Background:Some theoretical data suggest that the JAK-kinase blocker tofacitinib (TOFA) may have a direct analgesic effect. Objectives:to evaluate the clinical significance of the analgesic effect in patients with rheumatoid arthritis (RA) in real clinical practice. Methods:Hypothesis being tested: the analgesic effect of TOFA may be manifested by a decrease in the ratio of subjective to objective indicators of arthritis activity.Data from RA patients receiving biologics (bDMARD) and/or TOFA and included in the MUAR register were analyzed. The tender joints count (TJC), the HAQ-DI functional ability index, and the RAPID3 disease activity index were considered as subjective estimates. The swollen joints count (SJC) and the level of CRP were treated as objective indicators. The estimation of quotients from the division of subjective indicators into objective ones in all combinations is made.Taking into account the observational nature of the study, we searched for confounders for each of these ratios. Comparison of the calculated indices during the treatment with various targeted DMARDs (tDMARDs) was made with an adjustment for the detected confounders. Results:the analysis included 944 treatment episodes in 832 patients, including 93 episodes of TOFA treatment. The average age was 55.3 ± 12.4 years, women - 698 (83.9%), seropositive for RF -672 (80.8%). The analysis of the adjusted values showed that the ratios of the TJC, HAQ-DI and RAPID3 to the SJC during the treatment with TOFA was significantly lower than with tDMARDs on average. There were no significant differences in the ratios of objective indicators to the CRP level (Table).Table.Ratios of objective and subjective indicators during the treatment with tofacitinib and with tDMARDs therapy in generalConclusion:the severity of subjective feelings and functional disorders in RA patients receiving TOFA may be less with the same level of objective signs of arthritis compared with bDMARDs.Disclosure of Interests:Evgeniy Zhilyaev Speakers bureau: Novartis, UCB, Pfizer, Biocad, Abbvie, MSD, Roche, Galina Lukina Speakers bureau: Novartis, Pfizer, UCB, Abbvie, Biocad, MSD, Roche, Ekaterina Koltsova: None declared, Evgeniya Shmidt Speakers bureau: MSD, Novartis, Pfizer, Karine Lytkina Speakers bureau: Novartis, Eli Lilly, Pfizer, UCB, Abbvie, Biocad, MSD, Jonson&Jonson

Until the beginning of the 19th century, psychiatric patients did not receive specialized treatment. The problem that was posed by the presence of psychiatric patients in the Santas Casas de Misericórdia and the social pressure from this issue culminated in a Decree of the Brazilian Emperor, D. Pedro II, on July 18, 1841. The “Lunatic Palace” was the first institution in Latin America exclusively designed for mental patients. It was built between 1842 and 1852 and is an example of neoclassical architecture in Brazil, located at Saudade Beach in the city of Rio de Janeiro. In the 1930s and 1940s, the D. Pedro II Hospital was overcrowded, and patients were gradually transferred to other hospitals. By September of 1944, all the patients had been transferred and the hospital was deactivated. Key words: psychiatry, history, madness.

The hype around big data does not seem to abate nor do the scandals. Privacy breaches in the collection, use, and sharing of big data have affected all the major tech players, be it Facebook, Google, Apple, or Uber, and go beyond the corporate world including governments, municipalities, and educational and health institutions. What has come to light is that enabled by the rapid growth of social media and mobile apps, various stakeholders collect and use large amounts of data, disregarding the ethics and politics. As big data touch on many realms of daily life and have profound impacts in the social world, the scrutiny around big data practice becomes increasingly relevant. This special issue investigates the ethics and politics of big data using a wide range of theoretical and methodological approaches. Together, the articles provide new understandings of the many dimensions of big data ethics and politics, showing it is important to understand and increase awareness of the biases and limitations inherent in big data analysis and practices.

The role of museums in society has expanded significantly in the last decades: from temples of knowledge to forums for debate and discussion, from repositories of objects to people-centred institutions with social responsibilities and functions. This shift reflects an ongoing trend to democratise museums and make them more accessible to wider audiences and responsive to the public’s changing needs, in particular the interests of local communities, whose composition has changed in recent years to include migrants and people of different ethnic backgrounds. With annual migration flows to the EU as a whole projected to increase from about 1 043 000 people in 2010 to 1 332 500 by 2020, the question of how cultural institutions can contribute to effective integration and dialogue has become more relevant than ever. Funders and society at large expect museums to play their part in facilitating the integration and peaceful coexistence of newcomers, with financial resources being made available, also at the EU level, to support them in this effort. Many questions can be raised as to whether it is right and appropriate to charge museums with these responsibilities and whether this would push the boundaries of their work too far and give the social function an exceedingly prominent role over the traditional conservation and educational tasks museums already fulfil. But this discussion seems to be already obsolete in the light of the growing body of evidence on good practices available at the European level. This essay aims to illustrate some of them, as well as to discuss some underpinning theoretical issues and methodological approaches.
