Objective: The main objective of the present study was to find out visual outcomeof Low Vision Devices among patients suffering from different diseases causing reduced visualacuity in the area of Dera Ghazi Khan, Southern Punjab, Pakistan. Setting: OphthalmologyDepartment, Dera Ghazi Khan which is a tertiary care hospital. Period: 10th February, 2014 to31st December, 2014. Material and Methods: Patients (n=55) presented for their low visionassessment included two groups age wise as patients equal or more than 18 years age (n=22)and patients less than 18 years (n=33). LOG MAR VA charts, contrast sensitivity test, visualfields through arc perimeters, color vision with color pencils hue 16 test along with objective andsubjective refractions were applied. All the data were entered and analyzed using SPSS version16.00. Results: Patients (n=55) aged 7-90 years presented for their low vision assessment hadmean age 23.73+ 2.9 years. The patients were divided into two age groups. First group was22 (40%) patients equal or more than 18 years age and second consisted of 33(60%) patientsless than 18 years age group. Among the patients examined, 40 (72.7%) were males and 15(27.3%) were females. Our study results revealed that the low vision devices provision wasassociated with a statistically significant improvement in both near and distance visual acuitiesand with patients’ fulfillment. Conclusion: There was a significant improvement in far and nearvisual acuity of the patients suffering from a multiplicity of blindness caused diseases by whichpatients are able to improve their quality of life and able to do their tasks without getting helpfrom others.

Objective: To study the quality of scars following the use of butyl cyanoacrylate to close the skin during cleft lip repair. Design: Nonrandomized prospective study. Setting: Tertiary care hospital. Patients: Patients (n = 104) with cleft lip and palate in combination of various degrees of cleft with the age group ranging from 3 months to 5 years. Interventions: Use of butyl cyanoacrylate to close the skin during cleft lip repair after inserting dermal stitches with delayed absorbable sutures. Results: None of the 104 patients required revision of the vertical scar, which was excellent in nature. No patients had complications such as wound infection, dehiscence, nasal obstruction, or eyelid adhesion. Conclusions: Quality of the scar following the closure of skin of cleft lip repair using butyl cyanoacrylate was considered excellent.

Objectives: To identify the pattern and etiology of diseases in children presented hepatomegaly at Pediatrics Department in Hue Central Hospital, Vietnam. Material and Methods: The Present study was conducted to evaluate the possible etiology and clinical features of children presented with hepatomegaly. A total of 107 children, from one month to fifteen years of age with hepatomegaly admitted in the Pediatric Department of Hue Central Hospital, Vietnam from May 2018 to April 2019 were included in this study. Results: The most common causes of hepatomegaly were inflammation/infection of the liver (miscellaneous cause included), hematological diseases and congestion heart failure with the percentage of 50.5%; 27,1% and 12,1%; respectively. Among the hepatitis group, viral agents account for nearly 50.0%, of which the most common one was Cytomegalovirus accounting for nearly 30.0%. Distribution of causes by age group revealed most cases in 1 year-old or younger children was caused by inflammation (53.8%), and in the older than 1 year-old children were hematological disorders (47.3%) which has statistical significance (P<0.05). Conclusions: Our research observed that the most common causes of the group of ≤ 1 year old were hepatitis, followed by cardiovascular disease and biliary obstruction due to congenital biliary atresia, and in the older than 1 year group, hematological disorders was the most common.

ABSTRACT Background Bacterial vaginosis (BV) is highly prevalent among women especially in reproductive age group but it is found also in other age groups. Bacterial vaginosis is a great health concern in India because of its complications. Most of the healthcare centers in India are not well-versed in advance diagnostics for BV. Aim To assess the utility of conventional methods for diagnostic evaluation of BV in the patients of different age groups. Materials and methods In this 1 year cross-sectional study, the vaginal swabs were collected from women with signs and symptoms of BV like complaints of vaginal discharge, odor, itching, back ache, etc. attending the department of obstetrics and gynecology of a tertiary care hospital in Western UP. These specimens are subjected to vaginal pH determination and are sent to department of microbiology for Gram stain, wet-mount and whiff test. Amsel's criteria were used for confirming BV. Results A total of 206 women included in the study, n = 63 (29.61%) were found to be having BV based on Amsel's criteria. The mean age of the females were found to be 30.77 years. The most affected age group was found to be 26 to 35 years, n = 33 (54%). The vaginal pH was significantly higher in n = 61 (29.61%) women with BV. Vaginal discharge was prevalent in n = 108 (52.43%) women. Presence of clue cells and positive whiff test were found to be in n = 63 (30.58%) and n = 61(29.61%). Conclusion The characteristics of vaginal discharge, vaginal pH determination, clue cells and whiff test is relatively easier conventional methods for diagnosis of women with BV where better facilities are lacking. Background: This study was aimed to evaluate the age and gender related mortality and outcome of covid 19 positive patients in order to further strengthen the management of covid 19 patients as the insights from these data will be useful in determining preventive measures and treatment policies of covid 19 patients. Methodology: This was a retrospective observational study done in adult covid positive patients admitted in Tertiary Care Hospital from first May 2020 to 30 June 2021.The age related mortality and outcome in these patients was analyzed. Results: A total of 816 adult covid -19 positive patients were admitted in a Tertiary care hospital,GMC Anantnag from May 2020 to June 2021. Among the patients, 53.55 % were males and 46.44 % were females. The mean of the age involved was 57.96 years. The most common presenting symptom was fever (85%) followed by cough (70%) and shortness of breath (40%). Approximately 20% of patients presented with nonspecific complaints (including generalized aches, pains, body aches, myalgias etc). All age groups were affected equally (29% in 15-50yr, 37% in 51-65yr, 33% in 66-100yr). Maximum patients (70% ) affected were more than 50 yr of age. About 73.4% were discharged, 6% were reffered, 20% expired. So, overall hospital mortality was 20%. Further, Maximum (86%) of covid positive patients expired in age group of above 50 yrs. Further, Male and Female Mortality was also same in covid positive patients. Conclusion: Males and females were affected equally by Covid -19 disease with no sex predilection.. Fever was the main presenting symptom. All age groups were affected equally but mortality was more in elderly age group. Further, mortality is same in male and female covid positive patients with no gender predilection.

Adverse drug reactions (ADRs) are a major concern in clinical practice. Reporting of ADRs either through health care professionals or the patients themselves is of utmost importance to give an accurate estimate of the prevalence, severity and preventability of ADRs. Present study was conducted to evaluate the prevalence of adverse drug reactions in a tertiary care hospital in Hyderabad, Telangana, India. This was a retrospective observational study, extending over 3 months (December 2020 to February 2021). A total of 41 cases comprising patients of either sex or age group were studied. The data was collected using CDSCO ADR reporting form. A total of 41 ADRs were reported during the three months period of study. During the study period a total of 41 ADR reports were received from various departments of the hospital. We observed 25 ADRs in females and 16 ADRs in males from our study. Among the age groups, 3 ADRs in age group 0 – 19 Yrs., 28 ADRs were seen in age group 20 – 59 Yrs., and 10 ADRs were seen in age group >60 Yrs. Maximum number of ADRs came from General Medicine department 17 (41.46%). Drug therapy 41 (100%) and diseases 19 (46.34%) were the most prominent predisposing factors of ADRs seen in our hospital. Causality assessment of suspected drugs was assessed using Naranjo scale. According to Naranjo scale most of the reported ADRs were found to be probable 23 (56.9%) followed by possible 16 (39.02%) and definite 2 (4.8%). The severities of the reactions were done using Hart Wig Scale. Majority of the reactions were mild 25 (60.97%). Withdrawal of the drug 5 (12.19%) was the main line of the management of the adverse drug reactions in the present study. Majority of serious ADRs were preventable in our study. ADRs are a major cause of morbidity worldwide. Frequency of ADRs can be reduced by careful follow up and a robust hospital-based pharmacovigilance setup. Background: Guillain-Barré syndrome (GBS) is the commonest cause of acute flaccid paralysis worldwide, with an incidence of 0.6-4 per 100.000 inhabitants per year. It affects all age groups and carries an incapacity burden of up to 20%. The aims of this study were to evaluate the demographic and clinical presentation, hospital care and outcome with different modalities of treatment in adult patients suffering from GBSadmittedina tertiary care hospital in Dhaka.Methods: This observational study was done in the Neurology department of a tertiary care hospital in Dhaka.The studystarted in July-2011 and the first fiftypatients suffering from GBSwereenrolledconsecutively. Subjects included in this study were>18years of age.All the patients were interviewed and clinically examined. Relevant data were documented in a structured questionnaire. Nerve conduction Study (NCS) and Cerebrospinal fluid (CSF) study results were also documented. All of patients were watched for respiratory insufficiency and those who developed respiratory paralysis were transferred to ICU for respiratory assistance. According to clinical and electrophysiological criteria, the patients were classified into different variants of GBS. Patients were treated with intravenous immunoglobulin (IVIg), plasmapheresis, supportive care and outcome was observed. The patient who recovered and were discharged were later followed up on out –patientbasis for one month Results: The commonest age group affected was 31-50 yrs with male preponderance .The mean age of study subjects was 46years. In most of the patient their weakness progressed for 3-5 days. Clinical evidence of a preceding infection was present inforty-six percent of the patientsbutcausative organism was not identified.Clinical presentations were quadriparesis in 84 % of patients, cranial nerve palsy in 48% of patients. Severe respiratory involvement requiring mechanical ventilation developed in 10% of patientsafter admission. CSF study was done and only10 subjects displayed typical findings of albumino cytological dissociation. NCS wasabnormal in all study subjects and revealed Acute Motor Sensory Axonal Neuropathy (AMSAN) in58%of study population. Among the fifty patients 23received plasmapheresis and another 9 receivedstandard IVIg therapy. Of these patients 20(88%) and 7 (77.6%) patients made significant recovery, respectively. Residual neurodeficit persisted in 6 of the remaining patients who received treatment in the form of only physiotherapy at the end of one month follow-up.Conclusion: Rapidly progressive quadriparesis with cranial nerve involvement was the commonest presentation. In this group of patients NCS proved to be a sensitive study for early diagnosis of patients with GBS in both diabetic and non-diabetic population. The axonal variety of GBS was more common than the demyelinating type. Although not statistically significant, the better response to plasmapheresis seemedencouraging. Again the patients who received IVIg or plasmapheresis early in the course of disease had faster recovery as compared to patients who received only supportive line of treatment.Birdem Med J 2017; 7(1): 38-42

Objectives: The objective of the study was to analyze urinary 15-2t-isoprostane levels in epileptic patients with levetiracetam (LEV) and to compare the urinary 15-2t-isoprostane levels between the different age groups, smokers, and alcoholics. Methodology: The study was conducted in a tertiary care hospital. Urinary 15-2t-isoprostane levels were assayed in epileptic patients taking LEV. Twenty patients were recruited. Urinary levels of 15-F2t-isoprostane determined by ELISA. The patient’s age, gender, height, weight, smoking, and alcoholic history were collected using pro forma. Statistical analysis of data was performed using SPSS version 23.0. Results: A total of 20 participants recruited for the study. Among these, 70% were male and 30% were female. The mean weight and height were more in males compared to females. The mean 15-F2t-isoprostane level was significantly higher in males compared to females. More than 40% of the participants were in the age group of >40 years. However, the mean urinary 15-F2t-isoprostane level is higher in the age group of 19–30 years. Only 20% of the participants were smokers and alcoholics. Non-smokers and non-alcoholics had higher urinary 15-F2t-isoprostane levels compared to smokers and alcoholics. Conclusion: The urinary 15-F2t-isoprostane levels were more in males, the age group of 19–30 years, non-smokers and non-alcoholics. However, the levels are lesser when compared to earlier meta-analysis study. This concludes that LEV may have antioxidant effect. There is a need for further studies with more sample size and correlation of LEV and urinary 15-F2t-isoprostane levels.

The morbidity and quality of life after head injury is long and devastating.The symptoms may persist for lifetime and if not, then can be long enough to affect a person's health/the working conditions and definitely the monetary factor for coping up with this comes into a larger picture.It is also one of the leading causes of lifelong disability or death worldwide.The presentation may vary between a conscious patient on arrival to the emergency room to a person with seizures,ear bleed or drowsiness.The various types of presentation go hand in hand with different types of head injury, its management,prognosis and long-term impact on a person's life.Despite various studies and appropriate guidelines, the morbidity after head injury remains high.This study has a purpose to correlate various presentations of head injury, the epidemiology of head injury,types of head injury and its management options in a tertiary acre hospital. OBJECTIVES: a. To study the demographic profile and gender distribution of patients presenting to emergency room in a tertiary care hospital in western India. b. To study the mode and type of head injury,Imaging finding and its management. METHODOLOGY: It was a prospective study of consecutive 200 patients presenting to a tertiary care hospital: Mahatma Gandhi Mission's Medical college and hospital,Kamothe,Navi Mumbai.Institute Ethical committee approval was obtained before the start of study. All the age group was included. There were no exclusion criteria. Result was tabulated and analysed at the end of the study. RESULT:This study enrolled 200 patients in which male were more common than female,young adults more commonly affected.Majority of patients were treated conservatively and improved. CONCLUSION: The most common cause of traumatic brain injury is road traffic accident and relating it with the most common age group ,brings us to the conclusion of importance of safety measures on road amongst young adults.Hence , preventing the wrath of morbidity associated with the debilitating head injury.This also helps in improving the quality of life of the patient and the care giver

The descriptive observational study was conducted over six months among inpatients of the orthopaedics department. The aim is to observe the prescribing pattern of drugs, to find out the percentage of analgesics given and most commonly prescribed analgesic, to compare the prescribed drugs whether they are in NLEM, WHO list and calculate DDD/100 Bed-days and to analyze the drugs for WHO prescribing drugs. A total of 250 patients were included in the study. Out of this, 168 were male, and 82 were female. Most of the patients were in the age group of 31-40[45 in number with 18%]. The most common condition was found to fracture [113 in number with 95.2%]. Commonly prescribed drugs were analgesics 447 with 26.76%. Among all the NSAID's, PCM was most commonly prescribed analgesic with 34.4%. The highest no, of drugs was found to be 4drugs/prescription with 24%. Monthly one analgesic was prescribed per prescription with 38.8%. Utilization of analgesics in term of DDD/100 Bed-days was 55.26; Drugs will be evaluated per prescription as per prescribing indicators of WHO was done, the average no. of drugs per prescription was found to be 0.45, percentage of drugs prescribed by generic name was found to be 13.3%, percentage of encounters with antibiotics prescribed were found to be 94.4%, In ratio percentage of drugs prescribed from the national list of essential medicine was found to be 92.6%. This study would help to facilitate better health care delivery.

Abstract Background The coronavirus disease (COVID-19) pandemic has had a massive impact on individuals globally. The Chinese government has formulated effective response measures, and medical personnel have been actively responding to challenges associated with the epidemic prevention and control strategies. This study aimed to evaluate the effect of the implementation of a care transition pathway on patients that underwent joint replacement during the COVID-19 pandemic. Methods A quasi-experimental study was designed to evaluate the effect of implementing a care transition pathway for patients who underwent joint replacement during the COVID-19 pandemic in the orthopedic department of a tertiary care hospital in Beijing, China. Using a convenient sampling method, a total of 96 patients were selected. Of these, 51 patients who had undergone joint replacement in 2019 and received treatment via the routine nursing path were included in the control group. The remaining 45 patients who underwent joint replacement during the COVID-19 epidemic in 2020 and received therapy via the care transition pathway due to the implementation of epidemic prevention and control measures were included in the observation group. The quality of care transition was assessed by the Care Transition Measure (CTM), and patients were followed up 1 week after discharge. Results The observation group was determined to have better general self-care preparation, written planning materials, doctor-patient communication, health monitoring, and quality of care transition than the control group. Conclusions A care transition pathway was developed to provide patients with care while transitioning through periods of treatment. It improved the patient perceptions of nursing quality. The COVID-19 pandemic is a huge challenge for health professionals, but we have the ability to improve features of workflows to provide the best possible patient care.
