Introduction: Chronic rhinosinusitis may require referral to an ear, nose, and throat specialist for possible endoscopic sinus surgery if medical management fails. Vitamin D is one of the essential vitamins for the body that is effective in inflammatory processes. Therefore, it seems necessary to confirm the association between the deficiency of this vitamin and the occurrence of chronic rhinosinusitis with nasal polyposis. This study aimed to determine the relationship of vitamin D3 deficiency and chronic rhinosinusitis with nasal and sinus polyposis in patients referring to the Otorhinolaryngology Department of Valiasr Hospital, Birjand, Iran, in 2017. Methods: A case-control study was performed on individuals, including a group of patients with rhinosinusitis and a control group (n=20 each), referring to the Department of Ear, Nose, and Throat Diseases Department of Vali-asr Hospital. Among patients diagnosed with chronic rhinosinusitis, the cases that had polyps on endoscopic examination were included in the study. After completing the consent form, venous blood samples (10cc) were collected from the patients in fasting conditions. The electrochemical luminescence method was used for measuring the level of serum vitamin D. A questionnaire containing demographic information and clinical findings was completed by reviewing the patients' records. Data analysis was performed in SPSS software (Version. 22)using Chi-square and Mann-Whitney U-tests. Results: The mean and median scores of vitamin D level were obtained at14.13±12.99 and 10.25 in the case group, and 18.72±9.29 and 18.77 in the control group, respectively. The level of vitamin D was significantly higher in the control group than in the chronic group (P=0.04). In the case group, 16 (80%) patients lacked vitamin D and 3 patients had an insufficient level of vitamin D. In the control group, 13 (65%) patients lacked vitamin D and 4 cases had an insufficient level of vitamin D. There was no significant difference in vitamin D levels between the two groups (P=0.61). No significant difference was observed between the two clinical symptoms. Conclusion: It was revealed that the lack of vitamin D was likely to be an effective factor in the rhinosinusitis disease; therefore, proceedings need to be taken to cure the deficiency of this vitamin.

Introduction: Metabolic syndrome includes a range of disorders that increase the risk of cardiovascular disease and diabetes mellitus. In this study, we examined the serum level of vitamin D3 in diabetic individuals with metabolic syndrome compared with non-diabetic individuals without metabolic syndrome and the association of serum vitamin D3 levels with metabolic syndrome and atherogenic factor (LDL/HDL). Material and Methods: In a case-control study, we included 110 women with metabolic syndrome according to ATP III criteria and 127 healthy women as a control group. Serum concentration of total cholesterol, LDL-C, FBS, HDL-C and serum triglyceride determined by enzymatic method and colorimetric and, serum level 25-(OH) vitamin D determined by ELISA. Results: It was found that the two healthy and metabolic groups were significantly different in terms of total cholesterol levels, LDL and triglyceride levels, HDL, VLDL, FBS, atherogenic index (LDL/HDL) and vitamin D levels (p<0.05). All participants in the control group and the patient and the whole study population were divided into two categories of insufficient and sufficient based on their measured serum concentrations of 25-(OH) vitamin D. There was a significant difference between the group with insufficient levels of vitamin D in comparison with the group with sufficient levels of vitamin D in terms of total cholesterol, LDL and triglyceride levels, HDL, VLDL, FBS and atherogenic index (LDL/HDL) (p=0.000). Conclusion: The present results showed that there is a significant relationship between level 25-(OH) D and atherogenic index (LDL/HDL) and the incidence of metabolic syndrome.

Despite the abundant sunlight, vitamin D deficiency is prevalent in South Asian countries including Bangladesh. Information on vitamin D level is insufficient in adults particularly in female garment workers in Bangladesh. This study was designed to evaluate the status of vitamin D, parathormone (PTH), calcium, and alkaline phosphatase (ALP) among the female garment workers in Bangladesh. Blood samples were collected from female garment workers (n=40, case group) and general female workers (n=40, control group) in Dhaka. Serum vitamin D, PTH, calcium, and ALP were measured by chemiluminescence microparticle immunoassay. The mean level of vitamin D was significantly (p<0.001) lower in case (14.2±2.6 ng/mL) than in the control (22.4±2.4 ng/mL) group. No significant difference was found at mean of PTH and calcium between case (33.9±17.2 pg/mL;9.1±0.6 mg/dL, resp.) and control (35.9±16.3 pg/mL;9.3±0.6 mg/dL, resp.) group. The mean ALP in case (117.2±14.4 U/L) group was significantly (p<0.001) higher than the control group (80.5±30.6 U/L). Overall, PTH level did not show significant correlation with vitamin D. However, calcium and ALP levels showed a significant positive (p<0.05) and negative (p<0.001) correlation with vitamin D, respectively. This study indicates a high prevalence of vitamin D deficiency in the female garment workers in Bangladesh.

Objective: To determine the association of long term sodium valproate monotherapy and vitamin D3 levels in epileptic children Methods: This cross-sectional study was conducted in the Department of Pediatrics, Children Hospital, Pakistan Institute of Medical Sciences, Islamabad for six months from 15th February 2019 to 14th August 2019. A total of one hundred and thirty (n=130) children and adolescents of either gender between age 3-18 years who had a history of two seizures at least 24 hours apart in their life and were on sodium valproate monotherapy for more than one year were enrolled in this study through non-probability, consecutive sampling. Serum vitamin D3 (25-hydroxy vitamin D) levels were measured in all the patients at the time of enrolment into the study. All the demographic data and laboratory investigations were entered on the predesigned proforma and analyzed through SPSS version 17. Results: Vitamin D3 deficiency was found in 47 (36.2%) children which were significantly higher among patients with older age and longer duration of treatment (P<0.05) while gender and BMI of the patients did not show any significant difference (P>0.05). Conclusion: Significant percentage of epileptic children on sodium valproate monotherapy was found to have vitamin D3 deficiency. Therefore we recommend routine screening of vitamin D3 deficiency in all the epileptic children on long-term sodium valproate therapy followed by vitamin D supplementation in deficient patients.

Introduction: The International Diabetes Federation (IDF) Announces 415 million people around the world are suffering from diabetes. There is a high proportion of UDM (undiagnosed diabetes) at the level of the world and particularly in developing countries. The number of people with UDM in Iran in 2015 are 2197.96 per 1000. Vitamin D3 deficiency is associated with increased risk of type 2 diabetes and for a long time, been recognized as a risk factor for glucose intolerance. Salivary composition, as the mirror of oral health, its use as a diagnostic tool is increasing and diabetes is also can be effective on the flow rate of saliva and its compounds. Analysis of saliva can be used as part of the evaluation of endocrine function. Material and Methods: In this case-control study in 2016 in Hamadan, we selected 57 newly diagnosed patients with type 2 diabetes mellitus with mean aged 47.73 years and 57 healthy controls with mean aged 45.36 years. By using Spitting method, 5ml of completely unstimulated saliva samples were collected from diabetic patients and control subjects. The serum and saliva 25(OH)Vit.Dconcentrations were measured by ELISA. The results are analyzed by SPSS 16.Results: Significant difference was found in serum concentrations between the two groups (p&lt;0.001). But in unstimulated whole salivary 1, 25(OH) 2D3 concentrations between the two groups, difference was not significant and the relationship, was reversed. There was a significant correlation (P=0.013) between serum and saliva 1, 25(OH) 2D3concentrations in the control group. In other words, the correlation was approximately 0.33. Correlation between serum and saliva 1, 25(OH) 2D3 concentrations in case group was not significant. Serum levels of 1, 25(OH) 2D3 in 21.1% of control group, showed the inadequate level (Vit.D3= 20-29ng/ml) and in the case group, showed the critical situation and overall 91.2% of the newly diagnosed diabetics, suffered from lack of vitamin D3. According to the salivary levels of 1, 25(OH) 2D3, in general, a total of 58% of the case group, have deficiency of vitamin D3.Conclusion: The results was confirmed vitamin D3 deficiency in participating in the study population (both case and control). The findings showed that there is lower concentration of 1, 25(OH) 2D3 in serum and after that in saliva in diabetic patients that newly diagnosed. Saliva would play a helpful diagnostic role in the early detection, the monitoring and progression of diabetes, but still serum is the better method for detecting vit.D3 levels and more research needs to be done on saliva for detecting Vit.D3 concentrations.

Background: Vitamin D3 is synthesized in skin and sequentially metabolized in liver and kidney in humans. It is well known for its function in maintaining calcium and phosphorus homeostasis and promoting bone mineralization. The primary objective of this study was to evaluate vitamin D3 level in pregnant women who were suffering from gestational diabetes mellitus and comparing it with the control groups. Materials and Methods: This case control study was conducted on 100 pregnant women who were attending the inpatient and outpatient clinics of Obstetrics department, Tanta University Hospital, who were divided into two equal groups.Group A (control group): Fifty apparently healthy pregnant women at 24th-28th weeks of gestation. Group B (study group): Fifty pregnant women had gestational diabetes. Results: There is significant increase between the two studied groups according to HbA1c, also there is decrease between the two groups as regards VIT D. Mean HbA1c % was statistically significant higher in the study group versus control group. There was statistical significant difference noted between mean serum level of vitamin D among the two studied groups. A statistically significant negative correlation was observed between serum 25 OH vitamin D and HbA1c among our cases (r=- 0.745) (p ≤ 0.001). Mean serum vitamin D was significantly lower in cases with complications than those with normal outcome. Conclusion: Vitamin D deficiency may have a positive relationship with gestational diabetes mellitus.

Background: This study assessed the effects of weekly vitamin D (VD) supplementation on clinical and biological parameters after scaling and root planning (SRP) in the treatment of periodontitis and served to validate the VD dosage regimen. Methods: It was a monocentric, randomized, double-blind, placebo-controlled clinical trial with 6 months follow-up. Healthy Caucasian periodontitis patients presenting serum 25(OH) vitamin D3 below 30 ng/mL were randomly allocated to test group (SRP + VD 25,000 international units (IU)/week) or the control group (SRP + placebo). Results: A total of 59 patients were screened, 27 were included and 26 completed 3 months (M) and 21 completed 6M control. Test (n = 13) and control groups (n = 14) had similar 25(OH) vitamin D3 levels at baseline (17.6 ± 7.4 vs. 14.4 ± 5.2, respectively). After one month, there was a significant difference between groups (32.9 ± 5.2 vs. 16.1 ± 4.7), also seen at M3 and M6 (t-test, p < 0.001). Periodontal treatment was successful in both groups, since it resulted in a reduction of all measured clinical parameters at M3 and M6 (probing pocket depth (PPD), full mouth bleeding and plaque). However, the reduction in PPD was greater in the test group. Conclusions: In this short-term pilot study, no significant differences were observed between two groups. However, supplementation with VD tended to improve the treatment of periodontitis in patients with initial 25(OH) vitamin D3 < 30 ng/mL and proved safe and efficacious. NCT03162406.

Abstract Objectives We investigated the effects of vitamin D deficiency in the peripheral and central smell regions by magnetic resonance imaging (MRI). Methods This retrospective study included 29 patients (12 males, 17 females) with 25-dihydroxy vitamin D3 [25(OH)2D3] deficiency (group 1) and 34 subjects without 25(OH)2D3 deficiency (14 males, 20 females) (group 2). Using cranial MRIs, the peripheral (olfactory bulb [OB] volume and olfactory sulcus [OS] depth) and central (insular gyrus and corpus amygdala) smell regions were evaluated. Results The OB volume and OS depth values of the 25(OH)2D3 deficiency group were significantly lower than those of the control group (p < 0.05). For the central smell regions, the insular gyrus and corpus amygdala areas of the 25(OH)2D3 deficiency group were nonsignificantly lower than those in the control group (p > 0.05). There were positive correlations between OB volumes, OS depths, and insular gyrus and corpus amygdala areas bilaterally in the 25(OH)2D3 deficiency group separately and in all subjects (groups 1 and 2) (p < 0.05). In the 25(OH)2D3 deficiency group, as the 25(OH)2D3 values became lower, the insular gyrus area values decreased bilaterally (p < 0.05). In females, the corpus amygdala area values were lower than in males (p < 0.05). Conclusion Since vitamin D3 deficiency affected the peripheral and central smell regions negatively, we recommend evaluating patients' vitamin D levels as a health policy to prevent vitamin D3 deficiency-related cranial smell region problems. Moreover, sunlight exposure is very important to increase vitamin D levels, and the public should be informed about this topic.

<p class="abstract"><strong>Background:</strong> Allergic rhinitis (AR) is a common disorder seen in routine otolaryngology practice. There has been an increased interest in researchers worldwide regarding role of vitamin D3 in pathogenesis of allergy. This study intends to compare levels of vitamin D3 in patients with clinically diagnosed allergic rhinitis and subjects without allergic rhinitis. This study was conducted in rural South Karnataka where incidence of allergic rhinitis is on the rise. Main objective of this study was to assess vitamin D3 levels in patients with clinically diagnosed AR and compare it with vitamin D3 levels in control group where normal subjects were included.</p><p class="abstract"><strong>Methods:</strong> Total of 100 patients were included in this study. Case group comprised of 50 patients clinically diagnosed as AR and control group comprised 50 subjects without any symptoms of AR. All the patients underwent serum vitamin D3 estimation. Other parameters such as age, gender, occupation (outdoor vs indoor) and region of residence (urban vs rural) were also compared between the groups.  </p><p class="abstract"><strong>Results:</strong> This study did not find significant difference between two groups when all the parameters compared. Significant number of study subjects both in case and control group were found to be either deficient or inadequate in vitamin D3 levels.</p><strong>Conclusions:</strong> we did not find significant difference in vitamin D3 levels between cases and controls. Further studies with bigger sample size and robust study design may throw more light on association of vitamin D3 with AR.

Latar belakang: Rinitis alergi (RA) adalah peradangan atau inflamasi pada mukosa atau selaput lendir hidung yang didasari oleh reaksi hipersensitivitas tipe 1 dan diperantarai IgE. Rinitis alergi merupakan tipe rinitis kronis yang sering dijumpai. Sebanyak 30-50% penderita rinitis memiliki pemicu alergi dan prevalensinya cenderung meningkat. Vitamin D mempunyai peran memodulasi respon imun alamiah dan adaptif. Banyaknya pengaruh vitamin D dalam sistem imun diharapkan dapat mengatur aktivitas berbagai sel kekebalan, serta fungsi kekebalan tubuh dari epitel sel pada RA, namun konsensus sebagai bagian terapi RA saat ini masih belum ada. Tujuan: Mengetahui pengaruh pemberian vitamin D3 terhadap kadar vitamin D (25(OH)D) dan sel T Regulator (T reg) pada rinitis alergi. Metode: Penelitian eksperimental murni dengan rancangan penelitian randomized clinical trial (RCT) double blind, pre and post test control group dengan perlakuan berupa pemberian terapi sesuai ARIA (Allergic Rhinitis and its Impact on Asthma) dan vitamin D3 oral diberikan selama 4 minggu. Dilakukan penilaian VAS (Visual Analogue Scale), TNSS (Total Nasal Symptom Score), serta kadar vitamin D darah dan persentase sel T reg pada awal dan 4 minggu setelah perlakuan. Hasil: Pada kelompok perlakuan terdapat perbedaan bermakna kadar vitamin D sebelum dan sesudah perlakuan (p=0,01). Terdapat perbedaan yang bermakna dari peningkatan persentase sel T reg pada kelompok perlakuan dibandingkan kelompok kontrol (p=0,02). Terdapat hubungan antara peningkatan persentase sel T reg dengan kadar vitamin D pada kelompok perlakuan (p=0,01). Kesimpulan: Vitamin D3 dapat meningkatkan kadar vitamin D dan persentase sel T reg secara bermakna pada pasien rinitis alergi persisten sedang berat. Kata kunci: Rinitis alergi, vitamin D, sel T regulator ABSTRACT Background: Allergic rhinitis (AR) is an inflammatory disease of nasal mucosa based on type 1 hypersensitivity reaction and mediated by IgE. Allergic rhinitis is the most common type of chronic rhinitis. About 30±50% of patients with allergic rhinitis have trigger factors and its prevalence is increasing. Vitamin D has a function in modulation of innate and adaptive responses. The role of vitamin D in immune system is expected to regulate various immune cells activity, and the immune function of AR epithelial cells, but there were no consensus yet about vitamin D as part of AR therapy. Objective: To identify the effect of vitamin D3 on level of vitamin D (25(OH)D) and T regulator (T reg) cells in allergic rhinitis patients. Methods: This study was purely experimental research with randomized clinical trial, double-blind, pre and post test control group treated with appropriate therapy according to ARIA (Allergic Rhinitis and its Impact on Asthma) and vitamin D3 given orally for 4 weeks. Evaluations of VAS (Visual Analogue Scale) and TNSS (Total Nasal Symptom Score) value, vitamin D levels and the percentage of T reg cells were carried out at baseline and 4 weeks after treatment. Results: In the treatment group there was a significant difference between vitamin D levels before and after treatment (p=0.01). There was a significant difference on increased percentage of T reg cells in the treatment group compared with the control group (p=0.02). There was a correlation between the increasing percentage of T reg cells with vitamin D levels in the treatment group (p=0.01). Conclusion: Vitamin D supplementation could increase the level of vitamin D and the percentage of T reg cells significantly in patients with severe persistent allergic rhinitis. Introduction: Chronic rhinosinusitis with nasal polyposis (CRSwNP) is an inflammatory disease of paranasal sinuses and nasal coverage, which reduces the quality of life and overall health. Vitamin D regulates many cellular functions, including anti-inflammatory and immune-modulating functions. There is an increasing evidence of the relative role of fibroblasts in the accumulation of inflammatory cells in chronic rhinosinusitis. The aim of this study was to investigate the relationship between vitamin D levels and the proliferation of sinonasal fibroblasts in patients with chronic rhinosinusitis with nasal polyposis and its therapeutic role. Material and Method: In this case-control study blood and tissue samples were extracted from patients with CRSwNP (n=30). Control subjects with non-inflammatory conditions underwent endoscopic and non-endoscopic surgery (n=30). The serum level of 1,25 dihydroxyvitamin D (1,25(OH)2D) and proliferation of fibroblasts sampled from nasal and sinus tissue using ki 67 and vimentin were evaluated and analyzed using SPSS® 23 software. Results: The average level of vitamin D was 23.1±7.36 ng/ml in the case group and 31.19±7.02 ng/ml in control group, which was significantly lower in case group (P=0.008). The proliferation of sinonasal fibroblasts were positive in all cases patients and only in 20% of control patients using vimentin. The proliferation of sinonasal fibroblasts were negative in all control patients while only 10% of the case patients were negative using Ki 67. The level of vitamin D decreased with the increased rate of Ki 67. Conclusion: The results of this study indicate a reversed relation between vitamin D levels and the proliferation of sinonasal fibroblasts.Thus vitamin D has both prophylactic and therapeutic effects in chronic rhinosinusitis with nasal polyposis.
