Diseased environment, junk food habits, work in shift duties, anxiety etc. are the main causative factors for vitiation of Tridosha and the demonstration of diseases in today’s era. There is dearth of information about Dincharya and Ritucharya in common people. Due to which People mainly gets exaggerated their gastrointestinal system first, and then other symptoms come subsequently. According to Ayurveda Sheetpitta is described as Tridoshaj Vyadhi (Disease), but Vata and Pitta Dosha are predominant and Ras and Rakta are main Dushya. Sheetapitta is one among the Twak Vikara that have related Hetu of Kotha and Udarda. Vata and Kapha are two “Doshas”, which are primarily bothered which in turn is associated through Pitta resulting in Tridosha Prakopa causing to redness, swelling itching on the skin etc. Chief symptoms of Sheetpitta are reddish spots, inflammation on skin with moderate to severe itching at site. It is compared with urticaria in modern science and termed as primary cutaneous disorder. An episode of it may start with pruritis. Episodes of urticaria may continue to revert for days, weeks, months or year if not cured properly. Urticaria is calculated as allergic reaction due to certain food and have only symptomatic treatment and anti-allergic drug. Commonly antihistaminic medicines were used for urticaria. Ayurvedic Acharyas have mentioned so many Mukharaogas (diseases of the oral cavity). Among these Jihwagata rogas (diseases of the tongue) are described separately. Sushruta and Vagbhatta have mentioned five Jihwagata rogas. Upajihwika described by Sushruta and Adhijihwa described by Vagbhatta represent the cystic lesions on the floor of the mouth. Modern science describes a few diseases in which the patient presents with cystic swelling on the floor of mouth. Mucocele is a cystic lesion or a cavity filled with mucus. They are found on any mucosal surface where underlying accessory glands are present. Mucoceles are commonly found in the lower lip and are very rarely found on the tongue. Cystic lesions or mucoceles on the ventral aspect of the tongue are less frequent. Many modern treatment modalities as surgical excision, cryosurgery and electro-cautery are the only choice of treatment to completely remove the lesion and reduce the chances of recurrence. Herewith we report a case diagnosed as Adhijihwa according to Ayurveda. Ayurvedic internal medicines have provided promising results in reducing the cyst completely without causing any adverse effects within two months. Till this date there is no recurrence of the disease. Ayurvedic treatment helped in complete management without any surgical procedures.

Ayurveda described various skin diseases under the single heading Kushta. In Charaka Samhita, Acharya Charaka explained Ashtadasha Kushtas in two main categories namely ‘Maha Kushta’ and ‘Kshudra Kushta’. Kushta is tridoshaja vyadi, however, the symptoms appear according to the dosha predominance. Vipadika is one of the Kshudra Kushta with predominance of vata kapha doshas and it is charactrised by Pani Pada sputana (fissures in palms and soles) and Teevra vedana (severe pain). According to clinical manifestations, Vipadika is more similar to ‘Planto Palmar Psoriasis’ which is long lasting Auto Immune disease. It is found to be 3-4% of all Psoriasis cases. In modern science, many treatment modalities are available to treat the disease. They may cause many side effects and recurrence after subside is also very common which gave a big scope to alternative system of medicines to treat Vipadika. In the present case study, the line of management followed was mentioned by Acharya Charaka, according to dosha Predominance. The patient, initially administered sneha pana with Maha Tiktaka gritha for 7 days which was beneficial to pacify pitta dosha and kapha dosha utkleshana before Vamana. Later on, Vamana therapy was conducted to eliminate kapha dosha which gave good symptomatic relief to the patient. However, in view of the severity, chronicity and recurrence of the disease, Mridu Lavana jala Avagaha Sweda followed by Padhabyanga with pinda taila was also performed which resulted in excellent improvement in very short period of time.
