We estimated the costs and benefits of a community-led total sanitation (CLTS) intervention using the empirical results from a cluster-randomized controlled trial in rural Ethiopia. We modelled benefits and costs of the intervention over 10 years, as compared to an existing local government program. Health benefits were estimated as the value of averted mortality due to diarrheal disease and the cost of illness arising from averted diarrheal morbidity. We also estimated the value of time savings from avoided open defecation and use of neighbours’ latrines. Intervention delivery costs were estimated top-down based on financial records, while recurrent costs were estimated bottom-up from trial data. We explored methodological and parameter uncertainty using one-way and probabilistic sensitivity analyses. Avoided mortality accounted for 58% of total benefits, followed by time savings from increased access to household latrines. The base case benefit–cost ratio was 3.7 (95% CI: 1.9–5.4) and the net present value was Int’l $1,193,786 (95% CI: 406,017–1,977,960). The sources of the largest uncertainty in one-way sensitivity analyses were the effect of the CLTS intervention and the assumed lifespan of an improved latrine. Our results suggest that CLTS interventions can yield favourable economic returns, particularly if follow-up after the triggering is implemented intensively and uptake of improved latrines is achieved (as opposed to unimproved).

Port dredging to accommodate larger vessels can create substantial national economic benefits. However, how affected individual states fare economically with dredging is often unclear and can be an important issue. The benefits and the costs to Delaware residents of dredging—with the recent proposed deepening of the Delaware Bay and River main federal channel as a case study—are examined. Benefits include ( a) lower transportation costs that residents might receive on imported goods, ( b) profits that residents would realize if cost savings (e.g., on refinery products) were not passed forward to Delaware users, ( c) project costs that residents would bear as federal taxpayers, and ( d) benefits that residents would realize if the use of dredged sediments for planned beach renourishment created savings to the state. Sensitivity analyses are used to reflect uncertainty in outcomes. The estimated net present value to Delaware today of all future annual quantifiable benefits and costs ranges between $15,528,393 and $14,195,700 over 50 years at 5.875%. Stated another way, the quantified net benefits for Delaware imply a benefit-cost ratio between 2.07 and 1.89. Hence, for every dollar of the $7.5 million that Delaware would pay as a nonfederal cosponsor, estimated quantifiable net benefits to the state are $2.07 to $1.89. Some benefit and cost estimates are vehemently debated between interested parties, and not all benefits and costs can be quantified.

The Cooperative Research Centre (CRC) for Beef Genetic Technologies operated for its third successive 7-year term from July 2005 to June 2012. It developed new genetic and genomic technologies and non-genetic ‘products’ (practices, processes, tools and technologies) to improve profitability, productivity, animal welfare and responsible resource use of Australian beef businesses. In this paper we assess how well the third-term Beef CRC met its objectives, at the end of its funding period, using the Impact Tool software package developed by the CRC Program of the Commonwealth Government. The Impact Tool generates two commonly used measures of return on investment: the net present value (NPV) and the benefit : cost ratio (BCR). The NPV, the sum of discounted benefits minus the sum of discounted costs, was $233.2 m, when evaluated over the period 2005/06–2020/21. The BCR, the sum of discounted benefits divided by the sum of discounted costs, was 2.94, over the same period. Thus on both measures, investing in the Beef CRC is expected to have been profitable. We conclude by noting that the value of the Impact Tool is not only for ex-ante and ex-post evaluation of the impacts of particular technologies, but it also provides a very effective tool for RD&E project planning.

Currently, the pork industry is incorporating in-line automation with the aim of increasing the slaughtered pork carcass throughput while monitoring quality and safety. In Korea, 21 parameters (such as back-fat thickness and carcass weight) are used for quality grading of pork carcasses. Recently, the VCS2000 system—an automatic meat yield grading machine system—was introduced to enhance grading efficiency and therefore increase pork carcass production. The VCS2000 system is able to predict pork carcass yield based on image analysis. This study also conducted an economic analysis of the system using a cost—benefit analysis. The subsection items of the cost-benefit analysis considered were net present value (NPV), internal rate of return (IRR), and benefit/cost ratio (BC ratio), and each method was verified through sensitivity analysis. For our analysis, the benefits were grouped into three categories: the benefits of reducing labor costs, the benefits of improving meat yield production, and the benefits of reducing pig feed consumption through optimization. The cost-benefit analysis of the system resulted in an NPV of approximately 615.6 million Korean won, an IRR of 13.52%, and a B/C ratio of 1.65.

Abstract Background Methicillin-resistant Staphylococcus aureus bloodstream infections (MRSAB) cause significant mortality and often require extended antibiotic therapy. Vancomycin, the most common initial MRSAB treatment, carries significant monitoring burden and nephrotoxicity risks. We compared cost-effectiveness of vancomycin and other antibiotic regimens as MRSAB treatment. Methods We estimated cost-effectiveness of intravenous antibiotics (vancomycin, daptomycin, linezolid, ceftaroline/daptomycin, dalbavancin) for Veterans Health Administration (VA) patients with MRSAB using an exploratory decision-tree model. Primary effectiveness outcome was composite of microbiological failure and adverse drug event (ADE)-related discontinuation at 7-days. Results In base-case analyses, linezolid and daptomycin were less expensive and had fewer treatment failures than other regimens at 4 and 6-weeks. Compared to linezolid, daptomycin incremental cost-effectiveness ratios were ~$45,000 (4-weeks) and ~$61,000 (6-weeks) per composite failure avoided, respectively. In one-way sensitivity analyses, daptomycin (4-weeks) was favored over linezolid if linezolid microbiological failure or ADE-related discontinuation rates were >14.8% (base case: 14.0%) or >14.3% (base case: 14.0%), respectively, assuming a willingness to pay (WTP) threshold of $40,000/ composite treatment failure avoided. Vancomycin was favored if its microbiological failure risk was < 16.4% (base case: 27.2%). In two-way sensitivity analyses, daptomycin was favored if linezolid microbiological failure and ADE-related discontinuation rates were >19% and > 16%, respectively. Linezolid, daptomycin and vancomycin were favored in 47%, 39%, and 11% of 4-week probabilistic iterations, respectively, at $40,000 WTP. Conclusion Daptomycin or linezolid are likely less expensive and more effective than vancomycin or other initial regimens for MRSAB. More data are needed to support safety of linezolid in MRSAB patients. Disclosures Cornelius J.
