So far, there is no consistent and convincing theory explaining the pathogenesis of migraines. Vascular disorders, the effect of oxidative stress on neurons, and the contribution of magnesium-calcium deficiencies in triggering cortical depression and abnormal glutaminergic neurotransmission are taken into account. However, there are no reliable publications confirming the role of dietary deficits of magnesium and latent tetany as factors triggering migraine attacks. The aim of the study was to evaluate the influence of latent magnesium deficiency assessed with the electrophysiological tetany test on the course of migraine. The study included: a group of 35 patients (29 women and six men; in mean age 41 years) with migraine and a control group of 24 (17 women and seven men; in mean age 39 years) healthy volunteers. Migraine diagnosis was based on the International Headache Society criteria, 3rd edition. All patients and controls after full general and neurological examination were subjected to a standard electrophysiological ischemic tetany test. Moreover, the level of magnesium in blood serum was tested and was in the normal range in all patients. Then, the incidence of a positive tetany EMG test results in the migraine group and the results in the subgroups with and without aura were compared to the results in the control group. Moreover, the relationship between clinical markers of spasmophilia and the results of the tetany test was investigated in the migraine group. As well as the relationship between migraine frequency and tetany test results. There was no statistically significant difference in the occurrence of the electrophysiological exponent of spasmophilia between the migraine and control group. Neither correlation between the occurrence of clinical symptoms nor the frequency of migraine attacks and the results of the tetany test was stated (p > 0.05). However, there was an apparent statistical difference between the subgroup of migraine patients with aura in relation to the control group (p < 0.05). The result raises hope to find a trigger for migraine attacks of this clinical form, the more that this factor may turn out to be easy to supplement with dietary supplementation.

Background: Migraine is an important cause of headache and headache-related disabilities. It increases loss of working time, causes inability to carry out daily activities and disruption of family and social life. The pathophysiology of migraine is still poorly understood. On the other hand, the prevalence of obesity is constantly increasing worldwide. The consequence of overweight and obesity includes increased risk of diabetes mellitus, dyslipidemia, hypertension, cardiovascular disease and cancer.Objectives: This study was performed to assess the relationship between BMI and migraine by finding out the relationship between migraine frequency and duration in different BMI groups, comparing the socio-demographic variables in migraine and non-migraine patients and to find out the migraine related co-morbidities.Materials and Methods: This observational case-control study was conducted on 100 subjects aged 12–50 years in the Neurology Outpatient Department, Mymensingh Medical College Hospital, Mymensingh in the period of January 2011 to December 2012. Out of total subjects fifty migraine patients were selected as cases and fifty nonmigraineurs as controls. Subjects were then categorized in three groups based on BMI: <23, 23 to 25 and >25. Collected data were compiled and appropriate analyses were done by using computer based software, Statistical Package for Social Sciences (SPSS) version 16.0. For statistical analysis one way ANOVA tests were done for comparing means of quantitative data and Chi-square tests were done for qualitative data. A p value <0.05 was considered statistically significant.Results: In this study, majority (>95%) of the study subjects were between 10 and 35 years of age. Mean age of case group was found 25.55 ± 5.87 and that of control was 25.53 ± 4.22 years. Case group contained 10 (20%) males and 40 (80%) females whereas control group had 14 (28%) males and 36 (72%) females. Number of female cases and controls were higher than that of male. In control group 30 (60%) were unmarried and 20 (40%) were married. There was no significant difference in the financial condition between case and control groups. Students and housewives occupied the largest number of study subjects who were unemployed. Regarding residence, rural and urban patients were equal in case group and in control group 29 (58%) were urban and 21 (42%) rural. Mean duration of headache was 8.9 ± 7.5, 5.8 ± 6.7, 9.6 ± 14.3 years in different BMI groups (<23, 23–25, >25 respectively) in case group and 4.6 ± 5.3, 4.4 ± 3.4, 3.4 ± 1.4 years in control group respectively. There was significant difference in quality of headache. In migrainous group 58.6% in BMI <23, 30% in BMI 23–25, 36.4% in BMI >25 noted their headache as throbbing, in contrast most of the nonmigrainous described them as dull in nature. Mean frequency of headache per month was significantly higher in migraine group compared to non-migraine group (p=0.02). Regarding associated symptoms, nausea, vomiting, photophobia and phonophobia were observed significantly higher in migraine patients in BMI <23 group. Odd ratios (ORs) for vomiting, photophobia and phonophobia with 95% confidence interval (CI) were 23.385 (2.752–97.739), 16.500 (3.060– 88.971) and 13.000 (2.922–57.846) respectively. Smoking was found significantly higher in nonmigrainous group than migraine group in case of BMI <23.Conclusion: From the result of present study it can be concluded that there is statistically no significant relation of BMI with frequency of headache, but some relationship were observed for associated symptoms of migraine with low BMI.J Enam Med Col 2016; 6(2): 80-87

<b><i>Introduction:</i></b> This study aimed to reveal whether retinal nerve fiber layer (RNFL), ganglion cell layer (GCL) inner plexiform layer, and choroidal layer (CL) thicknesses differed in patients with migraine. Optical coherence tomography (OCT) was used to measure these neurovascular structural changes and determine the relationship between these structures and cranial white matter hyperintensities (WMHs). <b><i>Methods:</i></b> This retrospective comparative registry study included a total of 155 individuals aged 18–55 (mean, 33.50 ± 8.34), consisting of 110 migraine patients and 45 healthy controls. <b><i>Results:</i></b> RNFLs were thinner in the migraine group than the control group but not to a statistically significant degree. However, in both eyes, peripapillary RNLF thickness in some specific quadrants was found to be significantly thinner in the patient group than the control group. GCLs were significantly thinner in the migraine group than the control group. CLs were significantly thicker in the migraine group than in the control group. There was no significant difference between the OCT parameters of patients with and without WMH. An inverse correlation was found between disease duration and CL thickness. CLs were significantly thicker in patients in attack periods than those in attack-free periods. There was no significant difference between the OCT parameters of the migraine with aura and migraine without aura subgroups. <b><i>Discussion/Conclusions:</i></b> Retinal neural and vascular structures might be affected in migraine sufferers, including those in subgroups. Rebound vasodilation may cause alterations in CL thickness during a migraine attack. Factors other than hypoperfusion may contribute to the pathophysiology responsible for the formation of WMH.

Background Headaches and sleep disorders may influenceacademic performances, personality, memory, and interpersonalrelationships. Migraine is the most common headache type seen inadolescents. Although headaches and sleep disorders are believedto be related, there has been little study in this area.Objectives To assess the relationship between migraine andsleep disorders in adolescents and compare different types ofsleep disorders found in adolescents with migraine vs. healthyadolescents.Methods We conducted a cross sectional study in December 2009on students of three junior high schools in the Secanggang District,Langkat Regency, North Sumatera. We included adolescents aged12 to 17 years who suffered from migraines, as defined by theInternational Cftissificaticm of Headache Disorders, 2nd edition criteria(ICHD-II), and h ealthy adolescents who did not experien cemigraines as the control group. Parents filled questionnaires ontheir child's sleep patterns for one week.Results A total of 100 adolescents were enrolled in the study,consisted of 50 adolescents in the migraine group and the others50 in the control group. There was a significant difference in theincidence of sleep disorders between the two groups (76% and30%, in the case and control groups, respectively; P= 0.0001).Moreover, significant differences were also found in the prevalenceof different sleeping disorder types between the case and controlgroups, i.e. insomnia (62% and 30%, respectively; P= 0.003), sleepapnea (56% and 16%, respectively; P= 0.0001), restlessness (56%and 18%, respectively; P= 0.0001) , parasomnia (76% and 10%,respectively; P= 0.0001), narcolepsy (42% and 16%, respectively;P= 0.008), and excessive daytime sleepiness (50% and 26%,respectively; P= 0.023).Conclusions Migraine in adolescents is significantly associatedwith sleep disorders. Parasomnia is the most common type ofsleeping disorder observed in adolescents with migraines in ourstudy.

Abstract Objective Numerous studies have identified impaired decision making (DM) under both ambiguity and risk in adult patients with schizophrenia. However, the assessment of DM in patients with adolescent-onset schizophrenia (AOS) has been challenging as a result of the instability and heterogeneity of manifestations. The Iowa Gambling Task (IGT) and Game of Dice Task (GDT), which are frequently used to evaluate DM respectively under ambiguity and risk, are sensitive to adolescents and neuropsychiatric patients. Our research intended to examine the performance of DM in a relatively large sample of patients with AOS using the above-mentioned two tasks. We also aimed to take a closer look at the relationship between DM and symptom severity of schizophrenia. Methods We compared the performance of DM in 71 patients with AOS and 53 well-matched healthy controls using IGT for DM under ambiguity and GDT for DM under risk through net scores, total scores and feedback ration. Neuropsychological tests were conducted in all participants. Clinical symptoms were evaluated by using Positive and Negative Syndrome Scale (PANSS) in 71 patients with AOS. Pearson’s correlation revealed the relationship among total score of DM and clinical and neuropsychological data. Results Compared to healthy controls, patients with AOS failed to show learning effect and had a significant difference on the 5th block in IGT and conducted more disadvantageous choices as well as exhibited worse negative feedback rate in GDT. Apart from DM impairment under risk, diminished DM abilities under ambiguity were found related to poor executive function in AOS in the present study. Conclusions Our findings unveiled the abnormal pattern of DM in AOS, mainly reflected under the risky condition, extending the knowledge on the performance of DM under ambiguity and risk in AOS. Inefficient DM under risk may account for the lagging impulse control and the combined effects of developmental disease. In addition, our study demonstrated that the performance on IGT was related to executive function in AOS.

Abstract Objective: The unprecedented occurrence of a global pandemic is accompanied by both physical and psychological burdens that may impair quality of life. Research relating to COVID-19 aims to determine the effects of the pandemic on vulnerable populations who are at high risk of developing negative health or psychosocial outcomes. Having an ongoing medical condition during a pandemic may lead to greater psychological distress. Increased psychological distress may be due to preventative public health measures (e.g. lockdown), having an ongoing medical condition, or a combination of these factors. Methods: This study analyses data from an online cross-sectional national survey of adults in Ireland and investigates the relationship between comorbidity and psychological distress. Those with a medical condition (n128) were compared to a control group without a medical condition (n128) and matched according to age, gender, annual income, education, and work status during COVID-19. Participants and data were obtained during the first public lockdown in Ireland (27.03.202008.06.2020). Results: Individuals with existing medical conditions reported significantly higher levels of anxiety (p<.01) and felt less gratitude (p.001). Exploratory analysis indicates that anxiety levels were significantly associated with illness perceptions specific to COVID-19. Post-hoc analysis reveal no significant difference between the number of comorbidities and condition type (e.g. respiratory disorders). Conclusion: This research supports individualised supports for people with ongoing medical conditions through the COVID-19 pandemic, and has implications for the consideration of follow-up care specifically for mental health. Findings may also inform future public health policies and post-vaccine support strategies for vulnerable populations.

INTRODUCTION: By examining the relationship between changes in maximum mild-expiratory flow( MMEF) and specific airway conductance(sGaw), parameters with the change in FEV1 when evaluating the spirometer test and the bronchodilator response, we investigated their diagnostic contribution to the bronchodilator response in those with partial responses to FEV1. METHODS: The retrospective study sample consists of data from 112 patients between Jun 1, 2019, and Feb 1, 2020 who applied to the pulmonary function laboratory with a pre-diagnosis of bronchial hyperreactivity as well as body plethysmography test performed together with the reversibility test. RESULTS: MMEF% and ▲sGaw were linearly correlated with ▲FEV1 (respectively r = 0.752; p <0.001, r = 0.611; p <0.001). While there was a significant difference between ▲MMEF% and ▲sGaw between reversible, partially reversible, and irreversible groups (P <0.001), there was no significant difference in ▲sGaw between partial reversible and reversible groups in post-hoc comparisons (P> 0.05). In the binary logistic regression model created between the partially reversible and reversible groups, demographic characteristics, MMEF% and ▲sGaw variables, ▲MMEF was an independent predictor [OR: 1.132; 95% CI (1.036-1.238), p = 0.006]. The 24% threshold for absolute reversibility or partial reversibility calculated with MMEF% was significant, indicating significance at 86.2% sensitivity and 80.8% specificity (AUC: 0.811, 95% CI: 0.686-0.936; p <0.001). This value we found defined 81% of the partially reversible group as reversible. DISCUSSION AND CONCLUSION: We observed that ▲sGaw alone didn' have a determinant contribution for determining reversibility in bronchodilator response, which showed partial reversibility with respect to FEV1 change. We believe that ▲MMEF% can be an independent predictor between these two groups and the calculated threshold value of 24% can be used as a criterion for determining the reversibility in cases where FEV1 is not determinant.

AbstractA total of 104 children aged between 41 and 47 months were selected to study the relationship between language and false belief understanding. Participants were assigned to four different training conditions: discourse, labelling, control (all with deceptive objects), and sentential complements (involving non-deceptive objects). Post-test results showed an improvement in children’s false belief understanding in the discourse and the labelling conditions, but not in the sentential complements with non-deceptive objects or the control group. Furthermore, the most remarkable improvement in false belief understanding occurred in the labelling group. These results suggest that some types of linguistic experience promote the development of false belief understanding, provided that differing perspectives are confronted.

Background: Amplification of inflammation in acute exacerbation of chronic obstructive pulmonary disease (COPD) increases inflammatory mediators and oxidative stress in the airways, pulmonary and systemic circulation that are characterized by increased plasma level of IL-6 and MDA, resulting in worsening of clinical symptoms. Xanthones in mangosteen pericarp have anti-inflammatory and antioxidant effects, potentially as an adjuntive therapy in acute exacerbations of COPD. Methods: The aim of this study was to determine the effect of mangosteen pericarp extract to clinical improvements, plasma level of IL-6 and MDA of acute exacerbation COPD patients. A clinical trial of experimental with pretest and posttest was conducted on 34 acute exacerbation of COPD patients in Dr. Moewardi Hospital Surakarta and Dr. Ario Wirawan Lung Hospital Salatiga from April until May 2016. The sample was taken by consecutive sampling. Subjects were divided by randomized double blind technique into the treatment group (n=17) received mangosteen pericarp extract 2x1100mg/day and control group (n = 17) received placebo. Clinical improvements were measured in CAT score and length of stay. CAT score, plasma level of IL-6 and MDA were measured on admission and at discharge. Length of stay based on the number of days of care in hospitals. Results: There was significant difference (p=0,011) towards decreased of IL-6 plasma level between treatment group (-2,17 ± 3,46 pg/ mL) and control group (+1,67 ± 6,81 pg/mL). There were no significant difference towards decreased of length of stay (p=0,34) between treatment group (4,12 ± 1,54 days) and control group (5,24 ± 2,49 days), towards decreased of CAT score (p=0,252) between treatment group (-19,18 ± 3,96) and control group (-18,24 ± 2,75), and towards decreased of MDA plasma level (p=0,986) between treatment group (+0,03 ± 0,36μmol/L) and control group (+0,35 ± 1,58). Conclusions: The addition of mangosteen pericarp extract 2x1100mg/day during hospitalization was significantly lowered plasma levels of IL-6, but were not significant in lowering the CAT score, shortening the length of stay, and reducing the increase in plasma level of MDA.

Background and aims: Intermuscular lipolysis disorder plays an important role in insulin resistance and diabetes mellitus and perilipin PLIN5 and PLIN3 are the key proteins in regulating muscle cellular lipolysis. Therefore, the purpose of this study was to examine the relationship between the expression of PLIN3 and PLIN5 protein following endurance training in streptozotocin (STZ) rats. Methods: A number of 24 male Wistar rats were randomly divided into low endurance training group (n = 8), high-intensity training group (n = 8), and control group (n = 8). Diabetes was induced in every rat by STZ injection. Three days after injection, the blood samples were taken from the cut tip of the tails of the mice and animals with blood glucose greater than 300 mg/dL were considered diabetic. The training program included eight weeks of aerobic training at different intensities. Training in high- and low-intensity groups included 22-25 and 5-8 m/min of training. Finally, one-way analysis of variance (ANOVA) and correlation was used to determine the significance of the differences between variables, followed by utilizing Tukey’s post-hoc test for significance. Results: The comparison between the groups by ANOVA showed significant differences in PLIN3 (P=0.0006) and PLIN5 (P=0.012). The results of Tukey post hoc test also demonstrated a statistical difference between the mean values of diabetic control group and high-intensity endurance group regarding PLIN3 (P=0.01) and PLIN5 (P=0.009), but no significant increase was observed in the lowintensity exercise group as compared to the control group (PLIN3, P=0.067 & PLIN5, P=0.44). As regards insulin resistance, there was a significant difference among the three groups (P=0.0001). Eventually, the result of the correlation between PLIN3 and PLIN5 showed similar enhancement by increasing the intensity (P=0.0026). Conclusion: According to research results, high-intensity endurance training increased the expression of PLIN3 and PLIN5 in diabetic specimens and PLIN3 and PLIN5 followed a similar increase pattern in high-intensity training

Introduction. The relevance of the study is due to the lack of trained teachers - heads of school mediation services and future mediators. Reducing the deficit is possible only through increasing the competence of teachers in the field of conflict resolution and the formation of the ability and readiness to use mediation practices aimed at creating a conflict-free educational environment and harmonizing interethnic relations in a multicultural educational space. The authors presented a component-wise model of the formation of mediated competence of participants in the educational process from among the heads of school mediation services and future mediators and tested it. Materials and methods. The study used methods of measuring the components of mediation competence: communicative, information-analytical, organizational and managerial, emotional. The study involved two groups: students of the professional retraining program “Mediation. Psychology of Conflict Management ”55 people (experimental group) and 60 people (control group), teachers who are not trained in mediation programs. Comparative analysis of groups was carried out using the U-Mann-Whitney test. Results. Analysis of the effectiveness of the formation of mediation competence showed significant differences (p <0.05) on a number of scales that affect the effectiveness of the formation of components of mediation competence. Significant changes (p <0.05) were revealed in terms of the parameters of the severity of communicative skills, which indicates the formation of a confident partner style of behavior. Changes in the components of emotional competence are significant: emotional awareness (p = 0, 002), emotion management (p = 0, 045), recognition of other people's emotions (p = 0.021). There is a significant difference in the formation of information and analytical competence (p <0.05), primarily in the parameter of motivation (p = 0.000) of using mediation practices to resolve conflicts. Conclusion. It was revealed that the formation of the mediation competence of participants in the educational space of the Krasnoyarsk Territory can go through professional retraining programs and will be effective when combining the efforts of the academic community and practicing mediators, professional mediator communities and the cooperation of resources of various Siberian universities.
