Relevance. According to the study’s results, steady changes in various parts of the immune system: cellular, humeral, phagocytic activity is revealed in patients with type 2 diabetes. Hyperglycemia, hyperlipidemia, insulin resistance, and adaptive ehyperinsulinemia affect the cells of the immune system, promoting the development of metabolic immunosuppressant and forming a stable immunological disorder. The purpose of the study into the morphometric and morphofunctional state of peripheral blood leukocytes in patients with type 2 diabetes to determine their significance in the development of purulent-inflammatory diseases of soft tissues are research. Materials and methods. In this work, the analysis of morphometric, cytogystochemical and laboratory study results of the control (20 healthy volunteers) and thematic (47 patients with supportive soft tissue disease son the background of type 2 diabetes mellitus) study groups with the statistical processing of the obtained results was used. Results. The investigation of morphofunctional features of nonspecific and specific protection cells for purulent inflammatory diseases of tissues on the background of type 2 diabetes showed that the basis of diabetic complications are there vealed changes in the bactericidal activity of segmented neutrophils. The trend of indicators of leukocyte formula and the distribution curve of mononuclear cells by size, as well as the growth of the lymphocyticgranulocytic index, indicate a chronic formation of endotoxin synthesize of diabetes. Conclusions. Lowering the level of cationic proteins, the activity of myeloperoxidase and the NBT-test under stimulation of neutrophil granulocytes bylectinsis a sign of unsatisfactory state of the bactericidal cells system, which may be the cause of chronic and acute inflammatory processes in the body that accompany type 2 diabetes, to a functional exhaustion of the macrophage link in the organism immune defense. Patients with purulent-inflammatory diseases of soft tissues on the background of diabetes type 2 are resistant changes in various parts of the immune system. Hyperglycemia, hyperlipidemia, insulin resistance and adaptive hyperinsulinemia affect the cells of the immune system, promote the development of metabolic immunosuppression with the formation of stable immune dysfunction. The purpose of the study to investigate and analyze the metabolic state of immunocompetent cells in patients with purulent-inflammatory diseases of soft tissues on the background of DMD type 2. The study was conducted in 47 patients (the main group) with purulent-inflammatory diseases of soft-tissue on the background of diabetes mellitus, and 20 healthy volunteers (the comparison group) using the cytohistochemical restoration method of NST based on the percentage composition of neutrophils that have cytoplasm of formazan granules. The spontaneous neutrophil response was studied, as well as the parameters of the NST-test in stimulation of neutrophil granulocytes in vitro to assess the bactericidal, resource potential and their ability to complete phagocytosis. Results of the study showed that the decrease in the activity of myeloperoxidase and the NST-test in the conditions of stimulation of neutrophil granulocytes is a sign of unsatisfactory state of the bactericidal system of cells, which may be the cause of acute inflammatory processes in the body that accompany type 2 diabetes, which leads to functional exhaustion of the macrophage level of the immune body protection. At GZZMT against the background of DM 2 type compared with the norm, the “spontaneous” NST-test of neutrophils is high, which is due to the presence of inflammatory process. The index of stimulation of neutrophils is lower in the main group (1.6 times: 5.3±0.2% in the main group versus 8.4±0.4% in the control, p≤0.05), which is an indicator of a relatively weaker stimulation of those populations of neutrophils that were responsible for high indices in the “spontaneous” NST test, and the impaired state of cellular metabolism. So, in patients with type 2 diabetes, a violation of the bactericidal system of organism protection, which is one of the causes of the development of infectious and inflammatory processes.

In recent years obesity is treated as a civilization disorder. It is believed that it is the cause of diseases of many system; moreover, moreover obesity can promote the development of many types of cancers and is a major health hazard for insulin resistance and type 2 diabetes. Currently, the mechanisms underlying the development of obesity are not completely known. Over many years of experiments different factors contributing to the formation of obesity have been recognized. The discovery of resistin as a protein linking obesity to type 2 diabetes marked the beginning of a period of intensive research on this molecule. However, until now its role in the body has been controversial. In rodent resistin is responsible for the development of insulin resistance, but in humans this effect is ambiguous. This protein has strong proinflammatory properties, which can be associated with the development of inflammatory diseases, including atherosclerosis. It is possible that resistin is involved in the regulation of energy homeostasis, mainly by modulating the metabolism of carbohydrates and lipids; also, it acts as anorexigenic compound – it suppresses appetite. Resistin affects the functioning of many body systems, including reproductive and cardiovascular systems. Recent reports suggest that resistin can play a role in the development of resistance to leptin. The paper presents the current state of knowledge concerning the biological functions of resistin in the organism and its potential role in the development of leptin resistance. Leptin resistance together with hyperinsulinemia and insulin resistance all might lead to disorders of energy homeostasis and the development of numerous diseases.

Background: In recent times much is talked about of serum ferritin, an acute phase reactant a marker of iron stores in the body and its association with diabetes mellitus. Studies implicate that increased body iron stores and subclinical hemochromatosis has been associated with the development of glucose intolerance, type 2 diabetes and its micro as well as macrovascular complications. Material & Methods: This study was carried out to examine and to observe for any relationship between serum ferritin with Type 2 diabetes mellitus. Our study populations were included 163. Among them 81 type 2 diabetes patients as a case (M=49,F=32, mean 44.68 age in years)and 82 normal healthy individual as a control ( M=35, F=47 , mean 34.71 in years). Results: Majority were healthy outpatients who had come for regular checkup and were matched with controls. Serum ferritin and FBS were estimated and other investigations. Results showed that although Serum ferritin was in the normal range value it was increased in type 2 diabetes patients than in controls and was statistically significant, we did get a positive correlation with duration of diabetes. It can be concluded that there were positive associations between serum ferritin and FBG, age, sex among study groups. Conclusion: In conclusion our study shows that there is significant correlation between increased serum ferritin in diabetes compared to individuals with normal blood sugars in this part and hyper ferritinemia may be one of the causes for development of insulin resistance before overt diabetes. Abstract Backgrounds/Aim Sodium glucose co-transporter 2 inhibitors promote osmotic/natriuretic diuresis and reduce excess fluid volume, and this improves cardiovascular outcomes, including hospitalization for heart failure. We sought to assess the effect of empagliflozin on estimated fluid volumes in patients with type 2 diabetes and cardiovascular disease (CVD). Methods The study was a post-hoc analysis of the EMBLEM trial (UMIN000024502), an investigator-initiated, multi-center, placebo-controlled, double-blinded, randomized-controlled trial designed primarily to evaluate the effect of 24 weeks of empagliflozin treatment on vascular endothelial function in patients with type 2 diabetes and established CVD. The analysis compared serial changes between empagliflozin (10 mg once daily, n = 52) and placebo (n = 53) in estimated plasma volume (ePV), calculated by the Straus formula and estimated the extracellular volume (eEV), determined by the body surface area, measured at baseline and 4, 12, and 24 weeks after initiation of treatment. Correlations were examined between the changes from baseline to week 24 in each estimated fluid volume parameter and several clinical variables of interest, including N-terminal pro-brain natriuretic peptide (NT-proBNP) concentration. Results In an analysis using mixed-effects models for repeated measures, relative to placebo empagliflozin reduced ePV by − 2.23% (95% CI − 5.72 to 1.25) at week 4, − 8.07% (− 12.76 to − 3.37) at week 12, and − 5.60% (− 9.87 to − 1.32) at week 24; eEV by − 70.3 mL (95% CI − 136.8 to − 3.8) at week 4, − 135.9 mL (− 209.6 to − 62.3) at week 12, and − 144.4 mL (− 226.3 to − 62.4) at week 24. The effect of empagliflozin on these parameters was mostly consistent across various patient clinical characteristics. The change in log-transformed NT-proBNP was positively correlated with change in ePV (r = 0.351, p = 0.015), but not with change in eEV. Conclusions Our data demonstrated that initiation of empagliflozin treatment substantially reduced estimated fluid volume parameters in patients with type 2 diabetes and CVD, and that this effect was maintained for 24 weeks. Given the early beneficial effect of empagliflozin on cardiovascular outcomes seen in similar patient populations, our findings provide an important insight into the key mechanisms underlying the clinical benefit of the drug. Background: Type 2 diabetes needs a better understanding of etiological factors and management strategies based on lifestyle and constitutional factors, given its high association rate with many cardiovascular, neurological disorders, and COVID-19 infection. Purpose: The present study was undertaken to investigate the effect of Diabetes-specific integrated Yoga lifestyle Protocol (DYP) on glycemic control and lipid profiles of diabetic adults. Along with the DYP intervention, the individuals residing in Chandigarh and Panchkula union territories in the northern part of India were assessed for Ayurveda-based body–mind constitutional type. Ayurveda describes body–mind constitution as “ prakriti,” which has been discussed from two angles, namely physiological and psychological as body and mind are correlated. Methods: Cluster sampling of waitlist control study subjects was used as the sampling method for the study. A total of 1,215 registered subjects (81 diabetic) responded in randomly selected clusters in Chandigarh and Panchkula. Ayurveda physicians did Ayurveda body–mind constitutional assessment called prakriti assessment (physiological body–mind constitution assessment) in 35 participants (23 diabetic, 12 prediabetic) as a part of the study. Results: A group of 50 subjects was randomly selected for yoga intervention out of 81 diabetes mellitus adults, and 31 subjects were enrolled as waitlist controls. A significant decrease in the glycosylated hemoglobin levels from 8.49 ± 1.94% to 7.97 ± 2.20% in the intervention group was noticed. The lipid profiles of the DYP intervention and control groups were monitored. Three-month follow-up results of lipid profile diagnostic tests in intervention and control groups showed a significant difference between the two groups ( P < 0.05). Most diabetic and prediabetic individuals were found to have pitta dosha ( pitta controls all heat, metabolism, and transformation in the mind and body) as dominant constitution type. Conclusion: The study results demonstrated significant positive effects of yoga in diabetic individuals. This study has indicated the evidence for the safety and efficacy of the validated DYP for community-level interventions to prevent maladies like brain damage and stroke.

The bacteria that inhabit us have emerged as factors linking immunity and metabolism. Changes in our microbiota can modify obesity and the immune underpinnings of metabolic diseases such as Type 2 diabetes. Obesity coincides with a low-level systemic inflammation, which also manifests within metabolic tissues such as adipose tissue and liver. This metabolic inflammation can promote insulin resistance and dysglycaemia. However, the obesity and metabolic disease-related immune responses that are compartmentalized in the intestinal environment do not necessarily parallel the inflammatory status of metabolic tissues that control blood glucose. In fact, a permissive immune environment in the gut can exacerbate metabolic tissue inflammation. Unravelling these discordant immune responses in different parts of the body and establishing a connection between nutrients, immunity and the microbiota in the gut is a complex challenge. Recent evidence positions the relationship between host gut barrier function, intestinal T cell responses and specific microbes at the crossroads of obesity and inflammation in metabolic disease. A key problem to be addressed is understanding how metabolite, immune or bacterial signals from the gut are relayed and transferred into systemic or metabolic tissue inflammation that can impair insulin action preceding Type 2 diabetes.

Background: India has been recognized as the diabetes capital of the world by the year of 2025 Hypertension is a signicant danger factor for cardiovascular infection, stroke and ischemic coronary illness. Objective: The aim of this paper is prevalence of hypertension amid patients with type 2 diabetes mellitus- at a tertiary level hospital in central India. Methods: Present work is hospital based cross-sectional study was conduct in Chirayu Medical College & Hospital, Bhopal. The study population consisted of total 300 subjects. Data obtained directly from patients and patients medical les, and the following data were included in the study . Results: The results showed that the after using chi-square test we found that hypertension was signicantly higher p value(<0.05) among female patients (69%),patients with long duration diabetes (77%)10 years and more, also it was signicantly higher among patients with current smokers (69%). Conclusion: In this study the prevalence of hypertension is increasing in patient with T2DM. Hypertension was the most danger factor for micro vascular and macro vascular confusion of T2DM.
