The sustainability of European Long-Term Care systems faces the demographic and socio-economic circumstances, mainly the increasing ageing of the population, with its chronic disease conditions, and the simultaneous economic general crises, exacerbated by the recent COVID-19 pandemic. Beyond the increase in general rate of relative poverty, there is a higher risk of poverty among elderly and families in a high demand of care, especially if situations of Activities Daily Living (ADL) disability are present. Italian welfare, which is based on family care regimes and regional strategies, and is oriented to private or public care, is a relevant case study with which to analyze such a relationship. This paper aims to study the relationship between ADL disability and the socio-economic deprivation of families, that is, household poverty. Variables came from the ISTAT Health for All Italian Database and the INAIL Disability Allowance Database. A pool of statistical methods, based on bivariate and multivariate analyses, from bivariate correlation, through multiple linear regression to principal component factor analysis, were used to reduce the number of the variables and compute the indicators. The multivariate analysis underlines how ADL disability impacts on a household’s poverty, confirming the existence of statistical correlation between them. Moreover, the study identifies and measures two answer capability models to cope with household poverty. The answer capability of the formal system is the main tool for reducing poverty due to one family member’s ADL disability. Integration and collaboration between the formal system and family capabilities remains the main solution.

Abstract Long-term care is being prioritised due to population ageing, and hand in hand with the development of professional provision of long-term care, public expendi-tures will be increasing. Mainly countries with a sharp increase in the number of people aged 80+ will have to address the sustainability of long-term care systems and the pro-curement of relevant services. This paper aims to evaluate the forms of provision and financing of long-term care in selected OECD countries. Provision and funding of long-term care in terms of a formal system are assessed based on selected criteria using analytical methods (principal component analysis and TwoStep cluster analysis). Results of the evaluation carried out in 2008 and 2013 by means of the selected indicators of long-term care, using TwoStep cluster analysis, confirmed both similar as well as different approaches to the provision and financing of long-term care in the analysed countries. The most marked differences in the provision of care based on indicators LTC recipients aged 65+ and LTC recipients in institutions as a percentage of total LTC recipients were found between the first cluster (Australia and Korea with the highest share of LTC recipients) and the second cluster (Czech Republic, Estonia, with the lowest share of LTC recipients). In financing of long-term care (LTC expenditures on institutions as a percentage of total LTC expenditures), the most significant differences were observed between the first (Australia, Korea, with the largest share of LTC expenditures on institutions) and third cluster (mainly Nordic countries, with the lowest share of LTC expenditures on institutions of total LTC expenditures).

The construct validity and reliability of the 1987 Perceived Physical Competence Scale of Lintunen were examined to assess the applicability of the instrument for use with adolescents with disabilities. Subjects were 51 girls and 34 boys ( M age = 15.1 yr.) from several schools in central Finland. Principal component factor analysis with varimax rotation yielded the same two factors for adolescents with disabilities as reported for nondisabled adolescents in the related literature. Cronbach alphas for the two factors were .89 and .56. It was concluded that the scale is an appropriate measure for adolescents with disabilities. Statistical analysis indicated no gender differences for adolescents with disabilities. When compared with nondisabled groups in the related literature, these adolescents had perceived fitness similar to nondisabled peers but significantly lower than that of athletes without disabilities.

AbstractNearly 33 million female youths have an unmet need for voluntary family planning (FP), meaning they are sexually active and do not want to become pregnant. In Ethiopia, age at marriage remains low: 40% and 14% of young women aged 20–24 were married by the ages of 18 and 15, respectively. Despite increases in FP use by married 15- to 24-year-olds from 5% in 2000 to 37% in 2016, unmet need remains high at 19%. Supply-and-demand factors have been shown to limit FP use, yet little is known about how stigma influences FP use among youth. This study validates an anticipated stigma (expectation of discrimination from others) index and explores its effect on unmet need. A cross-sectional survey was implemented with 15- to 24-year-old female youth in Ethiopia in 2016. The analytic sample included married respondents with a demand (met and unmet need) for FP (n=371). A five-item anticipated stigma index (Cronbach’sα=0.66) was developed using principal component factor analysis. These items related to fear, worry and embarrassment when accessing FP. The findings showed that 30% agreed with at least one anticipated stigma question; 44% had an unmet need; 58% were married before age 18; and 100% could name an FP method and knew where to obtain FP. In multivariate regression models, youth who experienced anticipated stigma were significantly more likely to have an unmet need, and those who lived close to a youth-friendly service (YFS) site were significantly less likely to have an unmet need. Interventions should address anticipated stigma while focusing on social norms that restrict married youth from accessing FP; unmet need may be mitigated in the presence of a YFS; and the anticipated stigma index appears valid and reliable but should be tested in other countries and among different adolescent groups.

Health care system of rural familiesAbstract: The aim was to comprehend the health care system of rural families in the south of Rio Grande do Sul. Qualitative research with etnographic orientation, developed in 2014, with 25 interlocutors, residents of a rural territory. Data were analyzed through antropological hermeneutic approach. Health care practices, for these rural families, involve different knowledge, those from the formal system and those from the informal system of health. These practices transit through family care, biomedical systems and services, religiosity, not a single flow, but using different spaces and services, according to their needs. Thus, the realization of an integral health care of the individuals and their families requires professionals that know the community’s cultural context, understand it and value practices and care dybamics that integrate their health care system.Descripstors: Rural Population, Family, Culture, Nursing. Sistema de atención de salud a familias ruralesResumen: El objetivo era comprender el sistema de atención de salud entre las familias rurales en el sur de Rio Grande do Sul. Abstract Examine the composition of breakfast concerning weight status is essential for evaluating adolescent health and understanding this gap. This study aimed to identify breakfast patterns and investigate the relationship with weight status among Brazilian adolescents. We used a subsample of 7,425 adolescents aged 10-19 years from the 2008-2009 Brazilian Household Budget Survey. Breakfast eaters were those with intake of at least 50 kcal (209.2kJ) between 5 and 10 a.m. Breakfast dietary patterns were derived by principal component factor analysis with varimax rotation. We performed logistic regression analyses between breakfast patterns and weight status, considering the complexity of the survey sample design. Three breakfast patterns were identified explaining 44.8% of data variability: (1) the Cereal, protein, fruit beverages and Northern/Northeastern pattern, characterized by high consumption of cookies, meats, dairy products, preparations with corn, eggs, fruit juices/fruit drinks/soy-based drinks, tubers/roots/potatoes, and cereals, and negative adherence to cold cut meat and savory snacks/crackers; (2) the Protein-based pattern, characterized by positive loadings for cold cut meat, milk and cheese, and negative for cookies, fruit juices/fruit drinks/soy-based drinks, tubers/roots/potatoes, and cereals; and (3) the Mixed pattern, with positive loadings for cakes, coffee/tea, bread, fruit juices/fruit drinks/soy-based drinks, chocolate/desserts, and savory snacks/crackers. No association was found between skipping and weight status. Overweight adolescents had lower adherence to the Cereal, protein, fruit beverages and Northern/Northeastern pattern pattern (OR=0.67; 95% CI 0.47; 0.96). This is the first study to address dietary patterns at the meal level with adolescent population-based data, which requires further investigation.

Objectives: The study examined and identified the factors that affect lawyers’ attitudes to knowledge sharing, and their knowledge sharing behaviour. Specifically, it investigated the relationship between the salient beliefs affecting the knowledge sharing attitude of lawyers’, and applied a modified version of the Theory of Reasoned Action (TRA) in the knowledge sharing context, to predict how these factors affect their knowledge sharing behaviour.Method: A field survey of 273 lawyers was carried out, using questionnaire for data collection. Collected data on all variables were structured into grouped frequency distributions. Principal Component Factor Analysis was applied to reduce the constructs and Simple Regression was applied to test the hypotheses. These were tested at 0.05% level of significance. Results: Results showed that expected associations and contributions were the major determinants of lawyers’ attitudes towards knowledge sharing. Expected reward was not significantly related to lawyers’ attitudes towards knowledge sharing. A positive attitude towards knowledge sharing was found to lead to a positive intention to share knowledge, although a positive intention to share knowledge did not significantly predict a positive knowledge sharing behaviour. The level of Information Technology (IT) usage was also found to significantly affect the knowledge sharing behaviour of lawyers’.Conclusion: It was recommended that law firms in the study area should deploy more IT infrastructure and services that encourage effective knowledge sharing amongst lawyers.

Family care of the elderly is key to the long-term care system, and its importance has led to an abundance of research over the past two decades. Several methodological and substantive issues, if addressed, could create even more targeted and interpretable research. The present review critically examines methodological topics (i.e., definitions of family caregiving, measurement of caregiving inputs) and conceptual issues (i.e., family involvement in long-term residential settings, and the care receiver's perspective on care) that have received insufficient attention in the caregiving literature. Throughout this review recommendations are offered to improve these areas and advance the state of the art.
