Introduction: The inhalation route is widely used for the treatment of asthma. It is considered that inadequate inhaler technique is the leading cause of therapy failure. Objective: To determine the proportion of incorrect inhaler use and associated factors in asthmatic patients compromising quality of life. Methodology: It is a cross sectional study conducted at Specialist Chest Clinic Fatima Bhai Hospital Karachi from September 2010 to March 2011. A sample size of 215 was selected by Non-probability purposive sampling technique. Trained health care workers in presence Chest specialist requested patients to demonstrate their inhaler technique and assessed it according to the checklist. If any of the steps was missing or done incorrectly, it was marked as incorrect technique. Information was also collected through a structured questionnaire on socio-demographic variables such as age, gender, education, duration of asthma, duration of inhaler use, and patient education of inhaler technique. Data was entered and analyzed on SPSS-14. Results: There was an inverse relationship between education level and incorrect inhaler technique (p<0.05). Education of inhaler usage provided by doctors was more effective as compared to education provided by nurses (p<0.038). There was no significant relationship between age, sex, duration of asthma, duration of inhaler use, frequency of inhaler use and incorrect inhaler technique. Conclusion: Quality of life of a large percentage of patients is compromised by incorrect inhaler use and education level of patients had significant relationship with incorrect inhaler technique.DOI: http://dx.doi.org/10.3329/jom.v13i1.7980 JOM 2012; 13(1): 16-21

Nursing services greatly determine the quality of hospital services, but the services provided by nurses are still often complained of by the community. The highlight of nurse performance is a problem that must be addressed immediately. Performance in this case is closely related to how much workload, work stress and nurse motivation work in providing health services. Job stress is a common health problem and is often complained of by workers in various countries. The study design used quantitative analytical surveys with crossectional designs. The population of all nurses working in the Inpatient Room at Datu Beru Takengon Hospital was 304 people. The sampling technique uses Proportional Random Sampling. Data analysis was performed by univariate, bivariate and multivariate analysis with chi_square. The results obtained p-value = 0,000, 0,003, 0,000 <α 0,05, meaning that there is an influence between work stress, workload and job satisfaction on nurse performance, from the results of multivariate analysis found that the most influential variables in this study are load variables work with an Exp B value of 22.121 which means the workload is 22 times influencing the performance of nurses. Conclusions there is a relationship between work stress, workload and job satisfaction on nurse performance while multivariate analysis shows that the most dominant factor is the workload variable on nurse performance. It is recommended for nurses to use time more effectively in completing tasks that are burdened so that there is no accumulation of work so that it can be completed on time.

Keywords : Job Stress, Workload and Job Satisfaction, Nurse Performance

Stress is a dynamic condition of an individual faced with opportunities, limitations, or demands in accordance with expectations and results to be achieved in important and uncertain conditions. According to the National Institute for Occupational Safety and Health (NIOSH), nurses are professions that are very high risk of stress. Results of a survey by the Persatuan Perawat Nasional Indonesia (PPNI), where in 2006, approximately 50.9% of hospital nurses working in four provinces experienced work stress. This study aims to determine the relationship of job insecurity and mental workload with job stress contract nurses at RSJD Atma Husada Mahakam Samarinda. This research uses analytic survey method with cross sectional research design. The variables in this study are Job Insecurity, Mental Workload and Job Stress. The respondents of this research are contract nurses at RSJD Atma Husada Mahakam with total subject of this research is 74 respondents, sampling technique using Total Sampling. Data were analyzed by using Pearson Correlation test with α = 0,05. The result showed that there was a relationship between job insecurity (p = 0.0001) and mental workload (p = 0,004) with work stress. The conclusion of this research is job insecurity and mental work load is the factor causing the occurrence of job stress on contract nurses at RSJD Atma Husada Mahakam. It is advisable to pay more attention to the nurse's need to reduce the level of job insecurity and mental workload by raising nurse staffing status for nurses with more than 5 years working period and providing pension guarantee and providing training to improve nurse's ability.

Introduction: Environmental health (EH) services have a long history in Ethiopia, but data on environmental health services quality and the magnitude of environmental health professionals’ engagement has never been addressed. This study was conducted to assess the quality of environmental health services in different sectors and professionals’ level of engagement in Eastern Ethiopia. Methods: Institution based cross-sectional mixed study design was implemented. A cluster sampling technique was employed to select 83 participants. Data were collected using a pretested questionnaire and an interview guide. Descriptive, bivariate, multivariate, and thematic analysis was carried out. Results: Professionals’ performance in most services were reported to be average or low. Only 19.5% of participants responded as having good satisfaction in their job. The multiple logistic regression analysis showed factors associated with selected environmental health services. The odds of identifying environmental problems was associated with profession (adjusted odds ratio (AOR): 4.1; 95% confidence interval (CI): 1.3-7.6) and level of education (AOR: 3.1; 95%CI: 0.9-5.9). The factors contributing to introducing innovative solutions to EH problems were type of institution (AOR: 3.1, 95%CI = 1.6-9.3), profession (AOR: 3.4, 95%CI = 1.1-12.2), and level of support and emphasis offered (OR: 5.6, 95% CI = 2.2-11.9). Level of job satisfaction was also associated with the above-mentioned independent variables. Conclusion: The current study showed low level of professionals’ engagement and factors associated with the quality of environmental health services in different sectors. Therefore, Ethiopian Federal Ministry of Health and other concerned ministries, agencies, and authorities should intervene accordingly to improve the service and level of professionals’ engagement.
