Background: Physical inactivity and low resting heart rate variability (HRV) are associated with increased incidence of coronary heart disease. Heavy physical activity is associated with higher heart rate variability and reduces the risk of coronary heart disease Objective: To assess some time domain measures of HRV in order to compare Cardiac Autonomic Function between sedentary and heavy workers. Methods: This cross sectional study was carried out in the Department of Physiology, Bangabandhu Sheikh Mujib Medical University between 1st July 2008 to 30th June 2009 on 30 apparently healthy heavy workers aged 28-50 years from low socioeconomic condition (study group). For comparison 30 age, sex, BMI and socioeconomic status matched apparently healthy sedentary subjects (group A) were also studied. The study subjects were selected among rickshaw-pullers living in the slum areas nearby BSMMU, Dhaka and the controls were from fourth class employee of BSMMU, Dhaka. Heart Rate Variability were assessed by a Polygraph. Several time domain measures of HRV such as mean R-R interval, mean HR, SDNN, RMSSD were analyzed. For statistical analysis, Independent-Samples t-test, and Pearson’s correlation coefficient tests were done as applicable. Results: Resting mean heart rate (P<0.01), SDNN (P<0.01) and RMSSD (P<0.001) were significantly lower but mean R-R interval (P<0.001) was significantly higher in heavy workers than those of sedentary control. The mean R-R interval (P<0.05) showed significant positive correlation in heavy workers but significant negative correlation in sedentary workers with BMI. Conclusion: Cardiac autonomic nerve function status may be higher with parasympathetic dominance by increased physical activity. DOI: http://dx.doi.org/10.3329/jbsp.v6i2.9755 JBSP 2011 6(2): 77-83

Background: Altered cardiovascular autonomic nerve function with impaired sympathovagal balance is found in rheumatoid arthritis (RA). Heart Rate Variability (HRV) analysis is an important tool for assessment of autonomic nerve activity. Objective: To assess cardiac autonomic nerve function status in patients with Rheumatoid arthritis (RA) by time domain measures of HRV. Methods: This cross sectional study was conducted in the Department of Physiology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka from January to December 2010. Sixty female RA patients, age range 18-50 years were constituted study group enrolled from the Out- patient Rheumatology Wing, Department of Medicine, BSMMU. Age matched thirty apparently healthy females were studied as control. Time domain measures of Heart Rate Variability (HRV) such as Mean RR intervals, Mean HR, SDNN, RMSSD, NN50% and PNN 50% were recorded for 5 minutes by a Polygraph machine to observe cardiac autonomic nerve function activity. Data were analyzed by independent sample t test. Results: Mean R-R interval, SDNN, RMSSD, PNN50%, NN50% were significantly lower (p<0.001) but heart rate was significantly (P<0.001) higher in rheumatoid arthritis patients. Conclusion: Cardiac autonomic nerve function is impaired and characterized by reduced resting parasympathetic activity in female Rheumatoid Arthritis patients. The objective of the research was to develop new approaches to the prognosis, prevention and treatment of complications in patients at high and very high total cardiovascular risk based on the assessment of the state and effect on heart rate variability and heart rate turbulence.Materials and methods. There were examined 319 patients; they underwent a 24-hour monitoring of their heart rate variability and heart rate turbulence. All the patients were divided into three groups: Group 1 included patients with coronary heart disease (post-infarction cardiosclerosis) without concomitant risk factors such as smoking, obesity, metabolic syndrome; Group 2 consisted of patients smoking tobacco for more than 2 years; Group 3 included patients with metabolic syndrome and arterial hypertension; the control group.Results. In patients with coronary heart disease and concomitant phenomenon of heart rate turbulence, the activity of the parasympathetic nervous system was significantly higher in the active period of the day, while the passive period of the day was associated with the elevated level of stress. Due to this, among patients of this group, there was a decrease in the tension value of the total body regulation in the passive period of the day, while in healthy individuals, its increase was observed. In patients at high and very high cardiovascular risk according to the SCORE charts with the phenomenon of heart rate turbulence, the stress index was significantly higher throughout the day as compared to apparently healthy individuals. In patients at high cardiovascular risk and concomitant phenomenon of heart rate turbulence, the parameters of heart rate turbulence acceleration were significantly higher as compared to those of apparently healthy individuals.Conclusions. In patients at high and very high cardiovascular risk according to the SCORE charts with coronary heart disease and concomitant phenomenon of heart rate turbulence, the occurrence of ventricular extrasystole had a vagal nature; they were characterized by a higher level of stress in the passive period of the day as compared to the active one.

ABSTRACT: RELATIONSHIP BETWEEN PHYSICAL ACTIVITY AND FAMILY HISTORY WITH CORONARY HEART DISEASE AT PUSKESMAS  BANJARSARI METRO Introduction: Based on data in coronary heart events Metro City 2018 total which is 1,309 new cases. For new diseases 190 men and 22 for women cases,  for old cases 369 men and 528 women in 2019 have a very drastic decrease, with total of new and old 229 patients with heart disease , for new diseases which is 32 men and 57 cases and women, for an old cases which is 60 cases for men and 80 cases for women.Purpose:  to known relations between physical activity and family history with coronary heart disease at Puskesmas Banjarsari Metro in 2020.Method: quantitative research type, analytical survey research plan with cross sectional approach, population and sample in research patients with heart failure are22 respondents. Univariate and bivariate data analysis used chi square test.Result : From 23 respondents, 13 respondents (56,5%) with low activity, families at risk which is 14 respondents (55,0%),  12 respondent (52,2%) with coronary heart, results of statistical tests using the chi square test obtained p-value = 0,022(< 0.05) , p-value = 0,006(< 0.05).Conclusion : Results of using the chi square test obtained p-value = 0,022(< 0.05) that means there is a relations between physical actiivity with occurrence coronary heart disease in Puskesmas Banjarsari Kota Metro 2020. Results of using the chi square test obtained p-value = 0,006(< 0.05) that means there is a relations between history of family with occurrence coronary heart disease in Puskesmas Banjarsari Kota Metro 2020. For peoples who risk of coronary heart disease (CHD) to behave healthy living by maintaining a diet by consuming foods that conform to diet coronary heart patients, avoid smoking, not consuming alcohol beverages, maintain weight stability and exercise regulary. Keywords : Knowledge, Self Care, Quality of life.       INTISARI:HUBUNGAN ANTARA AKTIVITAS FISIK DAN RIWAYAT KELUARGA DENGAN KEJADIAN PENYAKIT JANTUNG KORONER DI PUSKESMAS RAWAT INAP BANJARSARI KOTA METRO  Pendahuluan: Berdasarkan data kejadian jantung koroner di Kota Metro tahun 2018 pada berdasarkan jumlah kasus baru dan lama  sebanyak 1.309 dengan pembagian penyakit baru, laki-laki sebanyak 190 orang, perempuan sebanyak 22 orang, dan kasus lama, laki-laki sebanyak 369 orang, perempuan sebanyak 528 orang, pada tahun 2019 mengalami penurunan yang sangat drastis, yaitu total pasien jantung baru dan lama sebanyak 229 orang, dengan pembagian penyakit baru, laki-laki sebanyak 32 orang, perempuan sebanyak 57 orang, dan kasus lama, laki-laki sebanyak 60 orang, perempuan sebanyak 80 orang.Tujuan: Diketahui hubungan antara aktivitas fisik dan riwayat keluarga dengan kejadian penyakit jantung koroner di Puskesmas Banjarsari Kota Metro Tahun 2020.Metode: Jenis penelitian kuantitatif, rancangan penelitian survey analitik dengan pendekatan cross sectional, populasi dan sampel dalam penelitian ini adalah pasien gagal jantung sebanyak 22  responden, Analisa data menggunakan univariat dan bivariat menggunakan uji chi square.Hasil : Dari 23 responden dengan aktivitas kurang sebanyak 13 responden (56,5%), riwayat keluarga beresiko sebanyak 14 responden (55,0%),  jantung koroner sebanyak 12 responden (52,2%), Hasil uji statistik menggunakan uji chi square didapat nilai p-value = 0,022 (< 0.05) , p-value = 0,006 (< 0.05).Kesimpulan : Hasil uji statistik menggunakan uji chi square didapat nilai p-value = 0,022 (< 0.05) yang artinya terdapat hubungan antara aktivitas fisik dengan kejadian penyakit jantung koroner di Puskesmas Banjarsari Kota Metro Tahun 2020. Hasil uji statistik menggunakan uji chi square didapat nilai p-value = 0,006 (< 0.05) yang artinya terdapat hubungan antara riwayat keluarga dengan kejadian penyakit jantung koroner di Puskesmas Banjarsari Kota Metro  Tahun 2020Bagi seseorang yang mempunyai resiko terhadap terjadinya Penyakit Jantung Koroner (PJK) untuk berperilaku hidup sehat dengan jalan menjaga pola makan dengan jalan mengkonsumsi makanan yang sesuai dengan pola diit penderita PJK, menghindari merokok, tidak mengkonsumsi minuman beralkohol, menjaga kestabilan berat badan dan berolah raga secara teratur. Kata Kunci      : Pengetahuan, Self Care, Kualitas Hidup

Heart rate variability (HRV) has been considered as an indicator of autonomic nerve function status. We aimed to find out the reference values of heart rate variability by power spectral analysis in our healthy population of different age. This cross sectional study was conducted in the Department of Physiology, Dhaka Medical College, Dhaka from the period of July 2012 to June 2013. For this, 180 subjects were selected with the age ranging from 15-60 years. All the study subjects were divided into 3 different groups according to age (Group A: 15-30 years; Group B: 31-45 years; Group C: 46-60 years). Each group contained 60 subjects of which 30 were male and 30 were female. The subjects were selected from different areas of Dhaka city by personal contacts. Analysis of HRV parameters were done in Department of Physiology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka. For statistical analysis, one way ANOVA, unpaired Students t-test and Pearson’s correlation coefficient test were performed by using SPSS (version-17) as applicable. LF nu, LF power and LF/HF were significantly (p<0.001) higher in group C in comparison to those of group A and B. Again Total power, HF power, HF nu (p<0.001) were significantly higher in group A and B in comparison to that of group C. Background: This study investigated whether work ability is associated with the duration of unemployment, heart rate variability (HRV), and the level of physical activity. Methods: Thirty-four unemployed persons (mean 55.7 ± standard deviation 33.3 years, 22 female, 12 male, unemployed: range 1–22.5 years) participated in the cross-sectional study. The Work Ability Index (WAI) and International Physical Activity Questionnaire (IPAQ) were applied. Short-term (five minutes) resting HRV (Low Frequency (LF), High Frequency (HF), Total Power (TP)) was collected. Results: Work ability was positively associated with the HRV: LF (r = 0.383; p = 0.025), HF (r = 0.412; p = 0.015) and TP (r = 0.361; p = 0.036). The WAI showed a positive linear correlation with the amount of total physical activity (r = 0.461; p = 0.006) as well as with the amount of moderate to vigorous physical activity (r = 0.413; p = 0.015). No association between the WAI and the duration of unemployment occurred. Conclusions: the relation between self-perceived work ability, health-associated parameters, the HRV and the level of physical activity points out the relevance of health-care exercise and the need of stress-reducing interventions to improve perceived work ability. Our results point out the need for the further and more holistic development of healthcare for the unemployed.

BackgroundAntidepressants reduce depressive symptoms in patients with coronary heart disease, but they may be associated with increased mortality. This study aimed to examine whether the use of tricyclic antidepressants (TCA) or selective serotonin reuptake inhibitors (SSRI) is associated with mortality in patients with coronary heart disease, and to determine whether this association is mediated by autonomic function.MethodA total of 956 patients with coronary heart disease were followed for a mean duration of 7.2 years. Autonomic function was assessed as heart rate variability, and plasma and 24-h urinary norepinephrine.ResultsOf 956 patients, 44 (4.6%) used TCA, 89 (9.3%) used SSRI, and 823 (86.1%) did not use antidepressants. At baseline, TCA users exhibited lower heart rate variability and higher norepinephrine levels compared with SSRI users and antidepressant non-users. At the end of the observational period, 52.3% of the TCA users had died compared with 38.2% in the SSRI group and 37.3% in the control group. The adjusted hazard ratio (HR) for TCA use compared with non-use was 1.74 [95% confidence interval (CI) 1.12–2.69, p = 0.01]. Further adjustment for measures of autonomic function reduced the association between TCA use and mortality (HR = 1.27, 95% CI 0.67–2.43, p = 0.47). SSRI use was not associated with mortality (HR = 1.15, 95% CI 0.81–1.64, p = 0.44).ConclusionsThe use of TCA was associated with increased mortality. This association was at least partially mediated by differences in autonomic function. Our findings suggest that TCA should be avoided in patients with coronary heart disease.
