Late adulthood or known as old age, is the final phase of the life development stage. Old age is often identified with a period of decline in various bodily functions and an impact on helplessness. Because of this, many elderly suffer from incurable and life-threatening diseases known as palliative diseases. Many families of elderly patients who have palliative diseases feel unable to care for their patients at home, and finally, choose to leave elderly palliative patients at the Wredha Home because patients depend on the help of others for care to fulfill their needs and to carry out their daily activities accompanied by a caregiver at the Wredha Home. This reasearch was to ascertain an expertises felt by caregivers in caring for palliative patients at the Tulus Kasih Home. This research uses qualitative methods, data collection using interview methods with structured guides, and a purposive sampling technique that includes four participants. The result of this study covered six themes: the caregiver's understanding of palliative patients, what the palliative patients needs, what difficulties the caregiver experiences when giving action to elderly palliative patients, the caregiver's response when having difficulty caring for palliative patients, what efforts are made by the caregiver when having difficulty caring for palliative patients, and the efforts made by the caregiver in increasing the action for palliative patients.

В статье рассмотрены вопросы развития фиброза и иммунного воспаления у больных артериальной гипертензией (АГ) с острым ишемическим инсультом (ИИ) в пожилом возрасте. Цель исследования - изучение возраст-ассоциированных особенностей концентрации маркеров фиброза (металлопротеиназы-9, тканевого ингибитора матриксных металлопротеиназ-1, их соотношения ММП-9/ТИМП-1), иммунного воспаления (TNF-α, IL-1β, INF-γ) у больных АГ с ИИ. В исследование были включены 86 больных АГ II степени, из которых 42 человека - среднего возраста (53±5 лет) и 44 - пожилого (66±5 лет), контрольную группу составили 22 пациента пожилого возраста с АГ без ИИ в анамнезе. Критерии включения - пациенты с АГ, поступившие в стационар в остром периоде первого церебрального инсульта. Установлено, что у пожилых больных АГ с ИИ показатели инфламэйджинга и маркеры фиброза были достоверно выше, чем у лиц среднего возраста. Уровень IL-1β был выше на 31,7 % (р<0,01), TNF-α - на 55,7 % (р<0,001), INF-γ - на 36,6 % (р<0,01), уровень ММП-9 - на 46,4 % (р<0,01), ТИМП-1 - на 21,2 % (р<0,01), ММП-9/ТИМП-1 - на 19,6 % (р<0,01) в пожилом возрасте по сравнению с больными среднего возраста с АГ и острым ИИ. Таким образом, установлено, что больные АГ с ИИ имеют нарушения процессов инфламейджинга, синтеза и деградации внеклеточного матрикса, особенно выраженные в пожилом возрасте. The article deals with the development of fibrosis and immune inflammation in patients with arterial hypertension and acute ischemic stroke in old age. The aim of the study was to study age-associated features of the concentration of fibrosis markers (metalloproteinase-9, tissue inhibitor of matrix metalloproteinases-1, their ratio MMP-9/TIMP-1), immune inflammation (TNF-α, IL-1β, INF-γ) in patients with arterial hypertension and ischemic stroke (AI). The study included 86 patients with arterial hypertension (AH) of the 2nd degree, of which 42 were middle-aged (53±5 years) and 44 elderly (66±5 years). The control group consisted of 22 elderly patients with AH without a history of AI. The criteria for inclusion in the study are patients with hypertension who were admitted to the hospital in the acute period of the first cerebral stroke. It was found that in elderly patients with hypertension with AI, the indicators of inflamaging and fibrosis markers were significantly higher than in middle-aged people. The level of IL-1β was 31,7 % higher (p<0,01), TNF-α by 55,7 % (p<0,001), INF-γ by 36,6 % (p<0,01), the level of MMP-9 was 46,4 % higher (p<0,01), TIMP-1 by 21,2 % (p<0,01), MMP-9/TIMP-1 by 19,6 % (p<0,01) in the elderly compared to middle-aged patients with hypertension and acute AI. Thus, it was found that patients with arterial hypertension and ischemic stroke have violations of the processes of inflamaging, synthesis and degradation of the extracellular matrix, especially pronounced in old age.

Introduction: Health problems of elderly are an emerging health burden throughout the world. Bangladesh is currently undergoing a demographic transition and the proportion of the population of 60 years and older is increasing rapidly. Health care providers and policymakers are highly concerned with this burning issue. Objective: To know the disease pattern among the elderly patients in Combined Military Hospital, Dhaka. Materials and Methods: This cross-sectional study was carried out from July 2015 to June 2016 among 152 elderly patients above 60 years of age admitted in Combined Military Hospital (CMH), Dhaka Cantonment. Data were collected by face-to-face interview with semi-structured questionnaire and checklist following purposive sampling technique. Analysis of data was done by Statistical Package for Social Science (SPSS, version 20.0). Results: Mean age of the elderly was 72.06±4.56 years with the range of 60-80 years and majority (90.8%) of the elderly was male. Out of total 152 elderly patients, by occupation majority (31.6%) were in the business group followed by 30.3% in the retired group and 9.2 % in the housewife group. Average monthly family income was BDT 17927.63±7360.75 with the range of BDT 6000-35000. With initial complaints elderly patients reported to doctors in private chamber (38.2%), private hospital (25.6%) and Govt hospital (5.9%). Among all of the elderly patients, majority (21.1%) had Diabetes Mellitus followed by Rheumatoid Arthritis (17.6%), Asthma (12.5%), Cataract (11.2%), ENT problem (6.6%), Malignancy (5.9%) and Benign Enlargement of Prostate 8(5.3%). Conclusion: The number of elderly people is expanding rapidly; it also presents multifaceted health problems and thus creates unique challenges for the national healthcareservices. Early identification of problem and ensuring the availability of health with economic and social support can have a control over the elderly health problems. Tukad Bindu at first was well-known as the dirty river and contained tons of household waste thrown away by society. Nevertheless, since 2018 it became one of recreation center in Denpasar City. Many activities carried out currently and the more crowded Tukad Bindu becomes, it is necessary to conduct a research which is expected to be able to perceive the characteristic and the meaning of recreation in Tukad Bindu to be a consideration for further development. The research method used is qualitative method with qualitative descriptive data analysis technique. The data resource of this research is from primary and secondary data. Data collection technique is conducted by observation, interview, questionnaire, and documentation. Sample determination is committed by accidental sampling using nonprobability sampling technique. Informant determination is committed by purposive sampling procedure.The result of this research indicates that progress level of Tukad Bindu is start on the development stage. The characteristics of Tukad Bindu visitors are perceived from socio-demographic characteristic, demographic characteristic, and psychographic characteristic turns out to be very diverse. The meaning of visitors’ recreation after having recreation in Tukad Bindy mostly are social meaning. Community should keep supporting activities committed by Tukad Bindu Foundation, particularly the youths there. It is expected that Tukad Bindu Foundation continues to hold regular annual events, such as Pekan Nusantara. Academics should synergize with each other in order to create an integrated recreation area among the fields of fishery, agriculture, and tourism.  Keyword:  Characteristics, The Meaning of Recreation, Leisure and Recreation

Background: Chemotherapy underuse in elderly patients (aged ≥75 years) with colon cancer has been reported in previous studies. However, these studies were mostly registry-based and limited in their potential to consider underlying reasons of such undertreatment. This study aimed to evaluate patient and hospital determinants of chemotherapeutic treatment in patients with stage III colon cancer, with a particular focus on age and underlying reasons for nontreatment of elderly patients. Methods: A total of 629 patients with stage III colon cancer who were diagnosed in 2003 through 2012 and recruited into a population-based study in the Rhine-Neckar region of Germany were included. Information on sociodemographic and lifestyle factors, comorbidities, and treatment was collected from patient interviews and physicians. Patient (with an emphasis on age) and hospital factors were evaluated for their associations with administration of adjuvant chemotherapy overall and of oxaliplatin specifically using multivariable logistic regression. Results: Administration of chemotherapy decreased from 94% in patients aged 30 to 64 years to 51% in those aged ≥75 years. A very strong decline in chemotherapy use with age persisted even after comprehensive adjustment for multiple patient factors—including comorbidities—and hospital factors and was also seen among patients without any major comorbidities. Between 2005 and 2008, and 2009 and 2012, chemotherapy administration in patients aged ≥75 years decreased from 60% to 41%. Among chemotherapy recipients, old age was also strongly associated with higher odds of nonadministration of oxaliplatin. The 2 most commonly reported reasons for chemotherapy nonreceipt among the study population were patient refusal (30%) and old age (24%). Conclusions: Age was the strongest predictor of chemotherapy underuse, irrespective of comorbidities and even in patients without comorbidities. Such underuse due just to older age in otherwise healthy patients deserves increased attention in clinical practice to ensure that elderly patients also get the best possible care. Patients’ refusal as the most frequent reason for chemotherapy nonreceipt also warrants further investigation to exclude misinformation as underlying cause.

Mobile learning and connecting development and trends are widely discussed globally. Literature evidences exist confirming mobile learning and its impact on higher education. Mobile learning opportunities can be one option for higher education institutions to reach a dispersed population of the Maldives. Although readiness of mobile learning has been analysed in several counties and contexts, this contributes as the first research on this subject in the Maldives. Therefore this article is intended to find out students' readiness for mobile learning (mLearning) in the higher education institutes of Maldives. Basic readiness and mobile usage readiness of students from three different higher education institutions contributed to the research. A total of 343 institutional students enrolled in flexible delivery programs responded to the online questionnaire survey. Considering the level of programs, cluster sampling technique was used in shaping the sample of the research. Data was analysed using descriptive statistics and SPSS to interpret the findings on student readiness for mobile learning. The findings indicated that the students were confident in using mobile and other mobile devices in daily activities. Also, findings indicated that majority of students were confident with the basic and advanced mobile skills of mobile learning.

This paper analyses the greeting system of Kisubi. It specifically seeks: to identify temporal greeting patterns in Kisubi; to determine whether age and gender do dictate the kind of verbal greetings, address forms and paralinguistic behaviours that are used in particular greeting events; to examine different kinship-specific verbal greetings, address forms and paralinguistic behaviours that are used in particular greeting events; and lastly, to assess the role of context in the Kisubi greeting system. Observation and face-to-face interview methods were used to gather data from 36 participants who were obtained using snowball sampling technique, and afterwards they were stratified into different strata depending on age, gender and educational status of a person. This stratification led to the attainment of authentic data. The obtained data were subjected to content analysis from which different themes emerged. It was established that there are four temporal chunks in Kisubi, each of which is associated with its own greeting term. Age, gender, context and kinship relations also determine the type of greeting terms and the honorifics or deferential terms to use. However, the use of honorifics and/or deferential terms is declining gradually.

BackgroundEndovascular therapy (EVT) for acute ischemic stroke (AIS) can be challenging in older patients with supra-aortic tortuosity. Rescue carotid puncture (RCP) can be an alternative in case of supra-aortic catheterization failure by femoral access, but data regarding RCP are scarce. We sought to investigate the feasibility, effectiveness and safety of RCP for AIS treated by EVT.MethodsPatients treated by EVT with RCP were included from January 2012 to December 2019 in the Endovascular Treatment in Ischemic Stroke (ETIS) multicentric registry. Main outcomes included reperfusion rates (≥TICI2B), 3 month functional outcome (modified Rankin Scale) and 3 month mortality. We also performed an additional systematic review of the literature according to the PRISMA checklist to summarize previous studies on RCP.Results25 patients treated by EVT with RCP were included from the ETIS registry. RCP mainly concerned elderly patients (median age 85 years, range 73–92) with supra-aortic tortuosity (n=16 (64%)). Intravenous thrombolysis (IVT) was used for nine patients (36%). Successful reperfusion was achieved in 64%, 87.5% of patients were dependent at 3 months, and 3 month mortality was 45.8%. The systematic review yielded comparable results. In pooled individual data, there was a shift toward better functional outcome in patients with successful reperfusion (median (IQR) 4 (2–6) vs 6 (4–6), p=0.011).ConclusionRCP mainly concerned elderly patients admitted for AIS with anterior LVO with supra-aortic tortuosity. The procedure seemed feasible, notably for patients treated with IVT, and led to significant reperfusion rates at the end of procedure, but with pronounced unfavorable outcomes at 3 months. RCP should be performed under general anesthesia to avoid life-threatening complications and ensure airways safety. Finally, RCP led to low rates of closure complications, emphasizing that this concern should not withhold RCP, if indicated.

This study aims to explain 1) the effect of psychological empowerment on innovative behavior, 2) the influence of organizational culture on innovative behavior. This research was conducted at UD Romo Jewelry Manufacture. The number of samples taken was 72 employees, with a non probability sampling method with a type of saturated sampling technique. Data collection is done through a survey approach with questionnaire and interview methods. The analysis technique used is multiple linear regression. Based on the results of the analysis it was found that psychological empowerment and organizational culture simultaneously had a positive and significant effect on innovative behavior. This shows that the higher the psychological empowerment given to employees, the higher the employee's innovative behavior. Likewise with organizational culture, the higher the organizational culture of a company, the higher the employee's innovative behavior. Therefore to foster innovative behavior in company employees is expected to pay more attention to psychological empowerment and organizational culture in order to improve innovative behavior.
