Today, the "healthy worker effect" is understood as organized (on the part of the employer) and/or unorganized (self-selection on the part of the worker, due to the functional capabilities and the state of workers’ health) professional selection of healthier workers in unfavorable working conditions, who remain in the profession. It represents one of the most important problems in epidemiological research in occupational medicine. The “healthy worker effect” and its intensity in various industries (in various professions) can indirectly characterize the harmful and unfavorable conditions. The “healthy worker effect” and its intensity determine the need for medical and social security, as well as rehabilitation measures, not only for people who remain in this production (in this profession), but also for people who left this production or specialty, to prevent the phenomenon of increased mortality among workers who left a given production or profession due to suspension from work based on the results of periodic medical examinations and self-selection, or due to intolerance to working conditions because of the state of health.

AbstractSeveral research works in the related international literature on sociology and health sciences deal with the state of health in one selected population. In these studies, the chosen sample is often connected with special jobs, especially with healthcare professionals and their working conditions. These studies predominantly examine the self-rated subjective health status using questionnaires. There are others that assess the state of health based not only on self-rated subjective indicators, but also using objective data gained by measuring. Considering the international experiences, we chose a special population in our research – healthcare professionals working in an institute for chronically ill psychiatric patients. Our choice was influenced by the fact that we wanted to include their unique working conditions when exploring and assessing their health status. Moreover, our approach was to assess the objective state of health alongside the subjective factors, as our hypothesis was that the majority of the indicators presumably coincided. The data were collected with the help of three questionnaires and some indicators of the objective health statuses were measured. The findings were processed using the SPSS 17.0 mathematical-statistical software package. Following the descriptive statistics, we applied hierarchic cluster-analysis based on results of the WHOQOLD-BREF26 life-quality questionnaire, the WHO WBI-5 Well Being Index, and on the body composition analysis. The results show the objective and subjective health status of population and the factors that influenced it; the working conditions and the interpersonal contacts in the workplace. The conclusion was that in the examined population the subjective and objective health status doesn’t coincide.

Pupils have a high level of chronic disease, the indicators of their physical development are reduced with increasing educational load. In this regard, of particular interest is the state of health of schoolchildren, additionally engaged in active sports in special clubs. Purpose of research. To study the state of health of schoolchildren of St. Petersburg attending the clubs of active sport, depending on age. Materials and methods. Data of medical examinations of 1122 schoolchildren 8-16 years old, engaged in sports schools in St. Petersburg. The Mann-Whitney U-test and the comparison of relative frequencies in two populations were used. Results of the research and discussion. Among children, the most popular sports are football (36.7%) and athletics (29.4%), the prevailing majority of children attend clubs from 1 year to 6.5 years. The proportion of pupils from the second health group was 84.9%, healthy children from the first health group – 13.1%, respectively. The most frequent deviations of musculoskeletal system were posture disorders (69.7% of children) and scoliosis (10.0%), at the same time, by the age of 16, the incidence of posture disorders significantly decreased (p < 0.05), and the proportion of scoliosis increased (p < 0.01). Myopia of weak and moderate degree was revealed in 13.3% of surveyed schoolchildren, the proportion of children with myopia by the age of 16 increased significantly (to 20.7%, p < 0.05). Among children attending sports clubs, fewer have myopia than among pupils enrolled in regular schools. The revealed dynamics of morbidity of the musculoskeletal system among children engaged in sports sections, indicates the need for continuous medical monitoring of the level of physical activity and the need to correct the training regimen in identifying children’s health abnormalities.

Aims - to analyze the historical aspects of the formation of the national normative framework in the field of primary and periodic medical examinations, to discuss the existing problems in the carrying out of primary and periodic medical examinations of workers and also to make suggestions for quality improvement of primary and periodic medical examinations and for introduction of occupational risk assessment into the practice of primary and periodic medical examinations. Conclusion. For quality improvement of primary and periodic medical examinations, for efficiency improvement of primary and secondary prevention of occupational diseases it is considered to be necessary to take the following suggestions: - fuller coverage of working people by periodic medical examinations; - thorough selection of organizations that perform the special assessment of working conditions; - the use of the results of the special assessment of working conditions for improving working conditions and informing workers about working conditions; - implementation of occupational risk assessment into practice of primary and periodic medical examinations; - the use of modern methods of molecular and genetic screening;

Introduction. The ”healthy worker effect” (HWE) is currently understood as organized (on the part of the employer) and/or unorganized (self-selection on the part of the worker, due to the functional capabilities and state of health of workers in unfavourable working conditions, is one of the most critical problems when conducting epidemiological studies in occupational pathology. Purpose and objectives. Analysis of the problem in occupational health and safety, the scientific and medical terminology used in the study of the issue of occupational health and safety, taking into account the possible inversion of the effect of exposure to harmful and unfavourable working conditions taking into account this phenomenon, as well as the intensity of occupational health and safety in various professional cohorts. Results. When conducting epidemiological studies in occupational health and occupational pathology, it is necessary to unify the scientific and medical terminology used in researching HWE and consider the possible inversion of exposure to harmful and unfavourable working conditions taking into account this phenomenon. Conclusions. HWE and its intensity in various professional cohorts can indirectly characterize the harmfulness and unfavorability of conditions. HWE, its degree of severity and intensity determine the need for medical and social security and rehabilitation measures in these professional cohorts.

The importance of occupational selection in prevalence of arterial hypertension in researches of professional cohorts is considered in the article. Because arterial hypertension represents not only the independent disease, but also one of the most important indexes of the functional status of a human body, prevalence of arterial hypertension in professional groups can be substantially caused by occupational selection. As an epidemiological phenomenon of occupational selection "healthy worker effect", known of scientific literature from the middle of last century is considered. Besides, it is offered to consider the regularities of professional selection opposite to "healthy worker effect", as "unhealthy worker effect". The professional conditionality of arterial hypertension considered from epidemiological line items can be distorted by occupational selection. Thus existence of "healthy worker effect" makes impression of absence of direct link between level of occupational influence and frequency of arterial hypertension. At the same time, the increase in prevalence of arterial hypertension not always testifies to its professional conditionality, and can be connected to "unhealthy worker effect". Possible solutions of problems associated with identification of relationships between occupational factors and prevalence of hypertension in the presence of «effects of the healthy» and «unhealthy worker» are discussed. The challenge of considered problem in occupational medicine despite the numerous offered methodological approaches of elimination or decrease in influence of occupational selection is noted.

Abstract. All management decisions in the healthcare are based on reliable data from medical accounting and reporting. Aim. Improve reporting forms in order to optimize accounting and reporting. Materials and methods. One of the main statistical forms of medical reporting that makes it possible to analyze indicators of the health status of workers and the activities of medical organizations in occupational medicine in the Russian Federation is the Form No. 30 “Information about a medical organization”. Results. The indicators analysis of the activities of medical organizations in the Russian Federation providing occupational health care was carried out on the basis of the Form No. 30 “Information about a medical organization”. The reporting forms about the examinations of occupational suitability and examinations of the connection between the disease and the occupation in the Russian Federation have been also developed and improved. The data analysis of medical organizations in the system for accounting and analyzing of occupational diseases «Occupational medicine» posted on the web-site http://profstat-irioh.ru is also carried out. Conclusion. It is proposed to improve the reporting documentation in order to optimize accounting and reporting if necessary. It is also proposed to keep statistics on the mandatory preliminary and periodic medical examinations for workers employed in jobs with harmful and (or) hazardous working conditions in the federal statistical form No. 1-zdrav “Information about the health care organizations”.

Relevance. Health maintanance and professional longevity are primary objectives of medical and psychological support for personnel of EMERCOM of Russia. Information about the state of health of military personnel and civilian personnel of rescue military units of Emercom of Russia, as well as military units for radiation, chemical and biological protection was not found in the available literature.Intention. Clarify data on the state of health of military personnel and civilian personnel of rescue military units of Emercom of Russia for developing evidence-based preventive and rehabilitation measures in the future.Methodology. The health status of the personnel of rescue military units of Emercom of Russia was studied on the model of the Federal State-Owned Institution Nevsky Rescue Center of Emercom of Russia (Kolpino, St. Petersburg) based on the results of periodic medical examinations in 2015–2016.Results and Discussion. The overall prevalence of diseases revealed in military and civilian personnel during the annual medical examination was similar, including the main classes of diseases, and averaged 2 diseases per 1 person. The overall prevalence of diseases in the older age group (30–44 years) of military personnel 1.2–1.4-fold (p = 0.021) exceeded that of the younger (18–29) age group of military personnel as well as of both age groups of civilian personnel. No convincing impact of professional experience on the prevalence of diseases was revealed.Conclusion. The levels and structure of pathology revealed during periodic medical examinations in military personnel and civilian personnel of the Nevsky Rescue Center had no statistically significant differences, suggesting the fundamental similarity of the nature and intensity of occupational factors. At the same time, age rather than the duration of professional activities influenced the frequency of the revealed pathology in the studied sample. Weighted indicators of public health over a longer period of time (5+ years) will help eliminate stochastic effects and make reasonable conclusions about the prevalence of diseases in various military rescue specialists.

Now in the Russian Federation negative tendencies in the state of health of children in all age categories are observed: high incidence, growth of prevalence of chronic pathology, existence of morfo-functional deviations, deterioration in the main quality indicators of health of children. Studying of data on incidence, death rate, disability, physical development and a role of the factors determining them in separate age groupes holds a specific place in assessment of health of children. For receipt of objective assessment of the state of health of the children’s population it is necessary to analyze in interrelation statistics on different sources: database of compulsory health insurances funds, medical examinations of children, accounting and reporting documentation of the medical organizations, computer methods of inspection of the children’s population. Social and hygienic researches on this problem are of great importance for practical health care consisting in development of recommendations about decrease in indicators of incidence, disability and death rate of children, offers of new organizational forms of work of treatment and prevention facilities. We in article provided the analysis of the children of 0-17 years this about the state of health inclusive of St Petersburg for the period 2011-2015. The main demographic characteristics of the children’s population are studied, features of physical development of children of 0-17 years inclusive, incidences, including prevalence of chronic pathology and socially important diseases among the children’s contingent, disability are revealed.
