- reading Tanzi and Riva's studies on "paranoia" (1884 - 1886) enriched by clinical cases masterly told with sharp sensibility, convey a thorough and deep analysis of an important notion in psychopathology. Their exemplary contribution is among the first definitions of the clinical picture in the context of the European psychiatric culture and its Kraepelinian inclusion in the construction of dementia praecox (schizophrenia). The Authors stand in a momentous time in the evolution of psychiatric knowledge and its needs for classifications. Their hypothesis, which follows "positivistic" ideas strong in Italy at the time, tends to interpret paranoia, its delusional expressions and, in particular, its peculiar personality organisation, in the context of the "degeneration" theory, in the belief that paranoia displays "atavistic" defects. Nevertheless, beyond this concept, linked to end of eighteenth century philosophical views, the Authors outline the subject of paranoia with precision, highlighting its essential features: the conservation of intellectual functions, despite the presence of disturbances affecting logic and language (with neologisms), and the complex interweaving of experiences caused by psychological events and existential confl icts. From this perspective, Tanzi and Riva's description maintains an undisputable importance whenever, in the superseding of nosographic stances, paranoia calls for an irreducible autonomy both in clinical practice, both in its theory.

Abstract Background Health service evaluation studies point to the low quality of care, however the permanent education of health professionals is a major challenge. Within the scope of the Open University for the Unified Health System (SUS), the Department of Social Medicine of the Federal University of Pelotas produced open access self-learning courses, for primary health care, based on interactive clinical cases, through distance learning. Methods The courses, with 45 hours, provides interactive clinical cases, medical calculators and textual materials for use by the professional or with the community. The cases are presented and followed by multiple choice questions, focusing diagnosis and treatment. The feedback is automatic. An expanded theoretical content ('learn more') that privileges the use of illustrations, images, infographics and flowcharts useful for professional practice can be accessed. Bibliographical references on the subject are also available. The courses are available in a responsive web client app, for various types of devices, providing control of progress and automatic certification. The app allows the work offline, enabling the permanent education of professionals working in remote areas. Results Six courses were produced for doctors, nurses and dentists, more than 30,000 users were certified. Conclusions This problem-based learning initiative allows the student to have a menu of cases, providing a choice of the subject, decision on the time allocated to the activity, repetition according to its need, access to a large bibliography and reading in the quantity and depth chosen by the student. In addition, the format is challenging and develops clinical reasoning. The proposition of questions promotes the reflection of the student in relation to his knowledge on the subject. Automatic feedback, 'learn more' and selected bibliographies provide formative assessment and subsidize students to define their study needs in each subject. Key messages Distance learning expands the possibility of qualifying the clinical practice of health professionals, especially those in remote areas. Interactive clinical cases provide the development of clinical reasoning, content review and promote formative assessment.

In Uruguay, Lacanian ideas arrived in the 1960s, into a context of Kleinian hegemony. Adopting a discursive approach, this study researched the initial reception of these ideas and its effects on clinical practices. We gathered a corpus of discursive data from clinical cases and theoretical-doctrinal articles (from the 1960s, 1970s and 1980s). In order to examine the effects of Lacanian ideas, we analysed the difference in the way of interpreting the clinical material before and after Lacan's reception. The results of this research illuminate some epistemological problems of psychoanalysis, especially the relationship between theory and clinical practice.

Translocation – Transformation is an ambitious contribution to the subject of mobility. Materially, it interlinks seemingly disparate objects into a surprisingly unified exhibition on mobile histories and heritages: twelve bronze zodiac heads, silk and bamboo creatures, worn life vests, pressed Pu-erh tea, thousands of broken antique teapot spouts, and an ancestral wooden temple from the Ming dynasty (1368–1644) used by a tea-trading family. Historically and politically, the exhibition engages Chinese stories from the third century BCE, empires in eighteenth-century Austria and China, the Second Opium War in the nineteenth century, the Chinese Cultural Revolution of the mid-twentieth century, and today’s global refugee crisis.

In the present day scenario prime need of Ayurveda is practical applicability of Ayurvedic fundamental principles and research should be more focused on all aspects where scientific inputs should confirm Ayurveda’s principles and philosophy. In Ayurveda, Gunas are described as the way of presentation of action without which no Karma (action) can be possible. Ayurveda has provided significance to every Guna so that they become useful in clinical practice. Clinical practice or research is a continuous process including a series of events which need to be performed in a sequential manner. Though there are many factors on which accomplishment of treatment depends, among these Charaka has given prime importance to Paradi Gunas. In clinical practice, Guna which are to be with Bhishak are mainly the Paradi Gunas which can also be called as miscellaneous Gunas. As rightly quoted by Acarya Charaka, for getting success in the treatment Paradi Gunas are the best. The Sutra quotes "Sidhyupaya Chikitsayam" which means that Chikitsa i.e. Dhatusamya will be done mainly with the help of Paradi Gunas. Knowledge of Paradi Gunas are required to be present in pharmacists, physicians and Researchers. Acharya Charaka says that Chikitsa Siddhi i.e. successful management of disease is not possible without the knowledge of Paradi Gunas. Paradi Gunas play an important role in selection, adaption and manifestation of drug as per condition of the disease and the patient in particular Desha and Kala. The ultimate goal of any clinical activity is to contribute in the knowledge domain and to improve professional practice. Thus this literary review of Paradi Gunas serves to explore the subject of the clinical practice.

The Laws of Hammurabi is one of the earliest law codes, dating from the eighteenth century BCE Mesopotamia (ancient Iraq). It is the culmination of a tradition in which scribes would demonstrate their legal flair by composing statutes on a repertoire of traditional cases, articulating what they deemed just and fair. The book describes how the scribe of the Laws of Hammurabi advanced beyond earlier scribes in composing statutes that manifest systematization and implicit legal principles. The scribe inserted the statutes into the structure of a royal inscription, skillfully reshaping the genre. This approach allowed the king to use the law code to demonstrate that Hammurabi had fulfilled the mandate to guarantee justice enjoined upon him by the gods, affirming his authority as king. This tradition of scribal improvisation on a set of traditional cases continued outside of Mesopotamia, influencing biblical law and the law of the Hittite Empire and perhaps shaping Greek and Roman law. The Laws of Hammurabi is also a witness to the start of another stream of intellectual tradition. It became a classic text and the subject of formal commentaries, marking a Copernican revolution in intellectual culture.

Clausewitz was an ardent analyst of partisan warfare. In 1810 and 1811, he lectured at the Berlin Kriegsschule, the war academy, on the subject of small wars. Clausewitz’s lectures focused on the tactical nature of small wars. However, the eighteenth-century context was by no means irrelevant for Clausewitz’s further intellectual development. On the contrary, he extrapolated from his analysis of the tactical nature of small wars their strategic potential, as well as their exemplary nature for the study of war as such. The partisan, in Clausewitz’s eyes, possessed exemplary qualities in that he acted autonomously and, in doing so, had to draw upon all his human faculties. As such, he was the paradigmatic antagonist to the regular soldier who displayed a ‘cog mentality’ fostered by the Frederickian military system.

In clinical practice there are few test that language therapist can use for the oral speech evaluation. The battery that has been frecuently used for the aphasia assessment, doesn´t usually give us this type of data, and when it happens, it is just a global description of the language of the subject, which doesn't make possible a detailed pursuit of the evolution. The relevance of linguistic production´s assessment becomes more evident in the study of patients with mild injury as well as when patient is in an advanced recovery stage. An assessment without a speech analysis uses to overestimate the subject's capacities and usually ends in an incomplete and inadequate intervention. The aim of the present work was to review different studies that include the narrative speech as part of the assessment, and to explore, with the study of a single case, some applications in this kind of studies for language evaluation in mild brain injury patients.

Individuals who excel in mathematics have always enjoyed a well deserved high reputation. Nevertheless, a few hundred years back, as an honourable occupation with means to social advancement, such an individual would need a patron in order to sustain the creative activities over a long period. Leonhard Euler (1707-1783) had the fortune of being supported successively by Peter the Great (1672-1725), Frederich the Great (1712-1786) and the Great Empress Catherine (1729-1791), enabling him to become the leading mathematician who dominated much of the eighteenth century. In this note celebrating his tercentenary, I shall mention his work in number theory which extended over some fifty years. Although it makes up only a small part of his immense scientific output (it occupies only four volumes out of more than seventy of his complete work) it is mostly through his research in number theory that he will be remembered as a mathematician, and it is clear that arithmetic gave him the most satisfaction and also much frustration. Gazette readers will be familiar with many of his results which are very well explained in H. Davenport's famous text [1], and those who want to know more about the historic background, together with the rest of the subject matter itself, should consult A. Weil's definitive scholarly work [2], on which much of what I write is based. Some of the topics being mentioned here are also set out in Euler's own Introductio in analysin infinitorum (1748), which has now been translated into English [3].

It is appropriate that an American should address himself to the subject of public opinion. For, in terms of quantity, Americans have made the subject peculiarly their own. They have also invested it with characteristically American concerns. Most of the work done on the subject in the United States is oriented by a certain theoretical approach. This approach is democratic and rationalist. Both aspects create problems. In this paper I wish to play down the democratic problem, viz., how many of the voters are capable of thinking sensibly about public policy, and emphasize rather the difficulties that arise from modern rationalism. Here I take a different tack from most historians of the concept of public opinion, who, taking note of the origin of the term in the mid-eighteenth century, stress its connection with the rise of representative government and democratic theory.

In psychiatric and psychotherapeutic practice, ‘boundaries' delineate the personal and the professional roles and the differences that should characterise the interpersonal encounters between the patient/client and the professional. Boundaries are essential to keep both parties safe. The author outlines the various types of boundary violation that can arise in clinical practice, their consequences (both clinical and legal), how professionals can avoid them and how health care institutions might respond, should they occur. He concentrates on sexual boundary violations, because these have been the subject of most empirical study.
