Abstract: Health has a great role in improving people's lives. The health system itself is all the activities that have the primary purpose of improving, repairing, or health care. Many things affect the health system in Indonesian as a developing country, among others is people's behavior. This can make the health system spending more responsive to local conditions and the diversity of disease patterns. Moreover, it can also result in the increase of regional disparities of health financing and reduced health information of national importance. This study aimed to determine the pattern of disease in the polyclinic ENT-HN Prof. Dr. R.D. Kandou General Hospital during January 2010 - December 2012. This was a retrospective descriptive study. The highest number of patients was in 2011 which was 2305 patients. The most common disease found in 2010 was obturans wax. In 2011 and 2012, the most common disease was otitis externa. During the period 2010-2012, the highest frequencies were among civilized employees, females, and aged 45-64 years. Conclusion: There were 10 diseases with the highest frequency during 2010-2012. In 2010, obturans wax was the most frequent cases, followed by otitis externa, sinusitis, chronic pharyngitis, acute pharyngitis, rhinitis, corpus alienum MAE, presbyakusis, acute rhinitis, and acute otitis media. In 2011, otitis externa was the most common disease, followed by obturans wax, chronic pharyngitis, sinusitis maxilaris, chronic purulent otitis media, allergic rhinitis, presbyacusis, corpal, and laryngitis. In 2012, otitis externa was still the most frequent, followwed  by obturans wax, chronic pharyngitis, sinusitis maxilaris, presbyacusis, chronic purulent otitis media, allergic rhinitis, laryngitis, and chronic rhinitis Keywords: disease patterns, outpatient, ENT-HN    Abstrak: Kesehatan berperan besar dalam meningkatkan derajat hidup masyarakat. Sistem kesehatan sendiri merupakan semua aktivitas yang memiliki tujuan utama meningkatkan, memperbaiki, atau merawat kesehatan. Banyak hal yang memengaruhi sistem kesehatan di Indonesia sebagai suatu negara berkembang; salah satunya ialah perilaku masyarakat. Hal ini dapat menyebabkan pengeluaran kesehatan menjadi lebih responsif terhadap kondisi lokal dan keragaman pola penyakit; selain itu, dapat juga berdampak pada meningkatnya ketimpangan pembiayaan kesehatan secara regional dan berkurangnya informasi kesehatan yang penting secara nasional. Penelitian ini bertujuan untuk mendapatkan pola penyakit di poliklinik THT-KL BLU RSU Prof. Dr. R.D. Kandou periode Januari 2010 - Desember 2012. Penelitian ini menggunakan metode retrospektif deskriptif. Hasil penelitian memperlihatkan bahwa kunjungan terbanyak pada tahun 2011 yaitu 2305 orang. Pada tahun 2010, penyakit tersering ditemukan serumen obturans. Tahun 2011 dan 2012, penyakit tersering ditemukan ialah otitis eksterna. Pada periode 2010-2012 pekerjaan pasien yang tertinggi ialah PNS, jenis kelamin perempuan, dan usia 45 – 64 tahun. Simpulan: Terdapat 10 jenis penyakit dengan frekuensi terbanyak. Pada periode 2010 secara berurut (mulai dari kunjungan tersering), yaitu: serumen obturans, otitis eksterna, sinusitis, faringitis kronik, faringitis akut, rinitis alergi, korpus alienum, presbiakusis, rinitis akut, dan otitis media akut. Pada periode 2011, yaitu: otitis eksterna, serumen, serumen obturans, faringitis kronik, sinusitis maksilaris, OMPK, rinitis alergi, presbiakusis, korpal, laringitis. Pada periode 2012, yaitu: otitis eksterna, serumen, serumen obturans, faringitis kronik, sinusitis maksilaris, presbiakusis, OMPK, rinitis alergi, laringitis, dan rinitis kronik. Objective: Otitis media (OM) is a common disease in childhood, but much less prevalent in adults, where it, however, tends to have a less benign course. The incidence of OM in children has decreased in recent years, which might be partly explained by the introduction of watchful waiting strategies, and by the introduction of conjugate pneumococcal vaccination (PCV) in children. Adult OM has been sparsely studied, and it is not known whether a herd immunity effect has been achieved after the introduction of PCV. Watchful waiting is usually not considered in adult OM patients. The purpose of this study was to use a national database to investigate the incidence of adult OM in Sweden during the last 20 years. Design: The yearly number of patients aged 20 and over, admitted to hospital with OM was obtained through a national database from 2000 to 2019. The corresponding numbers for outpatients were obtained between 2005 and 2019. Main outcome measures: incidence of acute otitis media in adults in in- and outpatient care. Results: Incidence rates decreased steadily during the study period. Comparing data on hospitalised patients from 2016-2019 to 2005-2008 (PCV was introduced in 2009), the incidence rate ratio was 0.49 (95% CI 0.45-0.53); p<0.0001. The corresponding figures for outpatients were 0.62 (95% CI 0.61-0.63); p<0.0001. The decrease in hospital admissions was most pronounced among younger patients, whereas the opposite was true for outpatient visits. Conclusions: There has been a continuous decrease in incidence of adult patients admitted to hospital with OM in Sweden during the last twenty years. Outpatient visits for OM have declined since 2009.

Objective: To determine the clinical features, subclinical and to evalute the treatment results otitis externa. Material and method: 51 patients with 53 ears were diagnosed otitis externa at Hue University of Medicine and Pharmacy. Methods: Cross sectional and propective studies. Results: Percentage of female (52.9%), male (47.1%). The most common age group is >15 – 30 years old (41.2%). Patients have antecedent with chronic otitis media is about 15.7%. 37.3% patients had used antibiotics, 21.6% had used corticosteroid. Acute otitis externa accounted for 43.4%, chronic stage accounted for 56.6%. Itching of the ear (67.9%), earache (41.9%), fullness (22.6%). External ear canal condition: earwax (58.5%), discharge (35.8%). 13.2% of patients has eardrum perforation. Isolation of microorganisms: fungi (60.8%), bacteria (29.4%), both fungi and bacteria (9.8%). Fungal results: Aspergillus (58.1%), Candida (16.1%), non-fungal culture (16.1%). Results of bacterial identification: S. aureus (80%), P. aeruginosa (15%). In acute otitis externa, the highest rate is bacterial infection (65.3%), chronic otitis externa, the highest rate is fungi (96.7%). In acute otitis media, the pain in the ear when pushed and of the pinna when pulled is 60.9%. Itching is the most symptom in infection by fungi (56.6%), ear pain commonly associated with bacterial infection (24.5%), fullness is most commonly caused by fungi (13.2%). Bacteria S. aureus in otitis externa are most sensitive to vancomycin(100%), gentamicin (76.5%), ciprofloxacin (64.7%). The proportion of patients responding to treatment is 90.6%. In external ear inflammation, the mean time at end of symptom is 6.2 ± 2.79 days. Conclusions: Otitis externa is a common disease, encountered at many ages and treatment has a good result. AbstractObjective:We report an advanced presentation of osteomyelitis of the temporal bone secondary to malignant otitis externa.Method:We present a case report and a review of the world literature concerning osteomyelitis of the temporal bone secondary to malignant otitis externa.Results:A 60-year-old diabetic man developed osteomyelitis of the temporal bone and a temporoparietal abscess as advanced complications of malignant otitis externa. He was successfully treated in our institution using a post aural incision after draining the abscess and excising the fistula, a modified radical mastoidectomy with canal wall down procedure with sequesterectomy and debridement of surrounding area done.Conclusion:The terms ‘osteomyelitis of the temporal bone’, ‘skull base osteomyelitis’ and ‘malignant otitis externa’ have not been clearly defined, and have in the past often been used interchangeably in the literature. Osteomyelitis of the temporal bone can occur secondary to malignant otitis externa, acute otitis media, chronic suppurative otitis media or trauma. Here, we present the management of an advanced case of osteomyelitis of the temporal bone.

<p class="abstract"><strong>Background:</strong> Chronic suppurative otitis media (CSOM) is a still common disease in developing country and is found sometimes difficult to treat. Different complications can develop inspite of availability of higher antibiotics. In pre-antibiotic era, complications of acute otitis media and CSOM were very common and lead to high mortality. Inspite of initial decline in the complication of CSOM due to higher antibiotics, the incidences are still on rise. CSOM remains a serious disease, particularly in developing countries and CSOM-related complications are still found life-threatening. The aim and objective was to study various clinical presentations and management of CSOM related complications.</p><p class="abstract"><strong>Methods:</strong> All patients of chronic supurrative otitis media with intra or extracranial complication who were admitted in Department of Otorhinolaryngology Head and Neck surgery, Sir. T. General Hospital, Government Medical College, Bhavnagar from July 2015 to December 2018 was included in this study. Data of clinical presentation, associated complication, management, and follow-up were analysed.  </p><p class="abstract"><strong>Results:</strong> Out of 250 patients of CSOM admitted during these 3 years in ENT Department, 36 patients presented with CSOM related complications. 15 patients presented with intracranial complications and 21 patients presented with extracranial complications.</p><p class="abstract"><strong>Conclusions:</strong> Inspite of availability of higher antibiotics, CSOM related complications are still common. In all the patients require higher intravenous antibiotics (which crosses blood brain barrier) followed by mastoid surgeries.</p>

Children are susceptible to acute otitis media (AOM) due to the shorter and horizontal anatomy of the eustachian tube. Allergic Rhinitis is one of the risk factos that caused acute otitis media in children.Purpose : The purpose of this study was to determine the correlation between Allergic Rhinitis and Acute Otitis Media in ChildrenMethod: This study used an observational analytic research with cross sectional research design. The sample in this study was 59 patients diagnosed with acute otitis media at RSUD Dr. H. Abdul Moeloek in 2019-2020. The sampling technique used total sampling. Data analysis using Chi-Square testResult: The results of this study indicated that there was a relationship between allergic rhinitis and acute otitis media in children with p value = 0,047(<0,05) with an OR = 0,146 (95%CI 0,17-1,232). Sampel sebanyak 59 pasien dengan diagnosa otitis media akut anak di RSUD Dr. H. Abdul Moeloek tahun 2018. Teknik pengambilan sample menggunakan total sampling. The problem of acute otitis media in children is relevant because of the prevalence, social significance, possibility of rapid development of both life−threatening intracranial and intralabyrinthine, mastoid complications, as well as acute or prolonged dysfunction of adjacent cranial nerves, especially auditory, vestibular ones. In order to determine the frequency of various forms of acute otitis media, the nature and extent of its complications in children of different ages there was conducted a retrospective analysis of medical histories of patients treated at the City Clinical Hospital № 30 Kharkiv within 2015−2019. There was stated that the biggest group was composed by the patients with a purulent form of the disease, a slightly smaller number of patients had secretory and viral processes. But when analyzing the data over the years, of interest is the fact that during the period when the number of purulent forms decreased, the number of viral ones increased. Complications include mastoiditis, acute sensorineural hearing loss, cerebrospinal fluid and hypertension and intoxication syndromes. It is important that in acute purulent and viral otitis there is an intoxication syndrome along with the manifestations of acute sensorineural hearing loss. Signs of vestibular dysfunction in children can be disguised as symptoms of intoxication, they are much more difficult to detect. Considering the fact that the snail and the labyrinth are washed by the same liquid, it is logical to assume that in this situation the vestibular analyzer suffers together with the auditory one. Therefore, along with otoneurological research it is necessary to apply instrumental methods for establishment of latent disturbances of function of the vestibular analyzer. According to the research results it was concluded that a more detailed study of vestibular analyzer dysfunction in children with acute otitis media, as they are disguised as symptoms of intoxication. Abstract Background Otitis diagnoses include acute otitis media (AOM), otitis media with effusion (OME), and acute otitis externa (AOE). AOM and OME occur primarily in children, whereas AOE occurs with similar frequency in children and adults. Treatment with amoxicillin or close observation without antibiotics is recommended for pediatric AOM, and oral antibiotics are not routinely recommended to treat OME or uncomplicated AOE. Data on otitis diagnoses in adults is limited. This study’s purpose is to characterize the incidence and antibiotic management of otitis diagnoses in adults. Methods. A retrospective cohort of ambulatory veterans who presented at one of six VA Medical Centers during years 2014–2016 with an ICD-9 or -10 code for AOM, OME, and AOE diagnoses was developed. Data extracted included demographics, vital signs, diagnoses, and antibiotic prescriptions. Incident density rates for adult AOM, OME, and AOE were calculated and compared with rates for acute rhinosinusitis. Antibiotic prescribing rates were calculated. Results. Of 4,759 otitis visits identified, the most frequent diagnoses included AOM (38%), OME (25%), and AOE (34%). A single otitis diagnosis was coded in 95.6% of visits and 13.0% had co-diagnosis of another acute respiratory infection (ARI). The incidence density (±95% confidence interval) was 5.4 (5.2, 5.7), 3.6 (3.5, 3.9), and 4.9 (4.7, 5.2) cases per 1,000 patient-years for AOM, OME, and AOE, respectively. For comparison, the incidence density of rhinosinusitis was 16.6 (16.2, 17.0) cases per 1,000 patient-years. Oral antibiotics were prescribed in 48% of visits: AOM (50%), OME (49%), and AOE (47%). Topical antibiotics were prescribed in 32% of AOE visits. The most common oral and otic antibiotics prescribed were amoxicillin/clavulanate (36%), amoxicillin (28%), azithromycin (11%), and hydrocortisone/neomycin/polymyxin (65%), respectively. Conclusion. Otitis diagnoses in adults were common independent of ARI co-diagnoses, but less frequent than rhinosinusitis. Almost half of the patients received an oral antibiotic including those with AOE and OME, indicating a possible focus for antibiotic stewardship programs. Studies to evaluate diagnostic accuracy and treatment of otitis diagnoses in adults are needed. Disclosures All authors: No reported disclosures.

AbstractObjective:Knowledge of ENT is important for many doctors, but undergraduate time is limited. This study aimed to identify what is thought about ENT knowledge amongst non-ENT doctors, and the key topics that the curriculum should focus on.Methods:Doctors were interviewed about their views of ENT knowledge amongst non-ENT doctors, and asked to identify key topics. These topics were then used to devise a questionnaire, which was distributed to multiple stakeholders in order to identify the key topics.Results:ENT knowledge was generally thought to be poor amongst doctors, and it was recommended that undergraduate ENT topics be kept simple. The highest rated topics were: clinical examination; when to refer; acute otitis media; common emergencies; tonsillitis and quinsy; management of ENT problems by non-ENT doctors; stridor and stertor; otitis externa; and otitis media with effusion.Conclusion:This study identified a number of key ENT topics, and will help to inform future development of ENT curricula.

AbstractObjective:To describe a relatively unknown clinical entity – inflammatory cast of the tympanic membrane after acute otitis media – and its simple out-patient treatment.Study design:Retrospective review of case series.Setting:Subspecialty practice at a tertiary hospital.Patients:Seven patients diagnosed previously with acute otitis media with perforation or otitis externa, and with persistent ear discomfort.Methods:Retrospective chart review.Results:The patients presented with weeks to months of persistent hearing loss after acute otitis media with perforation or acute otitis externa. Visits to their primary care physicians had been uninformative. After comparison of the affected and unaffected tympanic membranes, a thin, hard cast was identified and removed from the affected tympanic membrane. Improvement in hearing was documented in the three patients who underwent audiometric testing; the remainder had subjective improvement without audiometric evaluation.Conclusion:Otolaryngologists should be aware of the possibility of an inflammatory cast of the tympanic membrane following acute otitis media with perforation or otitis externa, and should carefully compare the unaffected and affected ears in such cases. Treatment – removal of the rigid cast – is both simple and effective.

Abstract Background Acute otitis media (AOM) and otitis media with effusion (OME) occur primarily in children, whereas acute otitis externa (AOE) occurs with similar frequency in children and adults. Data on the incidence and management of otitis in adults are limited. This study characterizes the incidence, antibiotic management, and outcomes for adults with otitis diagnoses. Methods A retrospective cohort of ambulatory adult veterans who presented with acute respiratory tract infection (ARI) diagnoses at 6 VA Medical Centers during 2014–2018 was created. Then, a subcohort of patients with acute otitis diagnoses was developed. Patient visits were categorized with administrative diagnostic codes for ARI (eg, sinusitis, pharyngitis) and otitis (OME, AOM, and AOE). Incidence rates for each diagnosis were calculated. Proportions of otitis visits with antibiotic prescribing, complications, and specialty referral were summarized. Results Of 46 634 ARI visits, 3898 (8%) included an otitis diagnosis: OME (22%), AOM (44%), AOE (31%), and multiple otitis diagnoses (3%). Incidence rates were otitis media 4.0 (95% confidence interval [CI], 3.9–4.2) and AOE 2.0 (95% CI, 1.9–2.1) diagnoses per 1000 patient-years. By comparison, the incidence rates for pharyngitis (8.4; 95% CI, 8.2–8.6) and sinusitis (15.2; 95% CI, 14.9–15.5) were higher. Systemic antibiotics were prescribed in 75%, 63%, and 21% of AOM, OME, and AOE visits, respectively. Complications for otitis visits were low irrespective of antibiotic treatment. Conclusions Administrative data indicated that otitis media diagnoses in adults were half as common as acute pharyngitis, and the majority received antibiotic treatment, which may be inappropriate. Prospective studies verifying diagnostic accuracy and antibiotic appropriateness are warranted.
