Oral diseases play a serious challenge to public health around the world. When left untreated, both the mouth and also the remaining the body would be adversely affected by oral diseases. Different treatment modalities are available for various dental diseases, but the most downside of those modern drug treatments are the frequent side effects related to their use. This has contributed to renewing interest in the development of novel plant-derived anti-infective natural compounds. Curcumin is an anti-inflammatory agent that occurs naturally, with different biological and medicinal properties. It has proven anti-inflammatory, antioxidant, antimicrobial, hepato-protective, immuno-stimulant, antiseptic, and anti-mutagenic properties. Due to these properties, it is especially useful in dentistry for the treatment of periodontal diseases and oral cancers. Turmeric can also be utilized in alternative treatments as a pit and fissure sealant, mouthwash and subgingival irrigant. Its gel form can even be used as a local drug delivery system. It is evaluated with a view to mitigate the human diseases, particularly in cancer and its potential to reduce cancer risk. Curcumin has only negligible side effects such as diarrhoea, diarrhoea, allergic skin reaction, gastric pain. The objective of this article is to review the efficacy and therapeutic properties of curcumin in maintaining oral health.

AbstractPeriodontitis is an immuno-inflammatory disease of the tissues surrounding the teeth. Various treatment modalities like mechanical debridement and use of antimicrobials have been followed in the treatment of such conditions. Introduction of local drug delivery system in the periodontal pocket is a promising therapeutic modality for achieving better clinical outcomes when used as an adjunct to conventional non surgical periodontal therapy. Intensive research efforts are now focussed on the development of new strategies for more effective treatment.

To study the anti-inflammatory effect of Punica granatum extract against the oral microbes. Oral diseases continue to be a major health problem worldwide. Dental caries and periodontal diseases are among the most important global oral health problems, although conditions such as oral and pharyngeal cancers and oral tissue lesions are also significant health concerns. Pomegranate extracts have been used for centuries in traditional medicine to confer health benefits in a number of inflammatory diseases, microbial infections and cancer. The anti-inflammatory activity of pomegranate extract was evaluated by protein denaturation assay, and the results were read spectrophotometrically. Denaturation of proteins is a great‐ documented cause of inflammation. As a part of the investigation on the mechanism of the anti-inflammatory activity, the ability to extract to inhibit protein denaturation was studied. It was effective in inhibiting heat induced albumin denaturation at different concentrations as shown in Table 1. Maximum inhibition, 70.12±1.12% was observed at500µg/ml. IC50 value was found to be 105.35±1.99µg/ml. Aspirin, a standard anti-inflammatory drug showed the maximum inhibition, 77.12±1.42% at the concentration of 200µg/ml. Hence it can be concluded that pomegranate extract has anti-inflammatory property and also can be used in products such as toothpaste and mouth wash etc.

In this article the review of foreign and domestic literary sources, which are devoted to the actual problem of modern dentistry – the treatment of inflammatory diseases of periodontal tissues: gingivitis and periodontitis are presented. The complex approach to their treatment involves the appointment of a significant amount of pharmacotherapeutic drugs. Therapeutic failures and iatrogenic complications have led to the fact that today the interests of doctors and population to medicinal products significantly increased. The purpose of the study is to analyze the data of scientific literature on the use of plant-based medicinal products for the treatment of periodontal inflammatory diseases over the past 10 years. Materials and methods. Comprehensive and systematic analysis of literature. Review and discussion. The analysis of information sources on the use of plant-based medicinal products in dentistry both independently and in the composition of medical and prophylactic means has established that the modern assortment of plant-based preparations in the pharmaceutical market of Ukraine to a certain extent is limited. The emergence of new plant-based species that have been tested in conditions of experimental pathology and require an evidence-based clinical base is noted. The composition of plant-based preparations used for the treatment of inflammatory periodontal diseases include vitamins, biologically active substances, glycosides, alkaloids, in connection with a wide range of action: antiseptic, anti-inflammatory, regenerating, hemostatic, antioxidative. The data on plant-based preparations that are most often used such as chamomile extracts, calendula, hypericum, plantain, kalanchoe, aloe, eucalyptus, milfoil, nettle, calamus and plant-based species are summarized. The medicinal agents considered are mainly recommended for local treatment of periodontal diseases in the form of dental care means, mouth rinse, gel, chewing gum, herbal liquer. It is known that the complex treatment of periodontal diseases includes a general influence on the body. The properties of green tea with its wide range of actions are investigated. With antioxidant properties, it can be a healthy alternative for controlling destructive changes in periodontal diseases. Attention is drawn to the proposed unique natural complex “Resverazin” due to a wide range of pharmacological action, low toxicity and relative safety. The drug produces antioxidant, anti-inflammatory, immune stimulating, vasodilative, neuroprotective action. Conclusion. Based on the literature analysis, it can be concluded that the accumulated experimental and clinical data on the therapeutic properties of plants prove perspective of their use in the complex treatment of inflammatory periodontal diseases. Future studies are mandatory for further confirmation of the effectiveness of these medicinal plants

Inflammatory processes are increasingly being identified at the core of many different disease states (e.g. heart disease, cancer, diabetes). As such, anti-inflammatory strategies available through drug delivery have undergone renewed interest. Due to the systemic side effects of steroidal drugs, non-steroidal anti-inflammatory drugs are often preferred for long-term treatment of inflammation in a variety of applications. While non-steroidal anti-inflammatory drugs are generally safe, there are some serious side effects that can be associated with their usage, particularly when given systemically or orally. Due to the high number of patients taking non-steroidal anti-inflammatory drugs, the reduction or elimination of these side effects, such as is possible through local drug delivery, could have a very powerful effect on patient quality of life. This review comments on a sampling of existing methods for localized or targeted delivery of non-steroidal anti-inflammatory drugs, with the goal of helping future research groups to focus on bettering methods shown to be effective and filling the gaps of knowledge in this field. Additionally, commentary is made on the field as a whole, and the standardization issues that arise from its expansiveness and diversity. Impact statement This work provides an overview of research currently being done exploring potential drug delivery device strategies for NSAIDs as an alternative to systemic delivery. Commentary on this field is made in an attempt to aid future experimental design, enabling researchers to determine the drugs and delivery vehicles which are most advantageous for them to pursue, as well as suggestions to standardize the reporting of such future research.

Currently, regardless of the age and social affiliation of the population, periodontal disease is the most common. Despite the modern technologies used in dentistry, the issues of prevention and treatment in dentistry remain unresolved. Dentists are interested in the fact that periodontal diseases are inextricably linked with systemic disorders in the body, which are responsible for its physiological functions. And the effectiveness of treatment for periodontal diseases is affected by all the factors that lead to the pathological process, i.e. diseases of internal organs and periodontal diseases are inextricably linked. Antioxidants are compounds that destroy the free radicals in the body, thereby preventing harmful oxidation-reduction reactions. Free radicals are chemical species possessing an unpaired electron that can be considered fragments of molecules and generally very reactive. They are produced continuously in cells either as accidental by-products of metabolism or deliberately during, phagocytosis. However, excess free radical production originating from endogenous or exogenous sources might play a role in many diseases. Antioxidants are man-made or natural substances that may prevent or delay some types of cell damage. Antioxidants are crucial for maintaining optimum health and well-being. Free radicals have been implicated in numerous oral diseases like leukoplakia, Periodontal diseases, and oral cancer.

Tobacco and its various forms cause major oral health problems. Tobacco either in smoked or smokeless forms is prevalent in Nepal and counts as a risk factor for the causation of various red and white lesions, premalignant lesions, oral cancers, gingival and periodontal diseases. Tobacco in conjunction with other risk factors adds a potential threat to oral diseases and its timely control is a cure to those threats. This article focuses on tobacco and its forms affecting oral health and also focuses on its prevention and control from the ground to the National level.

Nonsteroidal anti-inflammatory drugs (NSAIDs) are some of the most commonly used drugs to relieve a multitude of pain symptoms.F They are readily available and used extensively. There is a lot of concern about their adverse side effects namely cardiovascular (CV) and gastrointestinal (GI) side effects. It is important to have a good grasp of the pharmacology of these drugs in order to use them safely and effectively. NSAIDs work by inhibiting the cyclooxygenase (COX) enzyme system responsible for production of prostaglandins. Prostaglandins mediate pain inflammation and temperature regulation in the body. NSAIDS can be divided into selective and non-selective types. Three isoforms of COX have been identified COX-1, COX-2 and COX-3. Selective NSAIDs act on these isoforms. COX-1 is anti-inflammatory, COX-2 pro-inflammatory and COX-3, a variant of COX-1, does not produce prostaglandins. The CV side effects of these drugs can be wide ranging and include a rise in blood pressure (BP) and a higher risk of thromboembolic events. Patients also suffer from peptic ulcer disease or bleeding in the stomach as a result of their use. NSAIDs can cause liver and kidney toxicity and should be used with caution in patients with bleeding tendencies. New NSAIDs on the market include; lornoxicam (xefo®), meloxicam (coxflam®), celecoxib (celebrex®), parecoxib (rayzon®) and etoricoxib (arcoxia®). New ways of delivering NSAIDs to the body with minimal or no side effects are being researched. Novel technology in this field includes nano formulated NSAIDs; indomethacin (tivorbex®) and dicofenac (zorvolex), prodrugs and multi action drugs; cyclooxygenase inhibiting nitric oxide donors and hydrogen sulphide releasing drugs. Further exciting innovations are in the pipeline that could change the face of how we use these drugs. Until then they must be used with careful consideration and only if the benefits of use outweigh the risks.

Introduction: Inflammation is one of the body›s natural defence mechanisms alongside the cells or tissues damage due to various stimuli or infections. Pain is one of the greatest common symbols associated with different diseases. Therefore, researchers are trying to find new medications that bring patients relief with fewer side effects. Herbal medicine has been used for periods to treat acute and chronic pains. Objectives: This research evaluated the anti-inflammatory, antinociceptive and antioxidant properties of Astragalus fasciculifolius manna extract. Materials and Methods: The anti-inflammatory and antioxidant potency of the extract were determined by xylene test and ferric reducing antioxidant power assay. Results: Astragalus fasciculifolius manna extract at doses of 400, 800 and 1200 mg/kg significantly reduced acetic acid-induced writhing and formalin-induced paw licking (late and early phase) but did not affect mechanical hyperalgesia caused by heat. Moreover, naloxone failed to inhibit the antinociceptive activity of the extract. The extract of A. fasciculifolius manna also exhibited high anti-inflammatory activity against xylene induced ear edema. Treatment of mice with the extract did not improve serum antioxidant capacity. Conclusion: Results of the present research showed that A. fasciculifolius manna can prevent pain and inflammation because of the presence of high amount of saponin.
