There is paucity of information in the epidemiology and pathology of tuberculosis in swine though the endemic occurrence of bovine tuberculosis was elucidated in Ethiopia. A cross-sectional study was employed to investigate the epidemiology of Mycobacterium tuberculosis complex and  Mycobacterium avium complex in swine. The study was conducted from September 2016 to December 2017 using single intradermal comparative tuberculin test, gross pathology and histopathology. Tuberculin test was carried out in farmed swine of central Ethiopia while the necropsy at Addis Ababa Abattoirs Enterprise, Ethiopia. Of the total 329 heads of swine tuberculin tested by bovine purified protein derivative antigen, an animal level prevalence of 3% (95% CI: 2-6) and a herd level prevalence of 11% (95% CI: 1-49) were observed at a cut-off value of >2mm. Multivariable logistic regression analysis revealed the protective exposure effect of body condition to tuberculin test positivity in swine (Adjusted OR: 0.06; 95% CI: 0.0-1.1; P: 0.055). Swine were predominantly in close proximity with dairy cattle which is an important risk factor for the transmission of bovine tuberculosis from cattle to swine. Necropsy study of tuberculosis-like lesions in 640 heads of swine revealed an overall lesion prevalence of 4.1 % (95 CI: 2.8-6.0). In conclusion, the present study demonstrated the occurrence of swine tuberculosis in low prevalence in farmed swine and swine slaughtered atAddis Ababa Abattoirs Enterprise in central Ethiopia. Further in-depth study covering larger sample size and wider areas is warranted so as to identify the prevalent species and their zoonotic importance. Bovine tuberculosis (bTB) is a serious infectious disease of cattle with significant economic impact and public health risk. It is particularly important in Ethiopia where effective control measures are lacking. This cross-sectional study was carried out between April and July 2016 on 1038 cattle selected from 206 dairy farms in Adama city located in central Ethiopia to estimate the prevalence of bTB and identify the potential risk factors using comparative intradermal tuberculin skin test. Accordingly, the individual animal level prevalence was found to be 2.1% (95% CI: 1.35 – 3.22). Of the 206 dairy farms included in the study, 7.3% (95% CI: 4.29 – 11.98) had one or more 15 cattle positive for the tuberculin test. Furthermore, 5.5% of the examined cattle were found reactive for atypical Mycobacterium. Among the risk factors considered, age and lactation status of the animals were significantly associated (p< 0.05) with the prevalence of bTB. It was noted that the apparent prevalence of bTB positive reactivity was greater in young and non-lactating cattle than their counterparts. In conclusion, the present study revealed 2.1% prevalence of bTB in the dairy farms investigated and culling of positive reactors is recommended as a feasible control intervention. Background. Depression is a significant contributor to the global burden of disease and affects all individuals throughout their lifetime. Patients with schizophrenia are frequently attacked by depression during their total illness duration. Presence of comorbid depression in schizophrenia makes the patients more deteriorating and disabling course and poor outcome. Aim of the Study. To determine the prevalence of depression and highlight the associated sociodemographic and clinical factors in patients with schizophrenia in a specialized hospital in Addis Ababa, Ethiopia. Setting. This study was conducted at Amanuel Mental Specialized Hospital, Addis Ababa, Ethiopia. Methods. An institutional based cross-sectional study was conducted from May to June 2018. Depression was measured by Calgary Depression Scale for Schizophrenia on 455 samples of patient with schizophrenia and systematic sampling was used to select the study participants. Oslo Social Support Scale and Alcohol, Smoking, and Substance Involvement Screening Test were used to assess social support and substance use factors, respectively. A bivariable and multivariable logistic regression analysis model was performed to control the confounding factors. Odds ratio (OR) with the corresponding 95% confidence interval (95% CI) was determined to evaluate the strength of association. Results. A total of 445 patients responded to the questionnaire, which yields a response rate of 97.8%. The Magnitude of depression among schizophrenia patients was 24.9%. A multivariable logistic regression analysis model showed that being female [AOR 2.00, 95% CI: 1.25-3.18], divorced/widowed [AOR 2.39, 95% CI: 1.04-5.49], current substance use [AOR 1.95, 95% CI: 1.17-3.25], and poor social support [AOR 2.75, 95% CI: 1.35-5.61] were significantly associated with depression in schizophrenia. Conclusion. The magnitude of depression among schizophrenia was 24.9%. Being female, divorced/widowed, current substance use, and poor social support were associated with depression among patients with schizophrenia. Regular screening and prompt management of depressive symptoms among patients with schizophrenia is of particular importance to reduce the burden of the condition.

Background: Adolescents who have communication about sexual and reproductive health (SRH) with their parents are less likely to participate in risky sexual behaviors and to utilize modern contraceptives. However, communication on SRH issues between adolescents and their parents continues to be below the desired goals in low- and middle-income countries. Objective: To assess SRH communication between adolescent girls and their mothers among students in secondary and preparatory schools in Fiche town, Oromia, Central Ethiopia. Method: An institution-based cross-sectional study was conducted from March 27 to March 30, 2017, using a structured self-administered questionnaire. A total of 181 adolescent female students selected from 2 schools in the town participated in the study. Data were collected on sociodemographic characteristics, student-mother communication, and knowledge about and attitudes toward SRH issues. Multivariable logistic regression analysis was performed, and variables with a P value less than .05 were considered statistically significant. Results: A total of 103 (56.9%) of the students had discussed SRH issues with their mothers. The mothers’ educational status (ie, primary school and above, adjusted odds ratio [AOR] = 10.01, 95% CI = 3.55-28.19; the student living with friends, AOR = 5.29, 95% CI = 1.09-25.47; the students’ knowledge about SRH issues, AOR = 8.57, 95% CI = 3.61-20.35; and students’ attitude toward SRH issues, AOR = 2.79, 95% CI = 1.26-6.15) were identified as independent positive predictors of student-mother communication on selected SRH issues. Conclusion: More than half of the study participants had communication on SRH issues with their mothers. SRH communication was positively associated with maternal educational status, the students’ living with friends, and the student’s knowledge of and attitude toward SRH issues.

Bovine tuberculosis (bTB) continues to be one of the most widely distributed chronic infectious diseases of zoonotic importance, which causes a significant economic loss in animal production. A cross-sectional study was conducted to estimate the prevalence of bTB and its associated risk factors and type the Mycobacterium bovis isolated in central Ethiopia. A total of 65 dairy farms and 654 cattle were tested for bTB using a single intradermal comparative cervical tuberculin (SICCT) test. Data on farm management, animal-related characteristics, and the owner's knowledge of the zoonotic importance of bTB were collected using a structured questionnaire. In addition, a total of 16 animals from different farms were identified for postmortem examination. Lowenstein Jensen (LJ) culture was also conducted, and spoligotyping was used to type the M. bovis strains isolated. Chi-square test and logistic regression models were used to analyze the herd- and animal-level risk factors. Herd- and animal-level prevalence rates of bTB were 58.5% (95% CI: 46.2%−69.2%) and 39.3% (95% CI: 35.5%−43.5%), respectively. At the herd level, poor farm management was the predictor for bTB positivity (p &lt; 0.05). Animal breed, poor BCS, farm type, and poor farm management conditions were significant predictors of bTB positivity (p &lt; 0.05) at an individual animal level. All animals identified for postmortem examination were found to have gross TB-like lesions. A total of 14 M. bovis strains were identified from 12 animals that were positive for LJ culture. The strain with the largest number of clusters (five isolates) was SB1176, followed by SB0134 (three isolates), SB0192 (two isolates), and SB2233 (two isolates), and two new strains, each consisting of only one isolate. The majority (58.5%) of the respondents did not know the zoonotic importance of bTB. The result of this study showed a high prevalence of bTB in the Addis Ababa milkshed and a low level of consciousness of the owners on its transmission to humans. Therefore, the launching of acceptable control measures of bTB and the creation of public awareness about its zoonotic transmission and prevention measures are required.

A cross-sectional study was carried out between January 2012 and May 2014 to investigate the status of bovine tuberculosis in the state of Espírito Santo. The state was divided into two regions, and in each of them, 300 farms with reproductive activity were randomly selected and considered as the primary sampling units. In the selected farms, a fixed number of female bovines aged over 2 years were randomly selected to undergo a comparative cervical tuberculin test; an epidemiologic questionnaire was also applied. In the state of Espírito Santo, the apparent prevalence of tuberculosis-positive farms was 7.6% (95% confidence interval [CI] = 5.7-9.9). Prevalence at the herd level varied from 4.6% (95% CI = 2.6-7.3) in region 1 to 11.1% (95% CI = 7.7-15.3) in region 2. The apparent prevalence of tuberculosis-positive animals was 0.7% (95% CI = 0.3-1.1) in the state, and the prevalence varied from 0.3% (95% CI = 0.2-0.6) in region 1 to 1.2% (95% CI = 0.3-2.9) in region 2. The risk factors associated with tuberculosis prevalence in Espírito Santo were the number of adult females ? 10 (odds ratio [OR] = 2.40; 95% CI = 1.17-5.31) and milking type (milking machine/milking parlor) (OR = 2.88; 95% CI = 1.36-5.86]). The state of Espírito Santo should set up a surveillance system to detect and control bovine tuberculosis, taking into account the importance of dairy farms and animal trade in the state.

This study was conducted to determine the prevalence of cystic echinococcosis (CE) in small ruminants and humans in Addis Ababa, central Ethiopia. A cross-sectional study involving systematic random sampling was conducted to estimate the prevalence of CE in 512 small ruminants (262 sheep and 250 goats) slaughtered at Addis Ababa Abattoir Enterprise between October 2011 and March 2012. Hydatid cysts were identified macroscopically during postmortem examination and their fertility and viability were determined. CE was observed in 21 (8.02%) sheep and 17 (6.80%) goats. In sheep 13 (4.96%) of the lungs, 10 (3.81%) livers and 1 (0.381%) heart were found to be infected with hydatid cysts. Involvement of lung and liver in goats was found to be 10 (4.0%) and 8 (3.2%) respectively, with no cysts recorded in the heart. Of the total of 77 and 47 cysts encountered in sheep and goats, 33 (42.85%) and 15 (31.91%) respectively were fertile. Viability of protoscoleces from fertile cysts in sheep (29 [87.87%]) was higher than in goats (6 [40.0%]). For humans, retrospective analysis covering five years of case reports at two major hospitals in Addis Ababa between January 2008 and December 2012 showed that of the total of 25 840 patients admitted for ultrasound examination, 27 CE cases were registered, a prevalence of 0.1% and mean annual incidence rate of approximately 0.18 cases per 100 000 population. Liver was the major organ affected in humans (81.5% in affected patients) followed by spleen (11.1%) and kidney (7.4%). Logistic regression analysis showed that prevalence of CE varied significantly in relation to host age in the small ruminants (OR = 3.93, P < 0.05) as well as in humans (95% CI, R = 4.8). This epidemiological study confirms the importance of CE in small ruminants and humans in central Ethiopia, emphasising the need for integrated approaches to controlling this neglected preventable disease.

A cross sectional study was performed between June 2009 and March 2010 to determine the situation of bovine tuberculosis (bTB) in Rondônia. The state was divided into three regions and, in each of them, 300 farms with reproductive activity were randomly chosen and considered as primary sample units. In the selected farms, an epidemiologic questionnaire was applied. A fixed number of bovine females older than two years of age was randomly selected and tested through comparative cervical tuberculin test. Considering the State of Rondônia, the apparent prevalence of bTB positive farms was 2.3% (95% CI = 1.5–3.5%). The prevalence in the regions varied from 1.7% (95% CI = 0.7 – 4%) to 3% (95% CI = 1.6–5.7%). The apparent prevalence of bTB positive animals in the State of Rondônia was 0.12% (95% CI = 0.06–0.25%) and varied from 0.08% (95% CI = 0.04–0.18%) to 0.15% (95% CI = 0.07–0.33%) in the regions. The risk factor associated to tuberculosis in the State of Rondônia was the acquisition of animals (OR = 7.1; 95% CI = 1.6–31.1). The State of Rondônia should implement a surveillance system to detect bTB-infected herds to certify them as bTB-free. Moreover, an efficient health education program to inform farmers to test replacement animals for bTB prior to introduction in their herds should also be implemented.

The cross-sectional study was carried out to compare the hematological and serum biochemical profiles between bovine tuberculosis (bTB) affected and apparently healthy cattle during the period from March 2014 to October 2016. A total of 189 blood samples were collected from 93 bTB affected and 96 apparently healthy cattle of 3 Upazilas of Chittagong. The bovine tuberculosis (bTB) was diagnosed primarily by Caudal Fold Tuberculin Tests (CFTT) and confirmed by the Comparative Intradermal Tuberculin Test (CITT). The whole blood was analyzed for hematology, and serum samples were used for biochemical analysis. The study revealed that, the values for TEC, Hb and PCV were higher in infected cohort (based on CFTT) in compare to their counterpart control group. Among these three parameters only PCV values were statistically significant (P ≤ 0.05). Only basophil percentage was significantly (P ≤ 0.05) higher in infected cohort. The values of total protein, albumin, creatinine and SGOT were insignificantly lower and the values of calcium & phosphorus were also insignificantly higher in bTB affected cattle than control group of cattle.

Polymyositis (PM) and dermatomyositis (DM) are autoimmune-mediated multisystemic myopathies, characterized mainly by proximal muscle weakness. A connection between epilepsy and PM/DM has not been reported previously. Our study aim is to evaluate this association. A case–control study was conducted, enrolling a total of 12,278 patients with 2085 cases (17.0%) and 10,193 subjects in the control group (83.0%). Student’s t-test was used to evaluate continuous variables, while the chi-square test was applied for the distribution of categorical variables. Log-rank test, Kaplan–Meier curves and multivariate Cox proportional hazards method were performed for the analysis regarding survival. Of the studied 2085 cases, 1475 subjects (70.7%) were diagnosed with DM, and 610 patients (29.3%) with PM. Participants enrolled as cases had a significantly higher rate of epilepsy (n = 48 [2.3%]) as compared to controls (n = 141 [1.4%], p < 0.0005). Using multivariable logistic regression analysis, PM was found only to be significantly associated with epilepsy (OR 2.2 [95%CI 1.36 to 3.55], p = 0.0014), whereas a non-significant positive trend was noted in DM (OR 1.51 [95%CI 0.99 to 2.30], p = 0.0547). Our data suggest that PM is associated with a higher rate of epilepsy compared to controls. Physicians should be aware of this comorbidity in patients with immune-mediated myopathies.

ObjectivesTo assess the magnitude of internalised stigma and associated factors among patients with bipolar disorder attending the outpatient department of Amanuel Mental Specialized Hospital, Addis Ababa, Ethiopia.DesignInstitution-based cross-sectional study design.SettingAmanuel Mental Specialized Hospital, Addis Ababa, Ethiopia.ParticipantsWe recruited about 418 participants using systematic sampling technique for an interview during the study period.MeasurementData were collected by face-to-face interviews. Internalized Stigma of Mental Illness scale was used to measure internalised stigma. The Rosenberg Self-Esteem Scale and the Oslo-3 Social Support were instruments used to assess the associated factors. Bivariate and multivariate logistic regressions were performed to identify factors associated with the outcome variable. ORs with 95% CI were computed to determine the level of significance.ResultsThe magnitude of internalised stigma was 24.9% (95% CI: 21.2% to 28.9%). In the multivariate analysis, unemployed (adjusted OR (AOR)=2.3, 95% CI: 1.0 to 5.0), unable to read and write (AOR=3.3, 95% CI: 1.05 to 10.7), poor social support (AOR=5.3, 95% CI: 1.9 to 15.0), ≥4 previous hospitalisations due to bipolar disorder (AOR=2.6, 95% CI: 1.1 to 6.1) and low self-esteem (AOR=2.4, 95% CI: 1.1 to 5.1) had a significant association with internalised stigma.ConclusionsOne in four patients with bipolar disorder reported high internalised stigma. Unemployment, low educational status, low self-esteem, poor social support and being hospitalised more than three times before were significantly associated with internalised stigma. Thus, a stigma-reduction programme focusing on self-esteem improvement and psychological health of patients to increase their stigma resistance to counteracting effects of internalised stigma is essential.
