The article analyzes the disorders in homeostasis diagnosed during the development of acute viral infection SARS-CoV-2 in patients. Hematological changes in patients with coronavirus infection were characterized by the development of disintegration of humoral mechanisms of regulation with the initiation of a systemic inflammatory reaction with an increase in the leukocyte index of intoxication, activation of necrobiotic processes, an increase in the activity of macrophage-microphage nonspecific protection with a predominance of effector links of the immunological process. The development of immunocompromise in patients with a new strain of coronavirus infection is one of the determining factors in the course of the disease.

Prophylaxis, diagnostics and correction of nutritional status disturbances is considered as one of the main treatment methods of patients with COVID-19 infection-directed to resolve systemic inflammatory response and correction of metabolic response to a viral infection. Systemic Inflammatory Reaction (SIR) manisfestation as a result of viral infection leads to pronounced metabolic processes disturbances. The main metabolic manifestations of SIR is reflected as hypermetabolic-hypercatabolic syndrome with complex disturbances of protein, lipids and carbohydrates metabolism, increased consumption of carbohydrate-lipid reserves and breakdown of tissue proteins. Thus, adequate correction of metabolic disorders and a wholesome nutritional support, taking into account the clinical picture, severity of the disease, ongoing respiratory and intensive care therapy is an integral component in treating patients with COVID-19 infection which determines the efficiency of its treatment and reduction in mortality. Given the relevance of the problem, the authors decided that it was important to increase the COVID-19 treatment efficacy by producing guidelines based on the most fundamental provisions of the modern approach to nutritional support in critical patients with community acquired pneumonia, acute respiratory failure, ARDS, sepsis, multiple organ failure.

Objective — to analysis and systematization of literature data about pathogenesis of the inflammatory reaction and the clinical course of coronavirus infection caused by SARS-CoV-2. Materials and methods. Access to various full-text and abstract databases was used for the search query «coronavirus», «COVID-19», «SARS-CoV-2» and their systematic evaluation was carried out. The most complete database of available literature sources (about 70) was obtainedon the molecular pathophysiology of COVID-19. Results and discussion. The results of the analysis of the molecular pathophysiology of COVID-19 showed that the biomedical terms associated with COVID-19/SARS-CoV-2 form several clusters: cluster 1 is inflammation and the formation of a cytokine storm; cluster 2 — pathophysiological justification of the treatment of coronavirus infection, cluster 3 — comorbid conditions. Analytics of cluster 1 showed one of the most interesting working hypotheses today is model of bradykinin storm. This hypothesis can explain the multisymptomatic nature of COVID-19, including some of its strangest manifestations. The essence of the theory of bradykinin is that, when the virus begins to affect the regulation of renin-angiotensin-aldosterone system (RAAS) through the activation of angio­tensin-converting enzyme type II, it causes the mechanisms that regulate bradykinin levels to fail. Bradykinin’s receptors repeatedly are sensitized and the body also ceases to efficiently break down bradykinin. ACE break down bradykinin, but when the virus suppresses its activity, it cannot work with the same efficiency. The end result of such an imbalance is the release of excessive amounts of bradykinin, due to its mass unrestrained accumulation with the formation of a phenomenon — bradykinin storm. According to the bradykinin hypothesis, this particular variant of the storm is ultimately responsible for the polymorbidity of the clinical picture and the fatal effects cause d by COVID-19. Conclusions. The bradykinin hypothesis of the development of a systemic inflammatory response in SARS-CoV-2 virus is a model that contributes to a better understanding of the pathogenesis and course of COVID-19 and adds novelty to data that are already known. It predicts almost all known symptoms today and offers quality treatments for the disease. Analysis of the processed data from the literature of cluster 2, devoted to the pathophysiological rationale treatment of coronavirus infection led to the role of vitamin D — as a nutrient involved in regulatory processes with participation of RAAS.

The resorptive effects of 0.01 M cerium nanodioxide sol upon single intraperitoneal administration to rats have been studied. The acute exposure to nanoparticles was found to have a dose-dependent general toxic effect on the body (weight loss, inflammatory changes in the abdominal organs, modification of individual behavior, hematological changes, metabolic imbalance), which develops on the background of POL activation. The prooxidant effect of cerium dioxide nanoparticles is demonstratively manifested at relatively high exposure levels (80–8 mg / kg). The threshold dose for the general toxic effect (Limch integr) is equal to 0.8 mg / kg.

Since the outbreak near a fish market in Wuhan, China, in December 2019, researchers have been searching for an effective therapy to control the spreading of the new coronavirus SARS-CoV-2 and inhibit COVID-19 infection. Many countries like Italy, Spain, and the USA were ambushed by this viral agent. To date, more than 2.5 million people were infected with SARS-CoV-2. There is no clear answer, why SARS-CoV-2 infects so many people so fast. To date of April 2020, no effective drug has been found to treat this new severe viral infection. There are many therapy options under review and clinical trials were initiated to get clearer information, what kind of drug can help in this devastating and serious situation. The world has no time.

COVID-19, a disease caused by the novel coronavirus SARS-CoV-2, primarily affects lung tissue and disrupts gas exchange, leading to acute respiratory distress syndrome, systemic hypoxia, and lung damage. The search for methods of prevention and rehabilitation, especially after suffering from pneumonia caused by COVID-19, is on the agenda. This article discusses the possibilities of the interval hypoxic training (IHT) method for preventing infections by initiating nitric oxide production in the body. One of the main effects of IHT is the balanced stimulation of nitric oxide (NO) secretion. Over the past two decades, there has been an increasing interest in the function of nitric oxide (NO) in the human body. Nitric oxide plays a key role in maintaining normal vascular function and regulating inflammatory processes, including those leading to lung damage and the development of acute respiratory distress syndrome (ARDS). Our immune system destroys bacteria and viruses by oxidative burst, i.e. when oxygen accumulates inside the cell. This process also involves nitric oxide, a signaling molecule that has an antibacterial and antiviral effect, as well as regulates vascular tone and affects the permeability of the cell wall. Interval hypoxytherapy enhances endogenous oxidative protection and increases the amount of nitric oxide, thus allowing the body’s cells to resist infection more effectively. Mitochondrial NOS induction and mitochondrial NO synthesis increase under the action of pathogenic factors on the cell. By modulating the activity of mtNOS and the synthesis of mitochondrial NO, it is possible to increase the resistance to hypoxic effects. Interval hypo-hyperoxic training as an effective non-specific method of increasing the body’s defenses is indispensable not only in the prevention of viral infection, but also in rehabilitation after viral pneumonia, as well as as a method that reduces the severity of viral infection in the event of infection.

Oral fluid is an alternative biological material that confirms correlations with blood parameters in various pathological conditions of the body. In order to find a non-invasive approach to stratification of patients with COVID-19 disease, molecular biomarkers of the oral fluid have been determined in patients with moderate coronavirus infection in comparison with clinically healthy individuals. It has been shown that proteomic, carbohydrate, macro- and microelement profiles of the oral fluid in coronavirus infection can be used for diagnostics. The features of protein metabolism were revealed: an increase in the content of total protein, urea; increased activity of enzymes aspartate aminotransferase, gamma glutamyl transpeptidase, creatine phosphokinase, alkaline phosphatase; changes in carbohydrate metabolism, which is expressed by an increase in glucose and lactate levels, an increase in lactate dehydrogenase activity, sodium, chloride, calcium, magnesium, iron content.
