Latar Belakang: Rinitis alergi adalah suatu penyakit pada hidung yang ditimbulkan oleh reaksi inflamasi mpada mukosa hidung dengan perantara immunoglobulin E. Prevalensi rinitis alergi di dunia telah meningkat termasuk di Indonesia yang kini telah mencapai 1,5-12.4% dan cenderung mengalami peningkatan setiap tahunnya. Penelitian ini bertujuan untuk mencari prevalensi rinitis alergi dan hubungan antara jenis kelamin dan riwayat asma dengan kejadian rinitis alergi pada siswa sekolah di Palembang. Metode: Penelitian ini adalah penelitian analitik observasional dengan desain cross sectional. Penelitian dilakukan di SMP Muhammadiyah 3 Palembang pada tahun 2018. Data primer diperoleh dengan cara meminta subjek penelitian untuk mengisi kuesioner ISAAC. Kriteria inklusi penelitian ini adalah siswa-siswi berusia 13-14 tahun. Besar sampel yang digunakan sebanyak 80 responden, diambil teknik total sampling.Hasil: Pada penelitian ini didapatkan prevalensi rinitis alergi sebesar 51,2% dan jenis kelamin (p=0,014) dan riwayat asma (p=0,019) sebagai faktor risiko terjadinya rinitis alergi.  Perempuan lebih banyak menderita rinitis alergi dibanding laki-laki kemungkinan disebabkan perempuan lebih sering terpapar allergen berupa debu akibat sering melakukan pekerjaan rumah tangga seperti menyapu. Adanya paparan alergen terhadap mukosa hidung akan meningkatkan konsentrasi berbagai faktor yang terkait asma seperti eosinophil, interleukin-5 dan sel CD34 di darah perifer. Kesimpulan: Jenis kelamin dan riwayat asma merupakan faktor risiko terjadinya rinitis alergi pada anak.

ABSTRACT Background: Helminth infections remain endemic disease, mainly in tropical developing countries. Immunity against a number of helminth infections has been associated with anti-parasitic immunoglobulin-E (IgE). This study aimed to determine the correlation between total IgE level and intensity of whipworm infection in primary school children, Medan, North Sumatera. Subjects and Method: This was a cross-sectional study conducted at primary school, Tanjung Pura, Langkat, Medan, North Sumatera in Augustus 2019. A total of 19 students was selected for this study. The dependent variable was intensity of whipworm egg. The study subjects were selected by purposive sampling method. The independent variable was total IgE level. The data for dependent variable was measured by Kato-Katz technique. The total IgE serum concentration was measured by ELISA. Data were analyzed by Spearman correlation test. Results: There was a negative correlation between total IgE level and intensity of whipworm infection among primary school children in Medan (r= -0.64; p= 0.003). Conclusion: There is a negative correlation between total IgE level and intensity of whipworm infection among primary school children in Medan. ABSTRACT Over the past four decades, there has been a significant increase in allergy and asthma in westernized countries, which correlates with alterations in fecal microbiota (microflora) and widespread use of antibiotics (the “hygiene hypothesis”). Antibiotics also lead to overgrowth of the yeast Candida albicans, which can secrete potent prostaglandin-like immune response modulators. We have developed a mouse model of antibiotic-induced microbiota disruption that includes stable increases in gastrointestinal (GI) enteric bacteria and GI Candida levels with no introduction of microbes into the lungs. Mice are treated for 5 days with cefoperazone in the drinking water, followed by a single oral gavage of C. albicans. This results in alterations of GI bacterial populations and increased yeast numbers in the GI microbiota for at least 2 to 3 weeks and can drive the development of a CD4 T-cell-mediated allergic airway response to subsequent mold spore (Aspergillus fumigatus) exposure in immunocompetent mice without previous systemic antigen priming. The allergic response in the lungs is characterized by increased levels of eosinophils, mast cells, interleukin-5 (IL-5), IL-13, gamma interferon, immunoglobulin E, and mucus-secreting cells. In the absence of antibiotics, mice exposed to Aspergillus spores do not develop an allergic response in the airways. This study provides the first experimental evidence to support a role for antibiotics and fungal microbiota in promoting the development of allergic airway disease. In addition, these studies also highlight the concept that events in distal mucosal sites such as the GI tract can play an important role in regulating immune responses in the lungs.

AimThe biological mechanisms of work-related asthma induced by irritants remain unclear. We investigated the associations between occupational exposure to irritants and respiratory endotypes previously identified among never asthmatics (NA) and current asthmatics (CA) integrating clinical characteristics and biomarkers related to oxidative stress and inflammation.MethodsWe used cross-sectional data from 999 adults (mean 45 years old, 46% men) from the case-control and familial Epidemiological study on the Genetics and Environments of Asthma (EGEA) study. Five respiratory endotypes have been identified using a cluster-based approach: NA1 (n=463) asymptomatic, NA2 (n=169) with respiratory symptoms, CA1 (n=50) with active treated adult-onset asthma, poor lung function, high blood neutrophil counts and high fluorescent oxidation products level, CA2 (n=203) with mild middle-age asthma, rhinitis and low immunoglobulin E level, and CA3 (n=114) with inactive/mild untreated allergic childhood-onset asthma. Occupational exposure to irritants during the current or last held job was assessed by the updated occupational asthma-specific job-exposure matrix (levels of exposure: no/medium/high). Associations between irritants and each respiratory endotype (NA1 asymptomatic as reference) were studied using logistic regressions adjusted for age, sex and smoking status.ResultsPrevalence of high occupational exposure to irritants was 7% in NA1, 6% in NA2, 16% in CA1, 7% in CA2 and 10% in CA3. High exposure to irritants was associated with CA1 (adjusted OR aOR, (95% CI) 2.7 (1.0 to 7.3)). Exposure to irritants was not significantly associated with other endotypes (aOR range: 0.8 to 1.5).ConclusionOccupational exposure to irritants was associated with a distinct respiratory endotype suggesting oxidative stress and neutrophilic inflammation as potential associated biological mechanisms.

ABSTRACT The purpose of the present study was to determine the immunologic responses, particularly immunopathologic reactions, associated with nasal immunization with the mucosal adjuvant, cholera toxin (CT). BALB/c mice were nasally immunized with tetanus toxoid (TT) combined with CT, and the responses of these mice were determined. After nasal immunization, mice produce a serum antibody response, primarily of the immunoglobulin G (IgG) isotype of predominantly IgG1 subclass, against both TT and CT. Along with the antibody responses, we also found that inflammatory reactions, which could be potentially fatal, developed within the lung. Furthermore, IgE responses were also induced after nasal immunization, and these responses were associated with the detection of interleukin 5 in the serum. Thus, nasal immunization with TT plus CT likely results in the activation of Th2 cells, which may contribute to serious immunopathologic reactions in the lung.

Allergic asthmatic responses in the airway are associated with airway hyperreactivity, eosinophil accumulation in the lung, and cytokine production by allergen-specific, T helper cell type 2 (Th2) lymphocytes. Here, we show that in a cockroach antigen (CA) model of allergic pulmonary inflammation, the chemokine macrophage inflammatory protein (MIP)-3α is expressed in the lung within hours of allergen challenge. To determine the biologic relevance of this expression, mice lacking CCR6, the only known receptor for MIP-3α, were studied for their response to CA. CCR6-deficient mice were immunized to the same extent as their wild-type counterparts, as judged by cytokine production in antigen-challenged lymphocytes. However, compared with CA-challenged wild-type mice, challenged CCR6-deficient mice had reduced airway resistance, fewer eosinophils around the airway, lower levels of interleukin 5 in the lung, and reduced serum levels of immunoglobulin E. Together, these data demonstrate that MIP-3α and CCR6 function in allergic pulmonary responses and suggest that these molecules might represent novel therapeutic targets for treatment of asthma.

Chronic rhinosinusitis (CRS) affects 12.5% of the U.S. population. CRS can be divided into CRS with nasal polyps (CRSwNP) and CRS without nasal polyps. Some individuals with CRSwNP do not respond to standard-of-care medical and surgical management. For these individuals, targeted biologic agents are emerging as an important therapeutic alternative. In this review, we described the most-relevant studies that addressed the use of anti-immunoglobulin E (omalizumab), anti-interleukin 5 (mepolizumab and reslizumab), and anti-interleukin 4/interleukin 13 (dupilumab) monoclonal antibodies for the treatment of CRSwNP. In addition, we discussed the importance of some of these clinical trials in identifying new CRS endotypes based on distinct inflammatory profiles.

Background: Occupational exposure to metal dust particles leads to irritation and initiation of inflammatory responses in the respiratory system that subsequently onsets lung diseases. Lathe machine workers have a greater risk of having lung diseases due to continuously conducting metal dust particles. This study aimed to explore the pulmonary health status of lathe machine workers. Methods: In this cross-sectional study, 80 individuals (lathe workers) were adopted from a different area of the Tangail district in Bangladesh. A questionnaire’s survey was accomplished along with oximetry and spirometry. After that, blood was drawn for measuring Erythrocyte Sedimentation Rate (ESR), C-reactive protein (CRP), and Immunoglobulin E (IgE) test to the lung function abnormalities. Results: Among 80 lathe machine workers, 12% had breathing problems, 13.8% had coughing, 16.2% had to sneeze and 8.7% had a chest. In addition, spirometry results including, FVC, FEV1, FEV1%, PEF, PEF%, FEF25, FEF75, and FEF2575 were significantly lower in lathe workers compared to control. Moreover, higher levels of IgE (264.93±141) and ESR (11.5±4.77) were reported in lathe machine workers than in the control group. Not using protective masks and smoking were considered as risk factors for having respiratory diseases in lathe machine workers. Conclusion: This study concludes that exposure to metal dust particles can cause respiratory disease symptoms and lower pulmonary function in lathe machine workers. Furthermore, metal dust might also augment the blood IgE and ESR level.
