the group of deformed structures includes buildings that have received unacceptable subsidence and deformation during the period of their construction and especially operation, which, however, do not interfere with the perfor-mance of their main functions, but may eventually collapse. Their causes are errors in engineering and geological surveys and design; violation of the rules for performing construction work and operation of buildings and struc-tures. Long-term geodetic observations of the precipitation of the foundations of buildings on pile foundations have shown that both absolute and relative stabilized values of subsidence in the vast majority of cases are less than them and the normative limit values are calculated. Therefore, the group of deformed buildings on pile foun-dations includes somewhat less often similar objects with shallow foundations. The reasons for excessive subsid-ence of the foundations of pile foundations of buildings (and as a result, the occurrence and development of cracks and other deformations in load – bearing structures), in addition to these, are most often: unjustified use of increasing correction coefficients for the results of compression tests of highly acidic soils; the lower ends of the piles falling into layers of weak soil; the tip of the piles sinking from the design mark; overestimation of the bear-ing capacity of the piles due to non-compliance with the optimal time of their "rest" after immersion or erroneous interpretation of the graphs "load-pile sediment"; excessively close placement of neighboring piles in the plan, which when they are immersed, especially in the sand, leads to "pushing" up previously submerged; uneven load-ing of piles as part of the grillage; deformation of existing buildings and structures when driving piles near and tongue-and-groove, the development of pits, etc.

In order to determine the possibility of using the method of mathematical modeling for making long-term forecasts of channel deformations of trunk line underwater crossing (TLUC) through water obstacles, a methodology for performing and analyzing the results of mathematical modeling of channel deformations in the TLUC zone across the Kuban River is considered. Within the framework of the work, the following tasks were solved: 1) the format and composition of the initial data necessary for mathematical modeling were determined; 2) the procedure for assigning the boundaries of the computational domain of the model was considered, the computational domain was broken down into the computational grid, the zoning of the computational domain was performed by the value of the roughness coefficient; 3) the analysis of the results of modeling the water flow was carried out without taking the bottom deformations into account, as well as modeling the bottom deformations, the specifics of the verification and calibration calculations were determined to build a reliable mathematical model; 4) considered the possibility of using the method of mathematical modeling to check the stability of the bottom in the area of TLUC in the presence of man-made dumping or protective structure. It has been established that modeling the flow hydraulics and structure of currents, making short-term forecasts of local high-altitude reshaping of the bottom, determining the tendencies of erosion and accumulation of sediments upstream and downstream of protective structures are applicable for predicting channel deformations in the zone of the TLUC. In all these cases, it is mandatory to have materials from engineering-hydro-meteorological and engineering-geological surveys in an amount sufficient to compile a reliable mathematical model.

Abstract Various information technology (IT) infrastructures for biobanking, networks of biobanks and biomaterial management are described in the literature. As pre-analytical variables play a major role in the downstream interpretation of clinical as well as research results, their documentation is essential. A description for mainly automated documentation of the complete life-cycle of each biospecimen is lacking so far. Here, the example taken is from the University Medical Center Göttingen (UMG), where the workflow of liquid biomaterials is standardized between the central laboratory and the central biobank. The workflow of liquid biomaterials from sample withdrawal to long-term storage in a biobank was analyzed. Essential data such as time and temperature for processing and freezing can be automatically collected. The proposed solution involves only one major interface between the main IT systems of the laboratory and the biobank. It is key to talk to all the involved stakeholders to ensure a functional and accepted solution. Although IT components differ widely between clinics, the proposed way of documenting the complete life-cycle of each biospecimen can be transferred to other university medical centers. The complete documentation of the life-cycle of each biospecimen ensures a good interpretability of downstream routine as well as research results.

Based on the research results, coefficients in constitutive equations, describing the kinetics of dynamic, meta-dynamic, and static recrystallization in high-carbon bainitic steel during hot deformation were determined. The developed mathematical model takes into account the dependence of the changing kinetics in the structural size of the preliminary austenite grains, the value of strain, strain rate, temperature, and time. Physical simulations were carried out on rectangular specimens. Compression tests with a flat state of deformation were carried out using a Gleeble 3800. Based on dilatometric studies, coefficients were determined in constitutive equations, describing the grain growth of the austenite of high-carbon bainite steel under isothermal annealing conditions. The aim of the research was to verify the developed mathematical models in semi-industrial conditions during the hot-rolling process of high-carbon bainite steel. Analysis of the semi-industrial studies of the hot-rolling and long-term annealing process confirmed the correctness of the predicted mathematical models describing the microstructure evolution.

Opinion statementAbout 70–80% of early breast cancer (BC) patients receive adjuvant endocrine therapy (ET) for at least 5 years. ET includes in the majority of cases the use of aromatase inhibitors, as upfront or switch strategy, that lead to impaired bone health. Given the high incidence and also the high prevalence of BC, cancer treatment–induced bone loss (CTIBL) represents the most common long-term adverse event experimented by patients with hormone receptor positive tumours. CTIBL is responsible for osteoporosis occurrence and, as a consequence, fragility fractures that may negatively affect quality of life and survival expectancy. As recommended by main international guidelines, BC women on aromatase inhibitors should be carefully assessed for their fracture risk at baseline and periodically reassessed during adjuvant ET in order to early detect significant worsening in terms of bone health. Antiresorptive agents, together with adequate intake of calcium and vitamin D, should be administered in BC patients during all course of ET, especially in those at high risk of osteoporotic fractures, as calculated by tools available for clinicians. Bisphosphonates, such as zoledronate or pamidronate, and anti-RANKL antibody, denosumab, are the two classes of antiresorptive drugs used in clinical practice with similar efficacy in preventing bone loss induced by aromatase inhibitor therapy. The choice between them, in the absence of direct comparison, should be based on patients’ preference and compliance; the different safety profile is mainly related to the route of administration, although both types of drugs are manageable with due care, since most of the adverse events are predictable and preventable. Despite advances in management of CTIBL, several issues such as the optimal time of starting antiresorptive agents and the duration of treatment remain unanswered. Future clinical trials as well as increased awareness of bone health are needed to improve prevention, assessment and treatment of CTIBL in these long-term survivor patients.

ABSTRACT The purpose of study was to assess long-term clinical and radiographic aspects of dogs’ stifle joints which had undergone a modified tibial tuberosity advancement technique (mTTA). A total of 15 stifles that had undergone mTTA for CCL disease of 11 patients were included in this study. Assessments involved patient’s gait analysis, cranial drawer and tibial compression tests, stifle goniometry range of articular motion, thigh and leg girth and radiographic evidence of progression of osteoarthrosis. Variables were compared between operated and healthy limbs and among moments (M0) on the early postop; (M1) 120 days postop; and (M2) approximately 5 years following surgery. A questionnaire regarding owner’s perceptions after approximately 5 years of surgery was assessed. Most dogs presented positive response to cranial drawer and tibial compression tests on operated knees. There was also decrease on goniometry and thigh girth and increase in leg girth. Radiographic evidence of progression of osteoarthritis was seen especially on the long-term follow-up (M2). On gait analysis, most animals presented some degree of lameness in different conditions, in contrast to owners’ perceptions. Osteoarthritis still develops in dogs following mTTA surgery for CCL disease. However, owners were overall satisfied with their recovery and would be willing to accept indication of mTTA for dogs with ruptured CCL.

Abstract Background: This study explored the optimal time interval between staged bilateral total knee arthroplasty (BTKA) to minimise early complications of the second TKA and maximise long-term function of the first and second knees. Methods: We retrospectively reviewed 266 patients who underwent staged BTKA between 2010 and 2015. Groups 1–4 had time intervals between BTKAs of 1–6, 6–12, 12–18, and 18–24 months, respectively. Demographics, postoperative complications within 90 days of the second TKA, Knee Society Score (KSS), and Western Ontario and McMaster Universities Arthritis Index (WOMAC) score were compared among the groups. Results: In total, 54, 96, 75, and 41 patients were assigned to groups 1–4, respectively. Although group 1 had the highest overall complication rate (11.11%), there was no significant difference in the complication rate among the four groups. Also, no significant differences were found among the four groups in functional and patient-reported outcomes, in either the first or second knee at 5 years postoperatively, including KSS-knee, KSS-function, WOMAC-pain, WOMAC-stiffness, and WOMAC-physical function. The interval between BTKA did not influence complications or the function of the second knee. The TKA type (posterior-stabilised vs. medial-pivot) and age did not correlate significantly with any scores. Conclusions: There was no group difference in early complications of the second TKA, and postoperative function was equivalent between the two knees and did not vary by the interval between surgeries. Our results can help surgeons discuss the timing of the second TKA with patients, which should be based on their own preferences. If patients cannot tolerate severe symptoms in the contralateral knee after the first TKA, the second TKA should be performed as early as possible.

Our research was done on brown forest soil with long-term experiments and with a system of fertilizing which is in use for 40 years. Experiment variants with an increasing dose of nitrogen fertilizer were chosen for this research. Two experiments have been performed: experiment in pots supplied with ammonium nitrate labeled with a stable isotope 15N (11.8%) and experiment in the field. The aim of the research was to establish which plant and soil parameters group (obtained in the controlled conditions and/or in the field) could be considered as reliable for evaluation of aerobic and anaerobic incubation and of the best time for estimation of potentially mineralized nitrogen in soil. According to the determined correlative dependence, it could be concluded that reliability of aerobic incubation should be estimated in October by plant and soil parameters from field, anaerobic incubation should be estimated in early spring (March) by plant and soil parameters, from controlled conditions (pots) and from field.

ABSTRACT Purpose To report refractive, topographic and safety outcomes of corneal cross-linking (CXL) in patients younger than 18 years of age with progressive keratoconus. Materials and methods In this retrospective study, we enrolled 31 eyes of 21 children aged 11 to 17 years that underwent corneal riboflavin-ultraviolet A induced CXL due to progressive keratoconus at three different ophthalmology departments in Israel. They were followed for 3 to 48 months (average 23 ± 13.6 months). Evaluated parameters were uncorrected visual acuity (UCVA), best spectacle corrected visual acuity (BSCVA), manifest refraction, pachymetry, slit- lamp examination and corneal topography at baseline and at 1,3,6,12,24 and 48 months. Results We found a nonsignificant improvement in UCVA and BSCVA with a small reduction of manifest cylinder and no significant change in spherical equivalent or K-values. Following CXL, stability of UCVA and BSCVA at the last follow-up examination was found in 71 and 77% of treated eyes, respectively. No permanent adverse events have been recorded throughout the study period. Conclusion In our series, CXL was a safe procedure in the pediatric population. Stabilization of progressive keratoconus was achieved in visual acuity, refractive and topography parameters with no improvement in corneal indices in contrary to adult CXL treatment. AbstractStarting in 2016, we initiated a pilot tele-antibiotic stewardship program at 2 rural Veterans Affairs medical centers (VAMCs). Antibiotic days of therapy decreased significantly (P < .05) in the acute and long-term care units at both intervention sites, suggesting that tele-stewardship can effectively support antibiotic stewardship practices in rural VAMCs.
