Objective: to determine the clinical efficiency of antioxidant therapy in patients with liver cirrhosis and acute blood loss.Material and methods. The study included 286 patients with liver cirrhosis and acute blood loss of varying degrees of severity. The patients were divided into two series of studies. In the first series, the patients received the therapy according to the clinical diagnostic and treatment protocols, in the second series 1 according to the same protocols in combination with an intravenous administration of the antioxidant complexes “Reamberin” and “Fluimucil”. After 48 hours from admission, the antioxidant status of blood serum, the blebbing index of lymphocytes, the concentration of uric acid, and the level of hemoglobin were analyzed. The indicators of the clinical efficiency of the treatment were determined: risk of absolute mortality, risk of relative mortality, the number needed to treat.Results. The blood serum of the patients from the first series of studies was characterized by pronounced prooxidant activity, high values of the lymphocyte blebbing index, and a small amount of uric acid, which was indicative of the development of oxidative stress. The mortality rate in this series of studies was 26.0 %. The patients of the second series of studies were characterized by pronounced antioxidant activity of blood serum, low values of the lymphocyte blebbing index, and high concentrations of uric acid, which indicated a decrease in the activity of systemic oxidation processes. The mortality rate in this series of studies was 18.2 %. The duration of the treatment decreased from 16.5 days (first series) to 13.0 days (second series).Conclusion. The combined use of the “Reamberin” and “Fluimucil” complexes as part of the therapy according to the clinical diagnostic and treatment protocols leads to a relief of oxidative stress in patients with liver cirrhosis complicated by acute blood loss. The blood serum of these patients is characterized by pronounced antioxidant activity, low values of the lymphocyte blebbing index, and a high concentration of uric acid, which reduces the risk for the development of multiple organ failure syndrome. This therapy decreases the mortality rate to 18.2 % and the duration of the treatment to 13.0 days.

Background: Ursolic acid (UA) is a natural pentacyclic triterpenoid that is known for its benefits under several pathological conditions. Cisplatin (CP) is among the most preferred chemotherapeutic agents; however, its nephrotoxicity limits its clinical utility. Purpose: This study was aimed to determine the role of UA in the reduction of CP-induced nephrotoxicity and mitigation of pro-inflammatory cytokines and apoptosis in a rat model. Methodology: Male Wistar rats were randomized into vehicle control, CP (7.5 mg/kg), UA 10 mg/kg, and CP with UA 5 and 10 mg/kg groups. Kidney and blood samples were collected for assessment of renal function, measurement of pro-inflammatory cytokines, apoptosis markers, antioxidant activity, and tissue histology. Results: CP significantly increased the levels of serum Cr, BUN, and uric acid; it also induced histological damage reflecting the pathophysiology observed during nephrotoxicity. CP has also shown its pro-oxidant activity in kidney tissue because CP decreased the levels of GSH, SOD, and CAT; it increased the lipid peroxidation as measured by MDA content. In addition, CP significantly upregulated the activity of pro-inflammatory cytokines and expression of apoptotic markers, that is, there were increased levels of IL-1β, IL-6, TNF-α, caspase-3, and caspase-9. Two weeks of continuous treatment of UA showed significant recovery against CP-induced nephrotoxicity; UA decreased the levels of Cr, BUN, and uric acid and ameliorated histological damage. UA also downregulated the activities of IL-1β, IL-6, and TNF-α as well as expression of caspase-3 and caspase-9. Furthermore, CP-induced oxidative stress that was antagonized by UA—the levels of GSH, SOD, and CAT were significantly increased while MDA content was decreased. Conclusions: UA has a protective effect against CP-induced nephrotoxicity, which may be due to its antioxidant activity and mitigation of ILβ-1, ILβ-6, TNF-α, and markers of apoptosis.

There has been studied the effect of acute blood loss, which was modeled by a single loss of 30% of the circulating blood, on the fluctuations in the content of ascorbic (AA), dehydroascorbic (DAA), diketogulonic (DKGA) acid and their sum in the organs of rats in dynamics for the fifth, twelfth, nineteenth and twenty-sixth days after the blood loss. Acute blood loss caused a significant decrease in the content of all parameters of the system of metabolites of ascorbic acid – their sum, AA, DAA and DKGA – by 10–73 % compared to the control. The most significant decrease was in the content of AA, which was not restored in all organs until the end of the study period. The DAA content in all organs increased from the 12th day, and then decreased during the experiment. The content of the DKGA increased from the 19th day of the experiment. At the same time, it was found that on 26th day in the kidneys, the DAA content exceeded the control value by 42%, and the content of DKGA in the liver and blood – by 25–60 %. The content of the amount of ascorbic acid metabolites at the end of the experiment was almost restored, but this recovery occurred in various ways: in the kidneys – due to an increase in the DAA content, in other organs – by increasing the concentration of DKGA. The parts of AA from the sum of acids (in %) after blood loss significantly decreased, starting from the 5th day, and the process of its recovery began to occur only after the 19th day. The ratio of the amount of the vitamin component of the acids of the ascorbic acid system to the content of the non-vitamin DKGA was increased in the kidneys on the 12th and 26th days of the experiment, in other organs this index decreased 2.3–3.1 times in comparison with the control. The obtained data can be explained by the increased consumption of ascorbic acid to neutralize the effects of the intensification of oxidative processes under oxidative stress, which were activated by the action of acute blood loss, due to its reversible conversion to dehydroascorbic acid, and the latter irreversibly to diketogulonic acid.

The work is devoted to the research of the condition of the main components of the antioxidant system at the terms of the closed cranial cerebral trauma and at its pharmaceutical correction of Cerebrogermum.  It is determined that in case of using of Cerebrogermum the activity of superoxiddismutasa is rised in the blood serum in average on 35 % concerning the significations of the control group (the closed cranial cerebral trauma without treatment), on 72 hour of experiment the activity of this are fixed in the groups of both intact and reviewer rats. It does not also yield to reviewer preparation the ability of Cerebrogermum to resist to oxidizing stress because of the activity of catalasa. In relation to the estimation of antioxidant activity it considerably outweighs sizes for rats under influence of Cerebrogermum ( in average on 65 %) which are registered in the control group. The received in the experiment data give the basis to assert that the potential cerebral protector owns the exactly ability to resist to oxidative stress that is formed on a background of the mechanical damage of cerebrum.

Introduction. In the structure of injuries from gunshot wounds dominate the limb injuries that are often complicated by massive bleeding from the main vessels. The only means of rescue on the battlefield is a timely application of a tourniquet. This ensures complete exsanguination of the limb. The duration of exsanguination should not exceed 2 hours. It is proved that in conditions of acute blood loss complicated by ischemia-reperfusion of the limb, there are prerequisites for the development of dysfunction of internal organs, which in turn exacerbates dysmetabolic and functional disorders, stimulates endotoxicosis and may be complicated by multiple organ failure. However, the patterns of formation of the syndrome of endogenous intoxication in the conditions of ischemia-reperfusion of the limb, acute blood loss and their combinations have not been studied. There are no data on the effect of Carbacetam on these processes. It is known that Carbacetam has proven itself to reduce the manifestations of dysfunction of internal organs in the conditions of ischemic-reperfusion syndrome of the limb.The objective of research: To establish the dynamics of endogenous intoxication in the conditions of ischemia-reperfusion of the limb, acute blood loss and their combination. To evaluate the effectiveness of Carbacetam in the correction of identified abnormalities.Materials and methods. The experimental studies were conducted on 108 white nonlinear male rats weighing 200-220 g, which were housed in standard vivarium conditions. All animals were divided into five groups: control and four experimental. Under thiopental-sodium anesthesia in the first experimental group was simulated ischemia-reperfusion of the limb, in the second – acute blood loss, and in the third – these injuries were combined. In the fourth experimental group, animals with acute blood loss and ischemia-reperfusion of the limb were intraperitoneally administered Carbacetam at a dose of 5 mg per kilogram of animal weight. After 1 and 2 hours, as well as after 1, 7 and 14 days in the blood serum was determined the content of middle-mass molecules (MMM254, MMM280).The results and discussion. It was found that ischemia-reperfusion of the limb in comparison with the control group is accompanied by a significant increase of the MMM254 fraction in the blood serum content. From the first hour of the experiment with a maximum after 1 day and normalization of the index up to 14 days. The content in the blood serum of the MMM280 fraction became significantly higher, from the control only after 1 day with subsequent normalization in the following terms of the experiment. After simulation of acute blood loss, both studied fractions of MMM increased from 1 hour of the experiment, reached a maximum after 1 day and did not return to the control level after 14 days. For the first time it was found that two-hour ischemia-reperfusion of the limb significantly aggravates the course of acute blood loss, which revealed a significantly higher content in the serum of fractions MMM254 and MMM280 compared to other experimental groups in almost all periods of the experiment. Due to the use of Carbacetam, the content of both studied fractions of MMM in the serum compared with animals without correction was significantly reduced after 7 and 14 days of the experiment. Although the studied parameters did not return to the level of the Carbacetam control group, it can be considered a pathogenetically justified means of reducing the level of endotoxicosis in the conditions of ischemia-reperfusion of the limb and acute blood loss.Conclusions. Simulation of limb ischemia-reperfusion is accompanied by accumulation of MMM254-280 fractions with a maximum after 1 day of the experiment and normalization up to 14 days. Acute blood loss causes an increase in the serum content of fractions MMM254-280 from 1 hour of the experiment, which up to 14 days do not reach the level of control. The detected disorders are significantly exacerbated after a combination of acute blood loss and ischemia-reperfusion of the limb. The use of Carbacetam in comparison with animals without correction is accompanied by a significant decrease in the content of MMM254-280 fractions in the serum after 7 and 14 days of the experiment.

The article discusses the relationship between oxidative stress (OS) and pathological conditions, the possibilities and benefits of estimating OS considering the integral antioxidant activity (AOA) as an OS criterion, and using a simple accessible hybrid potentiometric method (HPM) with a mediator system for AOA monitoring. The results of AOA of blood serum in healthy volunteers and patients with various diseases are presented. Preliminary reference values are found. The lower levels of AOA of blood serum in patients with different diseases in comparison with the control group are observed. The potential mechanisms of changes in the AOA levels and it’s clinical significance are discussed from the position of biointerfaces interplay. With AOA equal or greater than 1.40 mmol-eq l–1 the person is healthy, the range from 0.95 to 1.40 mmol-eq l–1 indicates that the patient is at risk and needs to undergo a further medical examination. When AOA blood serum is below 0.95 mmol-eq l–1, detailed diagnostics and relevant treatment are required. The findings allow suggesting that the approach determine antioxidant/oxidant activity of biological fluids holds considerable promise for monitoring OS; it opens up new opportunities in expanding the use of analytical chemistry in medicine.
