Hormone profile and erectile dysfunction in men with vibration disease

Vibration disease (WB) is one of the leading diseases among workers at engineering enterprises, in the metallurgical, mining and construction industries, and in various branches of agriculture. The purpose of the study was to study the state of the hormonal profile and the severity of erectile dysfunction in men with vibrational disease. Violations of interaction in the system of the hypothalamic-pituitary-gonadal system in men with EF from the effects of general vibration, which is a harmful production factor in drivers of heavy vehicles, machine operators of agricultural enterprises, were revealed. The decrease in testosterone does not reach statistically significant changes compared with the control group and does not cause the naturally expected increase in the secretion of follicle-stimulating hormone and luteinizing hormone according to the law of feedback.

The article presents data (based on a questionnaire survey) on the impact of vibration disease from the action of local and General vibration on the erectile function of men. The main changes in the parameters of erectile function (frequency, need, etc.) are described in the case of first and second-degree vibrational disease caused by local vibration, and in the case of first-and second-degree vibrational disease caused by General vibration based on data from a questionnaire survey of male patients and control group.

Objectives - to study the severity of androgen deficiency and erectile dysfunction in patients with vibration disease and in its combination with arterial hypertension. Material and methods. 145 male patients with various forms of vibration disease were thoroughly examined. The following groups were defined - I stage and II stage vibration disease induced by local vibration; II stage, induced by local vibration with concurrent arterial hypertension; I and II stage vibration disease induced by whole body vibration; II stage vibration disease induced by whole body vibration with concurrent arterial hypertension. The control group included 30 patients. Evaluation of erectile function in men with vibration disease and its combination with arterial hypertension was carried out using the scale of severity of erectile dysfunction (International Index of Erectile Function, IIEF-5). The questionnaire for detection of androgen deficiency (Dedov II, Kalinchenko SYu, 2006) and the AMS (aging males' symptoms) questionnaire assisted in revealing the symptoms of aging in men. Results. According to the IIEF-5 questionnaire results the erectile dysfunction was defined in patients with vibration disease, most pronounced in II stage of vibration disease induced by whole body vibration and in vibration disease of any etiology with concurrent arterial hypertension. Introduction. The overwhelming number of publications contains only data on the content of individual antioxidants, but not on the overall antioxidant activity of the blood in patients with vibration disease.The aim of the study was to determine the total antioxidant activity of blood serum in patients with vibration disease.Materials and methods. Th e main group consisted of 30 people diagnosed with “Vibration disease” of 1 degree (n=21) and 2 degrees (n=9). Th e control group consisted of 30 clinically healthy men, comparable in age with the main group (p=0.66). Th e total activity of antioxidant systems of blood plasma was evaluated photometrically using the test system “Total antioxidant status-Novo” (“Vector-best”, Russia).Results. The indicator of the total antioxidant status (TAS) was 1,038±0.232 mmol/l in the examined main group, against 1,456±0.225 mmol/l in the examined control group (p<0.000001). Th e coefficient of variation (CV) in patients with vibration disease was 22.35%, 1.45 times higher than in the control group (15.45%). In the main group there was a positive correlation between age and TAS (R=0.525), in the control group there was no such relationship (R=0.095). Th e degree of decrease depended on the severity of vibration disease.Conclusions. 1. The development of vibration disease is accompanied by a decrease in the antioxidant status of blood serum. 2. Th e degree of decrease in the antioxidant status of blood serum correlates with the severity of vibration disease. 3. Reduction of TAS can serve as a pathogenetic justification of the need to include drugs and/or biologically active additives with antioxidant activity in therapy

The menopausal transition can be a challenging period for women’s health and a trigger of uncomfortable symptoms. Beer is the main food source of isoxanthohumol, a precursor of 8-prenylnaringenin, the strongest phytoestrogen identified to date. As phytoestrogens are reported to reduce perimenopausal symptoms, we evaluated if a daily moderate consumption of beer with (AB) and without alcohol (NAB) could improve menopausal symptoms and modify cardiovascular risk factors. A total of 37 postmenopausal women were enrolled in a parallel controlled intervention trial and assigned to three study groups: 16 were administered AB (330 mL/day), 7 NAB (660 mL/day), and 14 were in the control group. After a 6-month follow-up of the 34 participants who finished the trial, both interventions (AB and NAB) significantly reduced the severity of the menopause-related symptoms (p-value AB vs. Control: 0.009; p-value NAB vs. Control: 0.033). Moreover, AB had a beneficial net effect on psychological menopausal discomforts compared to the control group. As the sex hormone profile did not differ significantly between the study groups, the effects of both types of beers (AB and NAB) are attributed to the non-alcoholic fraction of beer. Furthermore, moderate NAB consumption improved the lipid profile and decreased blood pressure in postmenopausal women.

CONTEXT AND OBJECTIVE: Smooth muscle fiber has fundamental importance in erection. Alterations in its function or quantity may be associated with erectile dysfunction. The study objective was to assess the proportion of penile smooth muscle fiber in patients with severe erectile dysfunction. DESIGN AND SETTING: Clinical study, in the Sexual Dysfunction Group, Universidade Federal de São Paulo (Unifesp), and in the Anatomy Laboratory, Universidade Estadual do Rio de Janeiro (UERJ). METHODS: Twenty patients with severe erectile dysfunction were selected to form two groups of ten patients: one with normal arterial flow (age range: 44 to 78 years) and the other with altered arterial flow (age range: 38 to 67 years). These groups were compared with a group formed by ten cadavers aged 18 to 25 years that were presumed to have been potent. Quantification of the smooth muscle fibers was done by means of an immunohistochemical study. RESULTS: The proportion of smooth muscle fiber found was 41.15% for the control group. The patients with erectile dysfunction and normal arterial flow presented 27.24% and those with altered arterial flow presented 25.74%; 19 patients presented at least one chronic disease or risk factor for erectile dysfunction, with prominence for diabetes mellitus, systemic arterial hypertension and smoking. CONCLUSION: Among patients with severe erectile dysfunction, the arterial flow on its own does not present interference in the proportion of smooth muscle fiber. The diminution of the proportion of smooth muscle fiber may result from chronic diseases and vascular risk factors.

Aim. To study the relationship between the severity of erectile dysfunction and the anthropometric parameters of obesity in patients with arterial hypertension. Materials and methods. The study included 71 patients with different severity of erectile dysfunction, overweight and grade 1obesity and arterial hypertension. The anthropometric parameters were analyzed, the degree and characteristics of obesity were assessed according to data of multispiral computed tomography (MSCT), a preliminary assessment of erectile function was performed using the IIEF-5 questionnaire. Later the data of the questionnaire was compared with the in-depth urological study: the collection of anamnesis, examination and conduction of the pharmacodopplerography of the penis vessels. Vibration disease is one of the leading in the structure of occupational diseases. A more detailed study of the pathogenesis, expanding the possibilities of diagnosis of vibration disease, is necessary. The aim of the study is to determine the diagnostic value of stabilometry, quantitative sensory testing and neuroenergocarting in vibration disease associated with the combined effects of local and general vibration. Materials and methods. Two groups of patients were examined: with the diagnosis of vibration disease associated with the combined effect of local and general vibration – 50 people (age 48.7 ± 3.1 years), not in contact with vibration – 30 people (age 49.1 ± 2.8 years). The study of the equilibrium function, the determination of the level of the constant potential of the brain by 12 standard leads and the determination of temperature and pain sensitivity. Statistical processing of the results was carried out using the software package “Statistica 6.0” (StatSoft Inc., USA). Differences were considered statistically significant at p < 0.05. Results. In vibration disease, the vertical stance of patients is more unstable than in patients with no contact with vibration. In patients with vibration disease, there is a decrease in the threshold of cold sensitivity and an increase in the threshold of thermal sensitivity and thermal pain compared to the control group. According to our data, the increased level of permanent potential in patients with vibration disease is observed in the central (22.7 (12.6; 30.7) mV) and right central (20.4 (11.5; 27.1) mV), also the average level of permanent capacity in this group is increased – 17.1 (8.4; 25.8) mV.Conclusion. The results obtained indicate the involvement of the neurosensory complex united by single thalamic and cortical centers in the pathological process and confirm the diagnostic value of the methods used.
