ABSTRAK Salah satu faktor penting yang mempengaruhi kualitas hasil peledakan adalah tingkat keakurasian waktu tunda dari sistem inisiasi peledakan yang digunakan. Saat ini, peledakan di Pit PAMA CMD PT Kaltim Prima Coal (PT KPC) menggunakan detonator non electric (nonel) yang memiliki tingkat keakurasian waktu tunda sebesar ±97-98% dari waktu tunda rencana. Hasil analisis fragmentasi menggunakan software WipFrag™ dan pengukuran digging time pada peledakan nonel menunjukkan % passing 300 mm (P30) sebesar 80,64% dengan digging time 11,10 detik di Pit Pelikan dan P30 sebesar 82,66% dengan digging time 10,41 detik di Pit Kanguru. Ukuran fragmentasi pada peledakan nonel di Pit PAMA CMD aktualnya telah memenuhi standar ukuran fragmentasi yang diterapkan di PT KPC yaitu P30 ≥ 80%, namun masih terdapat potensi untuk meningkatkan hasil ini dengan menggunakan detonator elektronik DigiShot™ Plus (DS+). Penggunaan DS+ yang memiliki tingkat keakurasian waktu tunda mencapai ±99.8% dari waktu tunda rencana, selain dapat meningkatkan kualitas ukuran fragmentasi juga dapat memperbaiki digging time dan mengurangi penggunaan Lead in Line (LiL) dengan aplikasi remote firing. Hasil peledakan DS+ di Pit Pelikan menunjukkan P30 sebesar 86,24% atau 5,6% lebih tinggi dibanding peledakan nonel dengan digging time 10,22 detik atau 7,9% lebih cepat dibanding peledakan nonel. Sementara data yang didapat di Pit Kanguru menunjukkan P30 sebesar 87,55% atau 4,9% lebih tinggi dengan digging time 9,66 detik atau 7,2% lebih cepat dibanding peledakan nonel. Data ini didapatkan dari penggunaan DS+ sebanyak 8.541 unit di 89 lokasi peledakan di Pit Pelikan dan Pit Kanguru PAMA CMD PT KPC. Selaras dengan peningkatan pada kualitas fragmentasi dan digging time, peledakan DS+ di Pit PAMA CMD juga berkontribusi terhadap perbaikan produktivitas alat gali yang meningkat ±2,5% dan penghematan penggunaan LiL sepanjang 49,8 km. Dapat disimpulkan, penggunaan detonator elektronik pada peledakan sangat berpotensi untuk meningkatkan kualitas hasil peledakan dan penghematan biaya peledakan jika diaplikasikan dengan optimal. Kata kunci: elektronik, fragmentasi, digging time   ABSTRACT One important factor that influences the quality of blasting results is the accuracy of the delay of the blasting initiation system used. At present, the blast activity at PAMA CMD Pit PT Kaltim Prima Coal uses a non-electric (nonel) detonator which has a delay time accuracy of ± 97-98% of the planned delay. The results of fragmentation analysis using WipFrag™ software and measurement of digging time on nonel blasting showed % passing 300 mm (P30) was 80.64% with digging time of 11.10 seconds in the Pelikan Pit and P30 of 82.66% with digging time of 10.41 seconds in the Kanguru Pit.The size of the fragmentation in nonel blast at PAMA CMD Pit actually has met the standard fragmentation size applied at PT KPC which is P30 ≥ 80%, but there is still potential to increase this result by using the DigiShot ™ Plus (DS +) electronic detonator. The use of DS + which has an accuracy of delay time reaching ± 99.8% of the plan delay time, not only to increase the quality of the fragmentation size, but also improve digging time and reduce the use of Lead in Line (LiL) with remote firing applications. The results of DS + blasting in Pelikan Pit showed P30 was 86.24% or 5.6% higher than nonel blasting with digging time 10.22 seconds or 7.9% faster than nonel blasting. The data obtained in the Kanguru Pit showed P30 was 87.55% or 4.9% higher with a digging time of 9.66 seconds or 7.2% faster than nonel blasting. This data was obtained from the use of DS + as many as 8,541 units in 89 blasting locations in the Pelikan Pit and the Kanguru PAMA CMD Pit PT KPC. In line with the improvement in the quality of fragmentation and digging time, DS + blasting in the PAMA CMD Pit also contributed to improved digging equipment productivity which increased by ± 2.5% and savings in the use of LiL along 49.8 km. It can be concluded, the use of electronic detonators in blasting has the potential to improve the quality of blasting results and blast cost savings if applied optimally. Keywords: electronics, fragmentation, digging time

AbstractDue to the risk of adverse reactions to metal debris resulting from increased wear of the arthroplasty more than one million metal-on-metal (MoM) hip replacements worldwide are in active follow-up. Follow-up usually includes measurement of both whole blood cobalt (Co) and chromium (Cr) concentrations. Our experience is that Cr is seldom independently elevated. We wanted to ascertain whether blood Cr measurements could be omitted from follow-up protocols without lowering the quality of follow-up. We identified 8438 whole blood Co and Cr measurements performed without or prior to revision surgery. When the cut-off levels 5 µg/L and 7 µg/L were used, Cr was independently elevated in only 0.5% (95% confidence interval, CI, 0.3 to 0.6) and 0.2% (CI 0.1 to 0.3) of the measurements. The models with continuous variables showed that the higher the blood metal concentrations are the lower the percentage of measurements with Cr higher than Co. Our results suggest that whole blood Cr is very rarely independently elevated and therefore the authorities should consider omitting Cr measurements from their screening guidelines of MoM hip replacements. We believe this change in practice would simplify follow-up and lead to cost savings without decreasing the quality of follow-up.

Background: The etiology of benign prostatic hyperplasia and prostate cancer are unknown, with ageing being the greatness risk factor. Methods: This new perspective evaluates the available interdisciplinary evidence regarding prostate ageing in terms of the cell biology of regulation and homeostasis, which could explain the timeline of evolutionary cancer biology as degenerative, inflammatory and neoplasm progressions in these multifactorial and heterogeneous prostatic diseases. Results: This prostate ageing degeneration hypothesis encompasses the testosterone-vascular-inflamm-ageing triad, along with the cell biology regulation of amyloidosis and autophagy within an evolutionary tumorigenesis microenvironment. Conclusions: An understanding of these biological processes of prostate ageing can provide potential strategies for early prevention and could contribute to maintaining quality of life for the ageing individual along with substantial medical cost savings.

Introduction The impact of telemedicine on the access and quality of paediatric emergency care remains largely unexplored because most studies to date are focused on adult emergency care. We performed a systematic review of the literature to determine if telemedicine is effective in improving quality of paediatric emergency care with regards to access, process measures of care, appropriate disposition, patient-centred outcomes and cost-related outcomes. Methods We developed a systematic review protocol in accordance with PRISMA (Preferred Reporting Items for Systematic Review) guidelines. We included studies that evaluated the impact of synchronous and asynchronous forms of telemedicine on patient outcomes and process measures in the paediatric emergency care setting. Inclusion criteria were study setting, study design, intervention type, age, outcome measures, publication year and language. Results Overall, 1.9% (28/1434) studies met study inclusion and exclusion criteria. These studies revealed that telemedicine increased accuracy of patient assessment in the pre-clinical setting, improved time-to disposition, guided referring emergency department (ED) physicians in performing appropriate life-saving procedures and led to cost savings when compared to regular care. Studies focused on telepsychiatry demonstrated decreased length of stay (LOS), transfer rates and improved patient satisfaction scores. Discussion Our comprehensive review revealed that telemedicine enhances paediatric emergency care, enhances therapeutic decision-making and improves diagnostic accuracy, and reduces costs. Specifically, telemedicine has its most significant impact on LOS, access to specialized care, cost savings and patient satisfaction. However, there was a relative lack of randomized control trials, and more studies are needed to substantiate its impact on morbidity and mortality.

Data derived from the Automatic Identification System (AIS) plays a key role in water traffic data mining. However, there are various errors regarding time and space. To improve availability, AIS data quality dimensions are presented for detecting errors of AIS tracks including physical integrity, spatial logical integrity and time accuracy. After systematic summary and analysis, algorithms for error pre-processing are proposed. Track comparison maps and traffic density maps for different types of ships are derived to verify applicability based on the AIS data from the Chinese Zhoushan Islands from January to February 2015. The results indicate that the algorithms can effectively improve the quality of AIS trajectories.

Abstract Background In an effort to lower costs and improve quality of care, there is potential to change the treatment landscape for low-risk (i.e., less severe) ABSSSI patients who historically required inpatient management, a costly option. Outpatient IV treatment pathways have been shown to be a cost-saving option for hospitals and insurers. The objective was to quantify the potential opportunity for reducing cost of ABSSSI treatment in an insured Commercial and Medicare Aetna population. Methods Adult patients between January 2013 and July 2016 were identified with a primary ABSSSI claim (Table 1) in the Aetna fully-insured Commercial and Medicare insurance claims database. ABSSSI encounters were identified with insurance eligibility for the 7 months prior to and no evidence of ABSSSI in the 30 days prior to the ABSSSI claim. Demographic and clinical data were described, including length of stay (LOS) and allowed cost for inpatient encounters with data. Inpatient encounters without evidence of severity (e.g., codes for major complications or comorbidities) were considered potential candidates for an outpatient LAA pathway. A sensitivity analysis for LOS and cost was run including all ABSSSI patients with LAA dispenses through 2016 (i.e., inclusion/exclusion criteria did not need to be met). Results 194,023 ABSSSI encounters were identified, most receiving non-IV treatment (90%). 18,603 received IV treatment, where 83% initially presented to the emergency room and the majority were admitted (97%). Of the 28 encounters with LAA use, 7 were inpatient. Of all current inpatient encounters (N = 9,019 after Jan 1, 2015), the majority (N = 7,005; 78%) where considered potential LAA pathway candidates. Comparing inpatient encounters with vs. without LAA use, mean LOS and cost differed (Table 2: 4.1 days and $14,295 vs. 9.0 days and $23,194, respectively). A sensitivity analysis supported similar mean LOS and cost for all inpatient LAA dispenses. Conclusion Current use of LAA in an inpatient population is limited but resulted in potential cost-savings. Most of the inpatient population was identified as potential candidates for an outpatient LAA pathway. Background. Cost containment mechanisms, such as prepayment, are being considered or implemented in the US and elsewhere, but there have been few studies of the effects of such mechanisms on quality or outcomes of care for individuals with serious psychiatric disorders.Method. Key results from US studies on cost containment and their implications are reviewed.Results. Cost savings in out-patient mental health care can be achieved through increasing the share of costs paid by the covered individual or through prepayment, but individuals with the greatest psychological distress or poor people may achieve worse outcomes under greater cost containment. Quality of care may be poorer under some forms of prepayment than under fee-for-service care, yet a national prospective payment mechanism for depressed elderly in-patients was not associated with a marked drop in quality or outcomes of care among those admitted.Conclusions. Prepayment, relative to fee-for-service is not always associated with lower outcomes or quality of care for affective disorders. Under cost containment, quality and outcomes of care, especially for the sick poor, should be monitored to identify adverse consequences.

The decline in construction productivity across North America since the mid 1970s has been reported by many researchers. The potential exists to achieve major cost savings if the factors underlying this decline can be identified and quantified and solutions are found. The issue is complex, but it is generally acknowledged as an axiom that it is a management problem and within the control of management to rectify. This paper describes the results of a survey of the attitudes of construction professionals across Canada toward factors affecting construction productivity. Special emphasis is made to the differences between Newfoundland and the rest of Canada. Findings for different regions of the country are presented and contrasted. For the purpose of this study, the factors have been classified into six groups: (i) contract environment, (ii) planning, (iii) site management, (iv) working conditions, (v) working hours, and (vi) motivation. Major factors affecting productivity are identified. The survey questionnaire addressed these factors within the groups indicated. Respondents commented freely on factors that they considered to be of great importance to construction productivity, such as quality of labor skill, project communication, timeliness, and equipping of crews. Research indicates that contingency management (CM) has potential to improve a number of outcomes (e.g. substance use, treatment attendance, quality of life) among individuals with substance use and cooccurring disorders. However, multiple factors must be considered on a case-by-case basis in order to promote optimal treatment effects. The present study describes an individualized CM protocol for a US Veteran with substance dependence and cooccurring severe mental illness. CM targeted attendance at outpatient appointments and appropriate use of hospital resources. Effects of CM were assessed by comparing the 3-month baseline and CM periods. The CM intervention marginally reduced unnecessary hospital admissions, resulting in cost savings to the medical center of over $5,000 in three months for this individual. However, CM did not affect outpatient attendance. Several complications arose, highlighting challenges in using CM in populations with substance use and cooccurring disorders. Practical suggestions are offered for maximizing the effects of CM.
