Oral cavity diseases are common among small domestic animals; they can be of traumatic, inflammatory, bacterial, viral or mycotic origin. Pathologic process affecting superficial as well as deep layers of mucous coat of gingivae, ligamentous apparatus, periosteal coverage develops secondary to deterioration of metabolic processes or traumas and in any case is followed by decrease of immunological status of organism as a whole as well as local immune response to introduction of pathogen. Sharp pain syndrome and inflammatory edema of tissues troubles eating and drinking which additionally weakens the animal's organism and leads to even worse course of disease. The necessity of conducting research is dictated by the lack of information concerning the use of condylar blocking using immunomodulating and antibacterial drugs as a part of complex therapy of oral cavity diseases among small pets. Clinically disease declared itself in salivation of different intensity, hyperemia, anabrosis and ulcers with festering incrustations on mucous coat of oral cavity, swelling of submandibular lymphonodus, troubled eating and drinking or complete incapability to eat or drink as a result of sharp painfullness. In such cases the use of procaine block of supramaxillary and mandibular branches of trigeminus (V) which innervates skin and mucous coat of lips, gingivae, bottom of oral cavity, front parts of tongue, hard and soft palates and notal and ventral branches of buccal nerve (VII) which innervates cheeks, lips and nose muscles, joining together along their bodies and with branches of infraorbital nerve, temporal-auricular and mental nerve, sufficiently improves the state of animal by removing pain syndrome and as a result restoring blood circulation in damaged areas. Our work describes the methodology of condylar blocking with small pets as a part of complex therapy of diseases of oral cavity. Two new methods of blocking have been developed and tested. Anatomic assessment of effectiveness of different compositions of condylar blocks with cats and dogs has been made. In our work we studied the effectiveness of drug «Forvet®» as part of complex therapy of animal diseases of oral cavity using blocks of notal and ventral branches of buccal nerve (VII), supramaxillary and mandibular branches of trigeminus (V).

Introduction. Degenerative-dystrophic changes in the spine are the most common chronic human diseases. Pronounced clinical manifestations of vertebrogenic diseases are observed during active labor activity and represent one of the most frequent causes of temporary disability. It is also known that any pain syndromes are accompanied by the development of psychovegetative disorders, which reduces the effectiveness of treatment. In recent years, there has been a reasonable increase in the interest of the medical community to the non-drug treatment methods. It is caused by the polypharmacotherapy side effects, an increased numbers of allergic reactions, problems with polypragmasia, and a low level of compliance. At the same time, the question of the possibility of various non-drug treatment methods combining is quite acute.The goal of research is to substantiate the clinical effectiveness of the combined application of osteopathic correction and reflexotherapy in the complex treatment of patients with dorsopathy at the cervical-thoracic level.Materials and methods. A prospective controlled randomized study was conducted on the basis of the medical clinics of LLC «Family Health» (Kazan) and ANO «Center for SEB assistance» (Kazan) from April 2018 to May 2019. The study involved 52 people with dorsopathy at the cervical-thoracic level, aged from 25 to 45 years. In accordance with the exclusion criteria, 7 people dropped out of the study. The remaining patients (45 people), depending on the used treatment method, were divided by a simple randomization method using a random number generator into three groups of 15 people. The first group (main group) received osteopathic correction and reflexotherapyon the background of standard drug therapy; the second and the third groups (control groups) received only osteopathic correction orreflexotherapy, respectively, on the background of standard drug therapy. The osteopathic examination was performed for all patients regardless of the group, before and after the course of therapy, with the formation of an osteopathic conclusion.Also there was the estimation of the degree of pain syndrome severity,the asthenia, and the degree of accumulated emotional and energy charge that does not get out in a person.Results. It was found in the course of the study that the inclusion of osteopathic correction and reflexotherapy in the complex therapy of patients with dorsopathy at the cervical-thoracic level is clinically more effective than the isolated use of these methods. Such complex therapy allows to achieve a more significant reduction in the severity of the pain syndrome by VAS (p<0,05), relief of internal emotional tension (p<0,05), and increase the effectiveness of correction of dominant somatic dysfunction. Based on the obtained data, it could be assumed that reflexotherapy potentiates the effects of osteopathic correction.Conclusion. Based on the study, it can be concluded that the combination of osteopathic correction and reflexotherapy in the treatment of patients with dorsopathy at the cervical-thoracic level increases the clinical effectiveness of the standard drug therapy. At the same time, it is worth noting the potentiating effect of the combined use of two non-medicinal methods. The question of combining of various non-drug treatment methods is quite acute today, so this study will be continued.

Since the onset of war in Iraq and Afghanistan in April 2002, much attention has been given to the effect of war on servicemen and servicewomen who have now been serving in combat for over thirteen years, the longest sustained war in American history. Many service members have served multiple tours in Iraq and Afghanistan and suffered from the visible and invisible wounds of war. Much work has been done in the Veterans Administration, the Department of Defense, and the civilian sector after observing the effects of multiple deployments and overall military service on the service member. A survey of the literature revealed that the ethics of conducting research on programs to assist these brave men and women is fraught with ethical concerns based on a military culture that often precludes autonomy and privacy. While strides have been made in developing strategies to assist service members deal with their military service issues, a serious lack of information exists on the impact of a parent's service on the health and well-being of military children. A discussion of current research on services for children is presented with an analysis of the ethical problems that have precluded adequate study of those who need society's help the most.

Traumatic trigeminal neuropathy occupies a special place in the pain continuum. The clarification of genesis and clinical and neurophysiological findings makes it possible to perform differentiation treatment.Objective: to evaluate the clinical and neurophysiological efficiency of repetitive magnetic stimulation (RMS) and vitamin B complex therapy for traumatic trigeminal neuropathy.Patients and methods. The investigation enrolled 36 patients (26 women and 10 men) aged 25 to 35 years with inferior alveolar neuropathy following bilateral sagittal split osteotomy. The DN4 questionnaire was used to identify a neurogenic pain component. The intensity of pain syndrome was assessed using a visual analogue scale. A neurophysiological examination involving the recording of brainstem auditory evoked potentials (BAEPs) and trigeminal evoked potentials (TEPs) was made using a Neuro-MEP device (Neurosoft, Russia). Therapy including vitamin B complex was performed in 12 patients. Twenty-four patients received low-frequency pulsed magnetic field therapy using a Neuro-MS magnetic stimulator.Results and discussion. The clinical picture in patients with traumatic inferior alveolar neuropathy after corrective mandible surgery is characterized by the polymorphism of pain sensations and sensory disorders. The development of pain syndrome is due to a neuropathic component. The 10-day vitamin group B therapy cycle had no substantial impact on the time course of clinical and neurophysiological changes. After the 10-day RMS cycle, there were reductions in swelling and the intensity of pain syndrome and the severity of sensory disorders in the lower lip, chin, and mandible. The data on BAEPs showed shortening in the interpeak intervals III–V; those on TEPs demonstrated a decrease in the P1–N1 amplitude.Conclusion. Unlike vitamin B complex therapy, the RMS cycle in patients with traumatic trigeminal neuropathy makes it possible to reduce the intensity of pain syndrome and the severity of sensory disorders, as well as excitability of the nonspecific structures of the brainstem and the central structures of the trigeminal system.

The Aim of the Research. Based on the analysis of modern literature data and our own research to substantiate the principles of rational complex therapy with acute poisoning by thallium compounds (Tl+) at different stages of intoxication. Materials and Methods. The analysis of literature data and our own researches is carried out and the principles of rational complex therapy with acute poisoning by thallium compounds at different stages of intoxication development are substantiated and the algorithm of doctors’ actions at suspicion of acute Tl+ poisoning is defined. Results and Conclusions. The principles of complex therapy of acute Tl+ poisonings substantiated, which includes antidote therapy, methods of extracorporeal treatment (plasmapheresis, hemosorption, hemodialysis), treatment of toxic encephalopolyradiculoneuropathy, neuropathic pain syndrome and toxic liver damage. The sequence of appearance of the main symptoms of intoxication at low and high doses of Tl+ is determined, which allows to assume poisoning by this toxicant. Introduction. Radiculopathy is one of the causes of neuropathic pain. Among the causes of back pain, lumbar radiculopathy ranks second after non-specific musculoskeletal pain. Patients with radiculopathy have a marked decrease in the quality of life, a tendency to disability, which causes a large amount of diagnostic and therapeutic measures and associated economic losses. There is no unified view on the pathogenesis of radiculopathy, and the pain syndrome includes neuropathic and nociceptive components. The proposed treatment regimens for radiculopathy differ and are of a recommendatory nature.The purpose of the retrospective observational study was to study the effectiveness of the drug Neurobion® in the complex therapy of radiculopathy.Materials and methods. The study of outpatient records of 120 patients aged 25–65 years, suffering from lumbosacral radiculopathy was performed. The patients were divided into 2 groups and received basic therapy, which included NSAIDs, simple analgesics, muscle relaxants, and anticonvulsants. Patients of group 1 additionally received the drug Neurobion®.Results and discussion. Faster regression of clinical manifestations was observed in group 1 patients. The results of treatment in group 1 patients were higher both in terms of subjective feelings and objective criteria. The presence of two medicinal forms of Neurobion® allows a personalized approach to the patient’s therapyConclusions. The use of the drug Neurobion® is pathogenetically justified in the treatment of radiculopathy, provides earlier positive dynamics of subjective feelings of patients and objective indicators, improves the quality of life, leads to shorter treatment periods, reducing economic costs. The effectiveness and good tolerability of the drug Neurobion® allow us to recommend it for inclusion in the complex therapy of radiculopathy.

Introduction. Glaucoma is a chronic eye disease characterized by a constant or periodic increase in intraocular pressure, a decrease of visual fields and visual acuity, and by a special form of optic nerve atrophy with excavation in the disc area. The reflection of this slowly fl owing atrophy is a disturbance in the visual field and a complete irreversible loss of visual function. The prevalence of the disease increases with increasing age. The main methods of treatment are medical and surgical. Non-penetrating operations are recognized as the safest methods of surgical treatment of glaucoma. The most common complications of such operations include: detachment of the choroid, small anterior chamber syndrome, hyphema, cicatricial changes in the filtration cushion. Based on the analysis of literature data, it can be assumed that the inclusion of osteopathic correction in the complex therapy of patients with operated glaucoma can increase the effectiveness of therapy.The aim of the research was to study the clinical efficacy of osteopathic correction in the complex treatment of patients operated on primary open-angle glaucoma.Materials and methods. The study involved 20 patients (20 eyes) aged 70 to 75 years with developed and advanced stages of primary open-angle glaucoma, who underwent surgical treatment. The main (10 people) and control (10 people) groups were formed by simple randomization. Participants in the main group received standard medical therapy and osteopathic correction, while participants in the control group received only medical therapy. Changes in the following clinical parameters were evaluated: visual acuity, visual fi eld, intraocular pressure, thickness of the retinal nerve fiber layer, and severity of pain syndrome. Indicators were recorded at the beginning of the study (2 weeks after surgery) and at the end of the study (2–3 months after surgery).Results. Patients receiving osteopathic correction as part of the complex therapy after surgery of primary openangle glaucoma are characterized by a statistically significant (p<0,05) decrease in the severity of pain syndrome, an increase in the magnitude of the visual field and of the thickness of the retinal nerve fiber layer.Conclusion. The obtained results suggest the effectiveness of the inclusion of osteopathic correction in the complex treatment of patients with operated on primary open-angle glaucoma. It is recommended to continue the study with a larger sample size.

Subject. Microbial landscape of the oral cavity is a highly sensitive systemic indicator reacting to changes in different body organs and systems by qualitative and quantitative shifts. The unique ability of microbiocenosis of the oral cavity to self-regulate and maintain the ecological balance creates a personal health profile of an individual and assumes that a timely correction of microbiocenosis shall be added to required treatment patterns for various dental and somatic diseases. This approach ensures the protective effect of growth of opportunistic pathogenic bacteria improving quality of therapy and patient recovery time. Objectives. To study dynamics of clinical and microbiological changes in the oral cavity in patients with periodontitis and hypothyroidism on the background of complex therapy with inclusion of synbiotic Maxilac. Materials and methods. Based in the Ural State Medical University Department of Therapeutic Dentistry and Propedeutics of Dental Decease, the study of oral microbiology was carried out on 50 generalized moderate periodontitis patients aged 44 to 59 years during the period from 2017 to 2021. The average age of the subjects was 50.50±3.5 years. Two groups were formed: the main group (30 patients with “primary hypothyroidism” receiving hormone therapy) and the comparison group (20 patients). The patients in the main group were prescribed general therapy by the endocrinologist based on the phase, stage and the severity of the disease. During examination all patients underwent the complex clinical and laboratory evaluation before treatment, in its course and after it, as well they were subjected to full mouth debridement and the course of the complex periodontal therapy in accordance with clinical recommendations. Microbiota was corrected by active ingredient probiotic (lyophilized probiotic bacteria) — 4.5*109 CFU (colony-forming unit): Lactobacillus elveticus, lactis, rhamnosus, plantarum, casei; Bifidobacterium longum, breve, bifidum; Sreptococcus thermophilus. Results. The study of microbiocenosis of the oral cavity of patients with primary hypothyroidism demonstrated changes in species and the number of microorganisms in the oral cavity. Conclusions. Timely diagnostics and correction of disbiotic changes included in the dental treatment protocol will allow to select the most appropriate dental treatment strategy.

Atopic dermatitis is a complex multifactorial disease in which the essential microelement (zinc) is deficient, which adversely affects the course of the disease. The use of activated zinc pyrithione in the complex therapy of patients with atopic dermatitis is justified by a wide spectrum of it’s pharmacological action at the molecular level, which allows achieving it’s maximum absorption rate and increasing the ability to penetrate to deep layers of the skin. The use of activated zinc pyrithione in patients with atopic dermatitis leads to reduction of skin manifestations, and to decreasing of itch and the severity of the disease.


Summary: The problem of rational antibiotic therapy of pneumonia is among the most relevant in modern medicine. The aim of the study was to evaluate the clinical efficacy and therapeutic tolerance of parenteral application of clarithromycin in the treatment of patients with non-hospital pneumonia (NHP). Materials and methods. We observed 20 patients: 12 men - (60%), 8 women - (40%), the average age of patients was 46.1 ± 17.6 years. All patients showed clinical symptoms of severe infection of the lower respiratory tract. In 75.0% (15 patients) bilateral lung damage was noted, in 27% (5 patients) - unilateral lung tissue damage. Hemoptysis was observed in 4 (20%) patients. Clarithromycin-MB was included in complex therapy (detoxification, mucolytic drugs, multivitamins, and metabolices) for 6 patients with NHP after 3 days of ineffective starting therapy, 14 patients with emergency received clarithromycin-MB immediately after admission to the hospital. The drug was administered by intravenous drip for 60 minutes at a dose of 500 mg 2 times a day for 7-10 days. Result. The analysis of the obtained results of clarithromycin-MB therapy showed that clinical success was achieved in all patients. The positive dynamics of clinical indicators was noted already on the 3rd day from the beginning of antibiotic therapy with clarithromycin-MB, which was expressed in a decrease in body temperature until the 7th day of treatment, and in almost all patients the temperature returned to normal and only in 1 (5%) patient remained subfebrile. Also, during this period, patients noted a decrease in pain syndrome as well as signs of intoxication. According to the data of X-ray studies on the 10th day of therapy, 6 (30%) patients showed complete disappearance of infiltrative changes in the lungs, in 14 (70%) patients - a significant decrease in their severity. On the 15-24th (average 15.3 ± 1.2) days after the start of therapy signs of inflammatory infiltration in the lungs leveled off on the 15-24th in all patients. Conclusion. Clarithromycin-MB has good therapeutic tolerance, allows maintaining the required concentration in the focus of inflammation because of dosage regimen also affects the clinical and bacteriological effectiveness of therapy.
