ABSTRACT Objectives Fixed orthodontic appliances (FOAs) may cause a temporary deterioration of oral hygiene with corresponding gingival inflammatory changes. Optimal oral hygiene and dietary changes are essential in order to avoid periodontal inflammation. The present study investigates to what extent the recommendations made by the orthodontist are followed by young patients. Materials and methods A total of 67 adolescent patients with FOA were examined and interviewed. The control group consisted of 70 patients of the same age who had not undergone any orthodontic treatment. A specific questionnaire (assessing dietary habits, oral hygiene measures) was used and comprehensive oral findings along with oral hygiene status were evaluated. Results The analysis of the data showed that despite intensive oral hygiene and dietary advice in the patients with FOA, there was no change in diet or oral hygiene habits. There were significantly higher signs of gingival inflammation and amount of dental plaque in FOA patients. Nutritional recommendations and oral hygiene measures for young people with FOA were not adequately enforced. Conclusion The findings indicate that frequent recalls should be scheduled with repeated reinforcement of oral hygiene and dietary modifications in FOA patients. The present study aimed to evaluate the effect of the oral-hygiene regime on the incidence of enamel demineralization in young patients treated with fixed appliances. Research was conducted in a group of 144 patients aged 12–18 years, divided into 2 groups: orthodontically treated and control. The study was divided into three stages: before treatment (I), at 1 month (II), and at 6 months (III) for their follow-up. The International Caries Detection and Assessment System (ICDAS) was used for the visual assessment of white spot lesions (WSL). After 1 month, no new white spot lesions were observed. After 6 months of the treatment, new lesions were observed in 5% of the orthodontically treated patients and in 6% of the patients in the control group. New decalcifications were located on the proximal surfaces of the central incisors, first premolars, and first molars in the orthodontically treated group; and on the lateral incisors, first premolars, and first molars in the control group. We also observed new enamel demineralization on the vestibular surfaces of the canine and first premolar in the study group. The placement of a fixed appliance did not significantly affect teeth with the presence of new white spot lesions compared to the control group during 6 months of observation.

BackgroundThere is limited data on the beneficial effects of probiotics on the gingival health of patients undergoing treatment with fixed orthodontic appliances. This study aims to compare the effect of probiotic tablets combined with regular oral hygiene versus regular oral hygiene alone on gingival status in these patients. The effect of probiotic intake on plaque formation and salivary microbiome composition will be also assessed.Methods and analysisThis is a 3 month single-centre, single blind (clinical and laboratory examiners), parallel group randomised controlled two arm superiority trial. Fifty paediatric patients attending the Postgraduate Orthodontic Clinic at the Hamdan Bin Mohammed College of Dental Medicine (HBMCDM), Mohammed Bin Rashid University of Medicine and Health Sciences (MBRU), Dubai, United Arab Emirates, who meet the eligibility criteria will be recruited. Block randomisation with 1:1 allocation and concealment of allocation will be carried out. The treatment group will receive probiotic tablets containingStreptococcus salivariusM18 andLactobacillus acidophilustogether with regular oral hygiene versus the control group on regular oral hygiene alone. Clinical examination and collection of saliva for microbiome assay will be carried out at baseline and end of study. Self-reporting by patients will be used to document acceptability and adverse effects. Statistically significant decrease in gingival bleeding on probing in the treatment group will be classified as primary outcome of treatment success. Statistically significant reduction in Plaque Index, Gingival Index and shift in the composition of the oral microbiome in favour of beneficial bacteria are secondary outcomes indicative of efficacy of probiotic intake.Ethics and disseminationEthical approval for the study has been granted by the HBMCDM, MBRU, Institutional Review Board (Reference #: MBRU-IRB-2018–015). Good oral hygiene is an important factor in oral and general health, especially in orthodontic patients, because fixed appliances might impede effective oral hygiene and thus increase the risks of tooth decay, periodontal disease and general health complications. This study investigated the impact of fixed orthodontic appliances on the distribution of dental biofilm in teenagers. Supragingival plaque was assessed at T0, T1 and T2. The distribution of the biofilm was analyzed. Approximal Plaque Index (API) and Bonded Bracket Index (BBI) were used to measure the presence of dental plaque. After insertion of the fixed appliance, the dental plaque indices values in the orthodontically treated group were significantly higher (p < 0.05) than in the control group. Fixed orthodontic appliances caused significant changes in the distribution of the biofilm. This was characterized by the change of location of the dental plaque. In the orthodontic group, we observed an increase in the amount of the supragingival plaque on the vestibular surface of the teeth.

Background: Oral hygiene maintenance is difficult for patients who are receiving fixed orthodontic treatment. Objective: The aim of the study was to find out deterioration of oral hygiene status during treatment of fixed orthodontic appliances in the Department of Orthodontics in BSMMU. Methodology: This cross sectional study was carried out in the Department of Orthodontics at Bang-  abandhu Sheikh Mujib Medical University, Dhaka from July 2013 to July 2014. All the patients taking treatment with fixed orthodontic appliances for more than 6 months were included as study population. During treatment in the OPD all the information of the patients’ oral hygiene maintenance like brush-  ing, dental floss use, use of mouthwash, and history of sugar consumption, taking sticky food were recorded.  The  Orthodontic  Plaque  Index  (OPI)  was  calculated  and  recorded.  In  addition  to  that frequency of brushing, type of tooth brush, technique of brushing and inter dental brush were also considered. Result: A total number of 100 patients were studied. Mean OPI was 81.78 (22.97). Mean value of OPI was 86.35 (13.27) among patients who brushed their teeth once a day but it was decreased gradually as the  patients  increased  frequency  of  toothbrush  use  per  day  and  became  85.74  (±26.12)  among  the patients who brushed their teeth thrice a day. Mean value of OPI was 81.98 (±23.27) who did not use dental floss and 75.00 (±7.00) who used dental floss once a day. Mean value of OPI was 84.65 (±23.10) who used inter dental brush irregularly but 35.00 (±0.00) who used it thrice a day. Mean value of OPI was 85.70 (±22.43) who did not use mouth wash whereas it reduced to 46.00 (±0.00) among them who used mouth wash thrice a day.  Conclusion: All the indexes of oral hygiene are increased in patients treated with fixed orthodontic appliances in the Department of Orthodontics in BSMMU. Abstract: Fixed orthodontic devices are the most commonly used orthodontic appliances in the community especially adolescent because they have become an important part of lifestyle. Albeit, users of fixed orthodontic appliances do not realize that fixed orthodontic appliances could become a risk factor for poor dental and oral hygiene. This study was aimed to determine the relationship between the use of fixed orthodontic devices and the dental and oral hygiene status of students at SMA Kristen 1 Tomohon (senior high school). This was an analytical observational study with a cross sectional design. Subjects were 43 students who used fixed orthodontics appliances obtained by using total sampling method. The results showed that most subjects used fixed orthodontic appliance for less than one year (58.1%). Moreover, the dental and oral hygiene status of most subjects were in the moderate category. The Chi-square showed a p-value of 0,060 for the relationship between the use of fixed orthodontic devices and the dental and oral hygiene status. Conclusion: There was no significant relationship between the duration of use of fixed orthodontic devices and the dental and oral hygiene status of students at SMA Kristen 1 TomohonKeywords: fixed orthodontic appliances, oral hygiene, adolescents Abstrak: Alat ortodontik cekat merupakan peralatan ortodontik yang paling sering dipakai oleh masyarakat khususnya remaja karena sudah menjadi bagian dari gaya hidup. Namun pemakai alat ortodontik cekat tidak menyadari bahwa alat ortodontik cekat merupakan faktor risiko terganggunya kebersihan gigi dan mulut. Penelitian ini bertujuan untuk mengetahui hubungan pemakaian alat ortodontik cekat dengan status kebersihan gigi dan mulut siswa SMA Kristen 1 Tomohon. Jenis penelitian ialah observasional analitik dengan desain potong lintang. Subyek penelitian yaitu 43 siswa yang memakai alat ortodontik cekat yang diperoleh dengan metode total sampling. Hasil penelitian menunjukkan pemakaian alat ortodontik cekat terbanyak yaitu di bawah satu tahun (58,1%) dan status kebersihan gigi dan mulut berada dalam kategori sedang. Hasil uji Chi-square terhadap hubungan antara pemakaian alat ortodontik cekat dengan status kebersihan gigi dan mulut subyek penelitian mendapatkan nilai p=0,060. Simpulan: Tidak terdapat hubungan bermakna antara lama pemakaian alat ortodontik cekat dengan status kebersihan gigi dan mulut siswa SMA Kristen 1 Tomohon.Kata kunci: alat ortodontik cekat, kebersihan gigi dan mulut, remaja

Background/Aim. Orthodontic treatment enables the establishment of functional occlusion and improvement of oral health, however, it increases the risk of periodontal disease development. The aim of this paper was to examine the efficiency of the applied programme for the prevention of gingivitis in children undergoing the fixed orthodontic appliance therapy and to determine the most efficient devices and techniques for maintaining oral hygiene during orthodontic treatment. Methods. The study included 80 patients of both genders - 60 patients comprised the experimental group and 20 patients comprised the control group. All of them were patients of the Clinic for Orthodontics at the School of Dentistry in Belgrade, aged between 13 and 18. The Silness-L?e Plaque Index (PI) was utilised for the assessment of oral hygiene quality and Silness-L?e Gingival Index (GI) and M?hlemann Papilla Bleeding Index (PBI) were utilised for the assessment of gingival state. Checkups were conducted as a single-blind study at the beginning and after the first, the third and the sixth month of the preventive and prophylactic programme. Results. During the observed period, a statistically significant change in PI, GI and PBI values was noticed (p < 0.005), as well as the difference in the dynamics of value changes during the periods between the observed groups. Conclusion. The preventive programme, applied to children undergoing the fixed orthodontic appliance therapy, had a positive effect both on oral hygiene quality and gingival state. The values of the examined parameters of the patients from the experimental group were significantly lower in comparison with those of the patients from the control group. The most efficient combination of devices for oral hygiene during orthodontic treatment was: a Curaprox CP5460 toothbrush, CD Ortho 60 orthodontic toothbrush and Curaprox CPS 14 interdental brush.

Introduction. During orthodontic treatment, there is increased risk of periodontal disease and caries. Therefore these patients must be trained to maintain proper oral hygiene to minimize risks. The aim of this study was to examine the effects of oral hygiene training with different devices as well as a motivation and remotivation in oral hygiene using brochures, verbal methods and tablets for plaque identification. Material and Methods. The study included 80 subjects, both genders, divided in control (20) and experimental groups (60). All examinees in the experimental groups used toothbrush Curaprox CP5460 for oral hygiene. Subjects in experimental group 1 used mouthwash Curasept ADS 205, in the experimental group 2 orthodontic toothbrush CD Ortho 60 and proximal toothbrush Curaprox CPS14 while subjects in experimental group 3 used proximal toothbrush Curaprox CPS14 and mouthwash Curasept ADS 205. All participants were given tablets erythrosine and brochure in which was explained and illustrated the use of the aforementioned means for oral hygiene. Control check ups were conducted at the beginning, after the first, third and sixth month of the preventive program. Results. The values of the examined parameters, plaque index (PI), gingival index (GI) and gingival bleeding index (GBI), in experimental groups were significantly lower compared to control group (p<0.05). Conclusion. Motivation, compliance and implementation of all preventive procedures proposed by this program, as well as regular controls, contributed in maintaining gingival health during orthodontic treatment.

Background: Oral hygiene behaviours as well as dietary habits before bed can affect children’s dental health resulting in higher prevalence of dental disease. Dental disease can affect children’s health, development and even school performance. If left untreated, dental disease can progress and it can lead to extractions under general anaesthetic causing further distress for children and families. Consistent and appropriate oral hygiene behaviours and dietary habits can prevent dental diseases from occurring in the first place. Objective: This cross-sectional study examines the relationship between oral hygiene behaviours, dietary habits around bedtime and children’s dental health. Methods: A total of 185 parents with children between the ages of 3 and 7 years from deprived areas participated in the study. Data on bedtime routine activities were collected using an automated text-survey system. Children’s dental health status was established through examination of dental charts and dmft (decayed, missed, filled teeth) scores. Results: In total, 52.4% of parents reported that their children’s teeth were brushed every night. The majority of children (58.9%) had dmft scores over zero. In total, 51 (46.7% of children with dmft score over 0 and 27.5% of all children) children had active decay. The mean dmft score for those experiencing decay was 2.96 (SD = 2.22) with an overall mean dmft score of 1.75 (SD = 2.24). There were significant correlations between frequency of tooth brushing, frequency of snacks/drinks before bed and dmft scores (r = −0.584, p < 0.001 and r = 0.547, p = 0.001 respectively). Finally, higher brushing frequency was associated with a lower likelihood of a dmft score greater than 0 (Exp(B) = 0.9). Conclusions: Despite families implementing oral hygiene behaviours as part of their bedtime routines those behaviours varied in their consistency. Results of this study highlight the need for additional studies that consider bedtime routine-related activities and especially the combined effects of oral hygiene practices and dietary habits due to their potentially important relationship with children’s dental health.

ABSTRACT Objectives: The long-term skeletal effects of Class II treatment in growing individuals using high-pull facebow headgear and fixed edgewise appliances have not been reported. The purpose of this study was to evaluate the long-term skeletal effects of treatment using high-pull headgear followed by fixed orthodontic appliances compared to an untreated control group. Materials and Methods: Changes in anteroposterior and vertical cephalometric measurements of 42 Class II subjects (n = 21, mean age = 10.7 years) before treatment, after headgear correction to Class I molar relationship, after treatment with fixed appliances, and after long-term retention (mean 4.1 years), were compared to similar changes in a matched control group (n = 21, mean age = 10.9 years) by multivariable linear regression models. Results: Compared to control, the study group displayed significant long-term horizontal restriction of A-point (SNA = −1.925°, P &lt; .0001; FH-NA = −3.042°, P &lt; .0001; linear measurement A-point to Vertical Reference = −3.859 mm, P &lt; .0001) and reduction of the ANB angle (−1.767°, P &lt; .0001), with no effect on mandibular horizontal growth or maxillary and mandibular vertical skeletal changes. A-point horizontal restriction and forward mandibular horizontal growth accompanied the study group correction to Class I molar, and these changes were stable long term. Conclusions: One phase treatment for Class II malocclusion with high-pull headgear followed by fixed orthodontic appliances resulted in correction to Class I molar through restriction of horizontal maxillary growth with continued horizontal mandibular growth and vertical skeletal changes unaffected. The anteroposterior molar correction and skeletal effects of this treatment were stable long term.
