri- ods – 2002–2004 and 2014–2016 (n = 114 and n = 216, respectively). Patients of both periods were divided into three groups: the first one – cystitis, the second one – pyelonephritis and the third group – a combination of pyelonephritis and cystitis. An obligatory criterion for including the patient in the study was the presence of a pathogmonic combina- tion of leukocyturia and bacteriuria, 10 5 KOE/ml, for the infection of the urinary system. The main causative agent of urinary infection in children remains E. coli. An increase is marked in the specific gravity of E. coli, Enterococcus spp. and Klebsiella spp., Pr. vulgaris. The study revealed the growth of E. coli resistance to cephalosporins of II, III, IV generations; decreased resistance of Enterococcus spp. to ampicillin and gentamicin. In the study of the sensitivity of Klebsiella spp. an increase was detected in resistance to cephalosporins of the III generation. We identified the main pathogens of infections of the urinary system and determined bacterial resistance.

Results of experimental and analytical researches on restoration of stocks of crayfish of subfamily Astacinae Latreille, 1802 in water body of the Forest zone of the European part of Russia by introduction of larvae of the early stages received in artificial conditions are presented. Researches were conducted in 1995–2020 under programs of Rosrybhoz, Tver’rybhoz, the Russian Academy of agricultural sciences and the Ministry of the Science and Higher education of the Russian Federation. To restore the stocks of crayfish in the Tver Region, larvae of early age stages obtained under artificial conditions were placed in frame devices installed along the shore of the water body, covered with a fine-mesh netting with a small slit left near their bottom. The differences in the environment in the growing pond (fish farm pond) and in the larval introduction pond (Puiga River) were minimal. In devices filled with aquatic vegetation enriched with food organisms, the larvae had the opportunity to adapt to the new environment, find food organisms, and gradually get out through the slit into the water body. A survey of the results of this method of introducing of stocking material, carried out after 1.5–2 months, made it possible to establish the absence of larvae in the devices, and to find in the river backwater downstream a lot of larvae that grew up to the 6th age stage with a body length of 2 and more centimeters. The number of larvae in the backwater was established by calculation based on the results of an approximate calculation of their number per 165 copies/m² of the river bottom inhabited by crustaceans, the settlement area was 15.8 m². The number of found grown-up larvae allowed us to estimate approximately the survival rate of the universal planting material of cancer at least 36%.

The aim of the present study was to identify patients who were at particular risk of violent behaviour, over a period of one month in a psychiatric high dependency unit, and to examine whether particular patients could be identified at admission who would subsequently be violent. Medical records, and other documents for 52 inpatients were included. Demographic, historical and clinical data available at admission were noted. All violent incidents were recorded and classified. Seventeen of 52 patients committed physical violence, usually to the staff. Most incidents were carried out by a small number of individuals. Identifying the violent, using variables available at admission, was not possible. The ward admitted a group with a subsequent high rate of violence. Prediction within this high-risk group was not possible. Measures aimed at preventing violence on a local secure ward should apply to all patients and should not be targeted just at those that would appear to be the most likely to be violent.

The purpose of the study was to create a certification of the service of pediatric hematology-oncology of the constituent entities of the Russian Federation (RF) using infographics and digitalization. Materials and methods. For the first time, infographic maps were used as a tool for certification, as a way of visualizing a large amount of data on the organization of pediatric hematology-oncology service in each subject of the RF. They allowed to show the specifics of the medical and organizational structure of the profile service in the subjects of the RF, their interaction with other subjects and federal centers. Processing of the cartographic image of the subject of the RF was carried out in the Adobe Illustrator graphic editor. A modifiable tool for creating infographic maps created in the MS Power Point program, with saving the results in PDF and PNG formats. Results. In the course of the study, a “Passport of Service” was prepared, containing infographic maps of 77 constituent entities of the RF. The maps in a graphic form provide information on resources available in the subject of the RF: a medical organization of the 3rd level with bed structure and the possibility of visualization, its personnel; the number of multicenter protocols and clinical guidelines used in the subject of the RF; logistics of patients, taking into account the territorial distance from the specialized beds, the presence in the region of the station/department of blood transfusion; the possibility of radiotherapy in the region; the presence of institutions where they provide palliative and rehabilitation assistance; the name of the federal and regional specialized centers, with whom the interaction is carried out, the well-established telemedicine consultation system with the Dmitry Rogachev National Medical Research Center of Pediatric Hematology, Oncology and Immunology. The database of contacts of the main freelance children’s specialists of hematologists and oncologists of the RF was also updated. Conclusion.Thus, data were collected for visual display and to create specific conditions for developing optimal patient routing schemes and forming proposals for optimizing the service. The obtained “Passport of the Children’s Hematology-Oncology Service of the Russian Federation” can be used in the work of hospitals, scientific centers and professional communities.

This work describes the specific features of the influenza virus circulating in the period from October 2015 to March 2016 in 10 cities of Russia, the basic laboratories of CEEI at the D.I. Ivanovsky Institute of Virology “Federal Research Centre of Epidemilogy and Microbiology named after the honorary academician N.F. Gamaleya” of the Ministry of Health of the Russian Federation. The increase in the morbidity caused by influenza viruses was detected in January-February 2016. The duration of the morbidity peak was 4-5 weeks. The most vulnerable group included children at the age from 3 to 6; a high rate of hospitalization was also detected among people at the age of 15-64 (65%). In clinic symptoms there were middle and severe forms with high frequency of hospitalization as compared with the season of 2009-2010, but much higher in comparison with the season of 2014-2015. Some of the hospitalized patients had virus pneumonias, half of which were bilateral. Among these patients, 10% were children; 30%, adults. The mortality in the intensive care unit of the hospital was 46%. Almost all lethal cases were among unvaccinated patients in the case of late hospitalization and without early antiviral therapy. The predominance of the influenza A(H1N1)09pdm virus both in the Russian Federation and the major part of the countries in the Northern hemisphere was noted. The results of the study of the antigenic properties of influenza strains of A(H1N1)pdm09 virus did not reveal any differences with respect to the vaccine virus. The sequencing data showed the amino acid substitutions in hemagglutinin (receptor binding and Sa sites) and in genes encoding internal proteins (PA, NP, M1, NS1). Strains were sensitive to oseltamivir and zanamivir and maintained resistance to rimantadine. The participation of non-influenza ARI viruses was comparable to that in preliminary epidemic seasons.

This article focuses on the nature and role of small business in the sphere of consumer market and services as an important condition of economic growth. The article presents the analysis of the state of small business in the federal subjects of Russia - the Republic of Khakassia, and summarizes its development, the structure of small business by the number of enterprises in different activities. The authors estimate the small business in the sphere of consumer market and services and present the peculiarities and problems of development of entrepreneurial structures in the sphere of consumer market and services at the regional level (on the example of Republic of Khakassia). The consumer market remains one of the most important factors of economic growth in the Republic of Khakassia. The authors determine the factors contributing to the development of small business on regional level and their impact on economic growth. They note that just small businesses do not require large upfront investment and guaranteeing a high rate of turnover of resources and they can most quickly solve the problems of the formation and saturation of market with goods and services. Small businesses can quickly respond to changing consumer demand and thus they can provide the necessary balance in the consumer market. This study is limited to the territory of the Republic of Khakassia, however, the information can be applied and used in other regions of the Russian Federation, as well as in the preparation of textbooks and articles.

Background: Pain in the rectal, genital, and perineal area is a common condition treated by pain physicians. These chronic pain syndromes are therapeutically challenging because both interventional and drug therapies often are ineffective. Objectives: To determine if pelvic pain can be treated effectively with compounded topical amitriptyline-ketamine. Study Design: A retrospective review of medical records. Setting: A single academic medical center in the United States. Methods: We identified all patients treated with topical amitriptyline-ketamine from January 1, 2004, through November 28, 2011. Medical records were evaluated to determine the diagnosis for which the medication was prescribed. Treatment efficacy and any adverse effects were recorded. Results: Of the 1,068 patients who received amitriptyline-ketamine, 13 had the medication prescribed for genital, rectal, or perineal pain and had medication efficacy recorded. These patients were treated with a topical combination of amitriptyline 1-2% and ketamine 0.5%. Of these 13 patients, one (8%) had complete relief, 6 (46%) had substantial relief, 4 (31%) had some relief, and 2 (15%) had no response. One patient reported occasional irritation while using topical amitriptyline-ketamine with lidocaine; no other patients reported local or systemic adverse effects. Limitations: Retrospective review; lack of uniform system for pain grading; concurrent use of other medications. Conclusions: Topical amitriptyline-ketamine provided a high rate of pain relief with a low adverse-effect burden in patients with pelvic pain. This topical medication could offer an effective, noninvasive, nonopioid therapy for pain in the rectum, perineum, and genitals.
