Aim. Identification of epidemiological patterns of the SARS-CoV-2 spread among the population of St. Petersburg during the one-year COVID-19 pandemic period.Materials and methods. The performed analysis focused on the dynamics of COVID-19 cases in St. Petersburg from 2/3/2020 to 4/4/2021 and on the gender-age profile of patients. The information about patients (age, gender, type of the disease, hospitalization, social, and occupational status) was obtained from the database containing the materials from statistical data form No. 058/u.Results. After one year, the dynamics of reported cases of COVID-19 in St. Petersburg shows two cycles of seasonal surge (spring and autumn-winter) and 8 epidemic periods. It has been found that there are no gender-age differences among COVID-19 patients, which can be seen from the relatively similar number of cases among men and women per 100,000 people in each age group during specific epidemic periods. The strong association between clinical manifestations of COVID-19 and the patients' age was detected: Severe cases were more frequently diagnosed in patients over 70 years, regardless of their gender identity. Based on the social and occupational status, the people who were most exposed to the COVID-19 epidemic process were retirees and people whose occupation was associated with health and safety of St. Petersburg. Among the COVID-19 patients, retirees accounted for 13.69% (men) and 17.67% (women). The proportion of healthcare workers was 3.67% (men) and 9.41% (women).Conclusion. It has been assumed that COVID-19 tends to be a seasonal disease featuring annual autumn-winter epidemic cycles. The study addressed prospects of preventive vaccination against COVID-19 in Russia and the importance of tracking the complications pathogenetically associated with the acute phase of the disease in the system of epidemiological surveillance.

Systemic sclerosis (SSc) is a collagen-vascular disorder characterized by fibrosis and vasculopathy. Delta finger to palm distance (delta FTP) is an index measuring the distance between the tip of the third finger to the distal palmar crease in the flexed and extended position. The present study aimed to evaluate the clinical value of delta FTP and to assess the correlation of delta FTP with modified Rodnan skin score (mRSS) and forced vital capacity (FVC) over the 12-month follow-up. This prospective longitudinal study began with 50 participants who were followed for twelve months. Lowess smoothing and linear regression were applied to detect and assess the relationship between delta FTP and mRSS. p-values were adjusted by the Benjamini-Hochberg method (BHM) as a control for false discovery rate. Delta FTP was lower among patients with higher disease duration (p-valueadj: 0.008), diffuse cutaneous SSc (p-valueadj: 0.006), digital ulcers (p-valueadj: 0.003), telangiectasia (p-valueadj: 0.006) and dysphagia (p-valueadj: 0.036). The mRSS has a significant negative linear effect on the delta FTP at the baseline and the end of the follow-up (r: -0.31 and -0.40, respectively). Moreover, changes of mRSS and delta FTP showed a negative linear association over time (r: -0.22). These linear effects remained significant after regrouping the patients based on their SSc subtype. Delta FTP and FVC were not correlated either at the baseline or at the end. It seems that the delta FTP can be a valuable clinical index, supported by its correlated changes with mRSS and other SSc clinical manifestations over the one-year follow-up.

Abstract Background The care of people with dementia is usually carried out by their family members, which can cause objective und subjective burden and raise their risk of depressiveness. Thus, the aim of this study is to identify predictors of the change in depressiveness of informal caregivers over 1 year in order to be able to derive hypotheses for interventions that promise success. Methods The Bavarian Dementia Survey (BayDem) is a multi-center, longitudinal study conducted at three different sites in Bavaria, Germany. Participants were people with dementia and their informal caregivers. Data was collected at baseline and after 12 months by standardized face-to-face interviews in cooperation with local players. The informal caregivers’ depressiveness was assessed with the WHO-5. Data was also collected on the people with dementia’s cognition (MMSE), behavioral symptoms (NPI) and comorbidities (Charlson Comorbidity Index) as well as caregivers’ social inclusion (LSNS), time spent on care and care contribution (RUD). For statistical analysis, a multiple regression model was used. Results The data of 166 people with dementia and their informal caregivers was analyzed. Of the latter, 46% were categorized as “likely depressed”. The change in depressiveness over a year was significantly predicted by baseline depressiveness as well as an increase in the time informal caregivers spent supervising the person with dementia. Conclusions Informal caregivers of people with dementia are at high risk of depression. The time spent supervising the person with dementia has a significant impact on increasing depressiveness. This highlights the importance of support services to provide the informal caregiver with relief and possibly reduce depressiveness.

HCC epidemiologic data in Sverdlovsk region are presented in current issue. 1293 of new cases of HCC were diagnosed since 2015 till 2019. The incidence rate of HCC in 2019 was 6,53 cases per 100000 people and standardized incidence ratio was 3,48 per 100000 people. The maximum of new cases was among the people older than 70 years. HCC developed in cirrhotic liver in 70,2% of cases. The main risk factors were virus hepatitis (49,6%), regular alcohol consumption (23%) and obesity (9,9%). The post-mortem diagnosis exceeds 40% of cases. The one-year mortality was 72-73%. The number of patients with HCC sent to the regional oncology dispensary increased in the last 5 years in 5 times. There were 124 of such patients in 2019. The majority has HCC at BCLC D stage (56,5%). Only 16,7% of patients received specialized treatment. The introduction of multidisciplinary team led to a doubling of annual number of treated patients with HCC.

Key Russian experts representing applied medicine, scientific and educational organizations related to the vaccination hold the meeting on June 13 in Moscow. The main issue of the agenda was the discussion of the new approaches to vaccination of pertussis. It was noted that it is still a topical question for public health worldwide including Russia. The experts comprised the list of the unresolved questions related to pertussis surveillance and prevention. The most important fact is a continuing spread of pertussis due to the decrease of immune response 5–7 years after any type of pertussis vaccine, and a growing number of the people who have not suffered from pertussis before. Increase of pertussis incidence among one year old children as well as schoolchildren and adults attests for the circulation of pathogen and continuous spread of the disease. This makes necessary an introduction of additional pertussis vaccination boosters. The data of epidemiological surveillance in the countries which already introduced the second booster against pertussis for 4–7 year old children (more than 50 countries) shows essential epidemiological effect and decrease of pertussis incidence at the age from 4 to 10 years old, and has already demonstrated indirect protection for one year old children. The experts made final recommendations for Russia regarding the improvement of pertussis surveillance on regional and federal levels. Certain possible strategies of anti-pertussis vaccination were offered in accordance with the developing vaccines and technologies.

The increase in positive cases of COVID-19 in Indonesia since 2020 has led to various control measures, including prevention. One of them is through cultural approach and local wisdom that differs from one region to another, such as the one applied in Bali Province. The purpose of the study was to explore Bali’s experience in dealing with COVID-19 from the perspective of culture and local wisdom. A qualitative approach was used in this study using in-depth interviews with the Bali Provincial DisasterControl Office (Badan Penanggulangan Bencana Daerah, BPBD) and the Bali Provincial Health Office. Results of this study showed that the Bali Province has a strong religious approach, consisting of hupokara, which isthe faith in the Creator as disaster is believed to be a tremendous force that has to be managed through a ceremony called niskala, which giving offerings to God for health and safety of the people of Bali Province. The Balinese believe that the COVID-19 virus can be controlled with rituals and offerings. The presence of Pecalang as a traditional security unit looked up to and obeyed by the Balinese people also supports the successful management of COVID-19. The unique approach of Bali Province that is based on the local wisdom through mobilizing Traditional Villages, promoting custom-based cooperation through the formation of a task force at the Traditional Village level, and with good leadership from the Governorof Bali has led to successful COVID-19 control in Bali Province. Bali has learned from the COVID-19 pandemic that togetherness and mutual cooperation among people are needed to prevent the spread of COVID-19 inthe province.

Abstract Conflict in Gaza Strip involving Israel and Palestine has continuously attracted international concerns.  Under the strict authority of Israel, humanitarian issues materialize in the strip as Gaza’s inhabitants undergo crises including energy and subsistence while Israel fails to fulfill the basic needs of the people. In 2007, Jaber Al-Bassiouni Ahmed with his associates sought legal remedy to the Supreme Court of Israel to clarify their rights inhabiting the Gaza Strip. The purpose of this article is to explain whether international humanitarian law could be implemented to the occupying power of Israel in Gaza Strip and elucidate the limitation of the occupying power regarded by the law. Achieving this explanation requires a qualitative legal approach. It emphasizes on documents scrutiny using the perspectives of primary, secondary, and tertiary laws. The research result shows that under the regime of international humanitarian law, “alien occupation” is the key to regulate the Gaza Strip against the regime of the occupying power by Israel. The one-year rule in the Case of Gaza Strip indicates that Israel held the occupying power of the territory as not only did Israel act as a controller but also it participated in the physical activities to regulate the lives of Gaza residents in addition to the limitation of the functions of Palestinian Authority posed by Israel. Di bawah kekuasaan Israel, isu humaniter mencuat di jalur tersebut karena penduduk Gaza mengalami krisis energi dan mata pencaharian, sementara Israel tidak memenuhi kebutuhan dasar hidup penduduk tersebut. Pada tahun 2007, Jaber Al-Bassiouni Ahmed bersama rekan-rekannya mengajukan tinjauan hukum kepada Pengadilan Tinggi Israel untuk mengklarifikasi hak-hak mereka sebagai penduduk di Jalur Gaza. Tujuan penelitian ini adalah untuk menjelaskan apakah hukum humaniter internasional dapat diterapkan dalam kasus penguasa pendudukan di Jalur Gaza oleh Israel dan menjelaskan batasan kekuasaan pendudukan tersebut. Pendekatan kualitatif hukum berbasis studi dokumen dengan perspektif hukum primer, sekunder dan tersier digunakan dalam penelitian ini. Hasil penelitian ini membuktikan bahwa di bawah hukum humaniter internasional, “pendudukan asing” menjadi kunci regulasi yang dapat diterapkan di Jalur Gaza di bawah resim penguasa pendudukan yakni Israel. Aturan “Satu Tahun” di jalur tersebut menunjukan bahwa Israel memegang kendali wilayah tersebut karena Israel tidak hanya bertindak sebagai pengontrol melainkan juga melakukan kegiatan fisik untuk mengatur kehidupan masyarakat Gaza juga membatasi fungsi-fungsi autoritas Palestina. Abstract Coronary artery anomalies represent a heterogeneous group of congenital diseases with various clinical presentations. Over time, the subject of coronary anomalies has been constantly changing in terms of definition, morphology, clinical manifestations, prognosis, and treatment. We present the case of a male patient, aged 53, with coronary artery disease and a medical history of high blood pressure and diabetes mellitus, who had undergone a coronary computed tomography angiography during the one-year follow-up after a percutaneous coronary intervention with drug-eluting stent implantation for a critical stenosis in the middle segment of the left anterior descending artery. Axial images revealed a separate origin of the left anterior descending and circumflex arteries from the left aortic coronary sinus, with the absence of the left main coronary artery.

Abstract Rating patients with head trauma and multiple neurological injuries can be challenging. The AMA Guides to the Evaluation of Permanent Impairment (AMA Guides), Fifth Edition, Section 13.2, Criteria for Rating Impairment Due to Central Nervous System Disorders, outlines the process to rate impairment due to head trauma. This article summarizes the case of a 57-year-old male security guard who presents with headache, decreased sensation on the left cheek, loss of sense of smell, and problems with memory, among other symptoms. One year ago the patient was assaulted while on the job: his Glasgow Coma Score was 14; he had left periorbital ecchymosis and a 2.5 cm laceration over the left eyelid; a small right temporoparietal acute subdural hematoma; left inferior and medial orbital wall fractures; and, four hours after admission to the hospital, he experienced a generalized tonic-clonic seizure. This patient's impairment must include the following components: single seizure, orbital fracture, infraorbital neuropathy, anosmia, headache, and memory complaints. The article shows how the ratable impairments are combined using the Combining Impairment Ratings section. Because this patient has not experienced any seizures since the first occurrence, according to the AMA Guides he is not experiencing the “episodic neurological impairments” required for disability. Complex cases such as the one presented here highlight the need to use the criteria and estimates that are located in several sections of the AMA Guides.

Background: The aim of this study is to report the short-term results of catheter-directed foam sclerotherapy (CDFS) in the treatment of axial saphenous vein incompetence. Patients and methods: Data of all patients undergoing CDFS for symptomatic primary incompetence of the great or small saphenous vein were prospectively collected. Treatment results in terms of occlusion rate and patients’ grade of satisfaction were analysed. All successfully treated patients underwent clinical and duplex follow-up examinations one year postoperatively. Results: Between September 2006 and September 2010, 357 limbs (337 patients) were treated with CDFS at our institution. Based on the CEAP classification, 64 were allocated to clinical class C3 , 128 to class C4, 102 to class C5 and 63 to class C6. Of the 188 patients who completed the one year follow up examination, 67 % had a complete and 14 % a near complete obliteration of the treated vessel. An ulcer-healing rate of 54 % was detected. 92 % of the patients were satisfied with the results of treatment. We registered six cases of thrombophlebitis and two cases of venous thromboembolism, all requiring treatment. Conclusions: The short-term results of CDFS in patients with axial vein incompetence are acceptable in terms of occlusion and complications rates.

The main objective of this paper was to estimate and analyse transition-probability matrices for all 16 of Poland’s NUTS-2 level regions (voivodeship level). The analysis is conducted in terms of the transitions among six expenditure classes (per capita and per equivalent unit), focusing on poverty classes. The period of analysis was two years: 2015 and 2016. The basic aim was to identify both those regions in which the probability of staying in poverty was the highest and the general level of mobility among expenditure classes. The study uses a two-year panel sub-sample of unidentified unit data from the Central Statistical Office (CSO), specifically the data concerning household budget surveys. To account for differences in household size and demographic structure, the study used expenditures per capita and expenditures per equivalent unit simultaneously. To estimate the elements of the transition matrices, a classic maximum-likelihood estimator was used. The analysis used Shorrocks’ and Bartholomew’s mobility indices to assess the general mobility level and the Gini index to assess the inequality level. The results show that the one-year probability of staying in the same poverty class varies among regions and is lower for expenditures per equivalent units. The highest probabilities were identified in Podkarpackie (expenditures per capita) and Opolskie (expenditures per equivalent unit), and the lowest probabilities in Kujawsko-Pomorskie (expenditures per capita) and Małopolskie (expenditures per equivalent unit). The highest level of general mobility was noted in Małopolskie, for both categories of expenditures.
