Background: Premature rupture of membranes (PROM)is a rupture of membranes before any signs of labor and awaited before the labor occurs. The problem of PROM is a major obstetric issue, because the impact of PROM is an infection in the maternal and ends in an increased incidence of cesarean section due to a failed normal childbirth and neonatal. Factors that causePROMitself is not known for certain, but if we know earlier about the predisposing factors of PROM preventive efforts will be more helpful and reduce maternal and neonatal morbidity and mortality.Objective: Relationship of maternal age, parity, gestational age, and over distention with incidence of premature rupture of membranes in Yogyakarta Hospital.Research Of Method: This research uses retrospective study approach with data focus case control approach. The case population in this research is all maternal mothers who experience premature rupture of membranes in RSUD Yogyakarta in 2017 obtained from medical record. Sampling technique is by total sampling with the number of 427 respondents. Data analysis used is bivariate analysis.Result: Parity has nothing to do with the incidence of premature rupture of membranes due to the value of p value 0.142> 0.05, with the OR value of 0.814. Age of pregnant women has nothing to do with the incidence of premature rupture of membranes because the value of p value 0.671> 0.05, with the value of OR 1.062. Age pregnancy has nothing to do with the incidence of premature rupture of membranes because the value of p value 0.288> 0.05, with OR 1,207 nialai. Uterine overdistesi has nothing to do with the incidence of premature rupture of membranes due to the p value of 0.571> 0.05, with the value of OR 1.240. Conclusion: There is no relationship between maternal age, parity, gestational age, and overdistence with premature rupture of membranes in Yogyakarta Hospital. Every year the incidence of premature rupture of membranes ranges from 5-10% of all preterm preterm births and membranes occurring in 1% of all pregnancies.70% of cases of premature rupture of membranes occur in early pregnancy, prematurerupture of membranes is the cause of preterm birth as much as 30% (Manuaba, 2010). Atthe Derah Kepahiang General Hospital, the incidence of premature rupture of membranesis 219 cases in 2016.This research is analytical descriptive research with case controldesign. The sample in this study amounted to 138 people divided into two groups of 69cases and 69 controls. Statistical test using chi square test and binary logisticregression.Results of the study of 69 people with premature rupture of membranes atmaternal age <20 and> 35 years (46.4%), primiparity and grandemultipara parity(62.3%), fetal abnormalities (33.3%), gestational age aterm ( 63.8%) and those with ahistory of premature rupture (18.8%) in Kepahiang Hospital by 2016. Maternal age (pvalue0.013 OR = 2.646), parity (p-value 0.0,0 OR OR = 3.111) (p-value 0,0017 OR =2,950), gestational age (p-value 0,386) and history of premature rupture (p-value 0,167)at Kepahiang Hospital 2016. The most dominant factor causing premature rupture ofmembranes in RSUD Kepahiang year 2016 is parity (Exp (B) 2,806).It is expected thatthe hospital can use the research as input to suppress the incidence of premature ruptureof membranes, by counseling to explain the risk factors of premature rupture ofmembranes, especially maternal age, parity and fetal abnormalities.

ABSTRAKKetuban pecah dini (KPD) didefinisikan sebagai pecahnya selaput ketuban sebelum terjadinya persalinan yang terjadi pada atau setelah usia gestasi 37 minggu disebut KPD aterm atau premature rupture of membranes (PROM) dan sebelum usia gestasi 37 minggu atau KPD preterm atau preterm premature rupture of membranes (PPROM). Insiden ketuban pecah dini di Indonesia berkisar 4,5% sampai 7,6 % dari seluruh kehamilan. KPD Preterm terjadi pada terjadi pada sekitar 2-3% dari semua kehamilan tunggal dan 7,4% dari kehamilan kembar. Penelitian ini merupakan penelitian observasional analitik secara retrospektif deskriptif cross sectional. Pengambilan data diambil dari rekam medis pasien rawat inap kamar bersalin di RSUD.Ulin Banjarmasin pada periode januari– desember 2017. Pada penelitian ini menunjukkan bahwa sebagian besar variabel faktor risiko yaitu usia, pendidikan, usia kehamilan dan paritas menunjukkan hubungan terhadap kejadian KPD preterm. Hasil analisis menunjukkan terdapat hubungan yang signifikan pada usia ibu 18-20 tahun sebesar 63.6% dengan OR (95%CI)= 7.56 (6.206-7.687). Faktor risiko pada sampel pendidikan paling banyak pada tingkat pendidikan rendah sebesar 65,1% dengan OR (95% CI) = 7.244 (6.172-7.343) dibandingkan dengan ibu tingkat pendidikan tinggi sebesar 37.3%. Pada variable usia kehamilan sampel terbanyak ditemukan pada usia kehamilan 28-34 minggu sebanyak 100% dengan OR (95%CI) = 94.0 (78.2-130.3). Pada variabel paritas terbanyak pada multigravida yaitu 45.9% dengan OR (95%CI) = 2.26-2.32) namun hasil ini tidak signifikan dikarenakan p value >0.005. Ditemukan hubungan faktor risiko pada usia ibu 18-20 tahun, pendidikan rendah, dengan usia kehamilan 28-34 minggu, namun tidak signifikan pada paritas multigravida.Kata-kata kunci : Ketuban pecah dini preterm, kehamilan ABSTRACTPremature rupture of membranes (PROM) are defined as rupture of the membranes before the onset of labor and which occur at or after 37 weeks gestational age is called premature rupture of membranes (PROM) and before 37 weeks gestational age is called preterm premature rupture of membranes (PPROM). The incidence of premature rupture of membranes in Indonesia is from 4.5% to 7.6% of all pregnancies. Preterm KPD occurs at 2-3% of all single pregnancy and 7.4% of twin pregnancy. This study was a cross sectional descriptive observational analytic study. The data was taken from the medical records of Delivery Room in RSUD Ulin Banjarmasin at January-December 2017. In this study, the majority of risk factor, age, education, gestational age and parity showed a relationship to Insidence of PPROM. The results of the analysis showed that there was a significant relationship between the age of 18-20 years percentage was 63.6% with OR (95% CI) = 7.56 (6,206-7,687). The most sample at low education level was 65,1% with OR (95% CI) = 7.244 (6.172-7.343) compared to the high education level was 37.3%. Criteria for gestational age, the most samples were found at 28-34 weeks gestational age with percentage of PPROM was 100% with OR (95% CI) = 94.0 (78.2-130.3). Then at parity most Insidece in multigravida was 45.9% with OR (95% CI) = 2.26-2.32) but this result is not significant because p value> 0.005. ABSTRAK Ketuban pecah dini adalah pecahnya selaput ketuban pada setiap saat sebelum permulaan persalinan tanpa memandang apakah pecahnya selaput ketuban terjadi pada kehamilan 24 minggu atau 44 minggu. Kadar hemoglobin adalah ukuran pigmen respiratorik dalam butiran-butiran darah merah. Kelainan letak janin merupakan malpresentasi janin atau kelainan letak janin yang dapat membuat ketuban bagian terendah langsung menerima tekanan intra uteri yang dominan. Tujuan penelitian ini adalah mengetahui hubungan kadar hemoglobin ibu dan letak janin dengan kejadian ketuban pecah dini di klinik alisa talang keramat kenten Palembang Tahun 2018. Desain penelitian ini menggunakan metode survey analitik dengan pendekatan crossectional. Sampel berjumlah 63 orang. Pengambilan menggunakan teknik total sampling. Pengumpulan data menggunakan lembar checklist.Hasil analisis univariat didapatkan jumlah ibu yang mengalami ketuban pecah dini (22,3%), HB tidak normal (41,3%) dan letak sungsang (6,3%).  Dari hasil analisis bivariat dengan uji statistik Chi – Square ada hubungan bermakna antara kadar hemoglobin ibu p value  = 0,001 < α 0,05 nilai OR = 0,049 dan letak janin p value = 0,001 < α 0,05 nilai OR = 0,032 dengan kejadian ketuban pecah dini secara statistik terbukti. Melalui penelitian ini, diharapkan dapat memberikan pelayanan kesehatan yang lebih baik lagi pada ibu hamil dan bersalin sehingga tidak terjadi komplikasi.   Kata Kunci : Ketuban Pecah Dini, Kadar Hemoglobin, Letak Janin ABSTRACT Premature rupture of membranes isthe  rupture of the membranes at any time before the onset of labor, regardless of whether the rupture of the membranes occurs at 24 weeks or 44 weeks' gestation. Hemoglobin levels are the size of respiratory pigments in red blood granules. Fetal abnormalities are malpresentations of fetuses or fetal abnormalities that can make the lowest part of the membrane directly accept the dominant intrauterine pressure. The aim of this study is to know the correlation between maternal hemoglobin level and fetal location toward the incidence of premature rupture of membranes at klinik alisa talang keramat kenten The study design using analytical survey method with crossectional approach. The sample numbered 63 people. Taking by using total sampling technique. Data collection using checklist sheet. The result of univariate analysis showed that the number of mothers who had membranes rupturedearly was (22.3%), HB was not normal  (41.3%) and breech position was (6.3%). From bivariate analysis with Chi-Square statistical test there was a significant correlation between maternal hemoglobin level p value = 0.001 <α 0.05 value OR = 0.049 and fetal location p value = 0.001 <α 0.05 value OR = 0.032 with incidence of premature rupture of membranes was statistically proven. Through this research, it is expected to provide better health services in pregnant and maternity women so thereis no complications occur. Abstract Background Neonatal sepsis is one of the leading causes of neonatal morbidity and mortality. Despite implementing of different preventive interventions, the burden of neonatal sepsis is reporting in different areas of Ethiopia. For further interventions, identifying its determinants is found to be crucial. Objective This study aimed to identify determinants of neonatal sepsis in the Northwest part of Ethiopia. Methods Unmatched case-control study was conducted among 246 neonates admitted in neonatal intensive care unit, Northwest Ethiopia. Study participants were selected from February 1st to March 30th 2018. Data was collected through face to face interview and review of neonates’ medical records using pretested structured questionnaire. Data was entered into Epi Data version 4.2.0.0 and further transferred to SPSS statistical software version 25 for analysis. All independent variables with p-value < 0.25 in Bivariable analysis were entered into multivariable logistic regression analysis. Finally, variables with p-value < 0.05 were considered as determinants of neonatal sepsis. Results A total of 82 cases and 164 controls were included in this study. Neonates with gestational age < 37 weeks [AOR = 6.90; 95% CI (2.76, 17.28)], premature rupture of membrane [AOR = 2.81; 95% CI (1.01, 7.79)], not crying immediately at birth and have received resuscitation at birth [AOR = 2.85; 95% CI (1.09, 7.47)] were found to be predictors of neonatal sepsis. Conclusions and recommendations Premature rupture of membrane was found to be obstetric-related determinant of neonatal sepsis. Gestational age < 37 weeks, not crying immediately at birth, and have received resuscitation at birth were found to be neonatal-related risk factors of neonatal sepsis. Infection prevention strategies need to be strengthening and/or implementing by providing especial attention for the specified determinants.

Background: Premature rupture of membranes is defined as rupture of foetal membranes before the onset of labour. Management of cases of PROM still remains as one of the most difficult and controversial problems in obstetrics. PROM can cause maternal complications like chorioamnionitis, increased operative procedures, puerperal sepsis and neonatal morbidity and mortality. The present study is undertaken to study the labour outcome, maternal morbidity and perinatal morbidity and mortality in term PROM.Methods: 200 Cases of spontaneous rupture of membranes with gestational age >37 weeks with confirmed PROM by a speculum examination were selected. A detailed history was taken, and gestational age confirmed, general, systemic and obstetric examinations were done. Parameters of maternal and foetal well being were recorded. All study groups received prophylactic antibiotics. Single pelvic examination done, and maternal vitals recorded fourth hourly. All the data was analyzed and statistical significance was calculated using Chi-square test.Results: PROM was common in age group of 20-24 years (35%) with mean age of 22.6 years and SD of 2.8 years, and common in primigravida. Majority of women were admitted within six hours of PROM (41.5%) and Mean duration of induction to delivery interval was 12.9 hours. The mean duration between PROM to delivery was 20.2 hours which was statistically significant. Cesarean sections were more among primigravidas. Failure to progress was the common indication. Maternal morbidity was significant (17.5%). No maternal mortality in the study. Perinatal mortality was 1.5%. Birth asphyxia was the commonest cause. Perinatal morbidity was seen in 26%. Escherichia coli was common organism found in cervical swab culture.Conclusions: In present study, majority was primigravidas and the most common age group was 20-24 years belonging to low socioeconomic status. Maternal morbidity and neonatal morbidity was associated with increased duration of PROM to delivery and infection of the female genital tract with pathogens. Hence an appropriate and accurate diagnosis of PROM is essential for favorable outcome in pregnancy.

ABSTRACT: ANALYSIS OF FACTORS RELATING TO THE EVENT OF NEONATORUM SEPSIS IN THE PERINATOLOGY ROOM RSUD DR. H. ABDUL MOELOEK LAMPUNG PROVINCE IN 2020  Background : Maternal and child health problems are national problems that need to get top priority because they determine the quality of human resources in future generations. Children, especially infants, are more susceptible to diseases and unhealthy living conditions. Therefore, various efforts have been made by various parties to improve the health of infants, especially infants during the perinatal period (Ministry of Health Republic of Indonesia, 2018). Based on data from Dr. RSUD's medical records H. Abdul Moeloek from 2017 - 2019 saw an increase in the incidence of Neonatal sepsis in the Regional General Hospital (RSUD) Dr. H. Abdul Moeloek, where in 2017 there were 193 incidents, in 2018 there were 242 incidents and in 2019 there were 317 sepsis neonatorum incidents.Purpose : to find out the factors related to the occurrence of Sepsis Neonatorum in the Perinatology Room of RSUD Dr. H. Abdul Moeloek Lampung Province in 2020.Methods : This type of quantitative analytic research with a case control approach, the subjects in this study were all babies born in 2019 in RSUD Dr. H. Abdul Moeloek Lampung Province in 2020. The object of the study was the incidence of Sepsis Neonatorum, premature rupture of membranes, infection during pregnancy, LBW, and the sex of the baby. The research will be carried out after the proposal is approved.Research Result : It is known that out of 178 respondents, 144 (80.9%) of respondents did not have sepsis neonatorum, most of the respondents were 113 (63.5%) without PROM, most of the respondents had no infection as much as 158 (88.8%). Most of the respondents who were LBW were 99 (55.6%). There is a relationship between premature rupture of membranes and Sepsis Neonatorum in the Perinatology room of Dr. H. Abdul Moeloek, Lampung Province in 2020, (p value 0,000 OR 17,400) There is a relationship between infection during pregnancy and Sepsis Neonatorum in the Perinatology room of Dr. H. Preterm delivery is the greatest cause of neonatal morbidity and mortality. Preterm delivery can be accompanied by premature rupture of membranes. Preterm delivery with and without premature rupture of membranes are managed by conservative therapy to delay birth. Rupture of membranes causes inadequate protection of the fetus. This study aimed to determine the success of conservative therapy in preterm delivery with and without premature rupture of membranes at Regional Hospital (RSD) dr. Soebandi Jember. The study used secondary data from medical records of pregnant women with spontaneous single preterm delivery that has been given conservative treatment from January 2015-January 2017 period. As many as 149 samples were included for the study, which is collected using total sampling technique. This research used cross sectional research design. Chi-Square statistical test results showed a significance of (p = 0.001 <0.05). It can be concluded that there was a significant difference of the success of conservative therapy between the preterm delivery with and without premature rupture of membranes.
