Working conditions and occupational pathology of Kola Polar miners

Introduction. Despite the improvement of mining technologies and personal protective equipment for employees, working conditions in the mining industry create an increased risk of occupational diseases.The aim of the study is to compare the working conditions and features of professional pathology of miners in the production of iron, apatite and copper — nickel ore in the Kola Polar region.Materials and methods. The data of social and hygienic monitoring on the section “Working conditions and occupational morbidity” of the population of the Murmansk region in 2007–2017 were studied.Results. It was found that in 2007–2017, there was no significant improvement in the working conditions of the studied groups of employees of mining enterprises. The structure of professional pathology of miners was still dominated by vibration disease (37.0%), radiculopathy (23.8%), mono-polyneuropathy (15.5%), sensorineural hearing loss (15.5%), and among the etiologically significant harmful industrial factors — the severity of labor (54.1%), general vibration (23.8%) and noise (13.1%). During the observation period, occupational morbidity in the mining of iron and copper-nickel ore decreased significantly, while in the mining of apatite ore it tended to increase. There is no causal relationship between changes in working conditions and the level of occupational morbidity of miners.Conclusions. It is necessary to improve the quality of hygienic and clinical research when conducting a special assessment of working conditions and periodic medical examinations to obtain an objective assessment of occupational health risks for miners in the Kola Polar region.

Occupational morbidity indicators in the mining industry rank first among all types of economic activity in Russia. The aim of the study was to assess the risks of occupational pathology a posteriori in miners carrying out underground mining of copper-nickel ore in the Kola Arctic. We studied the data of periodic medical examinations and social and hygienic monitoring «Working conditions and occupational morbidity of the population of the Murmansk region.» In 2007, according to the results of medical examinations, 5,007 chronic diseases were identified in 2,042 miners, of which the most common were diseases of the musculoskeletal system (27.7 %) and eye (16.9 %). In 2008–2018, 166 (8.1 %) miners were first diagnosed with 303 occupational diseases, mainly due to increased labor severity (51.5 %) caused by imperfect technological processes (77.6 %). The highest risk of developing occupational diseases was noted among the operators of drilling and tunneling rigs, which significantly exceeded the rates among miners of all other specialties. During labor activity, diseases of the musculoskeletal system most often acquired occupational etiology (41.7 % of all cases). In the structure of nosological forms of occupational diseases, the first places were occupied by vibration disease (21.1 %), radiculopathy (18.5 %) and sensorineural hearing loss (17.5 %). The largest number of cases of occupational diseases per year per 10,000 workers was observed among drillers (540.5 cases) and crushers (441.6 cases), and the smallest — among locksmiths (18.5 cases) and engineering and technical workers (15.5 cases). At present, the modernization of ore mining processes and personal protective equipment, as well as the system of medical measures, do not provide solutions to the problems of preserving the health of the copper-nickel miners of the Kola Polar region.

AbstractAmong many studies on cytisine only a few have been controlled trials, and the aim of this study was to assess the efficacy of cytisine in a randomized controlled double-blind trial compared to placebo in medium-dependent smoking men working in mining industry. Materials and methods: 171 middle-aged smokers were randomised to either cytisine (25-days regimen) or placebo; both groups received individual counseling with brochure. Self-reported continuous abstinence was assessed at 8 and 26 weeks. Results: At the end of week 8 there were no differences in number of abstinent subjects, but at 26 weeks 10.6% of subjects were abstinent in cytisine group compared to 1.2% in placebo (p = .01). In both groups, we did not find any weight increase, but quality of life improved in both groups, and physical and social functioning improved in cytisine group. Conclusions: Cytisine may be an effective medication to help smokers quit even for those working in difficult working conditions with high relapse rate.

Aims - to analyze the historical aspects of the formation of the national normative framework in the field of primary and periodic medical examinations, to discuss the existing problems in the carrying out of primary and periodic medical examinations of workers and also to make suggestions for quality improvement of primary and periodic medical examinations and for introduction of occupational risk assessment into the practice of primary and periodic medical examinations. Conclusion. For quality improvement of primary and periodic medical examinations, for efficiency improvement of primary and secondary prevention of occupational diseases it is considered to be necessary to take the following suggestions: - fuller coverage of working people by periodic medical examinations; - thorough selection of organizations that perform the special assessment of working conditions; - the use of the results of the special assessment of working conditions for improving working conditions and informing workers about working conditions; - implementation of occupational risk assessment into practice of primary and periodic medical examinations; - the use of modern methods of molecular and genetic screening;

A survey of 1,254 miners engaged in mining of copper-nickel ore in the Kola Arctic was conducted with the purpose of identifying the prevalence, risk factors and clinical significance of obesity in this contingent of workers. As a result, obesity was diagnosed in 17 % of the examined persons, more often in women than in men (28.8 % and 16.3 %, p < 0.05). It has been established that the age of the employee and the length of service are the risk factors for developing obesity. On the contrary, there was no evidence of an impact on the obesity development of working conditions in any profession of miners, but the increased severity of labor (class 3.2) reduced the incidence of obesity (18.4 % and 13.9 %, p < 0.02).as well as the severity of labor. It has been shown that obesity increases the risk of developing cardiovascular (RR = 2.03, CI 1.65–2.50), endocrine (RR = 3.63, CI 2.02–6.52), and musculoskeletal (RR = 1.19, CI 1.09–1.30) diseases. The conclusion was made that during the health check-ups and other healthimproving activities it is necessary to pay attention to the issues of healthy nutrition of miners so that it corresponds to the energy costs of labor in modern mining industry.

Introduction. Over the past decade, in Russia and abroad, there has been a clear trend towards an increase in cases of sudden death in the workplace from the disease of the circulatory system. Material and methods. During the study, there were studied the databases of the State Labor Inspectorate in the Republic of Bashkortostan in 2014-2018, including accident investigation reports, information on a special assessment of working conditions, medical examinations, forensic medical examinations and other materials. Results. Diseases of the circulatory system (DSC) were the main cause of sudden death in the workplace in more than 90% of cases. The maximum number of cases of sudden due to DSC was recorded in the age group of 56-60 years. The overwhelming majority of the deceased cases were represented by working occupations - 82.8%, drivers of transport were in 13.7% of cases. The vast majority of deaths occurred in the morning (from 6 to 12) and daytime (from 12 to 18) hours. The working conditions of deceased DCS workers in most cases (59.8%) corresponded to the permissible class (class 2); under harmful conditions (subclass 3.1 - 3.3) 24.9% cases worked. Most often, some form of acute coronary heart disease (CHD) was mentioned as the immediate cause of death (64.0%). Among acute forms of coronary heart disease in medical documents in more than 50% of cases the code I 24.8 “Other forms of acute coronary heart disease” was established. “Acute myocardial infarction” was verified in 11.1% of cases and was the main cause of death in men. In the group “Cerebrovascular Diseases”, “Intracerebral hemorrhage” was most often diagnosed. Conclusion. The obtained results substantiate the need to develop measures to prevent sudden death in the workplace, aimed primarily at preventing the development, progression and early diagnosis of DSC.

Introduction. One of the urgent problems of medicine around the world is the growth of malignant diseases. The impact of the production environment in developing professional and production-related skin malignancies is a sensitive area of occupational health. The high prevalence of precancerosis and the increase in the incidence of skin cancer in people of working age indicates the need to improve diagnostic and preventive measures. One of the most effective recognized non-invasive differential diagnoses of skin tumors with high diagnostic accuracy and sensitivity is dermatoscopy. The study aims to substantiate the importance of dermatoscopic diagnosis of skin neoplasms in workers with industrial carcinogens. The researchers analyzed the available literature and statistical data on the impact of industrial carcinogens on benign and malignant skin lesions in workers in various industries. Specialists from Clinic of the "Izmerov Research Institute of Occupational Health" examined 1,136 people during periodic medical examinations. The features of the signs of neoplasms evaluate according to the principle of their presence or absence. There were two groups: group 1 (n=648) consisted of persons who have contact with industrial carcinogens according to the special assessment of working conditions (SAWC), group 2 (n=488) - persons who do not have contact with carcinogens in the workplace. We found that the frequency of benign skin neoplasms increases depending on the length of work in contact with a production carcinogen. Researchers developed the criteria of clinical and anamnestic signs (the main of which are the working conditions). Dermatoscopic criteria for the initial signs of malignancy of skin neoplasms allow us to form a group of dynamic observations of skin neoplasms in patients working with carcinogens during periodic medical examinations. We formed a group of 38 people (29 from the first and 9 from the second group) for monitoring the dynamics of changes in neoplasms with an appearance to a dermatovenerologist after three months. Specialists sent seven people for additional examination by an oncologist to clarify the degree of existing signs of dysplasia. The formation of a dynamic observation group with the risk of malignancy of skin neoplasms will optimize early diagnosis and ensure high efficiency of treatment in outpatient settings.

In the Republic of Bashkortostan, in 2019, despite the deteriorating working conditions at workplaces in almost all industries, extremely low rates of occupational morbidity were observed. At the same time, almost every third employee of the surveyed types of economic activity is engaged in work with harmful and (or) dangerous working conditions. In total, 46 cases of occupational diseases were found in the Republic in 2019. The occupational morbidity rate was 0.42 per 10,000 employees (2018 — 0.32), which is significantly lower than in the Russian Federation (1.03 per 10,000 employees). The highest levels of occupational morbidity are registered in mining, manufacturing, healthcare, and agriculture. The structure of nosological forms of occupational diseases was dominated by diseases associated with physical overload and overstrain of individual organs and systems: sciatica of the lumbosacral level, polyneuropathy of the extremities and occupational diseases from the influence of physical factors: sensorineural hearing loss and vibration disease. Against the background of the continuing deterioration of working conditions in the Republic and the low quality of periodic medical examinations, extremely low rates of registered occupational diseases are observed.

Introduction. Examining the disease’s connection with the occupation being the cornerstones in occupational health cannot be based on template solutions. When considering the aetiology of the disease, the entire document was the sanitary and hygienic characteristics of working conditions, the quality of which often determines the patient’s fate. Materials and methods. A retrospective analysis of the sanitary and hygienic characteristics of 647 inpatient medical records from 2013 to 2020, aimed at examining the relationship of the disease with the occupation: 562 cards with an established diagnosis, 85 - with an unspecified final diagnosis of an occupational disease. The contingent of the surveyed: 98.0% - employees of various industries and enterprises of the Republic of Bashkortostan, 2.0% - residents of other regions of the country, as well as the republic working in the Far North on a shift basis. Results. When preparing the characteristics in 73.0% of cases, the employees of Federal Service for Supervision in Protection of the Rights of Consumer and Man Wellbeing were based only on the data of a special assessment of working conditions, in which there was no complete information about the working conditions of the employee, which often leads to the inability to establish an occupational disease to the patient. In 7.0% of cases, the values of harmful industrial factors in the presented characteristics were on the border with the hygienic standard, which, with significant work experience and the presence of clear signs of occupational disease in the patient, also puts expert doctors in a difficult position. The situation mentioned above may be the cause of a legal dispute on the part of the employee if he refuses to establish an occupational illness and the employer if the issue is resolved positively for the employee. The reasons for delaying the terms of the examination were the need to request additional documents in connection with the data provided in the characteristics on the nature of the influencing harmful factors, based only on literary sources. Non-compliance with the deadlines for submitting the attributes to the Center of Occupational Pathology was observed in almost 100.0 % of cases. In 75.0% of cases, employees signed it without receiving explanations of quantitative and qualitative indicators. Conclusion. There is an urgent need to review the methodological approaches to compiling sanitary and hygienic characteristics of working conditions, develop an algorithm for its preparation, and the level of responsibility of all stakeholders for the quality of the document.

Introduction. The mining and metallurgical industries form the basis of the economy of the Murmansk Region. Working conditions at industrial enterprises, combined with extreme climatic conditions of the Arctic, pose an increased risk of occupational diseases. The purpose of the study was to investigate working conditions and occupational disease rates among industrial workers of the Murmansk Region in 2007–2017. Materials and methods. We studies socio-hygienic monitoring data on working conditions and occupational diseases in the population of the Murmansk Region in 2007-2017 provided by the Federal Center for Hygiene and Epidemiology of the Federal Service for Supervision of Consumer Rights Protection and Human Welfare, Moscow. Results. In 2008-2017, the most common occupational risk factors in the Murmansk Region included noise (25.8%), job strain (17.8%), and chemical factor (12.6%) while 39.2% of workers were exposed to a combination of occupational hazards. Over 10 years, the absolute number of workers exposed to dangerous and hazardous industrial factors decreased by 1.8 times. In the Murmansk Region, 84.3% of occupational disease cases were registered among workers of mining and metallurgical enterprises. In 2007-2017, the region experienced a significant decrease in occupational disease incidence mainly related to job cuts. The role of other factors (improvement of working conditions, health promotion) needs to be clarified; yet, it is possible to predict their positive influence on occupational morbidity in the next few years. Conclusion. To achieve a substantial decrease in occupational disease incidence rates, it is essential to keep improving comprehensive measures aimed at preserving workers’ health in the Murmansk Region.
