Introduction. Vibration disease (VD) is associated with an imbalance of endocrine regulation, the development of metabolic syndrome (MS), which leads to the development of neurological complications. One of the reasons is microcirculation disorders that involve the CNS and peripheral nerves in the pathological process. The aim of the study is to provide a comprehensive assessment of the state of peripheral nerves and afferent pathways in workers with VD, and patients with VD, burdened with MS. Material and methods. We examined: 41 patients with VD associated with combined local and general vibration (group 1), group 2 - patients with VD burdened with MS (29 people), control (group 3) - 33 people. Check somatosensory evoked potentials (SSEP) and electroneuromyography was performed on the NCS system «Neuro-EMG-Micro», «Neurosoft». Results. In group 2, violations of the central afferent pathways at the level of the cervical spine and the somatosensory cortex were detected, expressed in an increase in the latency period of components P25 and N30 and the duration of the interval N11-N13. When testing the motor and sensory components of peripheral nerves, patients with VD, burdened with MS, were found to have a more pronounced decrease in SPI for the sensory and motor components of the tibial nerve, and increased residual latency. Discussion. Changes in the central nervous system and peripheral nerves in MS are aggravated by the impact of vibration - changes in carbohydrate, protein, and enzymatic metabolism, triggering processes that lead to polyneuropathy. Exposure to vibration generates chronic stress (external factor MS) associated with activation of the hypothalamic-pituitary-adrenal system and the development of insulin resistance. Conclusion. In group 2, there was an increase in moderate autonomous-sensory polyneuropathy of the hands and feet and peripheral angio-dystonic syndrome of the hands. Violations in the state of the central afferent pathways at the level of the cervical spine and cortical structures of the somatosensory cortex were detected. Exposure to vibration and MS leads to an aggravation of demyelination of sensory and motor axons of peripheral nerves, expressed in the legs.

Introduction. The defeat of motor and sensory conductive structures, which ultimately leads to the development of demyelination processes and, as a consequence, the formation of polyneuropathic syndrome is natural under the influence of industrial vibrations. Exposure to vibration, along with other harmful working conditions, can potentiate the development of metabolic syndrome, which can lead to the development of diabetes, which is one of the most common causes of damage to the peripheral nervous system. Based on the above, it seems relevant to give a comprehensive assessment of the state of peripheral nerves and afferent conductive structures in workers exposed in their professional activities combined effect of local and general vibration, and patients with vibration disease, burdened with diabetes. Material and methods. 114 people were examined in clinical conditions. The observed cases included 50 with vibration disease (WB) associated with exposure to local and General vibration (group 1), the second group - 29 people with WB, burdened with type 2 diabetes. The control group (group 3) consisted of 35 conditionally healthy men, representative age (52.0±6.4 years) without contact with harmful factors of production. Results. In patients of group 1, changes were observed in increasing the time of postsynaptic activation of the posterior horns of the spinal cord and activation of neurons of the somatosensory zone of the cerebral cortex. In patients of group 2, changes in the state of the central afferent pathways were more pronounced, shown in increasing the time of the excitation wave from the cervical thickening to the thalamic nuclei, as well as violations in the state of the conductive structures in the thalamic region. Analysis of the peripheral nerves in group 1 patients revealed changes in demyelinating lesions of the motor and sensory components of the tibial nerve and changes in the speed indices of the nerves of the upper extremities. The examined 2 groups had similar disorders, there was a statistically significant decrease in the speed of sensory axons of the lower extremities. conclusion. It was found that the combined effect of vibration and diabetes mellitus leads to aggravation of the demyelination processes of both sensory and motor axons of peripheral nerves in patients. There is a statistically significant decrease in the amplitude of the M-response on the lower extremities.

We aimed to investigate the impact of metabolic syndrome (MetS) on the varicocele treatment. 101 patients underwent spermatic vein ligation between 2007 and 2010 were retrospectively analyzed. Those patients were divided into two groups as without (n: 56, Group 1) or with MetS (n: 48, Group 2). All the patients underwent left microsurgical subinguinal spermatic vein ligation. Groups were compared by the improvement on sperm parameters and spontaneous pregnancy rates at a mean of 19 (±4) months followup. When sperm parameters were compared postoperatively, the significant improvement in total sperm count, motile sperm count percentage, and normal sperm percentage was reported. The groups were compared to each other and the improvement seemed significantly better in Group 1. There was no statistically significant improvement difference in the normal sperm percentage between groups. Spontaneous pregnancy rate after two years was 45% in Group 1 and 34% in Group 2 (). Patients with MetS and varicocele improved after surgery, but not as well as the similar group without MetS. This may help to show that MetS can be a factor for male infertility.

Object Postoperative dysphagia is a well-recognized complication of the anterior surgical approach to the cervical spine. However, its incidence and etiology remain unknown. The aim of this study was to investigate the impact of automatic retractor use on the esophagus and to describe the related pathological changes that might occur during cervical spine surgery. Methods A single-level cervical discectomy was performed via an anterior approach in 16 skeletally mature female sheep. Continuous retraction was applied with an automatic retractor system during surgery. The sheep model was chosen because of anatomical similarities to the human esophagus. The esophageal tract in every animal was examined using contrast radiographic examination. Eight animals were killed 3 days after the operation (Group 1). The remaining sheep were killed 4 weeks after the operation (Group 2). The esophagi were removed for histopathological study, which was performed using H & E and Masson trichrome staining. The changes in esophageal innervation were examined with nicotinamide adenine dinucleotide diphosphate–diaphorase histochemical staining. Results Only 1 animal (a Group 1 sheep) demonstrated any postoperative radiographic abnormality. In Group 1 sheep, histopathological study of the esophagi at the treated level revealed edema between the muscular fibers in the outer longitudinal and inner circular layers of the muscularis propria. At some points, obvious signs of vascular congestion, vascular damage, and inflammation were observed. In the Group 2 animals, there was mild-to-moderate fibrosis extending from the outer surface of the esophagus to the longitudinal layers of the muscularis propria in the area to which retraction had been applied. Enzyme-histochemical staining revealed the presence of normal myenteric plexus and ganglion cells, and nitrergic innervation in all parts of the esophagus wall. Conclusions The results of this study demonstrate that direct pressure induced by the medial retractor blade on the esophagus wall leads to local injury. Postoperative dysphagia in human patients who have undergone anterior cervical spine surgery could be a clinical manifestation of this phenomenon.

Aims:Erectile dysfunction (ED) is a common condition affected by many factors. We aimed to show the impact of the metabolic syndrome (MeTS) on male sexual function based on VAI and the impact of increased levels of the VAI was investigated in patients with ED among the patients with and without MeTS. Methods:Participants who met MeTS criteria (Group 1, n=96) and without MeTS (Group 2, n=189) were included in this cross-sectional study. The MeTS diagnosis was made in the presence of at least three of the following criteria: serum glucose level higher than 100 mg/dl, HDL cholesterol level below 40 mg/dl, triglyceride level greater than 150 mg/dl, waist circumference greater than 102 cm and blood pressure greater than 130/85 mmHg. Demographic data were recorded; biochemical and hormonal tests were measured. Erectile and other sexual function scores were recorded. The VAI was calculated using the [(WC/39.68)+(1.88xMI)]xTG/1.03x1.31/HDL formula. Results:Mean age, smoking volume, T and T/E2 ratios of the groups were similar (p>0.05). Mean VAI was two-fold higher in patients in Group 1 (p<0.001) and erectile function score was lower in Group 1 than Group 2 (p=0.001). Other sexual function scores were similar (p>0.05). The METS was associated with an increased risk of ED (p=0.001). Logistic regression analysis showed that each integer increase of the VAI was associated with a 1.4-fold increased risk of ED (p<0.001). Higher T values were associated with a better erectile function (p=0.03). For the VAI=4.33, receiver-operating characteristic analysis showed a sensitivity of 89.6 % and specificity of 57.7 %. Conclusion:Compared to non-MeTS, the presence of MeTS has emerged as a risk factor for patients with ED with high VAI levels while the other sexual functions are preserved. Management of ED patients with MeTS should cover a comprehensive metabolic and endocrinological evaluation in addition to andrological work up.

The article deals with economic sanctions and their impact on the state and prospects of the neighboring partner economies - the European Union (EU) and Russia. It provides comparisons of current data with that of the year 2013 (before sanctions) to demonstrate the impact of sanctions on both sides. Despite the fact that Russia remains the EU’s key partner, it came out of the first three partners of the EU. The current economic recession is caused by different reasons, not only by sanctions. Both the EU and Russia have internal problems, which the sanctions confrontation only exacerbates. The article emphasizes the need for a speedy restoration of cooperation.

Sources of revenue and expenditure of APBD (regional budget) can be allocated to finance the compulsory affairs and optional affairs in the form of programs and activities related to the improvement of public services, job creation, poverty alleviation, improvement of environmental quality, and regional economic growth. The implications of these policies is the need for funds to finance the implementation of the functions, that have become regional authority, is also increasing. In practice, regional financial management still poses a complicated issue because the regional head are reluctant to release pro-people regional budget policy, even implication of regional autonomy is likely to give birth to little kings in region causing losses to state finance and most end up in legal proceedings. This paper discusses the loss of state finance and forms of liability for losses to the state finance. The result of the study can be concluded firstly,  there are still many differences in giving meaning and definition of the loss of state finace and no standard definition of state losses, can cause difficulties. The difficulty there is in an effort to determine the amount of the state finance losses. The calculation of state/regions losses that occur today is simply assessing the suitability of the size of the budget and expenditure without considering profits earned by the community and the impact of the use of budget to the community. Secondly, the liability for losses to the state finance is the fulfillment of the consequences for a person to give or to do something in the regional financial management by giving birth to three forms of liability, namely the Criminal liability, Civil liability, and Administrative liability.Keywords: state finance losses, liability, regional finance.

Subject. We assess the impact of tight financial and monetary policy of the government of the Russian Federation and the Bank of Russia on the level of household income and poverty reduction in Russia. Objectives. The purpose of the study is to analyze the results of financial and monetary policy in Russia and determine why the situation with household income and poverty has not changed for the recent six years, and the GDP growth rate in Russia is significantly lagging behind the global average. Methods. The study employs methods of analysis of scientific and information base, and synthesis of obtained data. The methodology and theoretical framework draw upon works of domestic and foreign scientists on economic and financial support to economy and population’s income. Results. We offer measures for liberalization of the financial and monetary policy of the government and the Central Bank to ensure changes in the structure of the Russian economy. The proposed alternative economic and financial policy of the State will enable the growth of real incomes of the population, poverty reduction by half by 2024, and annual GDP growth up to 6 per cent. Conclusions. It is crucial to change budget priorities, increase the salaries of public employees, introduce a progressive tax rate for individuals; to reduce the key rate to the value of annual inflation and limit the bank margin. The country needs a phased program to increase the population's income, which will ensure consumer demand.

Subject. This article explores the issues of business tourism clustering in Greater Moscow. Objectives. The article intends to justify the need to create a business tourism cluster in Greater Moscow to improve the investment climate in the region. Methods. For the study, I used a multivariate analysis, forecasting, and extrapolation. Results. The article shows a certain relationship between the efficient functioning of the business tourism cluster and the economy's development. Conclusions and Relevance. Certain types of tourist clusters can serve as platforms for attracting investors and implementing marketing plans. The business tourism cluster is a link between buyers and sellers in various industries. The results of the study can be used to improve the effectiveness of the cluster initiative in business tourism, as well as find ways of cooperation between the State and private investors when creating the business tourism cluster in Greater Moscow.
