Aims: To evaluate the association of coronary atherosclerotic severity in diabetic dyslipidemic patients of acute coronary syndrome.Methods: This was a cross sectional comparative analytical study, done in the Department of Cardiology, Sir Salimullah Medical College Mitford Hospital and Ibrahim Cardiac Hospital & Research Institute, Dhaka, during September 2009 to August 2010.Results: Most of the patients (57.5%) were in the age range of 40-70 years. Atherosclerotic severity in diabetic ACS patients was significantly higher with low HDL, high TC/HDL and high LDL/HDL ratio. Low HDL, high TC/HDL and high LDL/HDL ratio are indicators of the extent and severity of coronary artery disease. More frequent dyslipidemia in diabetic ACS patients were low HDL and it was about 54%.Conclusion: Atherosclerotic severity in diabetic acute coronary syndrome patients was significantly more in dyslipidemic group than non dyslipidemic group.Bangladesh Heart Journal 2016; 31(2) : 65-69

Background: Myeloperoxidase (MPO), an oxidative stress related enzyme is elevated in Coronary Artery Disease (CAD) and is involved in development of atherosclerotic plaque. Paraoxonase (PON) an enzyme protein associated with HDL serves as an antioxidant and plays an important role in preventing the formation of Oxidized LDL (OxLDL). This suggests a conflicting role of MPO and PON in development of cardiovascular disease and atherosclerosis. Aim: Present study was done to evaluate and compare MPO/PON ratio in Acute Coronary Syndrome (ACS) patients with controls. The study evaluates and compares the pro oxidant and pro inflammatory enzyme, MPO and anti-oxidant and anti-inflammatory enzyme, PON in ACS patients with controls. Oxidative marker, Malondialdehyde (MDA) and anti-oxidant marker, Reduced Glutathione (GSH) was assessed in ACS patients and compared with controls. An attempt was also made to correlate MPO/PON ratio to markers of oxidative stress (MDA and GSH). Methods: Cross-sectional study carried out in Dr. Somervell Memorial CSI Medical College, Trivandrum, Kerala, India.50 ACS patients from Cardiac Care Unit and 50 age and sex matched controls without CAD from Medical College Health Checkup was selected. Results: MPO and MPO/PON ratio were significantly high and PON  was significantly lower in ACS patients compared to controls. Total cholesterol, Triglyceride, LDL, MDA were significantly high in ACS patients. Statistically significant positive correlation was observed between MPO/PON and MDA. Significant negative correlation was observed between MPO/PON and GSH. Conclusion: Myeloperoxidase and MPO/PON ratio was significantly high in ACS patients than controls. This is suggestive of the role of MPO in oxidative damage to lipoproteins in CAD patients. Prooxidant, Paraoxonase ,and antioxidant, GSH is lowered in ACS patients as a result of the increased oxidative stress. This study suggests that MPO/PON1 ratio can be used as a predictive marker of ACS.

Introduction: The role of the immune and inflammatory pathways in coronary artery disease (CAD) is important but not complete understood. The aim of this study was to evaluate expressions of the interleukins 17th and 22th in patients with stable coronary artery disease. Hypothesis: Interleukins 17th and 22th are not increased in stable CAD. Methods: This is a cross-sectional, prospective, analytical study, conducted from August to December 2012. We included 40 patients (P) with stable CAD, CCS III or IV, ischemic myocardial scintigraphy, who had not been subjected to any kind of myocardial revascularization and with coronary stenosis equal or major than 50% according to current coronary angiography. There were 20 healthy volunteers (C), to take up comparison of expression of interleukins (IL). We evaluated the levels of IL 17th and 22th of the patients and controls. Interleukins were evaluated in serum of patients and after 48 hours of cells in culture with and without stimulus. IL concentrations were expressed in pg / ml. Statistical analysis was performed using the Mann-Whitney or Student t test. P ≤ 0,05 was considered statistically significant. Results: There were 26 men and 14 women in the group of the patients and 12 men and 8 women in the controls. The age was similar between the groups (63.2 ± 8.9 years vs 57.9 ± 9.4, p = ns). The comparison between the groups showed: Interleukin 17th: Serum: P = 3.9 (972.2 -- 2.93) vs C = 3.90 (28.8 -- 1.74), p = 0.5; culture 48 hours without stimulus: P = 3.90 (3.90 -- 3.90) vs C = 6.37 (3.90 - 11), p = 0.8; culture 48 hours with stimulus: P = 302.42 (2200 -- 3.90) vs C = 815 (1353 -- 3.90), p = 0.06. Interleukin 22th: Serum: P = 15.62 (64.72 -- 15.62) vs C = 15.62 (121 -- 15.62), p = 0.2; Culture 48 hours without stimulus: P = 11 (128.93 -- 7.81) vs C = 7.81 (7.81 -- 7.81), P = 0.8; Culture 48 hours with stimulus: P = 135 (2486.7 -- 7, 81) vs C = 322.86 (1319.11 -- 7.81), p = 0.4. Conclusions: There were no differences in concentrations of interleukins, but the trend of higher expression of the IL 17th in the controls after cell culture with stimulus. In conclusion, in patients with stable CAD the interleukins 17th and 22th did not exhibit increased concentrations.

AbstractMicroribonucleic acids (miRs) are small, noncoding ribonucleic acids (RNAs), which play an important role in the regulation of platelet function and activity. Several studies proposed a mechanistic role of platelet-related miRs in the pathophysiology of coronary artery disease (CAD) and atherothrombosis. Circulating, platelet-related miRs have been proposed as diagnostic, prognostic, as well as treatment response biomarkers in CAD and acute coronary syndrome (ACS). In this review, we summarize recent studies on the role of platelet-related miRs in the regulation of platelet function and activity. Furthermore, we review the studies investigating the role of platelet-related miRs as biomarkers in patients with CAD and ACS.

The SYNTAX score (SX score) is a useful score for assessing the severity of coronary artery disease (CAD). Previous studies have demonstrated a close relationship between SX score and inflammation. Procalcitonin (PCT) is an early inflammatory marker, especially during sepsis. Thus, in this study, we aimed to investigate the relationship between SX score and serum PCT levels. A total of 545 patients were enrolled in this prospective cross-sectional study and were divided into 2 subgroups, according to their SX score. Serum PCT and high-sensitivity C-reactive protein levels were measured. Serum PCT levels were higher in the high SX score group compared to the low–intermediate SX score group ( P < .001). Serum PCT levels were an independent predictor of a high SX score in patients with acute coronary syndrome ( P = .001). As patients with a higher SX score had increased serum PCT levels on admission, serum PCT may be useful for identifying patients with severe CAD.

Background:Acute coronary syndrome is a cardiac emergency. It is increasing dramatically and becoming a major burden in our health care system. Relation between serum lipid profile and acute coronary syndrome is well established. Our study tried to reveal association of high density lipoprotein cholesterol (HDL-C) with in-hospital outcome of patients with acute coronary syndrome.Methods: The study was a cross sectional comparative study. Clinical & biochemical evaluation was done in hospital settings. A total number of 271 patients were included in the study and divided into two groups. Patients with low HDL-C level were in group I and patients with normal HDL-C were in group II.Results: Group I populations had more complications & more in- hospital stay than group II (74.3% vs 28.9%, P<0.001 and 6.65±2.04 days vs 5.09±1.44 days, p<0.001 respectively).Conclusion: The study revealed significant association of HDL-C with outcome of acute coronary syndrome patients. Complications of acute coronary syndrome were more in patients with low HDLC level.Bangladesh Heart Journal 2017; 32(2) : 114-118

Introduction: Ischaemic Heart Diseases (IHD) are increasing at an alarming rate globally. Pharmacotherapy, the mainstay of management of IHD, has Dual Antiplatelet Therapy (DAPT) at its crux. DAPT, comprising of clopidogrel and aspirin, is used often for 12 months in medically managed Acute Coronary Syndrome (ACS). Clopidogrel being a prodrug, needs to be activated after ingestion, mainly by Cytochrome family of enzymes, for the desired pharmacological effects. Abnormal enzymes that convert clopidogrel to its active form can reduce the effectiveness of the drug. Therefore, Single Nucleotide Polymorphisms (SNPs) of the enzymes involved, would predispose the patient to therapy failure and recurrence of the illness. This study aims at identifying the presence of a genetic variant associated with clopidogrel responsiveness in patients with ACS. Aim: To estimate the frequency of CYP2C19*2 allele in patients with ACS presenting to Government Medical College, Thrissur, Kerala, India. Materials and Methods: Present study was a cross-sectional study done among 60 patients who presented to Government Medical College, Thrissur with ACS, between June 2018 to June 2019. Having obtained informed consent from the participants blood samples were collected in Ethylene diamine tetra-acetic Acid (EDTA) tubes. Samples were used to look for presence of CYP2C19*2 allele by Polymerase Chain Reaction-Restriction Fragment Length Polymorphism (PCR-RFLP). Results: Out of 60 patients 38 were males and 22 were females. Most common age group was 61-65 years. The frequency of CYP2C19*2 allele was 41% in the study population and the variant genotype (AA/AG) was present in 62%. Conclusion: The variant genotype was found to be present in a high frequency in the study population implying the high risk for therapy failure in the study population. This asserts the need to probe the matter further, as the gene is also implicated in the metabolism of several other drugs. The introduction of pharmacogenomic testing prior to initiation of therapy may be therefore recommended for better treatment outcome. However, to validate the same, further studies with larger sample size may be necessary.

To find out frequency of low HDL-C levels in patients suffering from acute coronary syndrome. Study Design: Descriptive cross sectional study. Setting: Department of cardiology Khyber Teaching Hospital (KTH) Peshawar. Period: 04/04/2016 to 04/10/2016. Material & Methods: By applying WHO formula for sample size calculation and 95% confidence interval, a total of 154 Patients admitted to coronary care unit (CCU) of Khyber Teaching Hospital with acute coronary syndrome were selected. Anticipated portion of low HDL in ACS was 73.3 % and absolute precision of about 7%. Results: In this study, mean age was 55.720 ± 8.901 years. About 38% patients were of female gender while 62% patients were male. About 20 % patients had UA, 9% patients had NSTEMI, and 71% patients had STEMI.  Patients with low high density lipoproteins constituted about 48% of the total patients admitted with ACS. Conclusion: Our study concluded that significant number (48%) of patients with ACS had low HDL levels.

Abstract: Acute Coronary Syndrome (ACS) is a set of manifestations or symptoms of coronary artery disease and thrombosis which can threaten the patien’s life due to interfering the blood supply of heart muscle. The infarct size is determined by assessing the coronary arteries lesion, stenosis. This study was aimed to determine the relationship between levels of leukocytes and the severity of coronary arteries lesions in patients with ACS. This was a descriptive analytical study with a cross-sectional retrospective design. The modified Gensini score was used to assess the severity of coronary artery lesion. The first leukocyte count examined when the patients was admitted to the hospital was obtained from the medical records period July to September 2015. Data were analyzed by using SPSS 20.0. The Pearson correlation test showed no significant relationship between the levels of leukocytes and the severity of coronary arteries lesions in patients with ACS (p >0.05). Conclusion: There was no significant correlation between the levels of leukocytes and the severity of coronary arteries lesions in patients with ACS.Keywords: leukocyte, severity of lesion, acute coronary syndrome, modified Gensini score Abstrak: Sindrom koroner akut (SKA) merupakan sekumpulan manifestasi atau gejala akibat penyakit arteri koroner dan trombosis yang dapat mengancam kehidupan pasien karena mengganggu pasokan darah ke otot jantung. Penilaian luas infark dilakukan dengan menilai lesi pembuluh darah koroner yang mengalami stenosis. Penelitian ini bertujuan untuk mengetahui hubungan antara kadar leukosit dengan severitas lesi pembuluh darah koroner pada pasien SKA. Jenis penelitian ialah deskriptif analitik retrospektif dengan desain potong lintang. Penilaian severitas pembuluh darah yang mengalami lesi menggunakan skor modifikasi Gensini. Hitung leukosit yang digunakan ialah saat pasien pertama kali masuk Rumah Sakit yang diperoleh dari rekam medis periode Juli-September 2015. Data dianalisis menggunakan SPSS 20.0. Hasil uji korelasi Pearson memperlihatkan tidak terdapat hubungan bermakna antara kadar leukosit dan severitas lesi pembuluh darah koroner pada pasien SKA (p >0,05). Simpulan: Tidak terdapat hubungan bermakna antara kadar leukosit dan severitas lesi pembuluh darah koroner pada pasien Sindrom Koroner Akut.
