BackgroundTeaching and learning how to perform examination of the ocular fundus is challenging. Smartphones can support to enhance students’ confidence and experience.MethodsFollowing an optional year-4 ophthalmoscopy practical using hand-held ophthalmoscopes, students completed a questionnaire using a visual analogue scale (VAS) investigating if students felt smartphone use aided learning and if student’s self-assessed confidence in visualising the ocular fundus had improved. VAS scores were compared using the Wilcoxon signed rank test (significance: P<0.05).ResultsAll 30 year-4 students attending the practical participated to the study. Confidence in performing direct ophthalmoscopy significantly increased after the practical. Confidence after the practical was 65.3 (±19.8) per cent compared with before the practical when confidence was 20.1 (±15.6) per cent (P<0.001). The perceived usefulness of traditional teaching was 62.3 (±23.8) per cent. The perceived usefulness of the teaching with the smartphone was 91.1 (±8.6) per cent. While students found both methods useful, they perceived the use of the smartphone to be significantly more useful (P<0.001). Free-text comments on the use of the smartphone were all positive and included ‘useful’, ‘fun’ and ‘good teaching tool’.ConclusionsThis study shows that students positively received the use of the smartphone, which can be a useful tool to teach the equine ocular examination to undergraduate veterinary students.

Forceps biopsies of airway lesions have variable yields. The yield increases when combining techniques in order to collect more material. With the use of cryotherapy probes (cryobiopsy) larger specimens can be obtained, resulting in an increase in the diagnostic yield. However, the utility and safety of cryobiopsy with all types of lesions, including flat mucosal lesions, is not established.Aims. Demonstrate the utility/safety of cryobiopsy versus forceps biopsy to sample exophytic and flat airway lesions.Settings and Design. Teaching hospital-based retrospective analysis.Methods. Retrospective analysis of patients undergoing cryobiopsies (singly or combined with forceps biopsies) from August 2008 through August 2010.Statistical Analysis. Wilcoxon signed-rank test.Results. The comparative analysis of 22 patients with cryobiopsy and forceps biopsy of the same lesion showed the mean volumes of material obtained with cryobiopsy were significantly larger (0.696 cm3versus 0.0373 cm3,P=0.0014). Of 31 cryobiopsies performed, one had minor bleeding. Cryopbiopsy allowed sampling of exophytic and flat lesions that were located centrally or distally. Cryobiopsies were shown to be safe, free of artifact, and provided a diagnostic yield of 96.77%.Conclusions. Cryobiopsy allows safe sampling of exophytic and flat airway lesions, with larger specimens, excellent tissue preservation and high diagnostic accuracy.

The impregnation of knitted Dacron grafts with biodegradable protein removes the need for preclotting at implantation. The effect of such impregnation on the thrombogenic potential of these prostheses is, however, unknown. We have compared the early thrombogenicity of two impregnated knitted Dacron grafts - Hemashield ( collagen-impregnated Dacron, Meadox) and Gelseal (gelatin-impregnated Dacron, Vascutek) -with conventionally preclotted knitted Dacron in an artificial circulation. Fresh, heparinised human blood, containing 111Indium-labelled platelets, was circulated around identical twin circuits of silicone tubing by a Sarns roller pump at 180 ml/min and 120 mmHg. Into each circuit a different 15 cm × 8 mm type of graft material was mounted. Changes in platelet count, platelet aggregation to collagen, plasma haemoglobin and blood radioactivity were recorded and after 60 minutes of perfusion each graft was sectioned and counted for radioactivity prior to scanning electron microscopy (SEM) of the luminal surface. The mean graft radioactivity divided by the mean blood radioactivity yields a Graft Activity Index (GAI) allowing comparison of platelet deposition and thus thrombogenicity.RESULTS: (mean ± s.e.m.) 8 examples of each graftSEM confirmed these findings with preclotted Dacron having the highest platelet deposition. Both impregnated grafts produced significant early haemolysis within 4 minutes of blood contact. Our findings indicate that the early thrombogenicity of these impregnated grafts is significantly less than conventionally preclotted Dacron, (p< 0.01, Wilcoxon signed rank test), despite the early haemolytic phenomenon and that Hemashield grafts are less thrombogenic than Gelseal grafts. (p<0.05, Wilcoxon signed rank test)

Aim of this trial was to assess (1) the accuracy and precision of electrochemistry-based glucometers CONTOURLINK, CALLA, and LINUS and (2) the diabetes control using Ambulatory Glycaemic Profiles (AGP) as markers of therapeutic effectiveness. Glucometers and COBAS INTEGRA 400 Plus analyzer were used by one laboratory professional to estimate P-glucose (PG) in 112 out-patients. There were 112 sets of 12 PG estimations analyzed. In each set, means of 3 capillary PG estimations on 3 respective glucometers and on INTEGRA analyzer were calculated. The statistical program SPSS, v. 15, was applied. The mean INTEGRA PG values ranged from 2.7 to 25.3 mmol/L. There were strong correlations between mean PG on INTEGRA versus CONTOURLINK, versus CALLA, and versus LINUS; PG deviations from INTEGRA were mostly within the range ±15%. Wilcoxon Signed Rank Test revealed differences between CONTOURLINK-INTEGRA and LINUS-INTEGRA; CALLA-INTEGRA showed no difference. SD INTEGRA = 0.061 mmol/L, SD CONTOURLINK/SD CALLA/SD LINUS were 0.256/0.290/0.286 mmol/L. All patients were trained to perform defined 10-point PG profiles to adapt food, exercise, and insulin doses. The PG differences between all tested glucometers and reference values were in borderline of ISO 15197 but worthy of consideration. AGP are helpful markers of diabetes control.

The aim of this study was to quantify the effect of tonsillectomy on the incidence of sore throats and its co-morbidity in adult patients. One hundred and nineteen adult patients were sent a standard questionnaire regarding their symptoms in the 12 months preceding and following their tonsillectomy. Outcome measures included the incidence of sore throats, total number of days with sore throat, amount of time taken off work or school, and number of visits to the general practitioner (GP). In addition, patients were asked to indicate the duration of their symptoms and whether or not they found the tonsillectomy effective in curing their sore throats.Sixty-six patients (55.5 per cent) returned completed questionnaires. The age of the patients ranged from 16 to 39 years. The mean duration of symptoms was 8.3 years. On average, patients had 8.1 different sore throat episodes, 42 sore throat days, 21.4 days of sore throat related absence from work or school, and 5.9 visits to the GP in the 12 months before their operation. For the 12 months after surgery, these reduced to 0.9 episodes, four days, 2.2 days and 0.6 visits, respectively. This reduction was very significant (p < 0.001, Wilcoxon signed rank test). After their surgery, more than half the patients achieved complete resolution of all the measured parameters mentioned above. Most of the remaining patients achieved at least 50 per cent resolution. Only three patients (4.8 per cent) achieved less than 50 per cent resolution. Ninety-five per cent of the patients found the operation effective in curing their sore throats and were glad they had had surgery. In conclusion, retrospective questionnaire data must be interpreted with some caution, but this study suggests that tonsillectomy is effective in reducing the incidence, duration and co-morbidity of recurrent sore throats in adults; this must be balanced against the post-operative problems in a minority of patients.

Purpose: To describe the biomechanical changes seen after thumb carpometacarpal (CMC) fusion with distal scaphoid excision (DSE), a surgical technique used for pantrapezial disease in high-demand individuals. Methods: A cadaveric study with 13 fresh-frozen cadaveric specimens was conducted. Fluoroscopic measurements in posteroanterior (PA) and lateral planes were performed. Thumb CMC fusion was then simulated. The same images were taken after fusion. The distal scaphoid was excised, and the measurements were obtained again. The angle between the index finger and thumb metacarpals was recorded. Statistical analysis was performed using Wilcoxon signed rank test. Results: The mean arcs of motion in the PA plane pre-fusion, post-fusion, and post-fusion with DSE were 48.5°, 25.1°, and 34.6°, respectively. An increase in arc of 9.5° was obtained after DSE compared with thumb CMC fusion alone. The mean arcs of motion in the lateral plane pre-fusion, post-fusion, and post-fusion with DSE were 53.4°, 22.4°, and 33.6°, respectively. A statistically significant increase in arc of motion of 11.2° in the lateral plane was obtained after DSE compared with thumb CMC fusion alone. This was a 20% and 21% increase in pre-fusion range of motion in the PA and lateral planes, respectively, from fusion alone compared with fusion and DSE. Conclusions: Thumb CMC fusion combined with DSE maintains motion lost in simple CMC arthrodesis.
