On the example of technologies of personalization of pharmacotherapy based on biomarkers of the processes of absorption, distribution, metabolism and excretion of drugs (ADME-processes), the methodology of the results of scientific research into the educational process within the system of continuous medical education of doctors into the real clinical practice is presented. This methodology has been developed and implemented in the Russian Medical Academy of Continuing Professional Education.

The continuous medical education requires development and implementation of modern approaches to enhancement of theoretical and practical training of personnel using modern educational technologies adapted to international requirement and needs of national health care and permitting to train for medical institutions personnel capable on high professional level to resolve problems of modernization of health care and development of quality of medical care ofpopulation. The article describes technological process of organization of webinars on clinical chair of medical university. The advantages and difficulties are discussed related to implementation of such form of interactive studies as webinar into system of continuous professional education.

Background Conventional tools for medical education are burdened by many drawbacks. Textbooks become rapidly outdated, meeting attendance is expensive, and results reported in journals are not easily suitable for clinical practice. Uptodate and Endotext are excellent online tools, but they have been developed for a clinical context far different from that of our country. Methodology Italian Association of Clinical Endocrinologists (AME) charged a task force aiming to develop an educational tool specifically tailored for Italian clinical endocrinologists. Required characteristics were clinical approach, modularity, continuous updating, full online availability (even by mobile devices), open sections and sections for registered members only, opportunity for individualization, indexing and search engine to facilitate browsing. Starting from an open-source platform, Joomla, several changes were implemented. Results Three editors, 2 central editorial secretaries, 30 section coordinators and over 350 authors have been involved in the writing of 21 different sections covering all fields of endocrine and metabolic diseases. The access to Endowiki is free for AME members and is fully open in sections for patients. All readers are requested to serve as referee, pointing to mistakes and need for revision. The system is attended daily by a mean of 250 individuals. Conclusions Endowiki stands alone as an opportunity for medical education in Italy. The big challenges will be the continuous updating and the link to the national certified system for CME.

Canadian governments and various stakeholder groups are advocating greater interprofessional collaboration amongst health care providers as a fundamental strategy for enhancing coordination and quality of care in the health care system. Interprofessional education for collaborative patient-centred practice (IECPCP) is an educational process by which students/learners (or workers) from different health professions learn together to improve collaboration. The educational system is believed to be a main determinant of interprofessional collaborative practice, yet academic institutions are largely influenced by accreditation, certification and licensure bodies. Accreditation processes have been linked to the continuous improvement of curricula in the health professions, and have also been identified as potential avenues for encouraging educational change and innovation. The purpose of this paper is to summarize the characteristics of the national accreditation systems of select Canadian health professional education programs at both the pre- and post-licensure educational levels and to show how these systems support and/or foster IECPCP. A review of the educational accreditation systems of medicine, nursing, pharmacy, social work, occupational therapy and physiotherapy was undertaken through key informant interviews and an analysis of accreditation process documentation. The results of this comparative review suggest that accreditation systems are more prevalent across the health professions at a pre-licensure level. Accreditation at the post- licensure level, particularly at the continuing professional education level, appears to be less well established across the majority of health professions. Overall, the findings of the review also suggest that current accreditation systems do not appear to promote nor foster interprofessional education for collaborative patient-centred practice in a systematic manner through either accreditation processes or standards. Through a critical adult learning perspective we argue that in order for traditional uni-professional structures within the health professional education system to be challenged, the accreditation system needs to place greater value on interprofessional education for collaborative patient-centred practice.

The features of evaluating data from real clinical practice are discussed. Approaches to material processing for decision support in medicine and health care are also given. The development of standardized methods of analysis with the possibility of obtaining a unified indicator for assessing data from routine clinical practice, combined with the development of information technology is the direction of development of the concept of result-oriented health care. The classification of information technologies used in medicine and public health is presented. The main characteristics and functioning features of the developed software modules for automated data evaluation of real clinical practice are presented: a program for the distribution of drugs on the levels of clinical efficacy, a program to assess the effectiveness of therapy for the specified period; a program to determine the interval of clinical efficacy of drugs.

The purpose of the study the attitude of nurses to the existing system of advanced training and determine ways to optimize it. Results. An analysis of the opinions of nursing staff on improving the system of additional vocational education in accordance with personal needs showed that most of the students of advanced training courses expressed a desire to be present in the training process of practical health care professionals from large medical organizations using modern and innovative teaching methods; introduction of distance learning into the educational process of nursing staff with a flexible schedule and an individual student's plan; the use of modern and innovative technologies, methods and forms of professional and educational activities, formed taking into account the opinions of the students themselves; maintaining a high professional level of the teacher, the main qualities of which are the ability to motivate to study, maintain interest in the classroom and create a favorable psychological attitude. Conclusion. In improving the quality of medical care for patients, specialists with secondary medical education, in the development of new innovative technologies, additional professional education is a key resource, which goes along with the specialist throughout his entire professional activity and is able to adapt them to new professional requirements.

Educational scholars are implementing technologies into the educational process. They use virtual reality, simulations, augmented reality and other immersive technologies that provide entertainment and an interesting, immersive environment as well as engage and motivate students. The aim of this research is to understand the scope and the nature of studies where serious games for medical education are prospective research subjects for the development of learning motivation. There are specific research questions that are of interest to the authors: (1) how many studies are devoted to exploring how serious games and simulations develop learning motivation; (2) how many studies are devoted to medical education, especially continuing professional education; and (3) which professional fields are most reflected in the search results, and is interdisciplinary learning observed? To answer the research questions, a systematic analysis of publications was chosen as the research method. Altogether, 132 articles published from January 1, 2014 to December 31, 2018 were selected for the review.

Background. Real clinical practice (RCP) exists in an evidence-based and regulatory framework, taking into account the social, political and economic situation in the country. Coronavirus pandemic (COVID-19) is the main challenge of modern RCP. Objective. To describe the modern features of the RCP. Materials and methods. Analysis of literature sources on this issue. Results and discussion. On December 31, 2019, WHO was informed about 27 cases of pneumonia of unknown origin. On January 1, 2020, the first WHO guidelines were issued. The COVID-19 outbreak was declared a health emergency on January 30 and a pandemic – on March 11. Experience with COVID-19 varies from country to country. In Germany, for example, pharmacies were allowed to produce disinfectants on their own, in Australia the telemedicine system was expanded, and in Poland a law was issued that provided the regulation of remote work, simplification of public procurement, and emergency pharmacy prescriptions. In Ukraine, the first information from the Ministry of Health on coronavirus was published on January 21. On February 19, a decision was made to procure medicines to combat COVID-19. On March 11, the export of personal protective equipment was banned, and on March 12, quarantine was imposed throughout Ukraine. On March 17, the first laws of Ukraine on combating the coronavirus were adopted. One in four patients who fell ill at the beginning of the outbreak was a health worker, which reduced the availability of medical care. The imposition of a pandemic on phase 2 of health care reform has limited health care and patients’ access to clinics and hospitals, and suspended planned hospitalizations and surgeries. Medicines without evidence were included in the COVID-19 National Treatment Protocol. An analysis of drug sales in pharmacies showed that quarantine had decreased the sales of cough and cold remedies, nasal irrigation solutions (due to a reduction in the number of socially transmitted diseases), and antidiarrheal drugs. Instead, sales of laxatives have increased (presumably due to changes in diet and limited physical activity). Sales of drugs for the treatment of sexually transmitted diseases also decreased. Quarantine in combination with the restriction of the availability of infusion therapy in the practice of the family doctor has led to a reduction in the appointment of parenteral drugs by half. In the absence of planned hospitalizations and surgeries, the volume of prescriptions for infusion drugs decreased by 13 %. There was a redistribution of drug consumption in favor of domestic drugs. “Yuria-Pharm” was in the top 3 among Ukrainian drug manufacturers. 6 out of 10 general leaders are domestic companies. “Yuria-Pharm” is a leader in blood substitutes and perfusion solutions prescribed by doctors of 16 specialties. The solutions were most often prescribed for pneumonia, mental and behavioral disorders caused by alcohol abuse, acute pancreatitis, cerebrovascular diseases, delivery, acute appendicitis, malignant tumors, insulin-dependent diabetes mellitus, chronic ischemic heart disease. For example, Tivortin (“Yuria-Pharm”) is most often prescribed by gynecologists, less often – by physicians / family doctors, neurologists, surgeons, cardiologists, anesthesiologists. In turn, Reosorbilact (“Yuria-Pharm”) is among the top 3 drugs administered by hospital doctors for the period 2014-2020. Repeated prescriptions for reimbursement were issued remotely, however, despite government programs, treatment in Ukraine still depends on the patient’s money. The National Health Service of Ukraine for 2021 proposed to increase the salaries of health care workers and reduce the catastrophic costs of medicines paid by patient on its own. At present, there is a need to transfer the results of clinical trials to the RCP, as the studies are conducted in specialized strictly controlled conditions, and the RCP allows to obtain more real results. There are several types of RCP studies: non-interventional, post-registration, marketing, pharmacoeconomic, and patient database and registry studies. Conclusions. 1. COVID-19 pandemic is the main challenge of modern RCP. 2. The imposition of a pandemic onto phase 2 of health care reform has limited health care and patients’ access to clinics and hospitals, and suspended planned hospitalizations and surgeries. 3. In the conditions of pandemic and quarantine there was a redistribution of drug consumption in favor of domestic drugs. 4. Reosorbilact (“Yuria-Pharm”) is among the top 3 drugs administered by hospital doctors for the period 2014-2020.
