Aim. Assessment of organs and tissues of oral cavity of professional athletes who use protective mouth guards. Methods. Dental examination of 340 professional athletes of Olympic reserve at the age of 18 to 32 years with a duration of sport experience of 9-13 years was performed. We investigated the effect of mouth guards on the condition of the marginal periodontium and microbial contamination. Changes in periodontal tissues were diagnosed by means of traditional methods of inspection, probing of the periodontal grooves, recesses, as well as determining tooth mobility. The control group included 20 athletes not using athletic mouth guards during training. Results. High prevalence of caries (in average, in all examined athletes the rate of carious teeth reaches 32.90%) and fairly high prevalence of severe forms of periodontal diseases in those patients were revealed. Evaluation of the structure of the index of teeth with decay and fillings and removed teeth showed major defects in timely dental care organization for professional boxers: their ratio of filled teeth does not exceed 40.36% in average. A significant number of extracted teeth (more than 23%) indicates that the examined patients require dental orthopaedic care. 30 examined athletes wearing protective mouth guards had the strains of P. gingivalis, A. Actinomycetemcomitans and P. oralis revealed. Conclusion. The study of dental status of professional athletes using protective mouth guards, showed high prevalence of inflammatory periodontal diseases and low level of hygienic state of oral cavity.

Aim. To reveal the rate and severity of clinical manifestations of common dental diseases and dentofacial anomalies in professional athletes Methods. The effect of intense physical exertion and overtraining syndrome on the state of the organs and tissues of the dental system were examined in 200 professional athletes aged 1825 years between 2014 and 2016. Five groups where each has the same number of athletes (n=40) included: team sport athletes (volleyball, handball), martial arts athletes (wrestling, boxing), gymnasts, swimmers, athletes. The control group consisted of 40 individuals who were not professionally involved in sports. The state of the oral cavity, in particular periodontal tissues, was judged by changes in the following indicators: prevalence of dentofacial anomalies, the incidence of dental caries, state of the oral cavity hygienic using by simplified oral hygiene index (OHI-S) ( Green J.С., Vermillion J.R., 1963), degree of gums bleeding by using the MuhlemannCowell bleeding index, degrees of severity of periodontopathy by using the papillary-marginal alveolar index (RMA) [Massler, Schour (1949) in Parma modification (1960)]. Results. According to the values of the simplified oral hygiene index the worst state of the oral cavity hygienic was in wrestlers (2.120.022 points) and boxers (2.030.029) compared to the control group (1.620.026, p=0.049 and p=0.001, respectively). Catarrhal gingivitis and generalized chronic periodontitis are most often diagnosed in the groups of wrestling (82.56.01%), gymnastics (77.56.60%), boxing (70.07.25%) and swimming (70.07.25%). Dentoalveolar anomalies were more often detected in boxers (77.5% of cases, p=0.001) and in gymnasts (34.0%, p=0.365) compared to the control group. In these groups, diastema was also more often diagnosed. Conclusion. Periodontal inflammation indicators were higher in wrestlers and gymnasts groups compared to the dental status of all other groups of athletes; caries and maximum level of dental anomalies are more common for boxers.

Aim. Evaluation of dental status in patients with inflammatory periodontal diseases. Methods. The study of dental status was performed with the use of clinical and historical data from 269 patients with inflammatory periodontal diseases at the age of 18-65 years. Among examined patients there were 75 (27.9%) people with gingivitis, 54 (20.1%) with mild periodontitis, 66 (24.5%) with moderate and 74 (27.5%) with severe periodontitis. Control group consisted of practically healthy donors at the age of 18-52 years (40 people) with sanitized oral cavity. Results. From history and interviewing it was found that 72% of the surveyed patients brush their teeth twice a day, 24% - once a day and 4% of patients do not brush their teeth. Family history of periodontal diseases among parents was recorded by 86% of participants. Dental status was characterized by the increase of the values of all dental indices with increasing disease severity regardless of gender. Need for correction of the depth of the vestibule, lips, cords, and bridles was identified in 51.7% of cases in patients with gingivitis and in 96.6% of cases of periodontal diseases. With increasing severity of periodontitis index (sum) of teeth with decay and fillings and removed teeth was increased by 1.5 times in gingivitis, by 2.2 times in mild periodontitis, by 2.6 times in moderate and by 2.9 times in severe periodontitis compared to control group. Partial adentia in gingivitis is 3 times more prevalent in males than in females and in severe periodontitis is 1.6 times more prevalent in females than in males. Conclusion. Dental status of the patients with inflammatory periodontal diseases was characterized by increased values of all dental indices compared to control group that demonstrates typical course of inflammatory periodontal diseases and decrease of local immunologic resistance of oral cavity and the organism in total; timely correction of local factors (local trauma), anatomical and topographical features of dentofacial system and status of oral soft tissues allows prevention of pathologic processes in periodontal tissues.

90 pregnant women with inflammatory periodontal diseases, aged 27 to 35 years were examined. The main group consisted of 60 pregnant women with gestational diabetes mellitus and the control one consisted of 30 patients with physiological pregnancy. All patients were examined before providing treatment and preventive measures at 14–15 weeks of pregnancy and after them at 37–38 weeks. Studies have shown deterioration in periodontal health, oral hygiene and increase in contamination of the oral cavity in patients of the main group as compared with patients of the control group. The use of the anti-inflammatory succinic acid based preparation is effective for inflammatory periodontal diseases in pregnant women with gestational diabetes. The improvement of periodontal tissues health was confirmed by the posttreatment indices of hygiene, immunoglobulins, microbiocenosis and periodontal indices that promote normalization of the oral cavity.

Periodontal diseases are one of the most important problems in dentistry, due to its very high dissemination. Inflammatory processes in periodontal tissues are more common among young people, which, in the future, without adequate treatment lead to irreversible changes in structure of cellular bone. This causes development of new effective therapeutic complexes aimed at eliminating etiopathogenetic factors development of periodontal tissues inflammatory diseases and the targeted influence on the pathological processes in these tissues. The objective of the study was to analyze the efficiency of suggested initial-I stage generalized periodontitis treatment among second-year students of medical university. Materials and methods. To achieve this goal, we examined and treated 50 students with initial-I stage of generalized periodontitis without somatic pathology, which were divided into two groups, depending on treatment method. First group included 25 students who received advanced treatment; second group included 25 students, who received usual treatment. Control group consisted of 20 clinically healthy students. The state of oral cavity prooxidant-antioxidant system was determined by the level of malone dialdehyde (MDA), diene conjugates (DK), superoxide dismutase (SOD) activity, glutathione-S-transferase (G-ST), and the activity of glutathione peroxidase (GPO). Results and discussion. Periodontal diseases, namely, initial-I stage of GP, are accompanied by an imbalance of oral cavity prooxidant-antioxidant system indicators. This is confirmed by an increase number of MDA and DK (p<0.001), and a decrease in SOD, G-ST, GPO (p<0.001) compared to clinically healthy students. Conclusions. The results of GP treatment in first group were significantly better in relation to the second one (p<0.05), indicating the effectiveness of the proposed advanced treatment of initial-I stage of GP.

The susceptibility to the development and progression of inflammatory periodontal diseases, which depends on genetic and external factors (smoking, stress, oral hygiene), varies widely. In the development of these diseases, an important role is played not only by the presence of periodontal pathogenic microorganisms, but also by the presence of congenital or acquired immunodeficiency, immunoregulatory defects. The immune system plays a key role in the physiological and pathological processes of periodontal tissues. In this regard, IL17, produced by CD4+ Th cells, which has both Pro-inflammatory and protective activity, is of particular interest in the pathogenesis of periodontitis. The aim of study was to identify the relationship between polymorphic loci of the IL-17A (rs2275913) and MMP-1 (rs1799750) genes and clinical manifestations of chronic periodontitis in petrochemical workers. Dental examination was performed in 92 ethylene oxide production workers with chronic periodontitis and 74 patients with chronic periodontitis who did not come into contact with chemical factors (control group). Genotyping of polymorphisms rs2275913 of the IL17A gene and rs1799750 of the MMP1 gene was performed by allele-specific real-time polymerase chain reaction (PCR). Hygienic assessment of the degree of air pollution of the working area with harmful substances was carried out by gas chromatography according to the guidelines for the determination of harmful substances in the air № 5098-89, № 3119-84. When comparing the results of studies of both groups, there were no statistically significant differences in the frequency distributions of allelic variants and genotypes of the IL-17A and MMP-1 genes. The AA/AG genotypes of the IL-17A gene were associated with an increased risk of severe disease compared to the GG genotype in workers in the main group (OR=6.1; 95% CI 1.33-28.5; p=0.021) and in the control group (OR=7.26; 95% CI 1.34-39.25; p=0.016). Carriers of the A allele in the control group increased the risk of severe chronic periodontitis by 2.4 times compared to carriers of the G allele (OR=2.41; 95% CI 1.19-4.87; p=0.014). During the dental examination of employees of the ethylene oxide plant, the clinical course of periodontal diseases was more severe in comparison with the control group, and the number of patients with severe periodontitis was twice as high. It was found that the AA/AG genotypes of the IL-17A gene and the carrier of the A allele are associated with increased susceptibility to the development of severe chronic periodontitis. The association between the MMP-1 gene polymorphism and the risk of severe forms of chronic periodontitis has not been established. A risk factor for the development of inflammatory periodontal diseases in employees of the petrochemical complex is a complex of harmful production factors.

With diabetes, there is a violation of the microvasculature, a violation of the immune status, an increase in destructive processes in the bone tissue, which leads to the development and / or intensification of pathological processes. In the oral cavity, the most frequently diagnosed inflammatory periodontal diseases, chronic injuries of the oral mucosa (COP), recurrent aphthous stomatitis, COP candidiasis. The aim of our study was to study the dental status of patients with type 2 diabetes mellitus. Materials and Methods: A descriptive evaluative epidemiologically controlled, non-randomized study was conducted to determine the effect of type 2 diabetes on dental status. The main group (“case”) consisted of 68 patients with type 2 diabetes in the compensation phase (IA) and 56 patients in the subcompensation phase (IB). The comparison group included 60 patients without somatic pathology. All patients underwent a comprehensive dental examination, bacteriological and microscopic examination to identify fungi of the genus Candida. Results: in patients with subcompensated form of type 2 diabetes, there was an increase in the frequency of complaints of plaque on the tongue, burning sensation in the COP, dryness and changes in taste. Periodontal indices indicate that the patients of the main group have a high prevalence of inflammatory and destructive processes in the periodontal tissues (indices of PMA, KPI, Mullemann indices) with poor oral hygiene (OHI-S = 3.5 ± 0.4), which confirms the Friedman criterion (χ2 = 116.27; p = 0.0000) based on the calculation of Kendall's concordance coefficient (0.743). The diagnosis of oral mucosa candidiasis was confirmed in 74.6% of patients in the main group by bacteriological examination.

Introduction. More and more researches dedicated to the communication of diseases of the oral cavity of pregnant women with diabetes. It is proved that the intensity of caries and inflammatory periodontal diseases (gingivitis and periodontitis) increase significantly during pregnancy, while the presence of comorbidities, such as diabetes, increase these indexes.Aim. The aim of the work was to study the dental status of pregnant women with diabetes.Materials and methods. The study compared women with gestational diabetes mellitus, type 1 diabetes, and type 2 diabetes, to a control group of pregnant women without diabetes. In addition to clinical research methods, liquid-based cytology of the contents of the gingival sulcus was performed.Results. The results of clinical and laboratory studies have shown that inflammatory diseases of periodontium and teeth within pregnant women with diabetes are more common than within the pregnant women without this disease. It should be noted that the frequency of occurrence and severity of these diseases in the pregnant women with type I diabetes is higher than in the other groups.Conclusion. Pregnant women with diabetes are at risk for dental disease and require more attention from dentists, endocrinologists and obstetricians. The use of liquid-based cytology method helps in the diagnosis of inflammatory periodontal diseases.

Aim. Evaluation of dental diseases prevalence in professional athletes engaged in power sports and of the influence of biologically neutral products based on propolis on the level of their dental health. Methods. The examination of organs and tissues of the oral cavity of professional athletes engaged for a long time in power sports was carried out. With the use of the developed specialized questionnaire dental status of the athletes was determined. Based on the analysis of the results of clinical and laboratory studies anti-inflammatory and anti-bacterial effectiveness of products based on propolis were evaluated. Results. The results of the survey showed that not all athletes are informed about the possibility of aggravation of pathological processes in soft and hard tissues of oral cavity during active training, mainly, in power sports. Lack of awareness of surveyed athletes about the need for timely individual oral hygiene procedures and regular dentist visits was revealed, especially during intense training before and after competitions. On evaluation of dental status of professional combat athletes high prevalence and intensity of inflammatory periodontal disease and caries were found. High anti-inflammatory efficiency of the product «Pomegranate Balm» for correction of dental status of professional athletes was proven. Conclusion. High incidence of inflammatory periodontal diseases of mild to moderate severity was revealed in athletes during prolonged and intense physical training; efficacy of biologically neutral products based on propolis was established.

Obesity prevalence has significantly increased especially in young adults, which is caused by a particular lifestyle, food quality and dietary behavior. Obesity leads to development of huge array of comorbid conditions such as arterial hypertonia, heart stroke, arthritis and other diseases. We conducted standard clinical examination of oral cavity of 154 young patients (18-21 years old) – all of them were students of Ukrainian Medical Stomatological Academy. It has been determined that the majority of patients with BMI >30 kg/cm2 have aggravated family heredity, in 66,2% cases one parent of obese students had obesity and 32,43% patients have both parents with obesity that is significantly higher compared with persons with normal BMI and overweight. There was a high prevalence of periodontal diseases about 74% and carious lesions of teeth - 97.4% among young people despite the age and contingent of examined patients - medical students. In patients with BMI >30 kg/cm2 prevalence of generalized forms of gingivitis and periodontitis was by three times higher compared with individuals with normal BMI. The values of oral hygienic indexes were poor in all examined groups, but their values were slightly lower in patients with normal BMI than in those with overweight and obesity. The prevalence of inflammatory changes in gums was higher in persons with obesity: all of them had a mild degree of lesions in periodontal tissues. Inflammatory processes in the gums were the most intense in patients with the second degree of obesity. According to the results of the study, the presence of the first and the second degree of obesity should be considered as a risk factor triggering periodontal tissues diseases. For persons with BMI >30 kg/cm2 with periodontal disease measures for the secondary prevention of inflammatory and inflammatory dystrophic periodontal diseases should be carried out and in persons without periodontal disease on the background of obesity measures primary prevention should be done.

Introduction: β-thalassemia major is a hereditary blood disease, a main pathogenetic factor of which is the disruption of the synthesis of β-chains of hemoglobin. Inevitable metabolic disorders that occur during regular blood transfusion form an additional risk of damage and cellular modification of organs and tissues, including the oral cavity. Aim: determination of the pathogenetic role of iron metabolism disorder in the development of chronic inflammatory periodontal diseases in patients with β-thalassemia. Material and methods: 12 patients with β - thalassemia major who had periodontal diseases were examined in the Republican Center of Thalassemia in Baku (average age is 18 ± 1.3 years). The control group consisted of 16 somatically healthy individuals with intact gingiva tissues (the average age is 18.0 ± 0.7 years). Serum Iron, ferritin, hepcidin, total iron-binding capacity (TIBC), level of Il2, Il6, Il10, TNFα were measured from peripheral blood samples. To determine the ultra-structural study of the periodontal tissues in patients with β-thalassemia major, the method of electron microscopy studies of gingiva biopsies was used. Results of the study: The study revealed an increase in the level of serum iron and ferritin against the background of a progressively decreasing TIBC in patients with β-thalassemia major. Also activation of pro-inflammatory cytokines - TNFα, Il6, Il10 was observed in homozygous patients. Metabolic indicators are compared with ultrastructural features of cellular elements of a gum. Signs of accumulation of iron in cellular elements both of its own plate and of the epithelial cover were compared. The presence of iron was recorded in immunocompetent cells - macrophages and lymphocytes. Conclusion: Summarizing the data obtained, we can conclude that deproteinization of ferritin cores and their detection both in the cytoplasm and in the nucleoplasm of immunocompotent cells leads to irreversible degenerative changes in cellular elements of the gingiva. The authors link the development of the generalized inflammatory - degenerative process in the periodontium with revealed exchange -structural violations.
