Background: Hypertension is a common health problem among the elderly. Approximately 50% have poor adherence to their prescribed medication, which leads to uncontrolled blood pressure. This study was aimed to understand causes of poor adherence and gain knowledge of patient's self-management in their home. Methods: In-depth interviews were conducted. Elderly hypertension patients with poor adherence to antihypertension medication and uncontrol blood pressure, between the ages of 60-79 were purposively selected to participate in this study. Home visits, including observation of patient's behavior in their home, were conducted. Content analysis was performed. Results: A total of thirty respondents were interviewed of which 17 were female and 13 were male. All understood that hypertension is a chronic health problem but most of them were unaware that nonadherence to their hypertension medication treatment would lead to negative consequence to their health. Most respondents had negative beliefs toward hypertension treatment. The reasons for poor adherence are: 1) personal beliefs or habits, which include forgetfulness and negative belief that the medication will not control blood pressure; 2) side effects of medication and 3) negative perceptions influenced by the community. Research results showed that most participants were not taking their prescribed dosage. Additionally, expired medication was commonly found in their medication inventory. High sodium diets and no regular exercise was commonly reported. Conclusion: Patients understood that hypertension is a lifelong condition but unaware of consequence of poor adherence. Personal reason and negative belief as well as wrong belief influenced by community. Forgetfulness and manage dosage of medication should be addressed when designing a strategy to improve medical adherence. Education on controlling hypertension and treatments should be addressed and incorporated into a holistic strategy to improve adherence to the patient's drug regimen.

AbstractHypertension is the most common degenerative disease suffered by the elderly. It caused by variuos factors, such as age, history of hypertension, high sodium consumption and lack of physical activity. Exercise suc as low impact aerobic can reduce heart rate related to decrease in cardiac output. Consequently, blood pressure will decrease gradually. The purpose of the study was to describe the effect of low impact aerobic exercise on blood pressure in elderly with hypertension. The study used a literature review, by accessing the Garuda and ProQuest database using the keyword “Hypertension” AND “Elderly” AND “Low Impact Aerobic Exercise” AND “Blood Pressure”. The result show that there is an effect of low impact aerobic exercise on blood pressure in the elderly with hypertension. After the interventions, the systolic and dyastolic blood pressure was decrease 10.16 mmHg ang 5,02 mmHg, respectively. Low impact aerobic exercise is effective in decrease blood pressure in elderly with hypertension. Low impact aerobic exercise can be an alternative to overcome the problem of hypertension among elderly in family, community and gerontology nursing settings.Keywords: Blood Pressure, elderly, Hypertension, low Impact Aerobics AbstrakHipertensi merupakan penyakit degeneratif yang paling bnayak diderita oleh lansia. Hal ini disebabkan oleh berbagai faktor, seperti usia, riwayat penyakit hipertensi, konsumsi tinggi garam dan kurangnya aktivitas fisik. Latihan fisik seperti senam aerobik low impact dapat menurunkan denyut jantung dimana terjadi penurunan pada cardiac output yang akhirnya dapat menurunkan tekanan darah. Tujuan dari penelitian ini yaitu untuk mendeskripsikan pengaruh senam aerobik low impact terhadap tekanan darah pada lansia dalam literature review. Metode yang digunakan dalam penelitian ini adalah literature review, dengan mengakses database Portal Garuda dan Proquest dengan menggunakan kata kunci “Hipertensi” DAN ”Lansia” DAN ”Senam Aerobik Low Impact” DAN ”Tekanan Darah”. Hasil penelitian ini menunjukkan bahwa terdapat pengaruh senam aerobik low impact terhadap tekanan darah pada lansia dengan hipertensi yang ditandai dengan adanya penurunan nilai sistole dan diastole setelah diberikan intervensi senam aerobik low impact. Hasil penelitian ini diketahui terdapat pengaruh senam aerobik low impact terhadap tekanan darah pada lansia dengan hipertensi, diharapkan dapat menjadi referensi di keperawatan keluarga maupun komunitas pada lansia untuk meningkatkan aktivitas senam aerobik low impact dalam mengatasi masalah hipertensi pada lansia.Kata kunci : Hipertensi, Lansia, Senam Aerobik Low Impact, Tekanan Darah

ABSTRAKHipertensi merupakan masalah kesehatan yang membutuhkan perhatian karena menyebabkan kematian yang tidak dapat diidentifikasi penyebabnya. Prevalensi Hipertensi yang ada di Indonesia dari tahun ke tahun mengalami peningkatan yang signifikan. Faktor utama penyakit Hipertensi di sebabkan oleh faktor pola hidup yang tidak sehat. Kegiatan pengabdian kepada masyarakat ini mempunyai tujuan untuk memberikan pemahaman kepada lansia agar dapat memahami dan mengetahui gejala hipertensi, faktor penyebab hipertensi, komplikasi dari penyakit hipertensi, dan cara menurunkan tekanan serta dapat membantu masyarakat dalam mengurangi angka peningkatan tekanan darah atau Hipertensi.  Pelaksanaan kegiatan dilakukan di Dusun Gupak Warak, Desa Sendangsari, Bantul Yogyakarta.  Kegiatan diikuti oleh 30 lansia dengan metode ceramah dan pemeriksaan kesehatan. Hasil evaluasi akhir dengan metode tanya jawab menujukan bahwa  terjadinya peningkatan pemahaman dan pengetahauan yang dimiliki oleh lansia tentang hipertensi. Kata kunci: hipertensi; penyuluhan kesehatan; lansia ABSTRACTHypertension is a health problem that requires attention because it causes death whose cause cannot be identified. The prevalence of hypertension in Indonesia from year to year has increased significantly. The main factor of hypertension is caused by unhealthy lifestyle factors. This community service activity has the aim of providing understanding to the elderly so that they can understand and know the symptoms of hypertension, the factors that cause hypertension, complications of hypertension, and how to reduce pressure and can help the community in reducing the increase in blood pressure or hypertension.  The activity was carried out in Gupak Warak Hamlet, Sendangsari Village, Bantul Yogyakarta. The activity was attended by 30 elderly people with lecture methods and health checks. The results of the final evaluation using the question and answer method showed that there was an increase in the understanding and knowledge possessed by the elderly about hypertension Kata kunci: hypertension; health education; elderly

Prevention of hypertension can help prevent cardiovascular disease and renal complications. Obesity, a high sodium and low potassium intake, physical inactivity, and high alcohol consumption all contribute to the development of hypertension, and randomized controlled trials have shown that appropriate lifestyle modifications are able to reduce blood pressure and/or prevent the development of hypertension. The major complications of hypertension are stroke, coronary heart disease, heart failure, peripheral artery disease, and chronic kidney disease. Multiple randomized controlled trials and their meta-analyses have shown that treatment with antihypertensive drugs reduces the incidence of fatal and non-fatal cardiovascular events. In addition, meta-analyses have shown that there are no clinically relevant differences in the effects of the five major drug classes on outcome, so all of them are considered suitable for the initiation and maintenance of antihypertensive therapy. Nevertheless, the therapeutic approach in the elderly, women, and patients with diabetes, cerebrovascular, cardiac, or renal disease deserves special attention.

ABSTRACTBackground: Hypertension is one of the most common diseases, because this disease is suffered byboth men and women, as well as adults and young people. Treatment of hypertension does not onlyrely on medications from the doctor or regulate diet alone, but it is also important to make our bodyalways relaxed. Laughter can help to control blood pressure by reducing endocrine stress andcreating a relaxed condition to deal with relaxation.Objective: The general objective of the study was to determine the effect of laughter therapy ondecreasing elderly blood pressure in UPT Panti Wredha Budhi Dharma Yogyakarta.Methods: The design used in this study is a pre-experimental design study with one group pre-posttestresearch design where there is no control group (comparison). The population in this study wereelderly aged over> 60 years at 55 UPT Panti Wredha Budhi Dharma Yogyakarta. The method oftaking in this study uses total sampling. The sample in this study were 55 elderly. Data analysis wasused to determine the difference in blood pressure before and after laughing therapy with a ratio datascale that was using Pairs T-TestResult: There is an effect of laughing therapy on blood pressure in the elderly at UPT Panti WredhaBudhi Dharma Yogyakarta marked with a significant value of 0.000 (P <0.05)

BACKGROUND. Increased salt intake is associated with a number of cardiovascular events, including increased blood pressure (BP) and the development of left ventricular hypertrophy (LVH). However, there is much evidence that a high content of sodium chloride in the diet does not always lead to an increase in BP, but almost inevitably causes cardiac remodeling, in particular, LVH. Many aspects of myocardial remodeling induced by high sodium content in the food have not been studied enough. THE AIM of the study was to trace the echocardiographic changes in Wistar rats fed the high salt ration and the high salt ration supplemented with soy proteins.MATERIAL AND METHODS. Echocardiography and BP measurements were performed on male Wistar rats, divided into three groups. The first (control; n = 8) included rats that received standard laboratory feed (20.16 % animal protein and 0.34 % NaCl); the second (n = 10) – animals that received standard feed and 8 % NaCl (high salt ration). The third group (n = 10) consisted of rats who consumed a low-protein diet containing 10 % soy protein isolate (SUPRO 760) and 8 % NaCl. The follow-up period was 2 and 4 months.THE RESULTS of the study showed that: (1) the intake of a large amount of salt with a diet does not necessarily lead to the formation of arterial hypertension; (2) despite the absence of a distinct increase in BP, under these conditions signs of cardiac remodeling, in particular, LVH, appear rather quickly; (3) supplementing a high-salt diet with soy isolates counteracts the development of LVH.CONCLUSION. High salt intake with food can cause heart remodeling, regardless of blood pressure, while soy proteins can counteract this process.

ABSTRAKMasih tingginya penderita hipertensi merupakan salah satu masalah yang berpengaruh terhadap gaya hidup dan sikap yang mendorong timbulnya hipertensi. Untuk menangani tekanan darah pada penderita hipertensi umumnya minum obat-obatan dari puskesmas, rutinitas ini sering tidak disukai oleh penderita. Penelitian ini bertujuan untuk mengetahui efektifitas air rebusan daun seledri terhadap perubahan tekanan darah pada lansia di Posyandu Ngudi Konco Desa Donomulyo Kecamatan Donomulyo Kabupaten Malang.Desain penelitian ini Pre-Experimental dengan rancangan One- Group Pre-Post test design. Populasi dalam penelitian ini adalah semua lansia penderita hipertensi di Posyandu Ngudi Konco Desa Donomulyo Kecamatan Donomulyo Kabupaten Malang sebanyak 30 responden. Sampel diambil dengan teknik total sampling. Variabel penelitian yaitu pemberian air rebusan daun seledri sebagai variabel independent dan tekanan darah sebagai variabel dependen. Data yang di dapat melalui lembar observasi tekanan darah kemudian di uji dengan menggunakan uji paired t Test.Hasil uji paired t Test menunjukkan bahwa p = 0,000 α= 0,05 Ho di tolak dan H1 diterima sehingga air rebusan daun seledri efektif terhadap perubahan tekanan darah pada lansia di Posyandu Ngudi Konco Desa Donomulyo Kecamatan Donomulyo Kabupaten Malang.Terdapat penurunan jumlah penderita hipertensi sebelum dan sesudah diberikan air rebusan daun seledri. Responden agar dapat meningkatkan informasi tentang pelaksanaan tekanan darah sehingga dapat mengurangi tekanan darah yang dialami dengan cara non formakologi sehingga responden tidak hanya menggantungkan pada obat-obatan farmakologi dalam menurunkan tekanan darah.Kata kunci : Air Rebusan Daun Seledri, Tekanan Darah, Lansia ABSTRACTStill high hypertension was  one of the problems that affect the lifestyle and attitudes that encourage the emergence of hypertension. This study aims to determine the effectiveness of boiling water celery leaves on changes in blood pressure in the elderly at Posyandu Ngudi Konco Donomulyo Village Donomulyo District Malang Regency.The design of this research was  Pre-Experimental with the design of One- Group Pre-Post test design. Population in this research was all elderly patient of hypertension in Posyandu Ngudi Konco Village Donomulyo District Donomulyo Malang Regency as many as 30 respondents. Samples were taken with total sampling technique. The research variable was giving boiled water of celery leaves as independent variable and blood pressure as dependent variable. The data can be through blood pressure observation sheet then tested by using paired t test.The result of paired t - test showed that p = 0.000 α = 0.05 Ho in rejection and H1 accepted so that boiling water celery leaf effective against blood pressure changes in elderly at Posyandu Ngudi Konco Donomulyo Village Donomulyo District Malang Regency. There was a decrease in the number of hypertensive patients before and after being given boiled water of celery leaves. Respondents in order to improve information about the implementation of blood pressure so as to reduce blood pressure experienced by non-formakologi way so that respondents not only rely on pharmacological drugs in lowering blood pressure. Key Words: Water Stew of Celery leaves, Blood Pressure, Elderly

Resistant hypertension (R-HTN) implies a higher mortality and morbidity compared to non-R-HTN due to increased cardiovascular risk and associated adverse outcomes—greater risk of developing chronic kidney disease, heart failure, stroke and myocardial infarction. R-HTN is considered when failing to lower blood pressure below 140/90 mmHg despite adequate lifestyle measures and optimal treatment with at least three medications, including a diuretic, and usually a blocker of the renin-angiotensin system and a calcium channel blocker, at maximally tolerated doses. Hereby, we discuss the diagnostic and therapeutic approach to a better management of R-HTN. Excluding pseudoresistance, secondary hypertension, white-coat hypertension and medication non-adherence is an important step when diagnosing R-HTN. Most recently different phenotypes associated to R-HTN have been described, specifically refractory and controlled R-HTN and masked uncontrolled hypertension. Optimizing the three-drug regimen, including the diuretic treatment, adding a mineralocorticoid receptor antagonist as the fourth drug, a β-blocker as the fifth drug and an α1-blocker or a peripheral vasodilator as a final option when failing to achieve target blood pressure values are current recommendations regarding the correct management of R-HTN.
