Influence of combined use of gas-liquid peeling and PDT on quality of life of patients with visible signs of involutive skin changes

Objective of the study. To develop a questionnaire to determine the quality of life of a patient with visible signs of involutive skin changes. To determine the effect of the combined use of gas-liquid peeling and PDT on the quality of life of patients with visible signs of involutive skin changes.Materials and methods. The study involved 50 patients aged 35 to 45 years. The state of the emotional sphere was assessed before and after the course of treatment using the following methods: a scale for assessing discomfort; ‘The Scale of States’ methodology by А. B. Leonova, Hospital Anxiety and Depression Scale (HADS), Dermatological Life Quality Index (DLQI). The condition was assessed before and after treatment (gas-liquid peeling and photodynamic therapy).Results. The use of the questionnaire developed by us made it possible to determine the index for assessing the quality of life in persons with agerelated skin changes and to reveal the influence of the latter on the emotional sphere and behavior of the respondents. A clear positive trend was observed when comparing indicators of discomfort, anxiety, depression, quality of life. The data obtained make it possible to recommend an assessment of the dermatological index of the quality of life in patients with age-related skin changes as a criterion for the effectiveness of the therapy, and also focus on the need for further study and correction of factors that cause deterioration in life in this category of women.

Dupilumab is an anti-interleukin-4 receptor monoclonal antibody that was recently approved for the treatment of atopic dermatitis (AD). In this single-center retrospective study, clinical baseline data of 117 severe AD patients treated with dupilumab were collected. At baseline and at weeks 4 and 16, disease severity was assessed through the Eczema Area and Severity Index (EASI) and quality of life through the Dermatology Life Quality Index (DLQI) questionnaire, Patient-Oriented Eczema Measure (POEM), Hospital Anxiety and Depression Scale (HADS), Peak Pruritus Numerical Rating Scale (NRS-itch), and VAS-sleep. Response to dupilumab was defined as an improvement of ≥75% in EASI from baseline (EASI75). At multivariate analysis, AD onset before 18 years [OR, 2.9; 95% CI, 1.2–7.2; p = 0.0207] and absence of hypereosinophilia [OR, 2.24; 95% CI, 1.03–4.86; p = 0.0412] were identified as significant predictive parameters for response to dupilumab in terms of EASI75 at week 4 but not at week 16. Significant reductions in EASI, DLQI, POEM, HADS, NRS-itch, and VAS-sleep were found between week 4 versus baseline (p < 0.0001 for all) and week 16 versus baseline (p < 0.0001 for all). Early AD onset and absence of hypereosinophilia may be suggested as predictive markers of early response to dupilumab. We confirmed the efficacy and safety of this agent along with the improvement of life quality in severe AD patients.

Introduction. The article provides latest data on modern methods of treating rosacea. The results of own clinical observations of patients with moderate to severe papulopustular rosacea receiving combination treatment and a comparative analysis of the efficacy of various therapy regimens are presented.Objective of the study. The aim of the study was to conduct a comparative analysis of the therapeutic efficacy of combination therapy using the ivermectin 1% topical drug combined with systemic therapy drugs (doxycycline, minocycline, isotretinoin).Materials and methods. We observed 37 patients with moderate to severe papulo-pustular rosacea subtype. The patients were divided into 4 groups (A, B, C, D). Patients in the control group received monotherapy with 1% ivemectin topical drug, patients in the other groups received combination therapy (1% ivermectin combined with low-dose doxycycline, minocycline and isotretinoin). The efficacy of the therapy was evaluated by measuring rosacea severity on the Scale for Diagnostic Assessment of Rosacea (SDAR), clinical manifestations according to the IGA (Investigator Global Assessment) criteria, and by assessing the patients' quality of life using the DLQI (Dermatology Life Quality Index) questionnaire before and after 3-month treatment.Results. The comparative analysis of changes in severity indicators of the skin process and quality of life in patients with moderate to severe papulopustular rosacea after topical and combination therapy showed that the results of the treatment in patients receiving combination therapy were more significant than those in the group receiving monotherapy.Conclusion. The concomitant use of 1% ivermectin and systemic drugs is most effective in patients with severe papulopustular rosacea subtype. The combination treatment tailored to the clinical forms and severity of rosacea allows to optimize the clinical results of the therapy, which significantly affects the patients' quality of life and opens up potential for an individual approach in the algorithms for the treatment of rosacea.

Indications to operation and choice of the operation volume were specified by morphological diagnostics refinement.

Background: Age-related macular degeneration (AMD) is a disease with an increasing incidence due to the general aging of the population that decreases the patient’s quality of life. This work aims to study whether selective cut optical filters improve the AMD patient’s quality of life. Methods: Prospective and longitudinal study in 79 patients. Visual acuity, contrast sensitivity, and the line differences in the Colenbrander test were measured. Patients answered The National Eye Institute 25-Item Visual Function Questionnaire (NEI VFQ-25), which measures the quality of life related to vision before and after using cut optical filters. Results: There was an improvement of 5.99 points (3.7–8.3) in NEI VFQ-25 after wearing filters. This improvement was 4.0 points for 450-nm filters and 12.7 points for 511-nm filters. For patients with visual acuity (VA) < 0.25, results of NEI VFQ-25 increased by 10.11 points (1.19–19.02) and for patients with late AMDs, results increased by 5.33 points (1.31–9.35). Conclusions: Selective filters improve the quality of life of patients with AMD. The success rate in the fitting of filters is better for those with VA lower than 0.25 and those with late or advanced AMD.

В статье рассмотрены вопросы изменения качества жизни лиц пожилого и старческого возраста с ХСН и промежуточной ФВ (ХСНпрФВ) в процессе длительной (12 мес) фармакотерапии. Были обследованы 377 больных, из них 129 - 45-59 лет (средний возраст), 128 - 60-74 лет (пожилой возраст), 120 - 75 лет и старше (старческого возраста). Диагноз устанавливали в соответствии с Рекомендациями по диагностике и лечению ХСН (2013, 2016 г.) Проведена оценка качества жизни по опроснику MLHFQ. Установлено улучшение качества жизни через 12 мес терапии во всех группах больных, но наиболее выраженные результаты получены в старческом возрасте. Максимальное снижение уровня тревоги наблюдали у мужчин средней возрастной категории, у женщин - в пожилом возрасте. В пожилом и старческом возрасте отмечено сопоставимое снижение баллов по шкале депрессии Цунга как у мужчин, так и у женщин. The article deals with the issues of changing the quality of life of elderly and senile people with chronic heart failure with middle range ejection fraction (CHFmrHF) during long-term (12 months) pharmacotherapy. 377 patients were examined, including 129 people aged 45 to 59 years (middle age), 128 people aged 60 to 74 years (elderly), and 120 people aged over 75 years (senile age). The diagnosis was made in accordance with the Recommendations for the diagnosis and treatment of CHF (2013, 2016). The quality of life was assessed according to the MLHFQ questionnaire. There was an improvement in the quality of life after 12 months of therapy in all groups of patients, but the most pronounced results were obtained in old age. The maximum decrease in the level of anxiety was observed in men in the middle age group, in women - in the elderly. In the elderly and senile age, there was a comparable decrease in the scores on the Zung depression scale in both men and women.

Background. Acne vulgaris is a common skin disease that can adversely affect the quality of life of patients.Objective. The aim of this study was to determine the quality of life in patients with acne vulgaris.Methods. This study was carried out on 70 patients with acne vulgaris (28 males, 42 females). All the patients filled out two Persian versions of questionnaires: short form 36 (SF-36) and Dermatology Life Quality Index (DLQI). The obtained data were analyzed by using SPSS software (version 17).Results. The scores for physical functioning, social functioning, and bodily pain domains in patients were over 70%, but the scores for role physical, general health, vitality, role emotional, and mental health in patients were under 70%. Scores on the DLQI in patients with acne vulgaris ranged from 0 to 22 (mean ± SD, 8.18 ± 4.83). After comparing mean score of DLQI with respect to gender and age, it was found that the difference between the two groups was not statistically significant.Conclusion. Acne vulgaris has a significant effect on the quality of life. There was not any significant gender or age related difference in QOL.

Balance disorders are complications of stroke survivors. Aim of this study was the establish effectiveness of the biofeedback approach. In this intervention study 245 patients with early diagnosis of acute disturbance of cerebral circulation (ADCC) were examined. Patients able to move independently were treated by standard conservative ADCC therapy on an outpatient approach, but they continued to have problems with coordination of movement in upright position. Then they were submitted to an increasing physical activity based on five sessions of biofeedback, i.e., a complex rehabilitation of patients with motor pathology "Trust-M" according to TU 9442-001-63704475-2010. Mobility rates were assessed using a web camera. Patients' quality of life was evaluated by SF-36 questionnaire and the Hospital Anxiety and Depression Scale (HADS). All parameters were recorded before and after 5 sessions of biofeedback. After treatment, the stability indicators improved and all patients showed a significant increase in motion rate and quality of life. At the same time, the severity of pain and of depression and anxiety decreased. Negative correlations of average strength between the quadrant and patient HADS scaling rates were obtained. In conclusion, our work shows effectiveness of the biofeedback technique for correcting coordination in stroke survivors.

Aim: to assess the psycho-emotional profile in patients with vulvar lichen sclerosus.Materials and methods. We examined 57 patients with a diagnosis of vulvar lichen sclerosus, the average age of 35.0 ± 0.6 (from 18 to 45) years. The control group included 45 healthy women without dystrophic changes in the external genital organs, who applied for a routine examination. The following methods were used to analyze the psycho-emotional profile: WAM questionnaire (well-being, activity, mood), Hospital Anxiety and Depression Scale (HADS), the Dermatology Life Quality Index (DLQI).Results. In patients with vulvar lichen sclerosus a decrease in indicators “well-being” (24.3 ± 1.7 versus 52.4 ± 5.6 scores) and “mood” (28.7 ± 3.6 versus 58.1 ± 6.1 scores) was revealed; “activity” was changed slightly (41.2 ± 3.2 against 47.4 ± 4.9 scores). Depressive (75.4 % versus 15.6 %) and anxiety (68.4 % versus 20 %) disorders were also more common in this group. Significant deterioration in the quality of life in patients was confirmed by a DIQL (18.79 ± 4.98 scores).Conclusion. Psycho-emotional profile of patients with vulvar lichen sclerosus is characterized by a change in most of the analyzed parameters. The presence of anxiety-depressive disorders can aggravate both the general somatic state and the course of the underlying disease. So activities aimed at improving the quality of life should be included in the rehabilitation of these patients.

Introduction: Silicosis is pneumoconiosis secondary to the inhalation of mineral dust containing silica. It represents a real public health problem in the city of Jerada.Purposes: To assess the functional parameters of silicosis patients (based on the 6-minute walk test (6MWT)) and their impact on our patients' quality of life (Saint George's respiratory questionnaire (SGRQ) and hospital anxiety and depression scale (HAD)).Materials and methodology: This is a cross-sectional study of 147 patients with silicosis, designed to collect their socio-demographic data, to assess their respiratory function (6MWT) and their quality of life (SGRQ).Findings: The average age of our patients was 64.08 +/- 6.46 years [46 -81]. At 6MWT, the mean distance walked by our patients is 326.8 +/-168.47 m (30 - 835) or 59% of the predicted distance, with a median of 360m. The mean total Saint George score (SGT) was 68.02 +/- 19.54%. The means of the other Saint George scores were: symptoms (SGS): 45.95 +/- 23.66%, activities (SGA): 79.58 +/- 19.8%, and impact (SGI): 68, Results: The average age of our patients was 64.08 +/- 6.46 years [46 -81]. At 6MWT, the mean distance walked by our patients is 326.8 +/-168.47 m (30 - 835) or 59% of the predicted distance, with a median of 360m. The mean total Saint George score (SGT) was 68.02 +/- 19.54%. The means of the other Saint George scores were: symptoms (SGS): 45.95 +/- 23.66%, activities (SGA): 79.58 +/- 19.8%, and impact (SGI): 68.32 +/- 22.33%. We found a statistically significant relationship between dyspnea, quality of life, anxiety and walking distance.Conclusion: The study demonstrated that silicosis is a pulmonary disease which causes significant functional limitation and deterioration of life quality.
