To update our understanding of the status of elderly health care within the context of the Eastern Mediterranean Regional Office’s Strategy Paper on Elderly Care (1995), a short questionnaire was sent to all Member States of the Region, except Afghanistan and Somalia. The questionnaire sought information on the proportion of the elderly in the population, the status of health care and the level of economic, social, cultural and physical assistance available to the elderly. Of the 21 countries in the survey, 18 (86%) responded. The findings of the survey are discussed here under the headings of demography, national policies on elderly care, social benefits, health care, social and community services, economic burden and the role of the non-government sector

This study aims to summarise the characteristics of elderly care system and analyse expenditures of healthcare for the elderly in Shanghai. The authors use medical records of 2015 and health account results of 2014 based on System of Health Accounts 2011 to describe the pattern of care expenditures for elderly. Individuals aged 60 years and above account for 19.5 per cent of Shanghai’s population but utilise 52.2 per cent of all outpatient visits and 45.3 per cent of all hospitalisations. Almost two-thirds of their medical expenditures occur in hospitals and 16 per cent in community health centres, corresponding to the status of resource allocation. The out-of-pocket payment ratio of the elderly is lower than that of the younger adults, which is attributable to the preferential reimbursement polices set by the insurance schemes. The leading causes of expenditures are cardiovascular disease, neoplasms and respiratory diseases. Care for the elderly costs more, and the elderly use more services than other age groups. The article recommends the monitoring of irrational utilisation of services, strengthening of primary level care and integration of services across different facilities to streamline care for elderly in Shanghai.

Abstract Reforms of the public sector, conducted in the spirit of NPM since the 1990s, are frequently studied by Western and Eastern scholars. The research shows national variations in how the NPM idea was translated and adapted into a country’s context and regulations. Care for the elderly is an interesting example of reforms conducted in the spirit of NPM, because it relates to welfare and health care and to the competences of provincial and local authorities in most European countries. This paper addresses the following questions: What do we know about the reforms conducted in the spirit of NPM and its practical implication within the field of care for the elderly during 1990 - 2010? What kind of knowledge about care for the elderly is still missing and should be developed in the future ? Th is paper conducts comparative research on what is known about the effects of the Swedish and the Polish reforms regarding care for the elderly. It argues that most literature points to negative effects, but also to the fact that there are still gaps in our knowledge about the effects of reforms concerning elderly care, especially regarding its organization. Hence, despite all the research done, we do not know what kind of social and health-care services for seniors represent the best practices for the future.

This article aims to nurses practising of Family Health Strategy related to actions directed at the elderly care. It is a qualitative study, converging-assistential, which data were collected between May and June/2012 through interviews and theme workshops with 20 nurses, who work in the Family Health Strategy of a Sanitary District of Florianópolis Municipality, SC. The data analysis involved processes of apprehension, synthesis, theorization and transference, resulting two categories: care to elderly; public Healthcare policy for the elderly. The results sustain discussions about the need of connecting the nurse work and proposal of government policies to the elderly health care. It is strongly recommended permanent education to the professionals already in service, so they can deal with the challenges of population aging.

To identify the influences of the professional training process for action in the Family Health Strategy (FHS) related to the health of the elderly man. Exploratory descriptive research, carried out with 20 professionals, from a Brazilian Northeast municipality from October to December 2014. The data collection was performed through a questionnaire, following the statements were processed in the software IRaMuTeQ, analyzed by the Descending Hierarchical Classification. Three classes emerged: Influences of the professional training process on elderly health care; Health care for the elderly; Actions developed in the family health strategy in the implementation of the national policy of comprehensive health care for the elderly. The findings suggest improved knowledge, lack of resources and medical specialties, evidenced the complexity of the actions developed by health professionals.

ABSTRACTObjective: to report the experience of Social Work Academic Outreach Program in Nursing in Health Care for the Elderly and Their Caregivers. Method: the present paper is a report on an academic experience of 3rd term undergraduate student  in Social Work at a workshop with caregivers of the elderly with dementia in an Extension Program of Nursing. It was important to record their experience with the group of the elderly caregiver’s. Results: the Elderly Rights Statute was presented as well as a brief survey of some institutions that provide population with free services. The interdisciplinary relationship between the Social Work student and nursing was an important contribution to develop this paper, which raised the possibility of having undergraduate students  broadening  their knowledge and world view of  life experience of people who depend on health care of public institutions to treat the dementia. Conclusion: thereby strengthening the bonds among the researchers and the participants. Descriptores: patient care team; nursing; social work; aged; health services for the aged; aged rights. RESUMO Objetivo: relatar a experiência do acadêmico de Serviço Social no Programa de Extensão a Enfermagem na Atenção à Saúde do Idoso e seus cuidadores. Método: o trabalho consiste em relato de experiência vivenciada por uma acadêmica do 3º período de Serviço Social na oficina com os cuidadores de idosos com demência em um Programa de Extensão de Enfermagem. Nesta construção foi importante registrar o vivido da discente e os assuntos trabalhados com o grupo de cuidadores de idosos com demência. Resultados: apresentou-se o Estatuto do Idoso e um breve levantamento sobre algumas instituições que prestam serviços gratuitos a esta parcela da população. A articulação interdisciplinar entre a aluna do serviço social e a enfermagem foi um ponto importante no desenvolvimento deste trabalho, criando uma possibilidade de ensino para alunos de graduação ampliando seus conhecimentos e sua visão de mundo sobre a experiência de vida de pessoas que dependem da assistência de saúde pública na atenção às demências. Conclusão: a experiência de integração da bolsista de Serviço Social com a Enfermagem, fortaleceu e trouxe novos conhecimentos, tanto a acadêmica quanto os participantes do grupo de cuidadores. La relación interdisciplinaria entre el estudiante de Servicio Social y de Enfermería fue un punto importante en este trabajo, facilitando la enseñanza de los estudiantes de pregrado para que estos amplíen sus conocimientos y su visión de mundo sobre la experiencia de vida de las personas que dependen de la asistencia pública en el cuidado a la demencia. Conclusión: la experiencia de esta integración entre la académica de Servicio Social con la Enfermería fortaleció y trajo nuevos conocimientos, tanto para la académica como para los participantes del grupo de cuidadores.  Resumo: Com o aumento da população idosa, e por ser uma fase da vida frágil e conforme os aparecimentos de doenças, com o encerramento trajetória profissional, perda familiar, mudança de hábitos, tudo isso vem o sentimento de frustações, é comum que nessa fase os idosos fiquem deprimidos. Tornando assim bastante importante o acompanhamento qualificado e humanizado por parte dos profissionais de saúde. Por tanto cabe a estes profissionais proceder em buas de uma assistência adequada. Objetivo desse estudo é analisar e descrever a importância da assistência em saúde ao paciente com depressão na terceira idade. Metodologia: trata-se de uma revisão integrativa da literatura, que foi realizada através da pesquisa bibliográfica com os seguintes descritores: Terceira idade, depressão, assistência da enfermagem e saúde do idoso.com os estudos dos artigos selecionados os resultados foram distribuídos em três categorias temáticas: Fatores contribuintes para depressão na terceira idade; Ações da enfermagem para prevenir e amenizar a depressão na terceira idade; A importância da assistência qualificada do profissional da enfermagem ao idoso com depressão. Notou-se o quanto é importante uma assistência adequada e humanizada da enfermagem ao idoso com depressão, elaborando assim ações que vá ajudar o idoso na prevenção à amenização dos sintomas da depressão. Espero que esse estudo ajude com futuras pesquisa relacionado a esse tema, fortalecendo o interesse dos enfermeiros a saúde do idoso.   Palavras-chaves: Depressão. Terceira idade. Assistência. Saúde do idoso.Abstract: With the increase of the elderly population, and because it is a fragile stage of life and according to the onset of diseases, with the closure of the professional trajectory, family loss, change of habits, all this comes with the feeling of frustration, it is common for the elderly at this stage get depressed. Thus, qualified and humanized monitoring by health professionals is very important. Therefore, it is up to these professionals to provide adequate assistance. The aim of this study is to analyze and describe the importance of health care for patients with depression in the elderly. Methodology: this is an integrative literature review, which was carried out through bibliographical research with the following descriptors: Third age, depression, nursing care and elderly health. With the studies of the selected articles, the results were divided into three categories thematic: Contributing factors for depression in old age; Nursing actions to prevent and alleviate depression in old age; The importance of qualified nursing professional assistance for the elderly with depression. It was noted how important an adequate and humanized nursing care for the elderly with depression is, thus developing actions that will help the elderly in preventing the alleviation of symptoms of depression. I hope this study will help with future research related to this topic, strengthening nurses' interest in the health of the elderly. Keywords: Depression. Third Age. Assistance. Elderly health.

AbstractHealth care for the elderly in Japan is financed through a pool to which all insurers contribute. We analyzed insurers' financial data to evaluate this redistribution system. Cost sharing affected financial performance substantially. The current formula for cost-sharing redistributes elderly health care costs unequally and should be changed.

For well over 20 years I have been arguing that someday we will have to ration health care for the elderly. I got started in the mid-1980s when I served on an Office of Technology Assessment panel to assess the likely impact on elderly health care costs of emergent, increasingly expensive medical technologies. They would, the panel concluded, raise some serious problems for the future of Medicare. The panel did not take up what might be done about those costs, but I decided to think about that question and wrote a book called, Setting Limits: Medical Goals in an Aging Society.


Nowadays, Thailand is stepping into an aging society. This research purposes developing the intelligence walking stick for the elderly in terms of the health care system by applied the IoT devices and biometric sensors in a real-time system. The heart rate, blood pressure, oxygen saturation, and temperature were measured at the finger of the elderly that holding the intelligence walking stick. All data can monitor and display on mobile devices. The intelligence walking stick system was evaluated by twenty users who are five experts and fifteen elderly in Ratchaburi province. As a result of the mean value at 4.88 and 4.85 by experts and elderly, respectively. It could be said that the development of intelligence walking stick by using IoT can help and improve the daily living of the elderly at the highest level.

BACKGROUND Time banking is a good mechanism to provide elderly care in community services with members having mutual benefits, besides social welfare and out-of-pocket fee payment mechanisms. With further integration with off-line works, mobile time banking may provide a better way, compared to traditional web access. On the other hand, blockchain technology has been long encountering difficulty in integrating with real-world economies or activities. Development of a mobile time banking system on blockchain (MTBB) may provide a realistic solution for community elderly care. Besides, the tracking mechanism from blockchain technology itself may also help track the elderly care service transaction records in order to measure better Sustainable Development Goals (SDGs) set by United Nations (UN). OBJECTIVE The aim of this study was to develop the MTBB, which enables tracking service transaction records in community elderly care through mutual helps. METHODS The MTBB was developed to empower organizations, either Corporate Social Responsibility (CSR) organizations, or Non-Profit Organizations (NPOs), to issue time tokens of their proprietary token types to the members who participate in the volunteer activities organized by the organizations respectively. In the service activities, members sign in and sign out before and after the services by using a smartphone app, and then get the time tokens afterwards. Members with time tokens can then exchange time tokens for elderly care services using the same smartphone app. MultiChain is used as the blockchain technology stack, as one of its features to support multiple token types is critical. RESULTS Database applications with smartphone apps integrated with MultiChain were developed. The whole set of the database schema was integrated with two smartphone apps, one for members, and the other for organizations, in addition to the two backend operations modules, one for organizations, and the other for managing all organizations and members. The MultiChain wallet was also integrated into the member app, as well as the organization backend modules for keeping track of the service transactions and time tokens. Metadata with the service transaction information is stored in the MultiChain blocks so that the transaction records are immutable and can thus be analyzed in the future. CONCLUSIONS The twelve characteristics of Cahn’s time banking are the guidelines of developing this MTBB with integration of MultiChain blockchain technology for tracking service transaction records. The study also combines the 1-to-1 member service exchange with organizations holding volunteer activities and issuing proprietary time tokens. With the blockchain transaction tracking mechanism, all of the elderly care service records through or within organizations can be tracked and analyzed to align with UN’s five SDGs.
