Due to the conditions that cause the spread of COVID-19, national health systems worldwide are under severe strain. Most countries face similar difficulties such as a lack of medical personnel and equipment and tools for diagnosis and treatment, overrun hospitals, and forced restriction of planned medical care. Public authorities in healthcare take the following measures due to increased pressure: limiting the transmission and spread of the virus (social distancing and quarantine), mobilizing medical personnel, ensuring the availability of diagnostic and treatment tools, and providing a sufficient number of premises, which are not always suitable for the provision of medical care (buildings and structures). To date, the stages of management decision-making to counter coronavirus infection and the risk of COVID-19 transmission at various facilities have not been analyzed. The authors propose a methodology for assessing the COVID-19 transmission risk at various social and transport facilities. A survey of 1325 respondents from Moscow demonstrated the most significant risk factors, such as visitation avoidance, infection risk, and facemask wearing. Risk categories were determined and objects classified according to high, medium, and low-risk levels.

Objective. Generalization of management decision-making experience and analysis of organizational measures taken during the preparation of a multidisciplinary hospital for mass admission of infectious patients with COVID-19. Materials and methods. The materials of the study included the generalized data on the results of work of a multidisciplinary hospital in conditions of mass admission of patients with COVID-19. The current guidelines, defining the key spheres of work of the head of medical organization and practical results of their implementation are analyzed. Results. The article analyzes the main activities of a multidisciplinary hospital in conditions of mass admission of patients with COVID-19. It is established that a set of measures, including preparatory preventive solutions, timely planning, re-profiling into a multidisciplinary infectious hospital with reserve beds (including resuscitation beds), creation of mobile teams to provide medical care in the foci of the territorial area of medical responsibility, timely training and reallocation of medical personnel to maximize the effective use of staff resources, reasonable routing of infectious patients, introduction of a three-level system for providing medical care to patients with COVID-19, made it possible to effectively use the available forces and resources of a multidisciplinary hospital during the COVID-19 pandemic. Conclusions. The measures taken proved to be effective, allowing a full provision of emergency medical care throughout the epidemic period, and resuming the provision of planned care in a full volume without reducing the safety of patients' stay in the hospital. Differentiation of patients flows, constant monitoring of the body temperature in both patients and medical workers themselves, constant use of PPE and motivated disinfection, regular check of employees for coronavirus infection (nasopharyngeal smear for COVID-2019 as well as blood ELISA for this infection) permitted to minimize the risks of spreading coronavirus infection in conditions of medical organization.

The aim of the study is to develop and apply a mathematical model for assessing the risks of contamination of medical personnel involved in providing medical care to patients with COVID-19 in a "red zone" environment. Materials and methods. Based on the analysis of informative signs and information on working conditions in the infectious disease department of the A.I. Burnazyan Federal Medical Biophysical Center of the Federal Medical and Biological Agency of Russia, a decision-making support system was developed to provide an objective assessment of the risks of infection for medical personnel when providing medical care in the "red zone". Results of the study and their analysis. The influence of various risk factors for infection of medical personnel involved in the provision of medical care to patients with new coronavirus infection COVID-19 was analyzed; the most significant risk factors were identified.

Subject. This article considers the issues of provision of supplementary education services for children and methodological tools for the formation of educational environment. Objectives. The article aims to assess the level of development of the system of supplementary education of children in Krasnoyarsk and offer a methodological approach to improving the management decision-making procedure in the formation of a portfolio of supplementary education services at the municipal level. Methods. For the study, we used the methods of theoretical, empirical, and logistic analyses, and sociological studies. Results. The article offers concrete solutions to the lack of a methodological approach to providing supplementary education services, considering one of the micro-districts of Krasnoyarsk as a case in point. It also offers tools that can be used by public authorities to organize educational space at the municipal level. Conclusions. A unified methodological approach should be used to provide a system of supplementary education, taking into account financial means, as well as differentiation in the distribution of educational facilities throughout the area. The relevance of the set of supplementary education services and consumer preferences should be taken into account, as well.

Currently, work in any medical organization carries a risk of coronavirus infection, and, first of all, this applies to medical organizations dealing with the treatment of patients infected with coronavirus. Medical workers are a group at increased risk of infection with the SARS-CoV-2 virus in the provision of medical care in modern conditions, which determines their incidence of COVID-19 [1-3]. The aim of the study is to explore the prevalence of cases of COVID-19 infection in medical workers providing medical care on an outpatient basis. Materials and methods. The analysis of the incidence of COVID-19 medical workers in one of the large medical organizations of the Samara region during the period of the pandemic was carried out. The medical staff of the medical organization includes 207 doctors. There are 11 subdivisions in the structure of the medical and prophylactic institution. Research methods: content analysis of the modern regulatory framework (2020-2021), statistical, expert. For the period from May 2020 to March 2021 71 cases of infection of medical workers on an outpatient basis with SARS-CoV-2 were identified, of which 32 doctors (45%) and 39 people (55%) nurses. Based on the results of the expert assessment, the insurance nature of the infection case was established in 50 people (70%). Among the medical and nursing categories of medical workers, the largest number of cases was made by doctors and nurses of primary contact - specialists of the district service: general practitioners (40%), pediatricians (32%), doctors - obstetricians-gynecologists (12%), nurses adults (76%) and children (20%) polyclinic departments. A third of health workers received inpatient treatment for severe COVID-19, an average of 60 days. Conclusion. Medical workers are a risk group of COVID-19 who need support measures, since the possibility of occupational infection has been sufficiently proven, and the risk probably depends on the work performed and the conditions of direct contact of medical personnel, which requires further study in the current persisting epidemic conditions.

Synopsis Used hypodermic needles were found in Stanton Convention Centre. The Centre’s department heads suggested installing secure needle receptacles. June Patterson, General Manager, quickly learned this was a divisive issue. Heated arguments focussed on two opinions: first, the Centre owed employees a safe working environment and needles constituted a significant risk to safety. Second, other department heads believed that presence of needle boxes would diminish customers’ perceptions of the Centre. According to one, “You wouldn’t find needle boxes in nice restaurants or golf courses.” Having promised a decision by the next meeting, Patterson mulled the question over and wondered how to proceed. Research methodology This case was written based on information obtained in interviews with the manager described in the case. The manager’s name, the name of the organization, and the city where it is located have all been disguised. Relevant courses and levels The authors use this case as the first contact with cases for our incoming MBA cohort in the summer pre-session to acquaint them with the basics of case analysis. However, this compact case could be used in many management courses, graduate or undergraduate, to illustrate difficulties in management decision making for different stakeholder groups. Theoretical bases The concepts most central to our discussions of the case are management decision making and the influences of diverse stakeholder opinions on those decisions.

Introduction. Among the problems of health care during the reform period, the problem of quality management of medical care and effective management of the medical organization is particularly highlighted. The purpose of the study is to determine the main priorities of effective management of the medical organization of obstetric and gynecological profile to ensure the proper quality of medical care to the population. Materials and methods. We carried out quantitative and qualitative evaluation of the perinatal center in the period 2015-2017. Studied the efficiency of the administration of the human resources capacity of health organizations quality indicators of the hospital bed Fund and the financial-economic activity of enterprises. Results. The effective activity of the institution in the conditions of the established perinatal center is Shown due to the chosen strategy and tactics of the phased development of new obstetric, neonatal, inpatient-replacement technologies, the provision of highly specialized care with the use of modern equipment and medicines, which allowed to expand the range and volume of Advisory and diagnostic and treatment services, to reduce the level of perinatal and maternal losses. Discussion. The strengths of the organizational activities of the perinatal center management include: the inclusion of women’s consultation in the structure of the perinatal center, ensuring the continuity of care for gynecological and pregnant patients; state social support for motherhood and childhood, changing the conditions and procedure for the provision of free medical care to the population; functioning on the basis of the perinatal center of the Department of pathology of newborns, Department of resuscitation and intensive care of newborns, consultative and diagnostic and gynecological departments. The weak side of the organization can be attributed to the shortage of highly qualified personnel (doctors, nurses and Junior medical staff). Conclusion. In order to effectively use the resources of the institution, it is important to change the approach to the provision of material resources and their rational use in the process of the institution’s activities. The chosen strategy and tactics of management decision-making makes it possible to improve the quality and availability of medical care for pregnant women, mothers, maternity and newborn children.

AbstractGlobal pandemics pose unprecedented challenges to healthcare. In the current situation, telemedicine appears to be a sustainable and effective solution in the implementation of precautions, prevention and treatment in order to discontinue the spreading of the coronavirus infection (COVID-19).The aim of this paper was to research and analyze the role of telemedicine in assistance to healthcare in the COVID-19 pandemic.Methods: The PubMed Database, Google Scholar, SpringerLink, ScienceDirect (Elsevier) and sites of public authorities have been accessed and reviewed.Results: Advantages, restrictions, policy and application of telemedicine in the conditions of COVID-19 pandemic have been studied and analyzed.Conclusion: Healthcare systems are put under an extreme test during global pandemics. Telemedicine is crucial during the COVID-19 pandemic because it guarantees that patients have access to medical care.

Epidemiologic situation remains a serious concern all over the world due to the coronavirus infection spread. It is vital to adhere to measure of non-specific COVID-19 prevention. According to literature data, the highest risks of the infection spread occur in public transport, retail outlets where foods and nonfoods are sold, medical organizations, and chemists’ shops. Our research objects were socio-hygienic and behavioral indicators that were risk factors of the coronavirus infection. Our research aim was to substantiate a score estimate of the COVID-19 contagion risk based on socio-hygienic and behavioral indicators. Questioning was applied to determine frequency of visiting public places and adherence to basic measurements of non-specific COVID-19 prevention; overall, 400 respondents took part in it. A questionnaire was developed by experts of the De-partment of General Hygiene at Sechenov University and contained questions aimed at revealing informative signs (risk factors) of the coronavirus infection spread. Cluster analysis was applied to group respondents’ questions and to identify informative signs for further development of a scale showing risk categories. Factor analysis in a form of principal component analysis was applied to questions that had the highest number of statistically significant indicators of Spearman’s correlation coefficient. We developed a procedure for assessing risks of COVID-19 contagion according to socio-hygienic and behavioral in-dicators and substantiated risk categories. The most significant risk factors were indicators related to mandatory mask wearing when visiting specific social objects (risk objects); when taking trips by various means of public transport and duration of such trips; keeping social distance when visiting social objects. We performed score estimate of risk categories regarding COVID-19 contagion.

The purpose of our work was to increase the efficiency of medical care provided to patients with the new coronavirus infection COVID-19 who were admitted to a multidisciplinary hospital, based on a retrospective analysis of the work carried out to reorganise material-technical and personnel support, organise operations in an unfavourable epidemiological situation in the period from 16.05–06.11.2020. During the specified work period, data on 677 hospitalised patients were analysed. The practical experience of the FGBUZ CCH RAS demonstrated the feasibility of a quick response to unfavourable development of the epidemiological situation associated with the spread of COVID-19 via solving the problem of reorganising a multidisciplinary hospital, taking into account the implementation of organisational measures that provide the material and technical basis for the diagnosis and treatment of patients and measures aimed at preventing the spread of infection among medical personnel and patients. Our data confirmed that complications related to infection with COVID-19 and which led to the need to use intensive care measures were more typical for people in the older age group: with an average age of 57 ± 6 years for men and 64 ± 7 years for women that were admitted to the hospital. A set of measures to ensure the safety of personnel involved in providing medical care to patients with the new coronavirus infection minimises the risks of personnel infection.

Natural disasters are caused by the exposure and vulnerabilities to natural hazards of people, infrastructure and economic activities. Analysis of these factors has permitted identification of countries and areas within them where disaster-related mortality and economic losses are likely in the future. These high-risk areas are candidates for increased attention to, and investment in, disaster risk identification, reduction and transfer. Plans are underway to further identify disaster risk levels and factors on national and subnational scales in high-risk countries to create evidence for improved risk management decision-making. In this paper, I review selected recent global and regional risk analyses to highlight findings, areas for improvement and next steps in the overall process of using disaster risk information for more effective risk management and cost-effective reduction of losses.
